Overall, I was well taken care of. I had major back surgery prior to coming there, so I needed a lot of help at first. The CNA's were so nice and helpful and cheerful, I can't say enough good things about them. The nurses were good, for the most part too. They were very nice and helpful. The physical therapy staff were top notch, and very lovely people. I felt well cared for there. If I had to go back there I definitely would. Friendly and welcoming and assisted me with our friend's needs. Answered all my questions. All the staff were professional and assisted our friend in a timely manner. When you entered the building, it is clean and orderly you are welcome by staff on entering the building. Alieem does an excellent job at the reception desk, especially during the very busy day shift. He remains professional, patient, and organized while handling a high volume of visitors and calls. I also want to recognize the rest of the reception team--Benny, Nelly, and Ubnesh--who consistently do a great job as well. Reception is a demanding role and they represent the facility very well. Given the workload and responsibilities of the day shift, Alieem in particular deserves recognition and a raise, and the reception team overall would benefit from both raises and additional support. The facility does a great job in taking care of my mom. I wish the menu had more exciting deserts and gourmet dishes. The facility could use better funding to help with that. I was so exceptionally pleased with the care my 89yo mom received here. I researched rehab facilities very carefully before deciding on Plum Tree: I only looked at facilities with high ratings on the Medicare recommendation sure, and I visited at least 8 in person. Plum Tree was by far the nicest looking facility, with the most comfortable looking rooms. Once in the facility, we found the staff to be exceptionally caring, and really well skilled, from the doctor to the nurses to the aides. And absolutely top marks for their social services group, who went above and beyond to help us all get the services we needed. I hope we never need them again, but if we do, we are going straight to Plum Tree. read more

My mom's has been at Plum Tree now going on 3 years. The staff is wonderful! Alim at the front desk is "on it" with a smile and a great greet as soon as you walk into the facility. The facility is beautiful and open and inviting to guests, the dining room is the center hang out for activities and lunch visits, always decorated lovely (especially on the holidays) and they have two big outside beautiful patios for visiting. The atmosphere is always busy and cheerful. We are very blessed to have met so many wonderful CNAs and Nurses that work there to take care of our families, it's not an easy job, and not a very glorified job, but they still do their best with what they are given and have a warm smile, and time for visits and hugs. Upon my grandmother's stay at the hospital, I was in search for a care service for her to recover in. her grandma's request she wanted to stay close to home. I took a look at the reviews online and didn't think twice about it due to the location of this facility. We're going on our third week now of her being at the plum tree facility. In the first couple of days, everything seemed to be great! The nurse was very nice and was always providing proper care as they still do today. Unfortunately, there are some residents there that like to scream at the top of their lungs all throughout the day and night, disrupting my grandmother's rest. Now I can understand here and there some loud noises, but the consistency of this seems to be getting worse. I can put myself in plum tree shoes and understand the fact that not everyone that needs care is going to be mentally stable, but I believe that after a little while they would figure out a solution and or close the door for those disturbing the peace. This may just be at bad timing for my grandma stay but at some times during my visits, we have to close grandma's door and can still hear loud and clear the screams. I would be happy to revisit and update this review once this problem has a solution. Again, don't get me wrong, the care is good. read more

My father went to Plum Tree after his stroke. although it was and adjustment, they provided the right amount of rehab, the right amount of medical care and the right amount of compassion that my dad and my family needed. the outside courtyard was a psychological bonus - we could go outside for a little with my dad and try to be just a family and gave us a reprieve from the situation. I would not recommend bring your family here. My dad was here for only a week before we wanted to move him. My dad is battling cancer and had lost the ability to walk during that time. He had to come here for a little to build his strength. Unfortunately during this time he needed assistance with the bathroom and the pretty much made him sit in his mess for 30-45 minutes multiple times. This place sounded like a psych ward and was completely run down and dirty looking. He did say the food was good, so there was that one good thing. read more

My mom was here for 3 weeks after heart surgery and got discharged today. We have made a fantastic decision to choose this place as a rehab. All staff were friendly, and helpful and took care of her very well. Especially, I do appreciate all the nurses who are always kind, professional and sympathetic. [Nurses here are best....( Minh Chau, Mamita, Ruksan, Vanessa....etc..)] Moreover, I thank to all OT/ OTA for helping and be patient with her every tough steps to be back to normal. Everyone is nice, but way, way, way over crowded, kept admitting even though they didn't have the staff to support patients the correct way. Patients suffer. Takes forever to get help, the call lights goes off and you can hear a lot of them going off down the hall. My showers were missed because " the aid was off" or didn't show up.... days later I got one because my daughter complained. The last straw was they would not help me walk to the bathroom because they "didn't have enough staff, to go ahead and go, and we will clean you up, no problem." WTF? I said, you want me to crap my pants, and clean it up later? I had no choice, no one to help me walk. After getting a rash, needing destin and a UTI, I finally went to another one and called MEDICARE and complained about the demoralizing environment. Stay away. read more

This facility is one of the best I've seen in terms of the quality of care. They provide tasty meals, compassionate assistance and entertainment at Christmas. When we visit our 94-year-old uncle, we appreciate the staff help in keeping us informed. He is healing from surgery following breaking his femur and cracking his pelvic bone. He is grateful for their attention to his pain. He finds the physical therapist to be helpful, professional and good company. The only thing that reduces my "star" assignment is that my uncle, who NEVER complains, has found himself sitting waiting to use the bathroom or to return to his bed for long periods of time. He finds the only way to get attention sometimes is to be noisy, which makes him feel badly. read more

Huey and Tessy, along with the other staff, have great patience to care for my parents. My Mom is incapacitated after a stroke and a heart attack so she can cry loudly for hours during the day and they know how to soothe her. I don't know what i would do if I didn't have Plum Tree to provide this care for my parents. They are in a room together and as comfortable as people can be in this difficult stage in their life. My Dad says he loves the food. I would recommend that Plum Tree partner more with the families and the nurses and doctors in terms of communication via a web portal in the future. Sometimes I receive phone calls and then try to call back and no one knows who called me. If I could handle the communication electronically and ask the doctor my questions that would really help for a better experience. read more

Skilled Nursing Facilities are understandably not places we typically want to experience. My elderly mother was released from the hospital, after a fall, to Skilled Nursing. The last experience we had with a SNF four years back was barely tolerable. Luckily our experience with Plum Tree was completely different and has changed my outlook that a SNF is a place to avoid at all costs. The staff were amazing and so kind. The intake team and Social Services were efficient and helpful throughout the process of admission through discharge. They even followed up to check on my Mom's transition after discharge. The Nurses, CNA's, PT , OT and Activity Director were very kind, helpful and responsive. The facility physician was open and easily accessible. Even the food looked and smelled good! My mother and I really appreciate the kind, respectful care that she received at Plum Tree. In the future, this will be the SNF that I request should one be needed. read more

My mother was transferred to this facility after receiving treatment in valley medical for second and third degree burns and recovering from a skin graft. The first night she arrived she said the nurses would ignore her, and would not help her get up to use the restroom which resulted in wetting herself. I called the facility and asked to speak to the head nurse, they said they'd give me a call back and I never received one. I decided to drive down to the facility & my mother's bed reeked of urine and she plead me to take her out. Aside from her arriving at 4pm the day before she was never bathed and mentioned they had yet to change her bandages covering her fresh skin graft, exposed skin that has yet to heal which were also covered in urine. She told me it took over a half an hour for a nurse to respond to her call button and when they would they would tell her they'd be right back and walk right out never to return. After reading other reviews it looks like ignoring patients is a habitual occurence. I asked the nurse why my mother had not been taken care of and she responded that my mother could verbally express what she needed, even though they had ignored her when she expressed as much. I asked how often they did routine checks on their patients & another nurse responded "All the time" but no specific time such as a every 30 minutes, every hour? They had no response when I again asked specifically how OFTEN. I videotaped the entire conversation of questioning the staff, including my mother's dirty bandages and bed sheets. The nurse even admitted that they only had 1-2 registered nurses on duty for 60+ patients and that was her reasoning as to why my mother sat in her urine from 10am until 2pm and hadn't been bathed when I asked where the nurse was she said "I don't know" and then called her over the speakerphone. It wasn't until I arrived that they started to come into the room to care for my mother, they suddenly rolled out a wheel chair and had the nurse on standby to bathe her. I feel if the patients here don't have someone on their behalf like family to speak up for them they are treated worse and neglected. I am calling the stateboard and will show them all of the video footage including where the nurse says "Well we do everything within state guidelines" as in the bare minimum. Understaffed and unqualified is what I would use to describe this horrible place. My family member went in for PT and OT for recovering from her time is hospital. While there, within 2 weeks of being there she got COVID, from exposure there. They kept putting her in rooms with roommates that would moan and keep their tv on loud every night. And the last room she was in before she was in before she was final able to leave (4 weeks instead of 2 because of Covid), had a broken glass in the window that was covered in tape and cardboard. Also, when she asked for help to get up to go to the bathroom, the staff person told her, they couldn't help her because they had a bad back.. It was an absolute nightmare. They used to be great, she had been there twice before - recovering from surgery- and they were good... but things have completely changed. We will never allow her to go there again. read more

If you care about your love ones then you should look for other places. We had our dad there about 18 days and from the second days his personal items started disappearing, when I talk to the social worker the first thing I heard was if it's not on the belongings list than we have no responsibility!! Care quality was poor, if your patient is not able to walk or talk could left alone and hungry. They never showered him during the whole time was there. One of the nurses was angry why my dad ask her to put his phone in charge and throw his charger on the floor. When we asked why she did that, they denied it. I even spoke with the family of the patient that was next to my dad and they were saying the same thing. When we asked to discharge our dad from there, social worker first said he must stay otherwise we will not give him any services, and when I asked her to give me the print of the report of what was done for my dad, she said I don't have time for today I will let you know when I get it done. They also charged my mom $13 for the print papers. I would not suggest anyone to take their family there. read more

My grandma got COVID 19 at this facility and almost died. They had a COIVD 19 outbreak and they NEGLECT their patients. Please AVOID at all costs... Here is my experience at Plum Tree Care Center: I decided to write this review after my grandma got discharged back to the hospital due to the COVID 19 outbreak they had so they do not harass her and treat her bad... otherwise I would of written this earlier. The first day she got transferred here from Good Samaritan hospital we get a call asking to come in and take care of her because she can not sleep. We go in and this ends up being a routine. John and the other workers kept calling us and harassing us telling us to go in 24/7 to watch her. They would call us about 3-4 times every single day. What are the workers getting paid for? They want free labor from us? They then asked us to hire more workers and for us to pay the workers out of pocket to take care of my grandma. This is crazy and nonsense! Why are your workers getting paid if you want us to pay them? Before we got her to go to this facility we let them know that she had alzheimer's. She likes to walk around, but she is not abusive or anything. John is super rude and has a very disgusting attitude. They locked my grandma into her room and she was banging on the door and screaming at the top of her lungs to get her out when we got there. This has happened 3x. This is NEGLECT. I would not schedule an appointment since I was helping them Susan stated "No appointment for you, come whenever you want." This is how I noticed that they would lock her up in her room. If anyone knows where I can complain to the state about this kind of neglect, please let me know. The amount of attitude they gave us about how much of a hassle my grandma is was ridiculous. John told us, to "Take her home and lock her in her house and leave her and turn off the gas so she does not light the house on fire." Isn't this illegal and neglect? Why would they advise us to do that? HUGE RED FLAG! SO ILLEGAL! They called once saying that, "By Monday you need to take her home or we will put her in the county." My mom's english is not good and she needs a translator. We hired a translator since Plum Tree could not help us and the translator called them and they ignored her. They were messing our her head and John and Susan both really gave us a hard time. We asked John for his card and he refused to give it to us. My grandma ended up getting COVID 19 and we told the staff she was sick. They kept ignoring us. They did not care at all. They wanted her to die. She was sick for 3 days. I kept telling the nurse she did not feel good and she was ignoring me and she was telling me that she is fine. A week later the ambulance showed up and took my grandma to the hospital and she tested positive for COVID 19. This is very irresponsible of the staff. We told them she was not feeling well and they did not do anything about it! She ended up being in the hospital for 3-4 weeks and in the ICU because they would not do anything about her COVID 19. They do not care if you have COVID 19 here! IF ANYONE has any INFORMATION about REPORTING Plum Tree to the state board, please message me as I am stuck here worried about other patients being mistreated. The pain that John and Susan and the staff caused us here tore my family apart and ruined our lives! We have all mental stress from this facility! read more

My mother was in rehab for 3 weeks after fracturing her pelvis and in the hospital for 2 weeks. She has dementia with agitated agressive behavior. She got progressively worse while at Plum Tree. After the prescribed medications by her geriatric doctor were not calming her down I repeatedly asked to have her tested for a UTI that can cause the symptoms we were seeing but should have been mitigated by the medication. They never performed the UTI test. A week later out of rehab and at home she showed signs of delerium and hallucinations. We admitted her to the hospital to find out that she had Pneumonia and a UTI which explained the delirium she had in rehab and she more than likely caught the Pneumonia while in rehab. I also submitted for arbitration to extend her rehab as her rehab was hindered by the not followng the geriatric doctors prescribed medications what we now believe was a UTI and Pneumonia but it was declined. Will never go back and will look for alternate rehab centers. read more

You have to chase the nurses, CNAs and even the housekeeping crew. Unfortunately my mom is here now and I never thought I should write a review about this facility. My mother was at Plum Tree for two weeks following a stroke in October. While I have a difficult time recommending any SNF, the staff at Plum Tree were all wonderful to my mom. Her time here was actually very pleasant for her, and she was actually sad to leave the Plum Tree when we moved her into a memory care facility. After a very bad experience at another SNF in San Jose, the Plum Tree was really a blessing - meals were better, staff tried to keep my mom feeling useful and involved, everyone was very friendly and informative. If my mom was not a wandering risk, requiring a locked facility, I would have tried to have her stay permanently at the Plum Tree. read more

This facility is great in terms of cleanliness and the staff is great, I took my mother here after she got Covid and needed help with her recovery to walks Giselle was extremely nice and helpful and all the nurses are caring especially miss tessi. The food is good as well ! My issue is with Nicole from the office, she's given me and my family so much stress and anxiety. Constantly threatening to kick my 99 year old mother out of the facility. My mother cannot walk on her own yet. She's calling on the daily threatening and coming up with ridiculous excuses To get my mom out of there. She recommended sending my mother to HOSPICE!!! I brought my uncle in to plumtree. He was weak fighting cancer. He was very ill. He didn't have much time to live. Because of Plum tree and the Amazing PT staff and RNA and nursing . And activities. Family involved. He Lived! He Loved the environment. . He couldn't talk. But the therapy staff recognized the only way he could talk was calling his phone. Cuz thats the way he would talk to his grandchildren. He would pick up the phone and speak! Wow! Therapy did it!he was happy for a while but he passed away very comfortably. We just brought my Father David home today from the Plum Tree Care Center...About 2 1/2 months between Good Sam and The Plum Tree...They got my Dad strong enough to make it home where he wants to be...When he is not at home getting well he loves The Plum Tree...The staff is Awesome...And, we appreciate the care they have provided my Father. My mother just spent 3 plus weeks at Plum Tree Care Facility for rehabilitation due to an injury she sustained in a serious fall. The facility was clean, bright, and very comfortable. The staff was upbeat, positive and very attentive to my mom and the other patients there. A special thanks to a few of the staff. Krian at the front desk always took the time to answer my many questions regardless of how busy she was. Pleasant and professional, Plum Tree is lucky to have her. Our nurse was extremely knowledgeable and very quickly made me feel comfortable that my mom was in good hands. Eva, the Occupational Therapist, had terrific rapport with my mom, and had her back on her feet and making steady progress with each passing day. Let me preface this review by stating there are some extremely caring people that work at Plum tree: Anne(csa), Amy(nurse), two of the front desk greeters, taller young woman and weekend woman. That being said, from the care conference on, Plum Tree was a total failure. Specifically Nichole Wojnowski(Director of Social Services) and Mark(Director of Physical therapy). My Mom was there during a Covid outbreak so Guests Covid tested every two days, while Employees tested twice a week. Testing was unsupervised so my Sister in law observed a woman not swab her nose and just put the solution in the testing device. After the outbreak, Guests were required to wear n95 masks while Nichole worked in her open to the lobby office without one, with an office mate next to her. During the care conference which was merely a phone call Nichole was clearly unprepared and unprofessional. There wasn't a nurse there, they had to find a physical therapy representative and all Nichole did was read from the Hospital discharge paperwork and provide misinformation. Plum Tree is very short staffed with many new Employees. Sanitation is not the best and they misreported how much my Mom was eating and drinking which was virtually nothing. Per my Moms roommate, my Mom yelled for help when she needed to go to the bathroom and it took forever for someone to come, which was true for most residents. Plum Tree was anxious to release my Mom to hospice because they claimed she did not want to participate in pt and was hostile to a therapist. This from a 90 something pound 99 year old. Nichole provided a list of Hospice agencies and we ended up selecting one that a Neighbor recommended. The Hospice agency spoke with Nichole in a lengthy conversation and my Mom was sent home. Two days later, the Nurse evaluated her and said she did not qualify for hospice and that Nichole knew this. Nichole never informed us and now we are left on our own to utilize home healthcare instead. Do not accept the opinion of Nichole W. In blind faith. read more
