I have spent years considering botox and lip filler and after I was over feeling insecure, I started my research on doctors. After reviewing countless resumes, I found Dr. Powers and was impressed. Within the first 5 minutes of meeting her, I knew I'd made the right choice. Dr. Powers is an artist! She is so thoughtful, detail orientated and truly has an eye for aesthetics. I loved how she took the time to ask me the right questions and for any questions I had, she was thorough with her answers. What I really appreciate about Dr. Powers is she is really friendly and incredibly knowledgeable, so it was easy for me to trust her and feel safe in her care. Throughout my visits with her she has always made me feel so comfortable and not pressured or rushed. I have done both botox and lip filler injections with her and did not bruise with either! Nor felt any pain during any of the procedures. My visits with her have been so therapeutic and reassuring because she takes the time to walk me through each step of the process and asks how I'm doing throughout. What I also think is amazing about her is hearing her observations and sharing her input in what would give me optimal results. The best example was when she handed me the mirror to look at how my lips looked after the injection, and she ever so kindly pointed out some areas where she felt that could use a little more filler in - AND I AM SO GLAD I listened to her because I LOVE HOW MY LIPS LOOK! They are better than what I imagined. I absolutely love how I look and feel about myself. I wish I met her sooner! Ciana does my laser hair removal and does a great job- she's professional and courteous and knows i have a busy schedule so she gets me in and out as quick as possible. It's pretty painless and definitely worth the investment. The only negative feedback relates to the check-out process. I tend to spend more time explaining to the receptionists (who are all super nice and cute) what I got done and that I have a membership than the time the actual procedure takes. I got my nose done by Dr Ward in January 2018. For cosmetic purposes as well as for functionality purposes. I've had chronically swollen turbinates since I was 20 years old, leaving me feeling claustrophobic in my body at all times. Barely being able to breathe out of a tiny hole in one of my nostrils makes exercising extremely difficult, and I do not sleep well because I'm forced to sleep with my mouth open. Dr. Ward was supposed to fix that. He was paid thousands of dollars to fix this problem. Although I love my nose cosmetically, inside of my nose is the same, if not worse. I was told to wait a year, that the swelling would go down. But it never did. His solution to the problem is to pay him more for his time to fix HIS mistake he made in my surgery. Well, money doesn't grow on trees when you're 27 and so I'm left feeling like a prisoner in my own body. Unable to breathe out of my nose, the same problems I had before dropping thousands of dollars. If you would like a beautiful nose, absolutely see Dr. Ward. If you'd like to have a functional nose, stay FAR FAR away. I feel absolutely robbed and devastated at having no hope for breathing correctly. Until I have hundreds of dollars to pay a different Dr. to fix Dr. Ward's mistake. read more

Update: it has come to my attention that Form no longer provides the services of Dr Chidester. I will only go where he can be the provider. You can visit a beautiful spa anywhere, but quality care and integrity in business is why I will take my money to where Dr Chidester is. Form Spa is absolutely beautiful! All the staff is so kind and friendly. Dr Chidester is so friendly and easy to talk to, yet very professional. He made me feel comfortable and answered all the questions I had. My experience was awful. Billing/insurance person, Annette is a joke. I received a free "just right" lip filler as stated on my surgery treatment plan & gift certificate. Two months after I had these services completed, Annette said my gift certificate & filler was not valid. She wanted me to pay for the service. They do not validate their own gift certificates or follow through on their own offers. The nurse was very kind though and apologized. read more

Went in for a "free" consultation. I was never informed during this visit that I would be billed to my insurance $490 having my out of pocket be $200 for a "free" visit. I called the spa as soon as I received this bill confused since I just went in for a consultation and decided not to follow through with my procedure. Initially when I called on Saturday 8/4/18 I was informed by their receptionist that this had to be a mistake and she couldn't see why they would have charged me for a consultation and told me she would have the billing office call on Monday, which they never did. After calling again today 8/8/18 I was then informed that since they decided my procedure was medically necessary they billed my insurance because it was now considered a medical visit and no longer free. Again, I was never informed of any of this. Even the front desk on Saturday didn't have that explanation and supposedly they are the ones who were supposed to inform me of this. They offered me absolutely no resolution except an apology of the misunderstanding and assuring me I signed documentation preventing me from fighting this charge. I would not recommend anyone but my worst enemy to work with this spa. read more

I recently saw Kayla for first-time Dysport injections and she was amazing! She took extra time to answer all my questions and made me feel comfortable. She was professional and cautious because of my concerns and went above and beyond. She also helped me with skincare, and at my follow up appointment, I mentioned that I had dropped the bottle at home and spilled some of the serum. She insisted on sending me home with a sample to make up for it. She has provided outstanding service you don't often see these days; I'll definitely be coming back regularly. read more
