After a rough start, things got better. Jasmine and Maggie were two personable nurses. The pt and ot leaders were open to where I began and what I needed to do. Expect to get 1/2 hour of physical therapy along with four or five other patients at the same time. Most of the food is out of a can or highly processed. I lost 25 pounds while I was there because of the poor food quality. Sometimes had to wait up to two hours to be cleaned up following an accident. read more

My husband was moved into this facility specifically to receive rehab after having pneumonia. They placed him in a shared room which seemed odd, especially for the other patron, then proceeded to be more concerned about mapping his body for bruises than his actual comfort. The room was substandard; broken handle on dresser, walker with someone else's name on it, broken headboard on the bed, holes in the screen and sewer smells from the open window, etc. There was confusion about his medications, that never did get straightened out. Several staffers standing around looking at You Tuber on their phones while patients called out for help. He was only there 2 days and I am glad we moved out of there. read more

Do NOT send your loved one here. Ever. One star is generous for this review. Bella Terra Lombard failed my mother in ways that have permanently impacted her health and future care. While under their supervision, my mother developed a severe sacral bed sore that was completely preventable. The wound was allowed to deteriorate to the point that it required surgical debridement and an additional week-long hospital stay. As a direct result, she is now being denied admission to higher-quality rehab facilities because no one wants to take on the liability of the wound care needs caused by Bella Terra's neglect. In addition to the bed sore, my mother also suffered a separate leg wound after cutting herself on a bed with exposed metal, because a protective cap was missing. This is a basic safety failure that should never happen in a facility responsible for vulnerable adults. The conditions were unacceptable: The facility smelled like decay and waste Administration was essentially absent Staff blamed my mother for incontinence, when in reality she was not assisted to the bathroom in a timely manner and thus was left sitting in her own feces, which directly contributed to skin breakdown and infection risk. This was not a one-off incident or bad day -- it was a pattern of neglect, lack of accountability, and failure to provide basic dignity and care. If you love your family member, do not send them here. The consequences of inadequate care don't end when you leave the building -- we are living with them now. I would never trust Bella Terra Lombard with the care of anyone I love. read more

My mother was a patient of Bella Terra in early 2025. She has now passed and I am still trying to process everything but this review has been a long time coming. My mother was a stroke victim age 62 and was sent here for rehab. Our choices were limited due to her state insurance. If you do not care at all about your loved one this is the place for you otherwise you are better off having your loved one live on the streets. I visited my mother daily here and it just kept getting worse. This place should be shut down! When you walk in there is a constant beep from smoke detectors needing new batteries. The staff does not acknowledge you whatsoever and is very quick to ignore and or be rude when you do go to the main desk. The place reeks of e coli and sadness. It is not uncommon to see elderly slumped over in a wheelchair to the point of almost being on the ground in their doorway. My mother went almost (2) weeks without a call button, sometimes did not receive her pills and when she did she was rushed and told " I don't have time for this! " my mother is a stroke victim she needs a moment and a lot of the times her meds were left bedside!!! I even have the photos and documented every occurrence we had here. One of the staff ate her food in the rec area I found her tray with her name and everything! One of the days my mother had fallen off the couch more slid and I asked the first nurse I saw to help me get her up and she said I had to find her nurse that was down the hall giving meds to other patients.... Are you kidding me! Then magically the next day the halls were filled with safety posters and how to handle falls. My mother also got sick and I could tell something was way wrong and she was puking what looked like old blood and I asked the nurse to please call for transport to the ER and I heard her on the phone state my mother was puking Dr. Pepper!! This was not the case, my mother was sent to Good Sam and my husband and I came back and backed up her room and NO ONE asked who we were, what we were doing nothing at all. This place should be closed down immediately; the staff is horrible and need to be educated on conditions that their patients have and how to help them properly not just collect a paycheck. Shame on you Bella Terra people deserve better! read more

Bella Terra Lombard is the best care facility in DuPage County! The staff - especially Gigi, Jasmine, Maggie & Hope, and social worker Jamie -- have always been gentle, caring & compassionate in their care for my brother. They work with Medicare and are transparent about costs & progress in therapy. They do amazing things! read more

I can't even believe what a poop hole this place was . I did not have to go, but leaving the hospital they said I may be able to get a wheelchair if I go to rehab at this nursing home for two or three days. That's why I want. I stayed for 36 hours . They never ever came up with my pain meds, I signed up for a shower and I don't think anyone had a shower . And I can't even describe how rude and cruel the staff was !! Please don't put your family in there if you love them at all . This is the Lombard facility I doubt the other ones are better but for sure Lombard is garbage pit . read more

Not the worst of the worst but definitely not the best. This facility must have changed corporate ownership sometime within the past couple of years. I visited a loved one here 10 years ago and the facility seemed fine. I visited a loved one here this year, and it was not good. They are chronically short-staffed. The staff (many of which are travel staff) aren't able to keep up with the need of their large patient loads. Staff do not update the whiteboards in the room and are not always familiar with the physical needs of the patient. I never knew who was caring for my loved one or how to contact them. When I pushed the call button, it took over 10 minutes for someone to answer. Staff are not always gentle with patients. They are busy and get their work done as quickly as possible sometimes at the cost of patient safety and comfort. The facility is also not in good condition. Lights in the rooms don't work and no one is available to fix the issues. Recommendation: If you have the choice to go elsewhere, it is best to explore your other options. This facility has gotten worse in the past couple of years. read more

This facility lies about the care they give patients. They claimed to have done therapy on my father in his medical records, when in fact the nurse told me face-to-face they were unable to because he was too tired or sleeping. They claimed to have kept my father hydrated and fed, when in fact the nurse walked in, dropped his "lunch" on his table and said, "You try and feed him he won't eat for me." He should have had an IV to keep him hydrated and he did not. They claimed he used the toilet and was able to walk. He never left his bed and wore diapers. His hands were covered in dried blood on our second visit. Have no idea why. (see pictures below) Do not trust this facility with your loved one. The devil and his minions run this facility. They do not care about human life. You've been warned. read more

My aunt is in the Bella Terra Lombard rehab facility. Her mattress is so thin and does not give her back any support. We visited her on December 3rd. The dry erase board in her room has Nov 18 written on it as today's date. The clock on the wall does not work. The room felt very neglected and ignored. I mentioned my concerns to an employee at the nurses station and I barely got a grunt out of her. No concern or attempt to remedy. I can't find an email address on the website to send an email to voice my concerns. So disappointed. read more

Don't judge this place by its looks. It's pretty rundown but hopefully they will be updating as they just recently bought the place from Lexington. My mother was there for the last five months of her life. The rehab floor was really bad. But then she moved up to the memory care floor and they took very good care of her up there. We had our mom there 10 days, she was not given any good care or kindness. She has a complex case, very sick for the last 10 years and fighting to overcome MRSA, and other infections as well as being on dialysis. There was one kind nurse, Noelle, who would do anything asked. Yet, we had to request everything. From wound changes- that were prescribed 2x's/day, to assistance in and out of bed. The staff were nothing but chatty, I had to request for her room to be cleaned, the toliet was dirty, the garbage over flowed and it took hours to reply to a nurse call. On Friday at 11am I requested to speak with the Director of Supportive Services and I sat outside her office for 3 hours, she returned and out a note on her door saying she was busy for another 2 hours. They knew we were requesting a transfer of care and they avoided us, by Friday at 430pm, it was too late to have any orders to transfer her and my mom had to wait to Monday. UNACCEPTABLE. The wound nurse said her infection was to the bone and blamed my mother for not staying off her knee, on Monday we took her to her PCP and wound specialist, there was NO Bone involvement and there was no attention to her cold legs, fluid on her lungs and we ended up bringing her to the hospital again. read more

If could give Lexington Healthcare Care negative stars I would give them -10. I would not send a dying dog to these people. In my opinion, based on my experience, incompetent. My mother was released from Pearl Healthcare in Rolling Meadows to me. She needed to have her lungs drained of fluid, daily. Lexington sent their nurse, Ray D., to my house to drain my moms lung---without the proper equipment. They had to use the one drain kit that Pearl sent home with. Their nurse, Ray D., said he would order the kits to drain my mom's lungs. Well, the kits he ordered came in 5 days---fortunately, I was smart enough to order them from Amazon so I could drain my mom as prescribed by the doctor. On arriving late, their nurse Ray D. said take her to the emergency room. Pretty unprofessional in my opinion. Further, their nurse Ray D. said that my mom's doctor (Doctor S.) said my mom no longer needed draining and that Lexington would not drain her. When I spoke with Doctor S., he said that he never cancelled the draining procedure for my mom. Looks to me like Lexington's nurse Ray D. lied. Lexington said they would investigate the matter and take action. When I requested documentation as to the action they'd taken against Ray D. they said it was an internal matter. So, I was given nothing and did not know what actions, if any, were given to their nurse Ray D. for lying to me about the Dr. S cancelling my mom's lung draining. I've spoken/emailed with their Vice President, Donna McN, about this incident. I was told that the $421 that I spent to purchase drainage kits to drain my mom's lungs was being refunded and processed around 1/25/2019. Another lie, I still haven't gotten reimbursed even though I've sent their Vice President Donna McN an email asking asking "where's my money". Would you want, in my opinion, leeches like this administrating healthcare to your loved ones? read more

Look people, let's get real. The Lexington system does not have the resources, training, oversight, or follow up in place (in fact across all their facilities) to keep their residents healthy from Covid -19 or any other outbreak that may happen in the future. The facts speak for themselves. Go on State of IL website and you will see how many cases and deaths their facilities have. There are many other similar long term establishments in the state of IL, in Dupage county, with no cases or deaths; they are not even listed on the website. Our loved one died at the Lombard facility from Covid-19. They are expensive and "for profit". Similar facilities do not have their problems. The people that work on the floors...in the trenches... are pleasant and appear dedicated to the elderly, but it is a revolving door of workers. Double shifts are not unusual for their workers...that is 16 hours straight. They love it as we were told, but this may not be in the best interest of your loved one. Do not expect follow through, nor communication between staff members. Reminders and overseeing the care of your loved one there will be your part time job. Do not expect your loved one to have consistent care. They have a systems problem: from lost or stolen clothes to lost wheelchairs. Bathroom hygiene is a problem. When other facilities spent the $$ paying their employees appropriately, screening employees, and being prepared for a pandemic, these steps obviously did not happen at these facilities. Are they better now with all the media attention and CDC recommendations being followed? Probably, but don't count on it. Will these employees be paid well enough to remain? Doubtful. Until the state of IL puts in regulations/laws for these nursing homes to follow, don't count on your loved ones being safe at this facility. Don't count on Lexington to be pro-active. They charge an egregious amount and are profitable...but, their resources are obviously not spent to where your loved one would be safe from these pandemics or outbreaks. The workers brought in the Covid-19 to the residents. Realistically with a revolving door of caretakers how in the world can the facility ensure training, oversight, and regulations being followed? Again, the facts speak for themselves. We cannot be any clearer. The state of IL has deferred their responsibility for Medicaid patients to these long term private facilities without oversight, and because IL is burdened with so many other budget constraints they don't look to prioritize the nursing homes with regulations. This is written not out of anger, but with the hope for change in all nursing homes in the future through state of IL regulations or through common sense management decision and priorities within the entire Lexington system. So if you decide to admit your loved one at this facility, just be realistic. read more

My mom is doing so well here at Lexington of Lombard that we placed another relative here also. We are utilizing Lexington long-term memory care in both Lombard buildings. We also had someone in their rehab for a few days. Staff members are kind, caring and patient with patents and family. Doctors and nurses are skilled, experienced and knowledgeable. BIGGEST PLUS is consistency of staff, meaning that you lived one has the same nurses and aides daily. Your skilled workers get to know your loved one, and your loved one comes to trust their caregivers. You don't have to tell new people every week what your mom (or whoever) needs, as you will in facilities with rotating staffs. Cleanliness is very good; my mom has not caught any of the contagious diseases that often run rampant in assisted living. The food is pretty good. If you want better care, you need to keep Mom at home or pay a lot more. [Note to other family members, from a family member -- and honest, I am family and not staff -- please consider leaving a good review here because people need to hear that this is a quality place.] read more

Find somewhere else is my strong recommendation. I was told this place had new management thus the reason for previous bad ratings on Medicare site. 1) Chatty nurses laughing and hanging around desk instead of answering call buttons. Very noisy. 2) Did not shave my 77 year old brother for a week. 3) Moved him to another room without notifying me first. 4) Strongly recommended that family sit 24/7 with him because he was on fall precaution. Hired outside help which cost us $270.00 per day. We finally got rid of them. And somehow they found someone to sit with him from nursing home. By the time they sent him home, still couldn't walk, yet did not send a wheel chair with him, or provide for a hospital bed, etc. which he didn't get until ten days after arriving home so he struggled without. Discharge person totally inadequate. Two weeks later with some home health care, my brother died. It shouldn't have been such a struggle for his family who attempted to get him good care. read more

I highly recommend this facility. The minute I came in for a tour I met the Assistant Administrator and the Admissions Director. The amount of information they gave me about the type of care my loved one would be getting made me much more calm about the whole situation we had been going through. I was shown the whole building and some of the current residents even talked about how much they liked it there. When my loved one admitted we were greeted immediately by the admissions director and manager on duty for the night. Within the hour multiple department heads greeted us and explained to us what we should expect. My loved one did amazing with therapy and progressed so well. We had one scary night and the nurses and aides were so responsive, not only for my loved one but for me as well. The nurses were knowledgable and used the telemedicine system to avoid my loved one from going out to the hospital. Our goal as a family was to have our loved one home and the staff at Lexington of Lombard made that happen. We could not have had a better experience. Although I hope we dont have any medical issues in the future, if it comes to that I will go back to Lombard in a heartbeat! read more
