I have never encountered such kindness, care, and attention as we did at SRH. From the moment he was admitted, the entire team made us feel supported and seen. Donna, the case manager, was a gift from heaven--she guided us through every step with so much compassion and clarity. Dr. Yabes and the other doctors were incredible--professional, attentive, and genuinely caring. Maria and all the nurses were beyond amazing--so gentle, patient, and dedicated. They treated my dad like family, and that meant everything during such a difficult time. Rick, the pharmacist, was also wonderful--thorough, caring, and great at explaining everything we needed to know. The Physical Therapists, Occupational Therapists, and Speech Therapists were the absolute best--encouraging, skillful, and deeply invested in helping my dad regain strength and independence. My dad stayed for about two weeks, and from beginning to end, our experience was outstanding. If you are considering this facility for your loved one, know that they will be in the best hands.     Sacramento rehab sent my dad to an ER to sit for 24 hours as an out so they didn't have to care for…read morehim anymore. Because of infections that happened during my dad's stay they sent him away so they wouldn't have to be responsible for him. This facility doesn't want to care for or help patients that may require extra support and more tailored therapies. The nursing staff acts in a way that is unbecoming of the nursing profession. They have laughed at, made fun of, and rolled their eyes when asked for help with my dad. I came in multiple times to find my dad sitting in his own filth in tears that nobody would help him. Sitting in wheelchairs that were too short so his legs were jammed into his chest, left there unable to move in excruciating pain. Having nurses laugh at patients, drag their feet to offer hands on care, and leaving patients filthy and unable to move directly violates ethics standards set out by the Nursing Practice Act. Professional conduct should uphold patients dignity and show compassion not demean people who have gone through life changing health issues. The therapist for the most part are kind and caring. Though there are a few who have no business working with people in this type of environment (I'm looking at you Jeff). The rehabilitation aspect is the only thing I trust this facility with which is why my dad had a family member there nearly 24:7. My sister stayed over night three nights a week. The one star is for them and nobody else! Case managers are not communicative and unavailable the majority of the time. Perhaps other people have better reviews and experiences because they are not seeing the type of care their loved ones are getting. all I can tell you is be present and do not trust that these people are properly caring for those you love. If your loved one had a stroke and can't speak clearly, walk, or take care of themselves do not leave them alone here. A shiny new facility is a great distraction for the patient neglect that is happening when you aren't looking.

This consistency has been essential to my recovery. CLIENT / PATIENT CARE Physical therapy is not a one-size-fits-all experience - it's a journey, and I'm still on mine. While I'm not fully recovered yet, I've come a long way, and that progress is thanks to the amazing team at Burger Physical Therapy - especially my Physical Therapist, Sam. Sam is absolutely incredible. She's compassionate, knowledgeable, and deeply attuned to the needs of each person she works with. She meets you where you are, both physically and emotionally, and walks with you - step by step - toward where you need to be. Communication is so important in PT, and Sam exemplifies this beautifully. Whether you're someone who is shy and reserved or more direct, she has this effortless way of adapting her approach to fit your personality and comfort level. I've had the privilege of watching her work with a wide variety of clients.     This place is now called "north pointe care center" ( it was confusing at first when looking up the…read moreplace) My father spent several months at North Pointe to complete his therapy after being in two other nursing homes that were not the right fit for his needs. What truly set North Pointe apart was their willingness to accept him when no one else would, due to behavioral challenges related to his illness. This facility gave our family hope at a time when we felt like we were running out of options. I vividly remember being at the hospital for the fourth time in just one month after more heartbreaking experiences at other nursing homes. During that stay, the nurse manager connected with North Pointe. She came back and said, "The first thing they told me was, 'Bring him in...let's make him comfortable.'" From that moment on, we felt like we had finally found the right place. They welcomed my dad with open arms, and the cycle of hospital admissions stopped. It was a huge weight lifted. From day one, the staff at North Pointe was exceptional. We were comforted to see the same nurses, CNAs, and therapists each day ...people who knew my father by name and took the time to understand both him and our family. Their consistency and compassion made all the difference. I especially want to thank the CNAs, nurses, and rehab team. They are incredibly experienced with patients living with Alzheimer's, dementia, or recovering from serious illness or injury. One thing that really stood out to us was how diverse the staff was. There were team members from many different cultural backgrounds, and many of them spoke Spanish;including nurses, CNAs, front desk staff, and even members of the cleaning crew. This made a big difference in creating a welcoming, inclusive environment where communication felt more natural and respectful, especially for patients and families who speak Spanish as a first language. The facility does a great job of helping residents stay active and engaged. The backyard area is one of the highlights...patients are able to walk outdoors safely, surrounded by large shade trees and grassy areas. There's even a big birdcage that many of the residents enjoy. Inside, there's also a piano, and music is played. It's comforting to know that patients aren't just confined to their rooms...they have opportunities to socialize and enjoy the outdoors in a secure setting. While the front porch offers a nice place to visit and provides some privacy, it gets extremely hot during the summer. Unfortunately, there's no designated indoor area for families to sit with loved ones privately, which made daily visits uncomfortable at times. The backyard is more of a shared space, and you'll often be surrounded by other residents. That felt a little intrusive in the beginning, but I quickly grew fond of many of the residents over time...they meant no harm, and it became a familiar, even comforting, part of our routine. The rooms are small, usually shared between three residents. That can be a challenge, but the staff works hard to match roommates based on compatibility. Everything is gated, which is a huge comfort if your loved one has memory issues or a tendency to wander. Safety is clearly a top priority. That said, the facility really needs renovations. The closets, dining room, and especially the bedding are outdated. We noticed torn linens and worn-out furniture more than once. It's time for ownership to invest in this space. Our seniors deserve not only excellent care...but an environment that reflects their dignity and worth. North Pointe has the heart and the team. With updated rooms, furniture, and better indoor common areas, it could become truly exceptional. Staffing was also stretched at times. While every staff member we interacted with was dedicated and hard-working, it was clear they were doing more than their fair share. They need more support from leadership. The social work team was also understaffed and could improve in communication and follow-up. I'm sharing all of this because I know how difficult it is to choose the right nursing home. We relied on honest reviews when we made our decision... and a lot of places had a lot of fake reviews....and I'm so grateful we chose North Pointe. It's not perfect, but what matters most...the staff are truly outstanding. They cared for my father with patience, skill, and compassion, and they gave our family peace during an incredibly stressful time. Thank you, North Pointe.

No one from UC Davis contacted me ahead of time to ask why I hadn't paid my bill. Apparently, according to them, my bill had been sent back to them after they mailed it to me. So they immediately handed it off to collections. This was in about a 2 week period after my visit. When I spoke to the people over the phone regarding the billing discrepancy, they treated me like I had made some sort of mistake with my address, despite having no previous problems. I was worried sick it would hurt my credit. Fortunately, the collection was soft so it didn't report. Still, I see it as a completely unnecessary step they took to escalate the situation, when all they needed to do was give me a phone call or send me an email. Again, this issue doesn't reflect on the staff at this particular location, but to the greater UC Davis medical group entity. This is corporate medicine In family doctor packaging.

    Two rep.s came to our Eskaton facility yesterday in Sacramento. They did not introduce themselves…read morewhen the presentation started. Presentation consisted of what they could remember about what PACE offered. If a question was asked about a particular service i.e. respite care, yes, they have it, but no further expansion of what their "respite" care really offered -- no details. Then the presentation became a banter between the two men about how awful care we were probably receiving from our existing healthy plan and physicians: doctors have to see a patient every 6 minutes so how much care are you getting; have to have another doctor assign a specialist for further care; surgery especially critical surgery scheduling would leave patients in pain until the surgery -- rep. knew because he had to wait for 3 weeks for his wife's broken angle. Just more bashing of every other local health plan. One man was nicely dressed in polo shirt and slacks; the other man was a slob -- old jeans and white shirt that maybe fit 20 lbs. ago, but not now -- needed a hair cut -- talked down to people who asked questions and asked for details. Presentation was a waste of time! Most everything PACE offers is all locked down anyway -- social services and activities are available through ZOOM and other computer programs -- oh, yeah, big fun! I can do that now and not pay $2,300 a month (maybe because they didn't know the rate and after consultation with other decided it was on a sliding scale) for services offered on a computer and by consultation with their one doctor director presently in Natomas.
