My Post Review for Berkley Post Acute June 8, 2026 My experience at Berkley Post Acute has been genuinely outstanding. As a former nursing assistant myself, it felt very strange and personal being admitted into a skilled nursing facility. From my very first night and morning, however, the staff made me feel comfortable, respected, and cared for, and that feeling has continued throughout my stay. The CNAs and nursing assistants here are exceptional. Karla has consistently gone above and beyond as a CNA, with Rosa being a very close second. Rosa, Jeffry, Jovani, Robert, and Sandra have all been AAA++ in helpfulness. Even when they are busy, they stop what they are doing to help without hesitation. The RN staff is equally impressive. Christine stands out as an extraordinary registered nurse, while Camille, Nasim, Jason, and Enrico have all been excellent as well. The truth is, the entire nursing staff is top-tier. Rosa and Christine especially have repeatedly proven to be people I could confide in and even vent to when needed. Their compassion and professionalism have meant a great deal to me Karla, Christine, Jovani, and Nasim were among the first people I spoke with during my first night and morning here. They immediately helped ease my anxiety and made me feel okay about my situation, especially considering how unusual it felt for me personally as a former CNA now receiving care in a facility myself. Joan, my speech therapist, deserves special recognition. If she could be cloned, the world would be a far more caring and thorough place. She has been amazing, compassionate, dedicated, and relentless in helping me improve. Aaron and Rachel in physical therapy have also been incredible. Aaron helped me before I even officially started PT. He listened to my concerns about my right hand and loaned me a hand exerciser that genuinely helped strengthen it. As a right-handed guitar player, that meant a lot to me. Rachel has also focused on hand and finger strengthening techniques that were important to my recovery and personal goals. Camille, the DSD, has also been a tremendous source of encouragement and inspiration. She has helped keep me motivated not to give in or give up on myself, which is something I admittedly struggle with when it comes to personal health and self-care. Rosa M. ("Mama Rosa") deserves a special shoutout as well. She takes on anything in her path and gets it done -- whether it is helping with patient grooming needs or picking up trash in the halls. She is truly a superstar. The RNA CNA's are perfect choices in their positions, because I tend to walk the halls strengthening my leg from morning until I go to bed. I constantly see the Restorative nursing assistants busy doing above and beyond their titles. Juan is a paticular stand out. The kitchen staff has been beyond remarkable. Everyone is friendly, helpful, and accommodating without hesitation. Lesette deserves honorable mention, and Lisa, despite her higher position, is still hands-on, caring, and willing to help anyone. The maintenance department has also been amazing and consistently helpful. I truly cannot say enough good things about them. Laundry services were equally caring. I arrived with only one change of clothes, and the head laundry staff member immediately helped me find clothing for the first few days until my niece could bring me more. That kindness meant a lot to me. The activities department has also been wonderful, especially Laura during my first days here. Although I do not regularly participate in the scheduled activities, they have all been eager to help with any questions or concerns I have had. Rhoda, from infection control, has also stood out for her helpfulness and willingness to guide me whenever I needed assistance. Greg, in the office behind Nurse Station 2, has been equally helpful. Tanya and Luiz have also been incredibly reassuring and supportive regarding many of my personal worries. Finally, Trevor, the administrator, deserves enormous credit. He introduced himself to me personally on my first day and genuinely listens to anything you have to say, whether it is something minor or something serious. I truly believe the compassion, professionalism, and positive environment throughout this facility are a direct reflection of his leadership. I sincerely appreciate everyone at Berkley Post Acute for the care, encouragement, compassion, and dignity they have shown me throughout my stay. read more

My father was admitted to Berkeley Post Acute after a hospitalization. I wanted to express my gratitude towards the staff and the administration at Berkeley Post Acute. Between both parents aging I've had to advocate for my parents in many similar facilities across Los Angeles but this truly was an exceptional experience for me and my father. While the adjustment period was a little bumpy at first once everyone was on the same page about his care plan it went great. He had daily physical therapy and the entire staff was patient and kind while pushing him to regain strength. ALL of the nurses were so kind to him and were so sad to see him leave (even though they were happy for him). Trevor the administrator and Tanya the social worker were so amazingly communicative. Any concerns my father or I had were immediately heard and addressed to the best of their abilities. Tanya helped point us in the direction of a new program called CAL-AIM that got my dad placed into an Assisted Living Facility that is partially paid for by Medi-Cal. Without this program there is no way he could afford to live in a ALF. If it wasn't for Tanya and her depth of knowledge of these types of programs he would not be where he is today. My family and I had a great experience from the minute we arrived; the staff made us feel welcome and cared for: the nurses, CNAs and therapists were all attentive, professional and genuinely compassionate. Every dept. was involved every step of the way. They took the time to get to know my grandmother personally and provided excellent medical and emotional support throughout her stay. All dept. managers made sure they introduced themselves and provided their contacts for any questions we had. Special thanks to the Social Services dept. for their excellent communication and resources they provided. My grandma was a patient here for about 2 months before I as the granddaughter, had to intervene. My grandma is diabetic and was recovering from a fall and they did not do proper checks on her extremities and it resulted in bed sores. I understand that they are busy and all of these places are understaffed and would have let it slide, until they gaslit me and my family saying they told us that these wounds were present when in fact, I alerted them to the issue. Then they tried to sweep it under the rug until I demanded they update their chart for her (had to get the director TREVOR involved). Furthermore, when I asked for a culture swab and antibiotics, they didn't do anything and made excuses. After a few days I asked for her to go to the hospital for evaluation and they flat out denied me until I told them I would sign her out AMA (against medical treatment) just to get her seen by a doctor, as they do not have doctors on sight. They were claiming the doctor "looked at her" which means nothing. After she was taken to the hospital she was on IV antibiotics for ABOUT A MONTH, along with excruciating pain during this experience that is 100% preventable. I understand mistakes and oversight, but failure to correct problems is a SEVERE issue. I would recommend that if you care about your loved one to not send them here. Besides all of the personal experience here, the "PT" is about 2x a week for 10 mins a day, if you're lucky. The facility is extremely loud and clearly overloaded with patients, your loved one will not experience peace and quiet here. The social workers that help with insurance and coverage will tell you things like to drop network providers, and say it's better to be on straight Medicare (which is true) but won't file for your loved one to have proper coverage after dropping network (even when they claim they did). Lastly, the dietician and food staff do not understand nutrition and your loved one will not receive proper dietary needs. Parking is also terrible and you will risk your car being towed/ a far commute to the facility. read more

If you love and care for your loved ones do NOT send them here. All I can describe this place as is ABUSIVE. They do not shower their patients and they will leave the poor people in dirty diapers. When my mother was staying here for a few weeks I was not given any phone calls on the status, instead they called my grandma even after my mother correcting them multiple times when I had gotten the phone number of her case worker I called heated and she decided to try to reprimand me I told her that I was a grown woman and that this facility was abusive and she tried gaslighting me. This place was great. My husbands aunt was here after hip surgery. The people here were really nice and took really good care of her. There were different nurses that took care of her. There was one nurse who gave medicine, her name was Nicky. She came in to check her blood pressure and told us what kind of medications she was giving to her. The other nurse said she does treatment. Her name was Maryl. She would come and change the bandages on where they operated on and she was nice enough to tell us what and why she was doing it. Sometimes she would have students following her and she was very patient with explaining to them what was being done. The other people that worked with my husbands aunt were also nice and careful when cleaning her, which was good to see. She had a good experience here in the time she spent at Berkeley before she went home. read more

My wife had taken a fall. She was released from Holly Cross Hospital Mission Hills. She need phyical therapy. So we picked Berkley. This place is very organized. When my wife push the buzzer for the nurse. The nurse was there. They are very professional and compassionate. My wife was there for a week and released. Everyone was great. read more

My husband spent 2 weeks in Berkley Post-Acute Care facility following lower back surgery. Like any other nursing care facility, you have to recognize your loved one's needs and ask for help from the facility's nurses, wound nurses, nurse assistants. I found this facility to be very clean, the patient rooms smelled good, and, in general, my husband was treated with respect and kindness on all levels. His was showered regularly, his gown and bed changed regularly, and had physical and occupational therapy 5 days a week as promised. You have to remember why your loved one is there initially and not expect everything to be perfect. read more

I was very emotional to leave my mother there but the staff did an amazing job in calling me to give updates daily.! My sister was admitted for a insurance pre-approved 3-week stay to return her to pre-hospitalization levels of function. She entered on transfer ambulatory with walker but very weak from USC Verdugo Hills Hospital where she he was minimally worked-up before transfer. 99 days later I picked her up and brought her north to live minimally responding and 100% nonambulatory. Once north, she ended up hospitalized acutely in a rural critical access hospital (a whole bunch less resources available than in Lis Angeles County), where she was immediately recognized as a medication nightmare in a toxic state. How did the Berkeley Post-Acute Rehab not even investigate the appropriateness of the medication profile and not in the least transfer her back to the acute setting for investigation as her condition deteriorated? It was very obvious she was declining both mentally and physically. Our brother was visiting at least 4 days weekly, trusting health care provision by certified/licensed health care providers. Once north with me, it took 30 days in an acute/swing bed setting to clear the inappropriate toxic medication effects physically, mentally, and psychologically that were manifested by the lack of attention to care of patient needs at Berkeley Post Acute. It took an additional 7 months of in family home care to return my sister to full independent function without any assistance. It took a real commitment to 3-times a week outpatient physical therapy sessions, mental health counseling, and several medical follow-up appointments. This should have been addressed in the 3 weeks converted to 99-day stay at Berkeley but was not. Once no longer needing Hoyer lifting, my sister had total continence with bedside commode and then advanced to full bathroom privileges. Why was an untrained family member able to achieve what Berkeley was a total failure for my sister's recovery. It is very concerning that close to a year of my sister's life was lost by a facility that was more interested in collecting insurance monies (Medicare and Medi-Cal) over the rehabilitation of their patient. Sadly, my sister was not the only patient stacked into a 3-bed room (converted from a semi-private 2-bed intended) and forgotten. Be an advocate for your family member and do not settle for a park and wait mentality, because that was my sister's experience in my opinion. No elderly person should ever be discouraged from recovering. My sister literally walked with walker aid to the bathroom upon arrival, left in incontinence briefs and barely responsive 99 days later. DO NOT send your family to Berkeley Post Acute and expect anything more than diaper changes, rather than transfers/help to bathroom privileges that involve time when call light has been properly utilized and answered. This is the most basic function of dignity any physical rehabilitation facility should baseline function provide. DO NOT send your family to Berkeley Post Acute. 1 out of 5 stars because she left alive -- barely. read more

I am a 79 year old lady and recently had a bad fall at home and broke my shoulder. I was taken to hospital and after a week was told I needed to go to a rehab unit. I had previously been in two rehab facilities and was disgusted with the care and cleanliness of both. I was very skeptical about going into a rehab and fought as hard as I could not to go. Common sense finally kicked in and I knew I could not manage at home because I needed 24/7 care. I decided on Berkeley Acute Care in Van Nuys for my care and rehab. It was quite a way from my home but absolutely worth the extra miles it takes to get there. I heartily recommend this facility to anyone. I found that it was very clean (and I am very fussy).The nursing staff were mostly excellent and caring. In every place there are 1 or 2 people who should follow different professions as they just don't have what it takes, but 99.5% of their nursing staff were really hardworking, knowledgeable, professional and caring. When I complained to the charge nurse about one nurse in particular that individual was removed from my care team immediately and replaced with someone else.Their rehab unit and therapists are really excellent. The administrator, Trevor and his staff are all very nice and helpful and try their best to accommodate their patients. I totally recommend this facility to anyone who is trying to decide which facility would work for them personally,or their family members. I and my family were very satisfied with the care I received there. Well done Trevor, you run a great care facility. read more

Berkley post-acute did an excellent job at treating and caring for my mom. As someone who works in customer service, I recognize and appreciate excellent service! The entire staff at Berkley were exceptional, caring, attentive, friendly and on top of things. The facility is exceptionally clean, and the patient's food is very tasty, and mom finished everything on her plate at every meal! Trevor, the on-site Administrator is outstanding, very attentive, and extremely knowledgeable. Nurse Rosa C. is wonderful and treated my mom like family and goes out of her way to help each patient! I highly recommend Berkley Post-Acute for any extended stay, physical/occupational therapy and we will definitely refer our family and friend to Berkley Post-Acute! This facility is full of compassionate staff and great customer service. My mom had a short stay here for PT, OT, and ST after being in the ICU for 2 weeks. The rehab staff encouraged my mom to participate specially on the days she did not feel like it. My mom was able to go home without oxygen and ambulating with a front wheeled walker. The administrator Trevor made it a point to introduce himself to my mom and made sure she knew he was a source of help if she needed it. The DON Carina educated my mom on resources such as how to apply for IHSS hours when she goes home. CNA Monnette S. made sure my mom's basic needs were met every day. She even took her time to fix my mom's hair after being matted and tangled from laying in the ICU for two weeks. Would definitely recommend this place! read more

What a well run convalescent home. There's not a ton of parking, but I've always find a spot when visiting my friend. The staff is amazing and friendly! They are always smiling and remember my name as well as my friends. I appreciate they introduce themselves and ask permission before doing any service (changing, showering, blood draws, medications, etc). As well as referring to everyone as residents over patients. They have daily activities for all and highly encourage all residents to participate in social gatherings. The doctors are prompt and friendly. The therapists are knowledge, gentle and kind. Facility is super clean and smells pleasant. I HIGHLY RECOMMEND this facility for anyone seeking rehabilitation whether long or short care! read more

Since my previous post things have definitely improved. Trevor, the administrator, has been very attentive and has listened to our concerns and has tried his best to find solutions. In addition, we have encountered some great staff like Dexter, the physical therapist, who has helped my father-in-law immensely. Dexter has been very patient and extremely cautious during the sessions. Another positive note, the facility is kept very clean and all visitors have to take a Covid test upon entering. I'm very happy that I can update my review with some positive feedback. -------------------------------------------- They tried being more attentive to my father-in-law on Friday (6/2/23). But the day before, on Thursday, they forgot to change his dressings on the wound and he is DIABETIC. Night care has been better and he is getting his medication. The day nurse changing dressings is amazing, facility needs to have more workers like her. BUT...... This morning (06/03/23) my father-in-law and the other patient in the room DIDN'T get breakfast, apparently they forgot the room. Breakfast was at 7is am it was already a bit past 9am. Please be the advocates for your loved ones. ------------------------------------------- The worst, please keep an eye on your loved ones if they are admitted here. My father in-law checked in yesterday (05/31/23). Not attentive at all. My husband had to remind staff about his pain medication, medication was supposed to be given 7pm, they didn't give him medication till midnight. They seemed bothered that my father in-law was even requesting it. Then...he asked them to help him rotate his leg, the leg that was recently operated on, no more than 5 days. The nighttime caretaker then came in and not only was she RUDE, she grabbed his leg and THREW it over, causing him extreme pain. By doing that she probably aggravated his injury. This incident was reported at the facility. read more

The staff is genuine caring for the patients. The follow up and keeping loved ones upto date is almost too good. Thank you to Joseph and the other floor people for calling least once a day to update me. Staff was very cooperative with my mom's primary doctor. I was very happy with the social services team Christina and Chris were responsive and thorough. This place is clean most of the staff is nice and they care for there patients. The food is okay I guess not horrible. I like how fast they got my mom doing her exercise and speech therapy. Bumping. These positive reviews are fake to improve the public outlook on it. Do not trust them. When I originally made the review, I had then been contacted by the facility's head and social worker, Gloria (at the time) to take it down. When she and her assistant showed up to my home to pick up my mother's wheelchair, she coerced me to take down the review and gaslit me. Fortunately, I snapped out of the shock I entered and had some aid from some family to reinstate the review. read more

Recently we had at my dad going to this place, it is a very nice clean place very organised. We had Nurse assisting (CNA) called Rosa Castro. She was the best caregiver that I can recommend. She took care of my dad constantly always asking him if he was okay, calling him papa, never not going to his room because she had other stuff to do. She always make time always make sure that he's clean and comfortable, you can't ask for anything else I recommend the place and anybody who will receive her as a nurse assistant is very lucky read more

I wanted to provide some positive feedback on Berkley Post-Acute (BPA). Mom stayed at Berkley for a couple of weeks due to a bad fall and the entire staff at BPA were excellent, very caring, and attentive to mom's needs and request. Trevor the on-site Administrator is an excellent Manager. He is very hands-on and stays on top of issues and does an excellent job at managing the team. I also wanted to recognize other team members that cared for my mom and made her stay very enjoyable while she recuperated. Nurse Rosa is an excellent nurse! Mercedes and Laura kept mom busy with non-stressful activities, Nutritionist Grace did an excellent job with the food selection, others like Charles, Hector, Luz, Tanya, Kristina all made mom's stay very enjoyable and stress free while she healed. Berkely is a wonderful place to recuperate and has great resources and can support and attend to anything you need. Excellent job everyone and please keep up the outstanding work! read more

My 100 year old mother has been in your facility for a few days now. Each time I try to call to talk to her, I'm left on hold for long periods of time. I'm on hold right now and it's been 30 minutes...so far. I'm going to have to end the call and call them back. The nurses tell me that there are only 2 cell phones for the entire unit. I don't understand this. Why aren't there phones in the patient's rooms? Relatives would like to call to speak to them and/or the caregivers and therapists. I'm hopefully my mother is getting the care and services she needs, but the experience of contacting her has been horrible. read more

My wife was sent here after colorectal surgery, for the purpose of having PT to help her gain strength back after months of illness. She was in the facility for 3 days before any PT session was held with her. The nurse assistants in a couple of cases were rude and unprofessional. When my wife asked for her pain meds, they often would arrive an hour or more later instead of in a reasonable time. On one occasion she asked the night nurse assistant "where are my meds, can I have them please?" the nurse assistant yelled at her, telling her to "stop screaming". My wife was not screaming, she was simply asking that she be given her meds on time and the correct one. Another instance with another night nurse assistant, my wife asked for a fresh hospital gown. The nurse assistant said to her "why, do you work here?" and then walked off. This same night nurse assistant neglected to empty the catheter bag of the patient next to my wife, in the same room. So it burst and flooded the floor with urine. This happened again a few days later. Twice in a period of eight days. When my wife got her first session of PT, it was very hard work and she was very sore afterward at her surgical site. The next day, a different PT nurse arrived to work with her, and told her the device the previous PT nurse had my wife use was not supposed to be used by patients who have had abdominal surgery. The bathroom in the patients room (3 beds) was often not cleaned and another patient in my wife's room complained about it a lot but it did not get cleaned in a timely manner. My wife had strict diet orders from her surgeon, and one of the restrictions was no fresh vegetables. All vegetables had to be well cooked. But on a few occasions her meal brought to her had raw lettuce mixed in with the chicken salad. People there do not read directions and pay attention to patient needs. A patient across the hall tested positive for COVID. It took them a day to move the patient out of there. The day I brought my wife home from this facility, we gave her a COVID test. She had what she thought was a head cold, mild sore throat, and coughing. She tested positive for COVID. Twice. So Berkley Acute gave my wife, 74 years old, COVID. The incompetence at this facility is staggering. read more

My brother has been in this rehab for a couple weeks now, and here are some things that I have noticed. They have an amazing Administrator that has helped me tremendously know have to navigate this rough situation we are in. He is extremely knowledgeable in regards to how to navigate the Healthcare system. On top of that, the staff are phenomenal. I'd like to shout out Tiffany for finding out how to motivate my brother to eat, Rosa for constantly being available to all his needs, Nikko for his attitude ofcompassion, Charles for his work ethic and always keeping me informed, Rhoda because she is sweet and knowledgeable, and so many others that it's hard to name. My mother just had a 2 month stay at Berkeley and was so happy with her stay. Every staff member from the nurses to the women that check the visitors in were wonderful. Christina in Social Services is a gem. She arranged all the transportation to my mother's appointments, and always returned phone calls promptly.The activity director made my mother feel so special. She always stopped by her room to chat and even gave her little gifts. I would definitely would recommend Berkley for anyone's rehab needs. read more

The place is clean and 97% of the nurses are nice and dedicated. The food is okay but the staff in front desk are very nice. I would go everyday, The only thing i had a problem was the parking they don't have enough parking. but beside that i would recommend it. They were fast on giving my mom physicals therapy which was very important. read more

As a health care professional, I have worked with Berkley Post-Acute and their staff on multiple occasions. I have always gotten positive feedback from patients that have been treated at Berkley. Most of my interaction has been with Gloria and she has always kept the best interest of the patient in mind and is ultimately concerned for her patients well-being. If my own mom was ever at Berkley valley I would know she is in good hands. That is the best compliment I can give. read more
