(Takes a good minute to fill out but it's standard in most offices so just be prepared). It's mostly information you know, signatures, DL information & payment at the end of course. Then you go in for your consultation, I waited for the doctor and then he does the consult and some work by the assistant, then off to the next room for an adjustment by the doctor and done. Ruby was the one who did my electric treatment and she was super sweet and kind! The doctor was very thorough, answers all your questions and explains his findings as well as recommended treatment.
