Due to circumstances I can't leave a full review currently. However, will in future. I can recommend advocating for your loved ones or self, keep track of all medicines given, get clear and precise information with physcians / nurses and caretakers about your stay, double check and verify care. Caregivers need to speak English!! When a resident has to go to the hospital, the staff should notify their family, etc that their belongings have been packed up and notice given to next of kin to pick them up, etc. The food leaves much to be desired. There's very little room to get around-especially in a wheelchair. The therapy team is amazing and the internests truly care. The activities department works hard to make life happier and better than many other places I've been. It's truly sad that a few can ruin a great place. read more

Lack of care and when you press the call button they have on several occasions taken more than 40 min and one time over 1 hour 18 minutes. You need to advocate for your loved one as other than when food is dropped off the staff is scarce. PT is dismissive and days can pass with no information on rehabilitation plan - when you ask they tell you they are understaffed. There seems to be plenty of staff on their phones in groups of three or more but don't expect attention or communication as other than day one when they parade in one person after the other you never see them again several days into a stay people are pretty much on their own through the say. One person who was excellent Was the nutritionist other than him they say they will come back to you but they so far never do. Definitely cannot recommend at all. No saving grace here. read more

I just want to say I was visiting a relative recently at Aviata of Countryside and the unit manager Kimberly was very friendly and helpful and the care she gave was excellent im so grateful she works there because I know my family member as well as the other residents will always get the care they need and deserve from her . read more

Couldn't be more pleased and exceeded my expectations. I wasn't looking for perfection! After researching several Palm Harbor SNFs, and speaking with families and patients, I found all had problematic issues. Grandiose DOES NOT exist in snf's. I sought a facility where I wouldn't get lost in the crowd, and I didn't! Aspire has, to its credit, harnessed an outstanding staff and especially an OT/PT team hard to beat. They provided me with aggressive physical therapy, as I requested. The facility's spotless cleanliness is something I attest to, despite the odor. Aspire Executive Director Jeff Comandini, is very personable. A hands-on director, fostering greater one-on-one personalized patient care. That's what I needed, and that is exactly what I received at Aspire Countryside. Aspire Shannon Sweeney (CNA). At 2am I arrived at Aspire. The first set of kind eyes and warm smile to welcome me were hers. Very intuitive, and very professional, she immediately put me at ease, settled me into my bed, helped me arrange my belongings, and answered all of my questions. Unforgettable in how efficiently she handled all of my needs. She exemplifies someone who loves her work. I looked upon her as the ideal mentor because she set such an outstanding example for others to follow. Aspire, Dawn Simmons. If the Angle Gabriel had a wingman, it would be Dawn. Highly Skilled professional caregiving specialist. Soft-spoken, keen to pick up vibes,for and, if something wasn't right, she immediately took action to fix it. Aspire Elvira Roko. A highly specialized, well-seasoned, dedicated coordinator of a million tasks. She is the "go-to gal". In getting things done right. You can't find people of this caliber. Yet it exists at Aspire. Aspire Dr. Vincent Dilella, what a terrific doctor! Always cheerful and asked what more he could do to add to my comfort. If ever there was a doctor that went beyond the call of duty. Dr. Dilella is surely the one. Aspire Lisa Kendall, APRN-C. Made sure my medications were in order. Never once did she greet me without a sincere smile. She made good on her promises. I could not have asked for better. Aspire Tarena, Lorena & JuLisa, they a powerhouse team of efficiency, making sure things run smoothly. Tarena is the Assistant Director of Nursing aka... the cavalry. Aspire Nisa Garcia - Wound care specialist. Plays a critical role at Aspire. Empowered with a gifted sense of compassionate caring. Well-earned accolades to describe her abilities would be exceptional, gentle, and experienced in the treatment of wound care. My surgeon was very impressed with the care Nisa gave to my injury. Aspire Wendy (Patient & Staff Coordinator). A sweetheart with a positive attitude and a drive to make sure all the needs of patients were being met. Even those who could never be satisfied. Aspire RNs in charge of Medications: Maria, Alfredo, Jose 1, Jose 2, Jose 3, Manuel, Marice, Elaine, Anabell, and Ana Fleets Roche. I sincerely thank you for your constant help. I could not have made it without you. Aspire Edgar- Social Worker Aspire Physical Therapy. They are responsible for putting Humpty Dumpty back together again. Ms. Omega customized my therapy to meet each new challenge. Madihra, blessed with endless patience, was a cherished joy to work with and helped me exceed my exercise count. Joyce challenged me, and I adored her unabashed willingness to be creative. Mandy had a talent for explaining every detail. Barry was talented and fun to talk with. Kaitlin taught me patience. The real cornerstone of my physical therapy was "Carly". Unsurpassable and played a critical role in my physical recovery. She taught me how to cope, how to position my hips, and how to climb stairs and so much more. I miss her specialized care. If you read this, know I emailed you. Aspire Without Caregivers-Aids, no SNF can exist without them. They are the true backbone of hospital care. my husband was neglected and ignored at this horrible place. There were a few caring people who were terribly overworked but most had poor English and stayed outside smoking. Management did nothing to improve care. I was there most of the day every day. He had trouble eating and they just sat the food down out of reach and left. His meds were sometimes inaccurate and I had to intervene. They did not answer the call button. Please never send a vulnerable person there. If you do then stay with them every minute. It has been almost seven months since my husband was at Aspire Rehab in Palm Harbor and the memory of the uncaring staff and awful negligent treatment is still vivid. He was put there for rehab of a torn bicep tendon. While there he contracted pneumonia and covid. The sanitary conditions were awful. No precautions were taken even though the floor my husband was on had numerous cases of covid. The first night he was there he had to call 911 twice as he could not get anybody to help him get to the bathroom. He was in terrible pain yet it would take forever to get an aid or nurse to bring him a pain patch. The aids were rough everytime they had to handle him and he screamed from the pain there rough treatment caused him. The toilet to his room clogged up and it took two days to get maintenance to fix it. When he began getting sick and coughing alot I asked to have a doctor see him. I was told there was no doctor available It was like pulling teeth to get him an xray and covid test. Then when he tested positive for covid and his xray showed pneumonia I told them he needed to go back to the hospital. They said he did not need to go as they could take care of him. Finaly my daughter in law an RN demanded that he be sent to the hospital. The hospital checked his XRay and also did another scan and discovered that he had undiagnosed bone cancer. Aspire should have noticed this on his Xray. We immediately placed him in Hospice where for the first time he had careing and gentle care takers. He died a week later. I will never forgive myself for placing him at Aspire where he was tortured and neglected on what was left of his life by an uncaring and calloused staff. I will never forget the morning when I arrived in his room and he was stark naked on the edge of his bed trying to get up by himself since he could not get anybody to help him. The physical therapist who arrived shortly after was shocked at the condition she found him in. She also called for help for someone to come dress him but she couldn`t rouse anybody that worked at this facility either. Even though it was not her job she felt so bad for him that she helped dress him herself and got him back in bed. This facility should be closed!!! read more

If I could give zero stars I would. My father was admitted to rehab a fractured hip. My father would be curled up in blankets sitting on the edge of the bed with no clothes on and a soiled diaper yelling for help to get to the bathroom. When searching for a nurse we were told good luck by one of the "student" aids. Once we found someone with a supply cart after thirty minutes of waiting we were given an attitude for asking for clean diapers. They don't witness the patients taking their medicine. Good luck finding anyone who speaks English or aren't glued to their cell phones. The nursing staff said my father was being combative which they felt was a reason to abandon him while there. Needless to say we pulled him out of this shithole and is back in the hospital. I don't know how a place like this can operate. Please, if you have to place a loved one in a rehab, stay away from this place. read more

I would do 0 stars if that was available. Our father went to Aspire because he had an infection that required a port, and the memory care unit wouldn't allow him to come in with a port. When my sister went on the weekends to visit it was nearly impossible to find someone that spoke English to address my father's lost clothes or abrasions from falls. The place is under-staffed, and my father was filthy most of the time. My father was medicated to calm him down after we told them that those medications make my father nutty and restless. I wouldn't recommend countryside to anyone ever. It was overwhelmingly evident that Countryside is purely a profit driven business with patient needs at the bottom of the list. My father lost his will to live at Countryside. read more

Can't think of too many things good about this place. My sister is there right now. Says whenever she requests something it literally takes HOURS before she gets any type of response. Needed ice... took about 4 hours. Same with water. She is suppose to have assistance to go to the bathroom but can never get anyone to help her. It's been like this since she got there!!!! read more

DO NOT bring your love ones/ family here! Worst facility in the area. Care givers are rude and treat the patients very bad. No respect what so ever. Definitely not welcoming for a visitor or patient. Worst care I've ever been around. read more
