This place put my mother on hospice who is not even terminal. They are being investigated by the adult protective services and Illinois department on aging Dr idell Woodson was her primary care who signed of on the hospice papers. Courtney Kent is the nurse who signed papers . The other nurse Michelle and the social worker Molly I believe her name is all are being looked into along with the inhibit doctor christopher m. The social worker hung up the phone on me when being questioned. I wouldn't recommend this company for a dog. Unethical. They took my mom who is not terminal off of her everyday medicine to keep her alive. Feel free to ask more questions I have all the paperwork including a video of my mother saying she does not want to die and she didn't know my sister maria Andrew's and Courtney put her on hospice read more

After a few months, we moved him over to The Arbor Memory Care because of change in his health. Again, the employees jumped over hoops to make sure he was happy. I truly feel funny calling them employees., because they are more like extended family to us as well as dad. Now, dad is in the final stages of his life on hospice at The Arbor, and I want everyone to know that there can't possible be a better place for his last days then being cared for by these wonderful people with such huge hearts. My grandparents…read morehave been through all levels of care from a 2-bedroom, independent apartment through the acute care center. The facilities are immaculate and the staff caring, friendly, and extremely competent. When my grandparents first moved in, my grandfather was just beginning to have his health failing, so they moved into a two bedroom apartment. They were extremely hesitant about moving into a senior care center such as this, having had very bad experiences in the past with you typical stereotype "Nursing Homes" in the 70s and 80s. However, we had really high hopes that conditions have improved. BOY HAVE THEY! Both of my grandparents really enjoyed the social activities, scheduled outings, movies and cultural programs, and even athletic programs such as aerobics and WII bowling. They also rave about the food. The chef and all of the dining staff, as well as the building staff in general are great. They really take an interest in the residents and treat them like family. They also have shuttle services to take the residents to doctors appointments, grocery shopping, and even occasional shows and outings. If I need a live in care center at that age, I will be happy if it is close to as great as Concordia.

For the first 10 years, they could not have been more satisfied. In 2019 a consistent number of personal and physical setbacks altered their situation and their needs. While upholding the policies of Concordia as far as moving from one level of care to another, they moved into assisted living for a short time. Just a few months into their move to assisted living, Dad's decline accelerated to the point of needing long term care. The staff were compassionate, attentive and supportive. Both Mom and Dad felt loved. is Concordia perfect? No. Where is?

We couldn't have asked for better care for my father-in-law during his hospice stay, the facility…read moreis older but in amazing shape, spotless, no smell, the staff are kind and caring and were attentive to his and our every need. The residents seem very happy and are kept quite busy with all the available activities. They had 2 musical performances during our stay and the living room was full of residents. From what I saw they encourage them to stay out of their rooms and in very social settings which is a far cry from Lewis who tried to rush them to bed after dinner. I wish we had found them for his rehab as the area is large, bright and a variety of equipment. It is a drive to Mt. Pulaski from Springfield but well worth it. I highly recommend if you or a loved one need care to consider them, we were impressed from the moment we hit the grounds. They have independent living around the outside and as needs change the resident can be moved to the main building. I'd like to thank you for the dignified care you gave him during his final days, you kept him comfortable physically, spiritually and emotionally.

Dr. Villafuerte is perfect but the office staff has not been. One gal told me that blood work was…read morenot done there and had never been when I knew good and well that was untrue and in fact did get my blood work done there that same day. Same girl asked me why I needed the blood work done; um, excuse me, that's none of your business! I don't expect office staff to be as perfect as the doctor but I do expect professionalism. This particular girl was layers lower than the doctor. Dr. Villafuerte deserves much better service from the staff so that she herself does not look bad. (This happened awhile back, to be fair. )
