I've been coming here for quite a while and have seen a few changes for the better. Seeing some…read moreimprovement in the appointment protocol and some of suggestions I made were listened to. When I was there before just the way a patient was treated from the waiting room to the examination room was horrible, no basic courtesy like being told where you were going , simply being told what exam room or stopping for a sample in the bathroom. I also had a couple of miserable appointments where I asked the nurse something simple and she answered ' I don't care ' which is a very bad response to anyone who is in PAIN. Fortunately they are better, but I don't know a pain specialist who is really interested, good or willing to try something different. But I think that a lot of it is because of the insurance companies not approving anything else but standard treatment and not entirely understanding how really miserable someone is after the trauma of bone, muscle, ligament and fascia damage. Not every injury is improved by the same protocol and it's no wonder chronic pain is a very unfortunate diagnosis

I provided feedback, then put on ignore for weeks. After weeks they finally called to schedule trigger point injections, for a few more weeks away, only to call me back again to cancel and wanted to schedule it for the 28th of March. I told them they could cancel everything and I would not be back there. They called again last week to schedule, and I did not schedule a new appointment. Today I received a call saying I missed an appointment last week. Again, I did not schedule an appointment, nor do I plan on returning to a business that is not focused on my healthcare. I have the same or worse pain level with my neck today as I did months ago when I started dealing with Commonwealth. I was sent to Commonwealth by the Veterans Hospital in Louisville, for community care. I will advise VA Community Care of this situation.

They get you in there and hooked on whatever pain meds they can then make up some bull crap reason you can't come back. This review is on Dr Hammad Malik practice. On 3/11/16 I was referred here for pain management. I…read morewas told by my primary care doctor it would probably be a couple of days before I was contacted for an appointment and possibly a month before I could get in. On 3/22/16 I called this office spoke to Charlotte she said she didn't have my referral paperwork. My doctors office faxed it again on 3/24/16. I contacted this office again on3/28/26 only to be told we didn't get a copy of your insurance card. I faxed them 2 copies on 3/29/16. On 3/30/16 I was told they didn't know if they accept my insurance. I said when I pull up my insurance account Dr Malik is listed as a preferred provider. Charlotte said she'd call me back on 4/1/16 to make my appointment, I said that's Friday and your office is closed. She said she'd call me anyway. NO CALL. On 4/3/16 I called again and said I see my primary care Dr today at 2:45 if you don't want me as a patient or can't see me just tell me so I can be referred somewhere else. Charlotte said I need a referral from your primary. I said it's been faxed to you twice, she said I need your SS# I said it's on my paperwork but gave it to her any how. She said I'll need your medical records I said I have everything from now back to 2007 including an MRI I had on 3/31/16 with the MRI report. She then said I need a valid ID I said I have a current drivers license. Charlotte paused and said is it a Kentucky license, I told her my husband travels for his job we live in a 5th wheel and have primary residence out of state. She then said I'd need the go get a Kentucky ID or drivers license. I said we travel from state to state for my husband's job we don't change our license for every state we he works in and I have a lease for the RV park I have valid ID. Charlotte said well Kentucky law says for a doctor to write any kind of pain medication you have to be a resident of Kentucky. I said that is not a true statement and I will call the Kentucky Medical Board to verify that because my primary doctor wrote my first scripts and I've lived all over the US and never run into this, she then she said well DR Malik requires it. So I can honestly say they just did not want me as a patient and only waited until I had been to the emergency room and low on medication to tell me. So now it's another referral another wait not only with my degenerative disc disease, and spinal stenosis with hands and feet going numb I'll now be out of medication. I've never in my life had the trouble getting medical care as I have in this state. We have excellent insurance, I have valid medical records and you want to call your office a pain management clinic but put me off any possible way so that I go somewhere else without just coming right out and saying we don't want you as a patient because you take medication, or the one where they say we only do injections and I never once ask for medication I actually needed injections for nerve inflammation. What a joke and if they didn't write medication send me somewhere that does. If there was a double 00 rating this place would have gotten it. So don't expect to be a true chronic pain patient and get into this circus of a practice.
