I was asked to submit a second form of proof since the birth certificate I submitted did "not have parent's names." I do not know a lot of birth certificates that are missing parent's names. Even though I disagreed I submitted another birth certificate by mail. Plus my Son is 16, according to US passport website, a 16 year old does not need proof of consent. He just needs "proof of parental awareness " which was provided when I signed his passport application and gave up a copy of my ID and passport at the local county clerk of court official records devision, the lady verified my identity and signed she witnessed I am the father. Yet the US passport office has yet to issue my son's passport. As previously stated, it is now more than 3 months. I have called and verified the website but the status stays the same. There is no help at all when I request an appointment to the closest passport office neither. Honestly I am exhausted of calling and verifying every day and seeing there is no progress.
