3/5 Stars (Could Have Been Higher) I was 40 years old when I stayed here, and by far the youngest resident. My roommate was close to 80 years old, which made for an interesting experience. Despite our age difference, I came to respect and admire him. What kept this from being a 5-star review was the food, the uncomfortable bed, the very small shared rooms, my television being much smaller and positioned much closer to the bed than my roommate's, and the nighttime noise. Some residents struggled with mental health or other conditions, and there were nights when patients would yell, making it difficult to sleep. I ended up staying for eight nights and nine days. Looking back, I believe another two or three days would have benefited my recovery, as I was still struggling with significant pain and difficulty walking when I was discharged. Although my insurance covered my stay, I felt that my Kaiser representative was too eager to move me out and didn't fully appreciate the severity of my condition. Unfortunately, that reminded me of my initial visit just days before I ended up in the emergency room, when I was only prescribed ibuprofen despite the pain I was experiencing. Some fellow patients encouraged me to appeal the discharge decision and fight to stay longer. However, I was physically exhausted, emotionally vulnerable, and simply didn't have the strength to advocate for myself at that point. In hindsight, I think a more formal discharge meeting that included me in the decision-making process would have been the better approach. Even if the outcome had been the same, I would have felt that my concerns and readiness to leave had been fully considered. That said, there were many positives. The staff, for the most part, were attentive, compassionate, and genuinely cared about the patients. I did have one negative experience with a staff member who fit every stereotype of someone who shouldn't be working in healthcare. I emailed the administrator about my concerns, and to his credit, he came to my bedside almost immediately, shook my hand, listened to me respectfully, and removed that employee from my wing. I appreciated how seriously he took my concerns. The physical therapy team was outstanding. They welcomed me from day one, even though I was hesitant to socialize because I was overwhelmed, heavily medicated, and had just been discharged from the emergency room. Their encouragement and professionalism played a huge role in my recovery. I was admitted after my back suddenly gave out to the point where I couldn't walk. At my lowest moment, I truly feared I might never walk again. Being surrounded by others who were also fighting their own battles gave me perspective and gratitude. I realized how fortunate I was to have the insurance and benefits that allowed me to recover in a facility like this. The location was also convenient, making it easy for family, friends, and coworkers to visit me during a difficult time. While there are definitely areas that need improvement, especially the food, sleeping accommodations, and room setup, the caring staff, excellent therapy team, and responsive administration made a difficult chapter in my life much more manageable. read more

Took great care of me Natalie the Treatment Nurse was also very Thorough on treating my wounds . Mahida and Daphne were very helpful as they were the Kaiser Hospital liaisons with the Kaiser Doctors!! The Kitchen Manager was nice and Creative with the Menu and the Food !!! The kitchen staff was inconsistent on portion size! The Physical and Occupational Therapy staff was Excellent and a Wealth of Information!!! There's even Entertainment and Bingo There !!! They have an Activity Director there that was really nice!!! This Skilled Nursing Facility is a. Baller On A Budget Approved!!! read more

Great family casual place - dogs came come on the outside patio- kids. Can walk around--food menu is terrific- we had a group and were able to order separately as families-- so not one big bill - it was already divided- read more

I recently had the pleasure of working with a wound care nurse by the name of Montana. When you put a family member in a skilled nursing facility or any type of a facility that is inpatient you want to make sure that your loved one is taken care of. Montana shows great leadership, professionalism, and outstanding customer service. My grandfather stayed at Canyon Creek for a little over a week. He was discharged here due to CCPA being a preferred partner with Kaiser Permanente. The nursing team, activity team, dietary, and environmental team are absolutely "GOLD STANDARD". As an 84 year old Indian male "nana" actually enjoyed the food. The DON and Executive Director are actively engaged in the care of all their patients. On the day I came into see Nana he was actually completing a ROM activity with the activity director!!!!!! I just wanted to thank the team!!! Nana is home and doing much better, thank you for being apart of his journey. read more

I am a young 67 year old woman who needed skilled nursing facility care for several weeks. I must say the staff at Canyon Creek Post Acute care are awesome. I made lots of friends here with staff and also the residents. The therapy department was also exceptional. I had one on one experience with them. They made the exercises fun and not painful. The activity room kept the residents entertained and happy. Bingo was the highlight of my day. Machu made it fun and she was able to connect all the residents. I'd like to give a special shout out to CNA Shewa and all the therapy staff. I definitely recommend Canyon Creek for Nursing and Rehab care. read more

Update: The Rehab center has agreed to reimbursement. There was apparently a lot of miscommunication. I caution everyone, communicating is key. Research this facility is smaller which is nicer. read more

I am very concerned about patient care at this site. Staff seems to be overworked and therefore unable to provide optimal quality care over quantity care. Some residents are not cleaned properly, which makes the rate of infection higher. Poor care management for residents with medical needs. Because of the limitations of some of the patients/residents, audits are unable to take into count the actual resident issues that loom at Canyon Creek PA. Consider alternate options when thinking of care for Loved ones. read more

I can tell that the facility has a legacy of quality. The care that I received was immediate and professional. The nurses were very attentive. The food was excellent and full of variety. They also have activities to engage you and fulfill the creative side. The physical therapy prepared me for my discharge and now I am home and thriving. I would recommend this facility to anyone who wants to have a full recovery. read more

SUGGESTION: Maybe they need to hire someone that just goes around and keeps track of when the call light was turned on and notifies the patient that assistance is coming and then prioritizes issue and makes sure people are not completely ignored and follows up with care team and patient to make sure they are cared for. How hard would that be. A high schooler or many people could do that little job (might even get a volunteer). It would take lots of pressure off the otherwise overly busy staff. Otherwise I am basically very happy with this place now that they removed most of the misleading pictures on the website. I have had relatives in many nursing homes over the years and most were 2-3 stars with one of them a hard 1. This is a decent place. I feel mostly the people are respectful and I do understand that the workers can get frustrated and every now and then be rude to patient or visitors. Which is unprofessional but the patients and family can be very demanding and rude. In fact unfortunately demanding and rude patients and visitors is probably somewhat common. All in all I really recommend this place. There are some very nice people there and management and workers clearly listen as long as the person they are speaking to returns the same courtesy. Negatives: Workers are busy and cannot always get the information needed and you may have an experience of a person telling you they will look into something and not hearing back about it. (This I am pretty sure will happen in 100% of nursing homes) Had complaint of rude staff at night but I am sure lots of stuff can cause that and that management has or will address that if they found it to be an issue. Parking can be limited, though I had no trouble finding a spot each time I went. Positives: nice location close to a Trader Joes and many other conveniences for visitors. Staff is friendly and work very hard (forgive anyone having a bad day. Spend some time there and you will understand how besides their assigned duties everyone is pulled in many directions all at once and they are not always treated very well by patients and visitors) Rooms are nice. Small not great for lots of visitors but very decent. Some have an outside little area to hang out in as well. Feel good about having a loved one here. Try to understand that they have many patients and they get treated equally and sometimes patience may be required. Needs remodeling and more staffing. The staffing is caring and loving. The facility is clean.

Only been here 1 hour! The website and pictures are misleading!!! They need to post the pictures of the inside of the rooms. Should have opted for home health aide instead. I don't believe my love one will get the right treatment here. I'll be here when visiting starts and when it's over!! read more

I usually do not post reviews, but in this particular situation I believe it is necessary. My father stayed here for about a month. He was admitted for physical therapy rehab. The time he spent here anyone could come in and leave the facility without anyone noticing. They are so under staffed that it can take hours to have someone check in on your loved one. One time my father had to go to the hospital for some tests and there the doctors had discovered that my father had 3 broken ribs. So sometime during his stay at Canyon Creek he fell in his room and no one noticed he was hurt nor had anyone communicated to us that he had fallen off the bed or maybe they didn't notice because they are under staffed. Naturally we took him out of this facility as soon as we found out of his broken ribs. Positive note is the speech therapist is awesome. She genuinely cared for my father and was very patient with him since he had dementia. The purpose of this review is in hopes that staff can be more cautious of their patients and for the facility to obtain more staffing where needed to provide better care for their patients. read more

My 91-year-old grandmother spent 18 days at Canyon Creek and I can honestly say they turned a tough situation into one of renewed hope. After two weeks of physical therapy, my grandma's ability to walk and climb stairs improved dramatically. The nurses and CNAs were kind and attentive to her needs. Many times, I witnessed Machu and the activities staff brightening the day with group trivia, coffee socials, bingo games, piano music, movies and other forms of enrichment. The vibrancy in that room was stimulating for my grandma and the other residents, and the positive socialization was a silver lining during this emotionally complex rehab process. The staff at Canyon Creek was friendly, attentive, and welcoming. I was pleased to see the recent renovations. I heard about this facility through a friend and agree that their nursing and physical therapy teams were great. I would recommend this facility to others looking for a safe place that their family members can recover. read more
