The Wonderful Staff at Creekside gave me my Mom(Linda) back! I toured the facility with Zharlene on short notice and was impressed with her and the facility. I liked that it had recently been renovated and had an Updated PT Department compared to what I had seen. My mom recently stayed at Creekside 9 weeks for rehab and she is a complex case due to severe osteoarthritis. Upon arrival she could barely stand up and walk 50ft. - she required 2 therapist during her PT sessions at first. It wasn't long before she was walking further and progressing in both OT and PT. The therapy team is Great! - Angelique, Anisha, Kiko, and Natalie worked with mom often and made amazing progress with her. Aloha Randy! Watching the entire therapy team in action everyday was impressive as to the range of skills they demonstrate patient to patient. Mom is home now walking around the house and outside. This was the primary goal for her being at Creekside. The CNA's are truly Special! Most of them treated my mom very well and kept her clean and if she needed something they took care of it. She had a couple favorites I would like to mention for well deserved recognition - Sauli - Tirhas - Carolina - Jonjie - Thank You for everything you did for my mom!! I really Appreciate YOU! Big Shout Out to RN's Sheri and Hafsa for getting mom her pain meds On Time! The kitchen Boss - Nakea checked in with mom to see if she wanted the menu item for the day or an alternative for Dinner - Very Nice! Annie is Awesome! She was moms Case Manager - All of the issues I brought to her attention were resolved quickly and professionally. I completely trusted her and valued her advise when needed. Emil and Annie make a Great Team and seem to run the place very well. Emil was moms Care Coordinator - Very Hard Working and easy to work with. We had weekly meetings regarding important details about mom's care. My mom had a private room during her stay at Creekside and I was with her most of the time to keep her company. I felt comfortable leaving her there alone at night. Her only complaints were the room was cold and the food not so good but we brought in outside food often. I was released to Creekside for post surgery care in 2 phases after breaking my leg. The staff were all amazing, considerate of my limitations but working with me to show me what I could do. The responsiveness of the nurses and CNAs insured I was well taken care of since I could not get up to do anything on my own. The Occupational and Physical Therapy teams were top notch - they really know their field and made sure I was well educated on my own care once I was released home. I highly recommend the facility, from the administration to the nutrition and housekeeping teams and everyone in between, you or your loved ones will receive excellent care! read more

Very disappointed with the quality of care and rehabilitation services provided to my mom. The physical therapy team and management lacked the professionalism, consistency, accountability, and commitment necessary to help patients make meaningful progress. Instead of providing a clear rehabilitation plan, setting measurable goals, and actively working through obstacles, there was often little guidance on how patients could advance toward recovery. Equally concerning, the physicians appeared reluctant to provide clear medical opinions regarding therapy needs, rehabilitation potential, and the most appropriate path to recovery, despite being in the best position to guide these decisions. A rehabilitation facility should be focused on problem-solving, collaboration, and helping patients achieve the highest level of independence possible. Unfortunately, our experience reflected a lack of leadership, coordination, and commitment to patient progress. read more

So as an update to my previous review Kaiser called us to let us know that creekside did not cancel the transportation so it was not their fault but that still doesn't change the fact they REFUSED to give my husband the transportation information before he left them a good review with their link sent to him . That doesn't change the fact the staff here don't care about the patients and staff can be seen roaming the halls not helping their patients on their phones and just worried about going to get their own food I never saw the CNAs helping feed patients good luck if your loved one has a special diet they don't provide any options for low sodium diabetic or otherwise. So like I said if your loved one is here check on them often because the staff dont. read more

This place is awesome right from day1 03/15/2025. I was admitted here due to fractured right hip. I nervous at first but was comfortable in less than 24 hours. The CNA's,PT's,OT's and RN's are so friendly oh and even the residents are really such a help with my emotional recovery. Was there for only two weeks but I believe I made a lot of friends. Actually on 2 fridays I was there we had this exercise right before lunch like people on rehabilitation. That I enjoyed so much that I cried and open my eyes. See I stopped all my meds 3 to 4 years thinking that if I get sick enough that my wife would take me back crazy right. At one point my A1c was at 19 which is unheard of. Coz I gave up on life already. It was here at this facility that Eloida witnessed to me telling me to ask God for what my purpose is. Coz God always spares my life. I felt like a new man when i left this place.Emil one of the Rns told me that I have a good heart Just need to learn how to forgive myself. So what I'm trying to say is this place is where I got my second chance in life. Love this place my goal is to comeback one of this days and visit on my own without a walker read more

I cannot say enough good things about this nursing home and the amazing team behind it. Every department plays a vital role: the nurses and CNAs provide attentive medical and personal care, the therapists support each resident's recovery, helping them regain physical strength and daily function, housekeeping and maintenance keep the entire place spotless and comfortable, and the kitchen staff ensure everyone enjoys nutritious, delicious meals. Together, they create a safe, clean, and truly welcoming home where residents feel cared for in every way. I'd also like to give a special shout-out to some of the staff here. Griselda in admissions is truly remarkable -- she never hesitates to take on extra tasks, even when they fall outside her role, and she always does so with a positive attitude and a willingness to help wherever she's needed. Cristina in admissions is an absolute powerhouse; she works tirelessly and is the key reason so many new residents choose this nursing home, thanks to her dedication and hard work. Zharlene in admissions is an excellent marketer who does a fantastic job building strong connections with other hospitals and healthcare providers, making sure our facility stays top of mind for those in need of quality care. Shelley and Sofia in social services are always on top of things -- they are constantly holding meetings, coordinating care plans, and going the extra mile to ensure that every resident's social and emotional needs are met with compassion and professionalism. Jocelyne, Dhalia, and Kimberly keep the business office running smoothly and efficiently; they handle every detail with care and make sure families and staff alike have the support they need behind the scenes. The nurses, therapists, and CNAs are the heart of this nursing home -- their dedication and hard work make all the difference. And at the center of it all is Gunner and Josie, the administrator and the DON, who wear many hats and go above and beyond to ensure the nursing home runs like a family. Their commitment and tireless efforts tie everything together. My brother was there for two weeks after his knee surgery at Kaiser, he was never given his pain medications on time until we decided to keep an schedule ourselves and agreed that he would start asking the nurse for pills to get them on time and regularly, on top of that he was giving an ice machine at Kaiser to ice his knee regularly so he brought it to the facility an it was his to bring home so they didn't like that and started to bring him ice bags so the machine was left in a corner but after some days it was gone and when my brother realized that and ask the stuff they said it was lost so yes it was lost for 3 days and he kept asking for his ice machine until it appeared on his release day but the knee pad that came with it was missing then when signing release papers the head nurse tried to convince me that the ice machine was brought without the knee pad from Kaiser from the beginning so the first two days that he was at the facility and used the machine and that I personally helped him with it are in our imagination or we make that up? That was insulting! It's sad that you go to one of this places to recover and needy of help but while there you must keep an eye on your belongings and that's how we felt after asking numerous stuff members for the lost ice machine and nobody saw it or new anything and then suddenly appeared three days after lost and incomplete. Last, when signing my brother's release I asked for the medicine I had brought from Kaiser and I was handed one bottle of pills when I confirmed the nurse that I had brought a bag with 8 bottles of pills/medicines she rushed outside the room and came back in with all the medicine that I had brought and explained to me that it was left in one of the medicine carts and that she didn't know about it.... Not a very organized system or don't know what to think. read more

My father was in Creekside for two weeks in March, 2025, but they released him prematurely as we felt he was very weak and had trouble standing up and getting off the toilet. As for the care, I have to say that this place seemed to have staff that were quick to act upon the patient needing something and we felt we had a rapport with the entire staff. Ok, my father, who is 95 got pneumonia and suffered from a bacterial infection when they prematurely released him. He was home for three days and went to Kaiser Hospital and was again referred to Creekside after two weeks or so in Kaiser. Though we felt they had released my dad too soon, we went along with the staff's recommendations and all seemed copacetic as before. Fast forward to the second stint at CS, dad was released on April 7 and was brought home by ambulance, as we thought he might feel weak and have trouble going up the very steep five steps that lead to our front porch. This time, we opted for the ambulatory transport and we thought all was well. Dad seemed stronger this time and had been home for three hours when we got a phone call from our county sheriff. The sheriff seemed puzzled as to the reason for the call, but told my mother that Creekside had called him and reported that my mother and I might be abusing my father, as we had gone "against medical advice" and refused to hire in-home caretakers for the aftercare phase. The sheriff claimed that he had never had a call of this nature before and didn't really understand why he was calling. We thought the call was misplaced and very odd, as we had voiced our intention to hire caretakers and we couldn't imagine why in the hay the sheriff would be getting involved. I called CS the next day and asked Miss S why they had felt the need to notify the sheriff, which in itself was downright bizarre. She claimed that since we were defying Creekside's directives and "going against medical advice" (AMA), they had simply used standard operating procedure and had by law (a complete lie) to notify law enforecement. I pointed out that this is not a police state and that I had spent over an hour with the discharge nurse who who no time, during our interraction told me that he or any of the staff felt we were endangering my father's life by "going against medical advice. First of all, patients have rights California Patient Bill of Rights which stipulate that patients have the right to refuse care or treatment at any time and can even reinstate care if they change their mind. I spoke with Emile on my way out, the day of discharge and told him that we were working on getting caretakers via Kaiser and that we would pay the $250 for the ambulance service. I discovered that the sheriff had called me and he told me that the reason for his call, he thought, was that I had abandoned my father after the discharge process and left my father at 95 without a ride home. This was false as the entire staff, including Emile KNEW FULL WELL that we were paying for professional transport. Miss S, whom I spoke with after the sheriff's call, did not mention anything about any abandoned party and or lack of a ride home for my dad after being released. I was in a state of shock and wondered whether anyone over there actually communicated with each other. Had they done that, they would have known, as at least three people at CS did, that we had previously arranged for the ride home and we had also informed them as mentioned before, of our plans to have in-home care. Today a social worker from Kaiser called us and informed us that she needed to visit and check out our home because she claims that Creekside told her that we were abusing my father who has been married to my mom since 1959 and whom we love dearly. What?????Here was yet another person making false claims against and why? To date, there has been no evidence of any abuse or crime or mistreatment against my father, no indicia of injury or starvation, and CS is harrassing us and depriving us of our right as a family and per the California Patient Bill of Rights, as well as the Informed Consent and mandated reporter statutes, and our civil rights. Both sets of rights allow patients to refuse care and make their own family decisions, be it with or against medical advice. We are not a police state, which I pointed out to Miss S and we should feel comfortable and free to make these decisions without law enforcement intervention (ONLY IN THE MOST DIRE CIRCUMSTANCES). Needless to say, though we thought the care was better than average we are dismayed, insulted, and upset with CS for their lack of judgment and for intruding in our family affairs. I really hope that no one of you and your patient's family members suffers the same treatment. To say we are disappointed would be the understatement of the decade. read more

Toured the facility recently. The staff greeted me warmly and the admissions team gave me a great tour. I really liked their rehab gym that had access directly to the big open backyard area. The therapy team were engaged with their patients. Would recommend this facility for short term nursing and rehab care. read more

My 94 year old mom was placed at Creekside Post Acute Rehabilitation Center for a fractured upper arm. She stayed for about 20 days. Staff nurses Nadia and Alyssa; CNA Erica and Aki; Physical Therapist Justin; and Case Manager Emil were super helpful, attentive, and compassionate. Although the facility is older and noisy, the staff are positive and caring. Some people there are long term care residents. The residents are diverse from different cultures. Food is carefully prepared by a staff nutritionist. The social worker and activity director work with mom to make sure she is well cared for. Front desk nurses are cheerful snd responsive, in coordinating transportation and communicating promptly with staff about her needs. Our siblings take turns staying with mom during visiting hours. Bottom line: Mom feels like she's at home at Creekside. She had tough PT exercises at first but it paid off in the end. She was able to walk with her walker by herself and left the facility in good spirit. She made lots of friends. Thanks for a job well done! read more

1- My 73 years old mother was recently sent to Creekside Post Acute right after her knee surgery. The first welcome we got as we arrived was warm but the warmth quickly faded as my mom unable to get up on her own was left in her pee and wet bed for hours. Pushing the call button did not get her help right away but 15 to 30 minutes of waiting, sometimes one hour. This put her in an unsafe condition as she had to manage to get up in her own to use the bathroom. Every morning, I got to the rehab early to discover my mom laying all wet in her bed. 2- When my mom first arrived at the faculty, her doctor called the faculty and told the staff exactly what needed to be done for my mom (Icing 6-8 times a day, type of medication to be given along with the time to take it and the amount). The doctor's requirements were not followed, my mom's medications were messed up many times, some given to her to take when she was not supposed to, and others not giving to her until I mentioned it. Staff also did not ice her knee consistently or at all at times. I ended up icing my mother's knee more often than they did. 3- Patients' hygiene is not a concern for Creekside staff, you are lucky if they change the bed sheets every day. Mom's first morning, she was given a toothbrush, toothpaste and a bucket, with no help while she couldn't get out of bed on her own to even use the bathroom. The first time she was offered help to take a shower was at day 8. Fortunately for my mom, I was there every morning helping her with her hygiene, many other patients were not this lucky. 4- This rehabilitation place couldn't even manage to feed my mom a diabetic diet. She was given sugary food, juice and snacks over and over again, even when I told the staff over and over again that she is diabetic and gave them a list of what she can eat and not eat. 5- Communication was not passed on from crew to crew properly as shifts end and new ones start, this was very frustrating. Mistakes were made over and over, and I had to repeat myself at each shift. 6- The place was very noisy, staff were talking to each other loudly across the hall throughout the day from early morning to night without consideration for the patients who were trying to rest. Multiple days I arrived the faculty at 6:45 am to hear staff loudly talking to each other across the hall. I would also leave the faculty as late as 8pm hearing the same noise. My mom had trouble sleeping due to the constant loud noise. 7- Lastly, I took my mom out of this faculty early because her roommate tested positive for covid and was removed from her room. I felt that my mom's health was not prioritized since she was left in that same room that her roommate with covid had been in. My mom was in this room for an hour after her roommate tested positive for covid until I got there and pulled her from the room myself. 8- The positive thing about Creekside Post-Acute is their PT/OT department, they are knowledgeable, compassionate and kind. They explained the exercises and what needed to be done to get my mom up on her feet and walking in no time. The person in charge of the PT/OT department showed us great understanding and was very helpful. The reason why I'm giving 2 stars and not 1 is because of the PT/OT department. read more

Quality of care - this place is understaffed and the doctors don't seem to be able to keep up with the quality of care. Awful
