My wife MARY entered Aviva in 2023 after her pelvis fractured. I saw the care was inadequate for her type of injury. During her stay she picked an infection that eventually took her life. The people who handled her care were not qualified to give to giver her the care sh needed. We spent months trying to stop the infections she had acquired at the center. Kaiser had placed her there and should have known the center was substandard. 12 operations later she passed at home. Her body was worn out trying to fight for her life. read more

My mother has been getting good care overall at Aviva. The nurses and CNA's are all very nice, smart and seemed to genuinely care about the patients. They are understaffed and it can take some time to get something from the nurses. She receives PT almost every day from some amazing therapists in a state of the art gym. I only wish she would get more speech therapy, she hasn't had any going on two weeks now and really needs that. They also don't seem to get (or acknowledge) orders from doctors at outside hospitals/medical centers, so there is a communication issue there if your loved ones have appointments off-site. They have a nice little coffee bar/cafe on the first floor. Reception and everyone else has been super friendly. read more

They do NOT care for your loved ones. They will leave them in pain and starving. The Nursing staff is RUDE and the CNA/LPN HELPERS whoever they are tell patients families that they are ''to busy'' to tend to your love ones. THINK TWICE about this place if you want your family member to get healthy. This is one BOOK you need to judge by the cover. The facility looks nice and these pretend 4.2 star ratings are even more deceitful. If you like watching your loved ones wait hours in time for someone to check on them or give the pain medication then go here....you will get all the NON support you are looking for. read more

I would not recommend this facility. I was transported from Swedish Medical Center Hospital to Aviva at Fitzsimons Nursing Care Center. I chose this facility due to the recent 5-star reviews and that the facility was clean, which is extremely important to me. Apparently being clean isn't enough. I arrived between 6:30 and 6:45 p.m. on Friday, May 9th. There was no one in authority there to check me in, other than a person who "appeared" to be one of the patients manning the front desk. She said everyone leaves at 6:00 p.m. It took about half an hour to get any staff member to receive me. I was finally taken to my room a half hour later. Once in my room, no one came to check on me. No one came to ask me if I needed to eat and no one came to check in on me for about 4 hours later when I'm assuming a nurse came to give me medicine which wasn't mine. I brought that to her attention. So she took the medicine away and finally brought mine maybe an hour or two later. The next morning my husband visiting me called to order me breakfast since I hadn't had anything to eat the whole night before. The cafe said that I could only have minced chopped up, cut up, mashed or blended food. I complained to the head nurse and eventually it was determined it was an error created by the attending physician at Swedish who sent orders that I could only have soft intravenous type foods by mistake. My personal doctor released me with the ability to eat whatever I want. This took the facility more than 3 days to get this corrected, even though my doctor had sent an order for them to remove the minced order and replace it with anything I wanted to eat. In the meantime they were waiting for a speech therapist to approve my eating regular food. I thought that's insane that a speech therapist had to watch me swallow. Even though my doctor had submitted an order to remove the error made by the attending physician at the hospital, who ordered me on soft intravenous liquid foods. Aviva wouldn't believe me that it was a vicious mistake and refused to feed me, forcing me to call my husband, family and friends to bring me food so I could eat. The whole time I was in the facility, the only interaction I had with the nurses was to give me my medicine twice a day and they gave me other people's meds on two separate occasions. It's a good thing I know what my medicines are. There were a few people who came to help me with physical therapy three times. But while I was in the facility, basically I had no help getting into or out of my bed, no one offered to help bathe me or assist me with anything. I literally took care of myself while I was in that facility. The bed was extremely loud, noisy and completely uncomfortable. So by Wednesday of the following week I was done with them and ready to leave. I left because it was pointless to be there. Though it was a clean facility, because house keeping came in to clean daily, the major reasons I would not recommend this care facility are as follows... 1. Starving me with the ridiculously long time it took to straighten out an error made in what food I could have, then refusing to accept my doctor's order sent to correct the mistake Swedish Hospital made, so they'd feed me the correct food. 2. Giving me the wrong medication. 3. Them not doing anything to help with my care which was the whole reason I chose to go to a sub acute care center, due to me being unstable and having the need to walk with a wheelchair or walker for assistance. 4. If my husband hadn't helped me, I would have never received assistance with a shower during my stay in this facility. 5. They must purchase the cheapest hospital beds in bulk. They are loud and extremely uncomfortable. Again, I would not recommend this facility and the only positive thing I can say is that it was clean. But that could be that the cleaning lady told me that the regular house keeper was out on vacation and she was standing in for her. However, the temporary cleaning lady's work was fire! read more

If you love your family don't take them here. The CNA's will wait 20-40 minutes to answer patient lights. The hallway is usually lit up like a Christmas tree with room lights, while the CNA's walk past them. We have filed multiple complaints with the state, and have talked with the Nursing Director and they don't care. CMS and Medicaid need to take a closer look at this facility. read more

I was a patient at Aviva Fitzsimons for rehabilitation after suffering a stroke. From the time I was met at the hospital by the driver, Reagan, to the day I was discharged the attention, care, kindness, and concern were absolutely first rate. The facility itself is attractive and meticulously maintained. It is decorated in a welcoming and comfortable manner. The staff is beyond excellent and I include everyone with whom I interacted from the front desk (Ana) to the maintenance staff (Michael and Ramon) throughout the Kitchen (Domonique), and housekeeping (Maria). I met others briefly who were also great. Then we get to the medical care staff all of whom were extremely dedicated to top flight patient care. Among these outstanding people are: Dr. Cahill, CNAs and nurses: Alecia, Claudia, Abigail, Michael, Tiffany, Layne, Nardos and others. All looked after my every need and were always available when I needed something extra. Then comes the rehabilitation therapists. All I can say is Wow! Every day I had at least three sessions of physical, occupational, and speech therapy. And by every day I include Saturday, Sunday and Christmas Day. The sessions were carefully planned to meet my specific needs and I saw the same attention to detail with other patients during my sessions in the gym. My therapists were: Mason, Mary, Ulrika, Man, Ana, Jocelyn, and Ashley. I know that I may have left someone off the list but this is due to my faulty memory not the professionalism and dedication of those therapists. Without exception every therapist was great. My sincere thanks to each of them. I am now home and progressing well but this would not be possible without the wonderful work of the entire staff at Aviva. If I have left someone off my list it is my oversight and I apologize to them. I am eternally grateful to my Friends at Aviva. Oh! I do have one issue with Domonique who is in charge of the kitchen: Your food is TOO good; I gained four pounds. Every meal was carefully prepared and quite delicious. Thank you. Once again my thanks to all of the Aviva family . Due to them I am well on my way to an amazing recovery from my stroke!! read more

Don't go here. Facility looks good on the outside, bad on the inside. My husband just had major spine surgery., involving 9 vertebrae. Rods, screws, cages. We were in the hospital two weeks. Looked like Aviva would be good, close to hospital just in case. I stayed with him at night until he received his meds. He did not receive all of them. He calls me at 4am, pain was 10. I was sleeping, didn't get call. Woke up and drove down to facility. I had to demand the get him to er. Tried to blame others about the medicine. We are currently in the emergency room. They have given him morphine. read more

A family member was here for rehab, after a hospital stay. We were very anxious to get him/her on the road to recovery. We knew it would be a long road, as my family member was very weak. We had our Care Plan Meeting, and things started out fine. The facility is pretty, and the food is good too. Meals are currently being delivered to the patient's room, which we appreciated, all due to COVID. After 4 days, there was a "kink" literally in his/her catheter. He/she was in quite a bit of pain. We told the nurse, who told the doctor. After 8 hours, the "kink" in the catheter was finally checked. Long story short, my family member had to be taken by ambulance to the hospital. He /she had developed a UTI. He/she was checked out of the facility that night, and we are never going back. Take whatever information you need from this review. I know it's not a true medical facility, it's a rehab facility. I only wish my family member had been given better care while he/she was there. Thank you read more

I only could put up with my mom being there for 1 1/2 days. We got her out of there after 4 days with all the paperwork for a transfer, and it was a weekend, it took a bit longer. My mom sat there for an hour before anyone even came into her room. And the nurse to come she did not seem to know how to hook up her feeding tube. It started beeping 20 minutes after she hooked it up, and started leaking, another nurse came in and got it right. When I came in the next morning, on a Saturday, she had been waiting between 20 and 40 minutes before her call light was answered. I went to complain and the nurse was scheduling flights to Southwest on her phone and ignored me as I stood there for about a full minute I told her I wanted to speak to her supervisor because I wanted my mom out of there. Damien came to talk to me, he basically made it sound of as if my mom and I were lying about the lack of care. He said I was making a mountain out of a molehill. He also said it was nothing he could do until Monday because it was a weekend so tough. Then he came in and said if I really wanted her out of there, I could call for an ambulance and get her out of there now, he is an administrative assistant, and he should know that's not possible without paperwork and phone calls made. I couldn't wait to get her to another facility. Some of the staff was nice, some were awful. Most of the nurses seemed like they were contracted and didn't really give a crap about what happened there. I am honestly not sure there are any good skilled nursing facilities out there to take care of someone after a surgery or an accident. Everyone I look up seems to have mostly bad reviews compared to good ones, that's a sad state meant for our healthcare here I got her into another one, and now she is sitting in the hospital with two different infections. read more

I would encourage everyone to visit this place BEFORE sending a loved one there. If they aren't equipped to care for someone at the very least they need to verbalize those concerns to the family. My mother was often times not brought meals, sat in a plastic bathroom chair with a new incision for at least 30 mins if not longer (she was unable to walk without assistance) and she had FOUR bed changes after her incision drained overnight. When I asked them if this was normal, they said yes! They are not equipped to care for patients that requite any sort of complex care. The bright spot was the Nurse Manager trying to help us AND the PT/OT people that tried to work with my mom AND the front desk employee, I believe her name was Anna. That's about it. My husbands visit here started out very rocky, thankfully it ended much better. The unfortunate thing about my husbands stay was that it was during the covid outbreak so I was unable to be there with him. In had to rely on being on the phone with him, and calling staff daily to help him out.Upon admission I was encouraged to call any time with any concerns or questions. I learned quickly that it's really not that inviting if you have complaints or concerns. I had a big concern with the response time on patients pushing their nurse call button. It never failed, any time my husband needed attention and pushed the call button, it would take anywhere from 20-30 minutes before someone would come into his room. There were a couple of staff members that would come and be helpful, some of the others would make it seem as if he was an inconvenience that he needed help. I expressed my concerns to a social worker and the help that I was offered was assistance in finding somewhere else for my husband to go. I had to let her know that we weren't uprooting him and starting this process over, I expect for them to do the work that was promised to us. My husband arrived at Aviva and pretty much needed to learn to walk again, so he depended on the staff for a lot of help. Because it took them so long to respond, or some act like they were incapable of helping him, my husband had quite a few bowel movements on himself. On one particular morning my husband kept telling me that he didn't feel good and no one was answering the call button. I called to the front desk to ask for the number to the nurses station for the wing that he was in, only to learn that there is no number dial to reach the nurses station, so once the receptionist goes home for the day, you have no way of contacting staff with concerns unless you call the charge nurse on her cell phone while she's home. My husband not feeling well kept being ignored until one day the Physican seen him and immediately knew that my husband wasn't well. He was taken back to the hospital and readmitted into ICU. My husband was suffering from internal bleeding but none of the staff there paid attention to his condition. When he was vomiting they just wanted to give him meds to make him stop vomiting. Thankfully the physician took quick actions and got him sent back to the hospital. While in the hospital my husband began physical therapy that was now working since his internal bleeding had been rectified. Once he arrived back to Aviva our whole experience turned around for the better. The staff wasn't taking as long to come if he needed to call them, they were more attentive to him now. He was now able to get the full on therapy that he needed so that he could come home. My husband also complained constantly about how uncomfortable the beds were. A part of me hates to give only two stars because I think after things got better, he received better than a two star rating, but A-Ok is just too much considering we had to go through as much negligence as we did his first week there. It's about 50/50 on my satisfaction with Aviva. Aviva May be ideal for someone a little more independent, but my husbands visit didn't become "enjoyable" until he gained independence and was more self sufficient. He did eventually get the therapy that he needed to gain enough strength to come home. Upon planning for discharge, there were a few communication balls dropped with the social worker and continual care after discharge. She neglected to follow through with the oxygen company so that things would be good to go the day of discharge, and the in home therapy also had not been scheduled. I had to do a lot of the work myself. So because of these things, i have to give Aviva a 2 star rating. read more

Looks like beautiful facility but it has many problems. The nurses station can not see the room numbers to help the patient. My husband was in there for 2 weeks and would have to roll his wheel chair into the hall to get some service. One time he sat in the bathroom on the toilet for 45 minutes because no one came. My husband recently was in St Joe;s hospital for sepsis then transfered there. He from being in the hospital and going to Aviva had a acquired a bed sore. They gave him Voltaren for him to apply on his bed sore on this lower back and buttocks. Finally he came home the nurse that came to our home. Bob gave him something to put on the bedsore. He was in and out of the hospital for 6 months. He had 4 operations at that time. His surgeons were the best of the best. I should have sent him to the one in Parker on Lincoln. The official Medicare website lists Aviva as a below average facility, with a 2 star rating and for good reason. But don't take my word for it. Visit the website and view the state inspection reports for yourself. And while there, I urge you to read the reviews from patients who have actually been a resident. Although they have a handful of positive reviews, there are more negative reviews, and the fact that almost every negative review states pretty much the same issue happening over and over again, lets me know that Aviva is uninterested in correcting the problem. And I stand by this statement, based off my recent experience with this deplorable organization. There are more facilities with higher ratings and staffed with people who actually care that can be found on the Medicare website, more deserving to care for your loved one because, unfortunately, Aviva at Fitzsimons is not that place. Kudos on the beautiful facility, though. It almost distracts you enough to not focus on all the bad. read more

I truly wish zero stars was an option here. I don't doubt that some people have had favorable experiences, this was ours. My father-in-law was there for about two weeks. He was sent there to hopefully recover some mobility so he might be able to return to in-home care. Due to the fact that he cannot hear without hearing devices or see well, they were quick to wash their hands of him. After a week, they wanted to release him. The only reason he stayed longer was due to his physician thinking he had contracted a respiratory infection. (This can happen when you are left to lay in bed all day every day.) The day I was there with my husband we asked the nurse at the station what the results of his test were, she said she would find out. Thirty minutes later, we went to ask again. This time she said, sorry I got sidetracked. After another hour, we went back to the desk just to find her sitting in the corner doing her own thing. Never acknowledged us. Never got any results. There was more, but I think I have made the point. We can see that he is deteriorating from dehydration and malnutrition, and it is decided to move him to hospice where he can receive hands-on care. When he arrived at the hospice center, he was soaked in urine and feces. Before you think, that could have happened in the transport, you also need to know that his back was covered in a rash from lying in urine for an extended amount of time. The nursing staff at the hospice had to bathe him and apply cream to the painful rash before he could be situated in his new room. There is absolutely no excuse for this type of neglect! To the nursing staff at Aviva: If you don't like the dirty part of the job, you need to find a different career. These are humans that you are trusted to care for! read more

I can't say enough great things about this rehab facility! Starting off with Isai, He explained everything from start to finish to my mother, making her feel completely comfortable about making the decision to go to rehab once again. We've had some rough rehab stays in the past so let me tell you it was very tough to convince her into giving another facility a shot. Not only is this facility beautiful but the care matches it as well. The transition from hospital to rehab was a smooth one. Nursing was outstanding! Joe the nurse was always very sweet to my mom and gave her amazing care. The nursing team was attentive and kind throughout her stay. rehab was amazing! She wasn't very motivated in the beginning but we quickly saw a turn around in her attitude and her strength and saw her reach many milestones throughout her stay. She's now home and able to continue to live independently. I would recommend this facility to anyone looking for great rehab experience. I wish I could give 10 stars!! We are forever grateful to the Team at Aviva!! read more

If I could I would give Aviva less than 1 star. Here is why. My 89 -year-old Father was taken to Aviva at the end of March, 2020 after a fall at his home that fractured his clavicle. After a month at Aviva, family members and I begged him to go home and continue rehab there, but he said he wanted one more week to get stronger. and decided to stay at Aviva. During what should have been his last week there, he was diagnosed with Covid that he contracted from the Aviva staff. Instead of going to the hospital to recover from Covid, they convinced him to stay at Aviva, because they said how safe they were, that he was not allowed visitors and "all precautions" were being taken. After 2 months at Aviva, although he no longer tested positive for Covid, his battle had weakened him so severely that he was on death's door with horrendous bedsores that went to his bone. After such horrible and neglect in care at Aviva, he was finally transferred to the Rose hospital. The doctor at the hospital said his severe bedsores did not start with the Covid. Bedsores that severe take several weeks to develop, and his dislocated shoulder fell out of it's corrected setting. How can a rehab center not rehab their patient nor rotate their patient nor treat such devastating bedsores? The lack of even basic care was beyond Inexcusable!!! My father passed away in the hospital 3 weeks after he left Aviva from being so severely weakened from the unset fracture, terrible bedsores and the Covid he caught while at Aviva. Aviva is a gorgeous facility with an amazing staff! Besides being brand new and state of the art, the food is great too. The staff provides quality, customized care. You can tell they care about each patient, their needs, their dignity, and they go above and beyond to give personable service. The staff are seeing patients through some of the lowest, hardest points of their lives but make sure both patient and their family feel cared for and involved in the process. I love this facility and would highly recommend it. From the moment you walk in the doors, you feel the happiness they've built into their culture and team and it is a reassuring feeling. read more

My husband was a patient at Aviva for one week from 7/24 to 7/31 , discharged from UC Hospital following major surgery and complications during recovery. He was too weak to travel home and the staff was extraordinarily helpful to work with him to progress physically to be able to make the trip. This is a beautiful new facility with every attention to an aesthetically pleasing environment including comfortable furniture and rooms, copious art work, outside patios frequently used for PT and common areas like a lounge and the dining room allowing for socialization. More important was the medical, PT and OT staff. My husband was visited several times by the staff physician and nurse practitioner. Nursing staff dispensed meds appropriately and were attentive to our needs. The CNAs were readily available and patients and family alike were greeted with smiles by everyone. The nutritionist was especially helpful dealing with digestive issues related to medications. He received the maximum therapy he could, including on a weekend, to give him the strength to return to our home in Durango. The only activity we were able to participate in was a delightful grilled lunch on the patio. We are deeply grateful for the excellent experience we had at Aviva at Fitzsimmons. read more
