My bad was in here for 28 days. He had a fall and was in the hospital for 10 days then transferred here for rehab. I have some good and bad things to say about the facility. Luckily my dad is walking without a wheelchair now just using a walker. CareOne is to thank for that because the physical therapy helped him to regain some independence in mobility. Good The social worker Janelle was top tier. She paid attention to detail, was patient and helped us navigate in finding wheelchairs, health aides, etc,. Bad My dad developed a bad blister on the bottom of his foot. It was so bad that even now in November he is still dealing with the effects of it. Nobody at the facility realized just how bad it was and now you can see all the way to the bone. He had no prior issues with his feet. My dad also has a L1 spine break that nobody caught until we paid for a MRI. The facility is extremely short staffed. There were 2 covid outbreaks in his last few days there. The sense of urgency with the patients was non existent. There were days when my dad would be sitting wet on the bed for a long while. Even though he was going to Physical therapy most days they did not encourage him to use the bathroom or any other things on his own so he was in bed 80 percent of the day. read more

Very close elderly relative of mine has been here 3 times in last 5 years. Usually recovering from a fall or hip replacement. It has gone from "ok" to not good. Physical therapists are very nice. Seem to know their jobs. But if you need a nurse or aid, be prepared to waiting 30-0 minutes for them to show up. People...please DO NOT LEAVE YOUR loved ONES IN THIS PLACE! My mother was there for 60 days, and not a day passed by when we did not have a complaint. The rooms were dirty, the floors were disgusting, and the bathroom stunk so badly from not having been cleaned properly or often enough. Service was not very good. With the exception of a select few, the vast majority of the staff are indifferent to the needs of the patients left in their care. My loved one (and other patients I've seen during my time there) were often left to lay in soiled sheets and garments for hours. Alerts to the staff were met with a true sense of annoyance and they clearly are burnt-out to the point that they did not move or respond to ANY request with any sense of urgency or empathy. Repeated requests for assistance were either totally ignored or met with hostility and callous indifference. If it were not for the attention of myself and my siblings, my mother would not have been showered or had here teeth brushed, or hair combed. There actions are deplorable. My mother often needs help eating and there were many times nobody wanted to assist in feeding her. On one occasion, a worker became so agitated and disgruntled at repeated requests for assistance the worker actually threw food at her. My mother only speaks Spanish and staff had no tolerance or patience with her. On another occasion a staff member was delivering breakfast and slipped on a dirty wet floor, dropping the tray of food. As such, no replacement was ever sent, and my mother had to wait until lunchtime to be fed. On more than one occasion, her medication was not given to her because they included water pills, and staff did not want her to go to the bathroom more frequently. Simply unconscionable and unacceptable. She also suffers from asthma and they failed to give her oxygen, resulting in her having to be taken to the emergency room. Healthcare workers do a necessary and often thankless job. But they are there to care for those who cannot care for themselves. I know it must be tiring and exhausting at times, but patients and their families expect dedicated professionals who demonstrate compassion and true concern for others. Sadly, the staff of Care One provided neither. I will be reporting all these incidents on all social media platforms and BBB as well. People needs to know what these places are doing with our love ones. This is a place that supposedly is a leading skilled nursing and senior care provider committed to delivering excellence in care....WHAT A JOKE! read more

UPDATE October 3rd, 2023. My mother was released today around 4pm and at around 10pm went back to the hospital. As I was cleaning my mother today when she was using the restroom, she was bleeding as if she had her period. Just a week and a half ago I had to argue with the nurses for nearly 2 hours about sending her to the hospital because she wasn't feeling good and felt it was an UTI. Well, it was a bad UTI and she was in the hospital for a few days. She returned to Careone for a few days to finish out her Medicare days (Careone exhausted all of them) and was released today, Tuesday, October 3rd. I asked my mom if this is the first time she was bleeding and she told me No and counted to three days. THREE DAYS?! So you're telling me a woman that is very dependent on someone else helping her in the bathroom was bleeding (a decent amount) and NOT one of the health aides noticed? I'm furious. Any doctor with a brain will tell you bleeding from that area with a history of re-occurent UTI'S is a sign to get medical attention, not send the patient home ill. I am disgusted. EVERY. SINGLE. TIME my mom has been released from this facility she ends up worse than when she went in. The neglect is A REAL issue in this facility and I have no other choice but to go a legal route against them. To top it off, my mothers clothes have been missing for a week and she has been stuck in her hospital gown since she came back. NO one notified me that they were still missing and I assumed she was clothed as no one got back to me about the clothes (I assumed they rectified the issue) nor called me asking for me to bring new ones. This place is a JOKE. DAYS ago they were suppose to set up transportation WITH the emphasis on 2 people needing to bring her UP THE STAIRS, yet that failed and they were scrambling to find someone from Modivcare to be able to do so. I have been so patient and understanding of the facility and the difficulty of this line of work, but I am beyond giving the benefit of the doubt. This is pure negligence. Last week I was told that my mother could walk with the walker 100ft. Then I get a call today on her discharge date that she is in the same shape she was when she left last time: UNABLE TO WALK AT ALL. CareOne, you are going to have a lot of state issues on your hands. Instead of renovating your building, you need to renovate your staff and policies. Thanks for nothing and exhausting my mothers 100 days of Medicare coverage. Now I have to have my mother clear from an ER for 60 days for her benefits to reset and her able to get any outside rehab she may need. NEVER AGAIN Update September 17th 2023. You've officially lost all stars from me even though the lowest I can go is 1. I am appalled at this place for how they have handled my mothers care. I gave them a 2nd chance and not only did they let us down, but they put my mothers health at risk. TWICE my mother has gotten covid19 at Careone (both Teaneck & Wellington) in a matter of 2 months. 20 days of that was in confinement which impeded my mothers progress by laying in bed all day, wasting away. The nurse told me someones family came to visit and knew they had covid but didn't wear masks. Did it have to take so many people getting sick at the facility to finally mandate that ALL who enter the facility need to wear a mask? Why aren't we still using certain protocols to protect the weak and vunerble? The real issue I have is again, the LACK of bathing and the fact my mother fell twice from her bed. For those who are a fall risk, there NEEDS to be barriers. It's not restricting (ffs it's not a stray jacket) it's a much needed safety measure for those who are fall risks. My final straw was when my mom, who has recently come back from the hospital after the 2nd fall in Careone, had been put in a neckbrace to wear for 6 weeks, was taken off for hours, leaving her neck vunerble. She can't take it off alone and when I asked why it was removed all I got was a hypothetical answer that perhaps it was taken off when they washed her. Washed her? She was filthy! She is ALWAYS filthy when I visit. Mind you, there was shit all on the toliet and sink that no one cleaned up. When she was in the ER with her last week and they were inserting a purewick, there was rashes all on her inner thighs and dried up feces! Common sense is putting the brace back on after "washing" since the doctor who put it on told me if she falls on her head again (as she did in CareNONE) she could potentially be paralyzed. The staff don't seem to care a bit about these people, just a paycheck. I don't trust the nurses or aides involved with my mom at all and it is outright neglect they are committing. I don't care if this place is understaffed. That's their issue they need to solve because these are lives in your hands, not a paycheck in your pocket. I was thinking of writing a review for a long time. My grandfather passed away several years ago. This was the place he spent his last months. I never forget the poor treatment he received. The CNA's were nasty and yelled at him when he never complained. The staff didn't move him and he got a stage four decubitus ulcer on his sacrum. The PT team allowed him to sleep instead of exercise him. He told them I was in D-Day I had enough exercise. Let me tell you, this place looks beautiful, but lacks good nursing care. wounds!!!! keep your loved ones out of here. never enough staff. the staff they do have are awful, not nice, will only come into the room when you ask them. they don't listen to the family and give meds even when told the patient was allergic. left the patient one week without an air mattress or wound dr to visit which turned into stage 3 wound. they are supposed to turn him every 3 hours. never happens. all this with family being here 12 hours every day. imagine what happens when you are not here . you have to flip out for anything to happen. This is the second time my mom has been here. First when she has a shoulder operation 10 years ago and now at 88 after an accident where she had second degree burns on her leg and required rehab to walk again. Our experience both times 10 years apart has been positive. Although my mother says there are a couple of employees there who seem to have an attitude problem, overall she feels the staff is professional and friendly. I've been to the center several times and it always appears clean and my mother agrees. She is a tough customer when it comes to these things as is my sister who visits her often. Both of them think highly of Care One in Teaneck as do I so I would recommend this rehab center. read more

Recently, they sponsored an event for the Northern New Jersey Holocaust Memorial & Education Center which featured filmmaker Nancy Spielberg. The event centered around the film that she produced called, "Who Will Write Our History", about the Warsaw Ghetto. The proceeds went to the organization. They also have holiday parties and weekly prayer services for the residents. We look forward to their continued support. read more

I'm giving this place 2 regrettable stars. I wish I could've given them 3 stars but I just can't do it. My 82 year old mother came here after having a partial hip replacement. After about 4 weeks, due to crappy government insurance, my mother was sent home. My mother made very little improvement while being here but that's not necessarily their fault. The nurses, physical therapists and aids were all VERY VERY GOOD. VERY VERY VERY GOOD! They were gentle, caring and trustworthy. They went above and beyond and to them, our family is very grateful!The 2 stars is for their administrative staff. They ARE THE PITTS! They offered no help with the process of going forward, NONE WHAT SO EVER! Basically, they hung my mother out to dry. Pathetic! read more

I was a resident at care one for 5 weeks. I cannot say enough about everyone in each department. The therapists were great Robert was my O.T, Ngozi my P.T.their patience and knowledge was what got me discharged. The negative comment although not the fault of Care One is that my insurance allowed 90 minutes of therapy 5--6 times a week so there is so much time I was there without P.T. or O.T, I did ask the therapists to work with me on activities I can do myself in my room as there is so much down time. My nurse Ronnie along with his CNA Joyce helped me start each day. They are a great team and truly did get me going from 7 A.M. When their shift changed, Oliver and Jesus picked up where they left off. Throughout the day the activities director Mark offered almost non stop activities to get involved in. His very positive energy is felt on the 2nd and 3rd floor, The food was not always to my liking but for each meal, alternative menus were offered. I have visited two other facilities before choosing Care One in Teaneck and hands down this was the most well appointed facility with bright lobby and nice sitting area. Andrea at the front desk could not have been more helpful. She even booked my hair salon appointments every Tuesday. The administrator Gene has an open door each day so when any issue arises, she is willing to hear from everyone. Although this worked very well for me as I was there in the summer, there is very nice outdoor space when you prefer to eat out with friends and family. I'm home now and so appreciated my time there to get the necessary rehabilitation. read more

A wise man told me that you can tell a persons true character by how they treat you when you're at your weakest. I'm going to tell you my mother's first hand experiences while at Care One Teaneck and at her weakest. She was put in a room with a roommate that who would sleep most of the day and blast the TV at night which was both unhealthy for the room mate who suffer from that condition that old people fall into and for my mother who could not get rest to convulses and becoming sleep deprived. So the answer of the staff when pleas to the roommate to lower the TV fell on deaf ears, was to lower the TV which the roommate raised the volume and then when left the room And when the staff got sick of the game with the roommate one nurse or nurses aid said to my mother,"Your ass is as big as your mouth." So for a PUNISHMENT they put her in a room near the mechanical room (laundry, Heat and AC) with a room mate who blasted the TV 24/7 even louder and had to listen to the machines as well. So when she complained and brought it to their attention they suckered her and said you want to go back and since she was convalescing and sleep deprived she said the other one was better. And so they PUNISHED her and sent her back. So after a few days she couldn't stand it and said to the nurse that she wants to leave and called me Friday mid day. I contacted another Care One which never got back to me and another facility which said to fax the clinical info to them this was Saturday. The head nurse at Teaneck told me she couldn't do that and I had to wait for Monday. I called Monday and left a message to fax the clinical info and found out that the social worker wouldn't be in till Tuesday. I faxed the paperwork I had and on Thursday the other facility said they couldn't take her because they were an acute rehab a sub acute rehab. But just to note Teaneck didn't sent the clinical info. So to PUNISH her they put her in another another room where the roommate had the heat on during the heat wave in May 2021. The TV was loud in the daytime and snored at night. And further PUNISH her by having a nurse or aid that would tell an 80-year-old with 2 knee replacements and a hip replacement and at the facility for ankle surgery to push her to go to the bathroom and insist that she goes into the wheel chair by herself wheel herself to the bathroom then get up go on the toilet and then reverse the process or PUNISH her by leaving her wet for hours then getting mad. So she had enough and I took her out. Mom is a nice sweet person who was used to fill a un fillable rooms of who are long term residents not short term residents. read more

My mother was at CareOne twice and each time was worse than the next. The first time she had a nurse who we couldn't understand and her English was broken. My mother was so sick and I couldn't communicate with her nurse!!! Additionally, I had to stay there most of the time because otherwise we wouldn't see anyone for literally hours on end. The weekend staff was especially horrible and not one nurse checked on my mother for over 2 1/2 hours. The assistants also would rarely come in unless I asked, so to get anything done I literally had to chase down someone to help me. The rehab wasn't bad but the medical care was horrible. THe doctors on staff admitted that they had no idea what to do to help my mother when she was sick. They kept sending her to the hospital instead of consulting with her regular doctors. I was disgusted with this facility and the lack of competency and compAssiOn they showed not only to my mother but to all their patients. Do not send anyone you love to this horrible place!!! read more

Don't let your family members be at the mercy of this uncaring negligent staff. You come out with illnesses and other issues you didn't come in with. Love love love ! I can't say enough good things about Care One Teaneck. Both of my parents spent time here for rehab after the hospital, and we couldn't be more pleased. The physical therapy was excellent, the ambiance was so nice, the fireplace was so nice to sit around during our visits, and the outdoors patio area was so pleasant. The nurses and aids are compassionate, attentive and always respond in a timely manner when I call them. The therapy has been excellent and I have made great progress in therapy. I have been working at CareOne at Teaneck for a few years now. I am very appreciative in being part of a company that sets the standard for superior care, particularly in a long-term care setting. I feel the health and safety of residents and staff continue to be a top priority for CareOne during these challenging times. Attention to detail continues to be the norm, especially in making sure that residents are well nourished. Working in the dietary department and assisting in making sure that our elders receive daily three nutritious kosher gourmet meals, really makes me feel fullfilled. The nurses have attitudes. The administrators have attitudes. Even the receptionist has an attitude. The only person who treated us well was the Occupational Therapist, Jiro. My father was admitted on a Thursday. We were hopeful for his recovery. We really were. By Tuesday he was gone from this world. If this review can influence one person into not bringing their loved one to this facility, I will have done my job. read more

My brother was staying there following surgery. He experienced a problem and tried to get attention. He finally called 911 as a patient. He was frantic. Wound up going to the ER and ultimately is now paralyzed due to not being taken immediately to the hospital and being ignored. I pray he recovers but its not looking good. If I could only give this place negative stars I would. They took my Mom on 3/21/20 from Holy Name after stating "they had room" (despite the Covid-19 outbreak going on in Teaneck). They put my mom on the 3rd floor and not even a week later I got a call saying she was being moved to another floor because there were "positive Covid patients" on floor 3. So they admitted my Mom to a floor that already had symptomatic patients (because we all know you dont just wake up one day with Covid symptoms and the testing before getitng a "positive" takes at least 2-3 days.) So to the 2nd floor she went and would complain to me often that the nurses were not attending to her. Not even a week after she was moved to the 2nd floor did she then start to experience Covid symptoms herself and NO ONE WOULD ANSWER THE PHONE!! I left messages for nurses, the administrator, the intake department, and even the receptionist at the facility said she was getting calls from other concerned family members and didn't know what to do. Only after I threatened to call the police to do a "wellness" check because my Mom sounded completely out of breath and only then did she finally get attended to and shortly after was "presumed positive" and moved back up to the 3rd floor where she passed away 4 days later. 2 weeks after her passing I get a call to do a "survey about my Mom's discharge" and after telling the "surveyor" my mom wasn't discharged, she passed away, the person said "oh im sorry, is this not a good time, i can call back in a couple of weeks". WTH??? I then write a emotional post on Facebook and a couple of weeks later get a call from an "administrator" apologizing about my moms passing. This place is run by incompetents who in the course of 16 days took an otherwise healthy woman (only came to CareOne after broken hip surgery - with NO other health issues) where she contracted Covid in their facility due to their negligence, and caused her to pass away on 4/7/20. Stay far far away from this place. All they care about is collecting money. read more

the care here is horrible, please do not send your loved ones here. my father went it not speaking and we were told they would have an ent doctor check him. 3 weeks and we are still waiting. and now we are being told he needs to discharged. they do not take care of them the way they should. the night staff takes forever to get to the patient. they don't even care. they hang their call buttons while the patient is sleeping some of the nurses and aides are great. but they cant help working in an environment like this. the nursing manager is horrible with communication and getting things done for the patients. please do not admit your family member they will come out worse than the way they came in. read more

Never ever send your family member here for rehab...mom was sick and they still sent her down for rehab...i even told those stupid physical ed. personal that my mom is sick send her back...they didn't, saying she's fine. All they did was have my mom sit in a wheelchair and they put her against the wall the whole time...no rehab was done...they did this for 3 days before my mom was sent back to the hospital...all they cared about was charging the insurance for the rehab session when no rehab was done...this is insurance FRAUD!!!!!!!!!!!! I don't know what the rehab is like, we only looked into having our family member stay there. That was because of their horrendous rule about confiscating any food brought in that's not kosher. If you want to eat Kosher, fine. If you want to serve your patients Kosher, also fine. But if I bring something for myself to eat (especially because the food in these cafeterias is always overpriced and often not very good) not fine! If I bring my family member something to eat, like their favorite cookies or something, especially not fine! For patients with elderly dementia like my loved one, things that remind them of home can ease their anxiety and confusion they experience when they're staying in a strange place. And confiscating someone's property, even if it's only food, quite frankly, is actually stealing!!! read more

Last review in 2008 said ZERO care. As of today June 2010, the care is SUB ZERO. The nurses who actually work, are so busy doing not only their shift but the shift of the afternoon and night nurse who do as little as they can. Some of the nurses, if you ask for care throw it back and make themselves a victim if they cannot deal with a medical request. Doctors come by swarming like ants-except no one bothers to tell you that each visit no matter how miniscule is billed. They lack the legal numbers of RNA's, but are overrun with "social workers" who like the nurses have a word they love to use: "protocol".They do not consider the patient needs patient "protocol". If you do not have a person who is with the patient 24 hours you can forget about "care". One last thing. Physical therapy, is overcrowded, noisy. Keep far away. read more

PLEASE DO NOT ADMIT A FAMILY MEMBER HERE!!! This was the most horrific experience I have ever had with a skilled nursing facility, and I work with nursing homes routinely as a professional in case management. The level of incompetence is beyond anything you might imagine. My family and I had to deal with this facility not once, but twice unfortunately because my mother was sent there directly from Holy Name hospital, which is right done the street. The second admission was worse than the first and my mother was weaker and less capable at discharge than when she went in. The social work staff are completely without knowledge empathy, or ambition to do anything related to discharge planning. The nursing staff are unprofessional, untrustworthy, and dishonest in there charting and patient care information. The physical therapy staff do little to engage patient's in rehabilitation therapy and put patient's at risk for re-injury which is exactly what happened to my mother. Overall, this place is all about taking insurance money, and private payment without giving anything more than the bare minimum of care, if that. I could go on and on with the horror story but I am so sickened by the thoughts I must stop. I just want to spare any other family from enduring what we did. read more

My father was catheterized at the hospital and brought to Care One a week ago. His personal physician visited him there two nights ago and wrote orders to remove it, but to monitor him. If after eight hours he fails to void, reinsert. After nine hours, (now 7am) I called there to follow up, and they still had not reinserted it and he had not voided. I promptly got the nurse in charge on the phone and she promised d me that she would have the oncoming nurse reinsert it immediately. I went to work. At 1pm, I called to check on him, and he didn't answer the phone. I asked for the nurses station and they told me that no one was available to speak with. I insisted on the receptionist getting me a nurse, no matter who. Finally, the desk nurse got on the phone, checked his chart for me, and proceeded to tell me the catheter had still not been put back in. And the aides said he had not voided. And 16 hours had passed!!!!!!!!!!!! I was livid, demanded to speak to the administrator. He told me he would investigate immediately. Upon entering my dad's room, I am told that he found him lying on his back on the floor (obviously that's why he didn't answer my phone call). I called back five minutes later, and the administrator told me I should have him removed from the facility, as his staff just keeps f...ing up. I was aghast. Totally unprofessional and inappropriate comment to an upset family member. That's when I got the ass't head nurse of the Care One on the phone who has now taken charge of my father's care. Her boss, the head nurse, is also having the staff report directly to her as well. Here's the kicker, when I questioned the moron nurse who had come in at 7am, and was told to reinsert the catheter, he says he questioned my father, asking him if he had any discomfort. When he was told no, HE, this nurse, made the insane decision to go with an altzheimer patients comment, rather than follow doctors orders. AND, when I insisted on speaking with this nurse at 1pm, I was told he had left for break. And this was at the 16 hour mark. I now have 24 hr aides sitting with him, and will be bringing him home as soon as his private physician says I can. Do NOT allow your loved one to enter this facility unless you are sure they know exactly what is going on and can fend for themselves. Care Zero is not the place to be if you Care for your loved one. read more

Do not be fooled by the fancy hallways and decor. This place is not here for you to get better but to keep you there as long as the can to get as much money as they can from you.. I had a knee replacement and was instructed to use a cpm machine for faster recovery. They refused to let me use it. I was constipated for 6 days. No one cared to ask me about that read more
