    Ruptured/Torn Achilles - Post surgery

    Ruptured/Torn Achilles (8/22/17) - I'll update my review as I progress, but I just finished surgery and have allot of time so thought I'd start a review. I ruptured my Achilles on a Thursday playing basketball. It was pretty obvious at the time because a co-worker of mine went through the same process and detailed how it felt (like a kick to the back of your ankle... but nobody touched you). You may be able to move your foot up and down alittle, or not at all, it will be really tight. I went to urgent care immediately that night. Urgent care can only asses the injury, do X-rays, and provide crutches. Crutches were good and I still use them. They cannot recommend an MRI which is needed to ensure you have torn you Achilles. The doctor that night said it was pretty obvious I tore a tendon, but didn't break any bones. The next morning I went directly to my primary care physician, only to find out she cannot recommend an MRI, and that I needed to find a specialist (orthopedists). This may seem obvious to some, but this is my first time facing this type of injury. I yelped 'orthopedists' and found that Doctor Heinen was both highly rated and seemed to have several reviews with surgical testimonies. After explaining my immediate need for consultation and my background, I was able to see him early afternoon (Friday). The building location (Arcadia) is close to my home (Temple City), looks modern and clean. Staff is friendly. I only waited about 30 min after arriving before getting into the office and seeing the Doctor. Doctor Heinen was able to confirm the ruptured Achilles without an MRI, and suggested I save my money, but I could still have it completed if I like. Very friendly and nice and helped explain how the tendon works. He explained both options (surgery or natural heal), recovery plan, and probabilities of re-rupture for both option. The timeline seemed similar, but with surgery, I have a less likely chance of re-rupture. I had a mindset of getting surgery from the beginning and this only made my choice easier. The staff worked quickly to verify my insurance would cover the cost of the boot, MRI, and surgery, which they do. Unfortunately I was unable to have the surgery until Tuesday (5 days after rupture). Living through the weekend with a ruptured Achilles is something I wish upon no one. It wasn't very painful unless you accidentally extend it past its conform zone (which is small). Doctor Heinen did offer me a protective boot and pain medicine, but at the time it didn't seem like I needed it (I'm an idiot!... remember, my first time too). Surgery - 9:15a on Tuesday @ Specialty Surgical Center. Can't eat or drink after midnight the day before. Place is COLD so bring a sweater or jacket despite how hot it is outside. Nice and modern building and the surgical center is ground floor so very convenient. Staff friendly and experienced it seems. Surgery takes approximately 45 minutes, but I was knocked out another 1 afterwards. Everything went well from what the nurses and what Doctor Heinen said, but I'll learn more on Friday during the follow-up with Doctor Heinen. I'll keep you posted. UPDATE! 5/25/2018 Super late to update but long story short, everything went amazing. I was walking again in 2 months (not recommended by doctor or physical therapy... but I had to get on with life and it felt good). After walking again I dropped out of physical therapy and seeing the Doctor, but overall I think everything went great. The Achilles' feels really strong. I can obviously walk, light jog/run, jump, go bowling, carry my 3yr girl, etc. I did most of that at 3 months. I could play alittle ball but mostly just pass... no driving to the hoop yet. Keep in mind that I don't regularly stretch or work my Achilles like I should. Something to note. After the surgery, while putting on the long term cast, Dr. Heinen positioned my foot at 90 degrees from my leg. Normally your foot would be extended like a ballerina, then after the cast is removed, you would spend several months in physical therapy slowly working your foot towards a 90 degree angle from your leg. Usually at a pace of 1in/week. By moving my foot at 90 degrees before cast, he saved me about 6 week of physical therapy. The process did HURT, but again... I was walking in 2 months (priceless). Also Note. After surgery, you MUST BUY the iWalk 2.0! This thing gave me back my life. It is worth it's weight in gold. I used it at work, at home, everywhere. If I lost my leg permanently, this would be physically attached to me.

I decided to have my 2nd appt with Dr. Moscarello. What a huge difference. He spent about 3 x the time with me and went over the makeup of the quad tendon, how it tore and explained things from my MRI with me as well. Just a very knowledgeable guy. Very nice gentleman too. I left there feeling informed and like my time was well spent with him. Thanks Doctor!

I received good care for my knee arthritis, but was dismayed that they did not use ultra-sound for PRP injections. So I went elsewhere. My back is where it went bad. I was diagnosed with scoliosis, spinal stenosis, spondylolisthesis, osteoarthritis, degenerative disc and halitosis :) I received cortisone epidurals, the second one was 9 months after the first. When I requested the second injection the doctor said I was getting too many shots and I needed surgery. Fact, you can get 3-4 injections per year. The fact the doctor lied to me was the last straw. Went to Pasadena Ortho, they answer the phone and scheduling is easy. They supplied me with exercises for my back and knee as well as a medical knee brace which I no longer need. Imagine my shock when the back doctor after a very thorough exam said my back pain was actually from lumbar facet joint syndrome. He referred me to an anesthesiologist/ pain specialist in their office. He did a very thorough exam and confirmed the diagnosis. Surgery was not required and all I needed was facet joint ablation (radio frequency kill the nerves being squeezed) which last 9-12+ months. I am astounded that Risser misdiagnosed me, lied and I could have had an unnecessary surgery.

Even here, the problem isn't necessarily with my doctor. Rather, it's with the staff of both places, but especially at the Arcadia location. In Arcadia there is the medical office on one side and the physical therapy area on the other side of the waiting room. The physical therapy area is perfectly fine. I did rehab on my ankle there, and everyone was great. However, when I had horrible arm troubles with my hands suddenly seizing up, I talked to my ankle doctor and got the name of the one doctor at both locations who handle arms as their focus. I called Arcadia, and they couldn't get me in despite it being an emergency for at least 2 months. I called the Pasadena location, and they also had a wait. I asked about a wait list at both places, but when I asked Pasadena, they put me on hold and apparently called Arcadia. Suddenly I could get in a couple of days later. Thank God! After my first visit, things seemed to get better until I took another horrible fall and feared I had hyperextended my other arm at the elbow and shoulder. I called about having an accident and once again was told it would be 2 months. I asked about a wait list and was told to call back every day. I still had to wait about 5 weeks, which is forever when you've had a devastating fall. It took another couple of weeks to get my MRI done there due to waits. And then no one called me with results. One week went by. Then another. So I called and asked if they got my results. They had. Well why didn't the doctor call me? Well they don't call you when they get the results. It's up to you to call back and set up an appointment if you want to hear the results. What?! Even better, the doctor was apparently going on vacation for two weeks, so I'd have to wait about 6 weeks at the earliest to get in. I freaked out on them. I mean did I take crazy pills. They had the doctor do a telehealth call with me in a day or two, but then when I asked him if not calling the patient was actual proper protocol, he was just defensive and said he had been on vacation for weeks (not that he was going on vacation the following week). The whole office is a clusterf*ck! The doctors are knowledgeable and some are better than others, but the office is run by clueless maniacs. If you can find another decent place, go there and not here.

As I was leaving the office my Dr called and said to make sure I get an X Rey and I went back from the front door back to ask for an X Rey, they said I have to wait til all the patients are done because I've checked out and basically I'm in the back of the line. So I left. Then they called me to say they were going to do my x ray now. I am not a child for them to treat me like that. Sad place!!

Front office is awesome, submitting authorizations seemingly as soon as I left the office, didn't waste any time. Due to a job change our insurance was running out and once approved Lisa the scheduler got me in asap. Same situation with my other knee 6 weeks later with the exception of it needing replaced instead of revised. Got me in and done in a couple weeks. All the follow ups have gone very well, due to a scheduling misunderstanding (I forgot and missed a follow up showing up the following day at their lunch time) they still got me in and seen ON THEIR LUNCH!     I came in to go over my options after injuring my right knee in a fall a few years ago. The injury…read morecaused a meniscus tear, and I haven't been able to fully straighten my knee since then. However, I had limited knee pain and no difficulty walking; the main issue was painful compensation on my left side. He outlined several treatment options, but the conversation seemed to focus on dismissing alternatives in favor of a total knee replacement which felt far too aggressive at this stage. It came across more like a sales pitch than a recommendation based on my best interests. There is no patient portal, nothing digitized, no texts, nothing. I personally find these features immensely helpful in understanding and organizing my ongoing treatment. Seems outdated since I don't think I have any providers without a portal these days. The few pages of pre-surgery information they sent me by mail seemed painfully inadequate. When I made the initial appt, they told me they accept Blue Shield PPO, but when I looked at the claim in the BS portal yesterday I noticed they were out of network. When I called to ask about it, I was told I should have asked or looked into it on my own, but I very rarely ever run into providers who aren't in network so I didn't think of it. So I guess that's on me. Maybe I'm being picky, but I feel that is a point worth mentioning especially when considering the huge expense of major surgery. Some of the portals that I mentioned above also let you know your insurance summary *before* the appt. To be fair, he was easy to talk to, listened to my concerns, and did not rush through the appointment. Clearly he has helped a lot of people so he's doing something right. But after considering all of the above, I decided not to move forward with the surgery.
