I have had wonderful service from the Nurses Maryann and Jill. They have cared for both my husband after surgery 7 days a week and for my mother with multiple issues. When they say they are going to come they show up no issues. They show great concern, are available for questions by text and have aided in the recovery of my husband not getting an infection and in care of my mother with multiple health issues and falls. My mother is 91 and also has bath aide with Yolanda, this has been a huge help to me to have this assistance twice a week and has take some weight off my shoulders. She is so very kind and patient with my mother which is beyond wonderful. We have had Stan for PT for both my husband after surgery and my mom after falls and he is OUTSTANDING, compassionate and keeps my mom laughing which helps her spirit. If we need PT we always request Stan! My reason for only giving 3 instead of 5 is NOT the Nurses, Aides or PT it is SUPPLIES - they take months and I have to purchase supplies out of my own pocket then the supplies show up 2-3 months after ordered and WRONG things come, not even what was ordered. For example my husband had foot surgery and supplies were ordered by Jill, followed up at least 5 times and finally THREE MONTHS later they show up and there are multiple things never ordered in the box. In the meantime to avoid an infection I had to purchase supplies. Our recent order for a fall for my mom Jill placed an order on 07/10 and today is 07/22 - NOTHING - multiple follow ups by Jill and Maryann and still nothing. I am not sure why it takes so long for the supplies that are urgently needed for treatment to stop infections. If it is the company Bryam then SWITCH - but this has been an issue for over a year and is very concerning. I have spent hundreds of dollars purchasing supplies that either show up months later or never show up at all. My insurance is being billed for these supplies, but they show up LATE, if treatment was held off waiting on these supplies my husband could have lost his foot from infection. This place is a freaking joke. I am currently in the hospital for almost 2 weeks now because these dolts can't get their crap together. Ordering supplies is apparently something that can't handle. If it was for my nurse who has taken great care of me, I would have left this sh!t show a long time ago. I've literally done all the work for them and they still can't figure it out. The hospital staff is dumbfounded as to how disorganized this place is. I wouldn't normally leave a review unless it's a good experience but this has been such a clown show that I can't stay quiet. Not sure how the he'll they have decent reviews if THEY CANT EVEN HANDLE A SIMPLE SUPPLY ORDER. There has been a problem each time I've needed supplies since I started with them in Sept 2025. Do yourself a favor and avoid dealing with these imbeciles. THE INFECTION IN MY FOOT CAME BACK DUE TO NOT JAVING THE PROPER SUPPLIES QHEN I NEEDED THEN which is why I'm currently in the hospital now. This home health agency has been extremely disorganized. On multiple occasions, appointments were scheduled by one staff member without other team members knowing who was coming or when. There appeared to be little internal communication, which is alarming when you're dealing with medically vulnerable patients. After my mom's cancer surgery, she was losing a significant amount of weight. I specifically requested for her home health visits from Green Meadows that she be weighed at every visit so I could monitor whether an ER visit might become necessary. That request was not followed by the home health PT, John. During multiple home visits, her weight was not documented or communicated to me. When I contacted the nursing manager, Tiffany, to inform her that my mom had now declined to 66 pounds and was being hospitalized at the ER because of it, I expected at minimum concern or acknowledgment in addition to an apology. Instead, there was no apology, no expression of urgency, and no accountability. I was simply told, "I already brought this up to the case manager." The tone of that response was dismissive and indifferent, especially given that my mother had just been admitted to the ER after severe weight loss that was never flagged during their home health visits. At no point were we alerted to her decline despite having requested for weight updates after every visit. When you are caring for patients recovering from major surgery, basic monitoring and communication are not optional. Families depend on these services to catch exactly this kind of deterioration. Unfortunately, that did not happen here. I hope this agency implements better coordination, clearer accountability, and more compassionate communication moving forward so other families do not experience what we did. Update: Shortly after our family terminated services when my mother was admitted to the hospital and we informed management that we would be transitioning to a different home health company, representatives from this agency still continued contacting us. Despite clearly notifying one of their managers that services were ending, a different physical therapist reached out the following day asking to schedule another visit as if care were still ongoing. Around the same time, different staff also continued asking which hospital she had been taken to, even though they were no longer involved in her care. This was deeply uncomfortable and intrusive, as our family expected our decision and our privacy to be respected once services were terminated. read more

Green Meadows came to evaluate my mother after her stay in the hospital. They came had her sign the paper work and then came a couple of times and that was it. It was almost like they just wanted the payment from the Insurance. When the nurse did come out she put ointment on the wound that was supposed to get iodine to dry the wound not moisten it. When I asked her about it she said to trust her? Then she never showed up on the time she said she would and then just stopped coming. My advice to you is if Green Meadows is assigned to your mother or loved one, call your primary immediately and have them schedule another home health care facility. Avoid Green Meadows at all costs. Amir has been the perfect therapist for my 77-year-old mom. She had been struggling with mobility after a hospital stay, but Amir got her standing up and walking in a couple sessions. Horrible place. I had a surgery for brain aneurysm and it took this place a week for someone to come out for an assessment. After that initial assessment but the nurse another week has passed and no update about PT/RN/OT. I had to call them to see what was going on. The coordinator then said "oh we will reach out to the PT". So PT came once and no follow up week after, I haven't even heard from the nurse or OT again or PT for follow up. Worst home care ever!!!!!!! I was required to give a star rating, if I could I would've came ZERO stars. I have worked in 3 homecare before and not one of them were ever this negligent!!! read more

This is the worst Home Health Company I've ever dealt with! My 90 year old mother almost died under their care! I called them at 5:00 pm last night to come and look at my mom they told me they do not have any nurses available after 5:00 pm!!! The entire purpose for home health is to be available 24/7. My mother ended up in hospital with an infection and 103 degree fever ! I would never use them again! I'm appalled by their careless irresponsible attitude! After my moms stay at the hospital Green Meadows staff immediately reached out to schedule nurses OT & PT appointments and take the burden off me and i am truly so grateful. Amir her PT has been amazing. He saw and changed things in the first appointment to help aid her getting around eaiser and help minimize her pain. Then they disconnected me. I called back 3 times and all 3 times they disconnected. Then one last try and Verizon Wireless said the call could not be connected! This review is for home health care Caseworkers, Carmen and Brian. My dad had knee surgery and it has been a roller coaster since then. First, I was told by his surgeons office my dad was supposed to get a nurse visit along with PT. I spoke with Brian at Green Meadows. He listened to me and my Concerns. He helped me in trying to Look for the authorization as to why the surgeons office has it, but Green Meadows doesn't. With him I felt I was being heard. Two days later after going back-and-forth I spoke with someone else when I requested to speak to a manager at the surgeons office. I was then told that there was never an authorization for the nurse visit even though someone else told me there was. The office gave me the wrong information which I proceeded to let this person know of the Mess and stress this has caused for the past two days. I then called Brian to let him know what has happened. He was very kind and understood. I thanked him for all his help. Second incident, The same week. After taking my mom to her doctors appointment while I was out to dinner I get a frantic call from my stepmom. She then informed me that the PT that was supposed to come that day (which would have been the 2nd visit) will not be coming due to the authorization for PT being canceled. Not knowing why I call Brian. I explain the situation and then I spoke with Carmen. She informed me that she's aware of the issue. she is looking into it and she will help us. This happened on a Friday evening. Monday came. I took my dad to the doctors. Guess what the doctor said oh he doesn't need at home PT anymore. Needless to say my family and I were very frustrated. I want to point out that not once from any of the interactions that I had with Brian or Carmen did I feel like they did not care. It's stuck in my mind with Carmen said no I won't leave him hanging like this. I am very grateful and appreciative for their help. As well as their understanding and taking the initiative. Beware. I have filed a complaint with the Medicare fraud department about Green Meadows Hospice. They persuade elderly individuals to enroll in the Medicare hospice program by offering "free perks" such as meals, bathing assistance, in-home doctor visits, and prescriptions delivered to their door. To qualify these individuals, they falsify diagnoses on the Medicare application. Once enrolled, the elderly people receive unnecessary medical equipment like wheelchairs, oxygen tanks, and pumps, without being informed or asked if they need or want these items. This is a common tactic in Medicare fraud. Dishonest hospice companies then bill Medicare for these services and supplies, often inflating the costs. Contrary to the promises of "free" services, it comes at a cost. When the elderly person passes away, Medicare attempts to recover the expenses through its Estate Recovery Program. This program files claims against the deceased's assets and takes money from the sale of their property. These hospice companies never inform the elderly individuals about this recovery process. Instead, they falsely claim everything is free and without cost to the patient. It is a deceptive and unethical practice. read more

My father was "cared" for by Green Meadows for over a year and had endless troubles with getting the appropriate amount of medication and having it delivered in a timely manner. It wasn't until we decided to switch to a new hospice that we realized just how sub-par his care had been. His nurse was nice enough, but in over a year he never met with an actual doctor. Save yourself a lot of frustration and go with another hospice company to care for your loved one. read more

Our nurse, Mary Ann is amazing. She provided excellent service treating my daughter at our home with cleaning the wound, new bandages, foam dressing pads at every visit, hooked up the wound vac therapy system and replaced hoses and containers. Helped with ordering the correct supplies. She is the only nurse that could properly install the wound vac system so it would not beep due to an air leak. She explained the process of healing and treatment to family members and kept us up to date on scheduled visits. Mary Ann even came to our house on the weekend and said she is only a phone call away. My daughter is fortunate to have Mary Ann as her nurse. Also, we did speak with Carmen and Brian who helped get authorization to continue treatment. The only problem we had was the Quick-Connector tubing came out of the wound vac system a couple of times and the wound vac supplier did not provide extra components to replace a non-functioning hose.
