Before my dad was transferred there, I stopped by there without an appointment to check out the facility. I was immediately greeted and welcomed by the friendly staff and given a tour of the facility. The facility was clean and pleasant smelling. The rooms were very nice and spacious, the shared rooms looked as though the residents had plenty of space, and did not look cramped at all. Once my dad arrived, he was put in a very nice private room with a ton of space. All of the office staff, his nurses and nursing assistants, therapists and doctors have all been very sweet, and do such a wonderful job taking care of my dad. The physical, occupational, and speech therapy my dad receives have been top notch. He had been in another facility a couple months back and the therapy he received was the absolute minimum and didn't help him at all. The therapists at Hawthorne truly care about their patients making progress. The speech therapist, Brittany is so sweet and caring. I get to spend more time with her because I am often there when she comes to his room to work with him on his eating. She lets me bring in his favorite banana cream pies and she stores them in the fridge, so she can feed him when working with him on his eating, instead of the usual apple sauce. When I to go to the therapy gym, it is very busy, and all the therapists are working hard with their patients. My dad has already made a lot of progress in just the couple of weeks he has been there, as they really push him. They all make him feel so welcome and always happy to see him. My dad has dementia, so it can be scary leaving him at a new place, knowing he cannot communicate that well. I visit my dad at different times every day, and every time I go, I always find he has been well taken care of. I also enjoy taking my dad outside in his wheelchair so he can feed the ducks at the pond that is located in front of the entrance. If you are looking for a skilled nursing rehab facility for your loved one, I cannot recommend Hawthorne enough.

After visiting several facilities in the Brandon area, I am grateful to Miss Henderson and Miss Woody who quickly responded to Mother's need for placement and put me at ease regarding the quality of care that she would receive at Aviata Oakfield. They have kept every promised made. 6. A plan of care was never presented to me for consideration and planning. 7. I am also thankful for Paulette and Nurse Tye who were the first to provide services (intake) to Mother. 8. It is admirable that the Administrator is hands on with staff and clients. 9. Grateful I am for Clara (Business Office) who patiently assist in guiding me through the steps necessary for permanent placement. 10. Nurse Jerome excellently guides the team responsible for providing care of my Mother. 11. I am pleased with staff with whom I have had the pleasure of engaging in conversation or requests for service. 12. Discovered that I have a dear friend confidentially admitted. 13. The driveway needs immediate attention to repair pot holes. 14. The exterior of the building needs attention in some areas. 15. The grounds need some tender loving gardening care. 16. Better system of the patients personal clothing needs to be made. Though carefully marked, many pieces of clothing are missing. 17. Customer friendly training is obvious with most of the employees. 20. CLOSING: Overall, I am pleased with the Facility and have established a personal connection not only with Staff but also with many of the patients. I have a family member that developed cellulitis and this was left unattended. After over a week of having advised the staff of the condition, my family member ended up in the hospital with an advanced infection in the bloodstream and in delicate condition. Do not send your loved ones here - staff is agreeable but that does not solve much when patients do not receive appropriate care and there's no communication from one attendant to the next. No one seems to have any information when asked about what care is being provided. I don't recommend.

But once again, the owners are "absentee" so they probably aren't even aware of how poorly the facility is being run. We met with one of the sales people in assisted living who was rather helpful and when we went back a couple of days later she too was gone! Then there's the "rehab". This section should be under investigation for 'senior abuse'! They are desperately understaffed therefore the patients very poorly cared for. There is NO actual attention given that would be considered 'rehab' because the little staff that they have is continuously preoccupied administering pharmaceuticals. If you are considering bringing your loved one here, don't be fooled by the outer condition of the buildings because it's what's inside that counts. And inside is not good.

Then, there was minimal rehab at best. If she was ever in need of anything (pain pill, bathroom, etc), she had to ring the bell an hour ahead, because she learned that it would take that long to get service. Then, upon her release, the did absolutly nothing to arrange for post-release rehab. It finally went back to her primary care doctor, but she went another 2 weeks after release because they never made the arrangements. Terrible, just terrible. Yes, we can say they're understaffed but is that really a good excuse for poor treatment?

We did not have a positive experience. The food was bad on a good day and inedible many days. My…read morerelative had come as a result of an emergency and they told us nothing about how things worked. We spent days looking for their clothes, or medicines or someone to change them. This last was especially bad as they would go hours without being changed. Many of the staff were nice to us, but nasty when we weren't there. Ultimately, we believe that the facility sabotaged our choice of assisted living by not letting them actually meet with my relative, just telling them negative comments in order to keep them. It was a relief to get them out of there. The actual facility itself is all right and PT was good. Everything else was wanting...and how does food even GET that bad?
