The memory care unit has the most vulnerable residents and I have seen animals treated better. I have a full-time job, but this quickly became my second and not as an employee. I wish I could have had the opportunity to move my mom sooner. There are better places out there. Like countless others, It was one of the hardest decisions I had to make for my mom, placing her care and giving blind trust to strangers, but boy did they sell me in the beginning. I quickly learned how poorly it was managed, starting at the TOP, a constant revolving door of staff, never knowing who is in charge. The caregivers that cared were left without the proper tools or assistance to do their job. Overworked, underappreciated and ultimately many left throughout this time. The others seemed to be there just for a paycheck, more interested in their smart phones than the residents. A certificate of any sort is not required to work in a CBRF facility. I could list all the terrible quality of care issues and occurrences, but this review would be even lengthier so here are just a few. When I couldn't be there to do ALL of the care for her, she was left without being provided basic daily needs. My biggest issue was her medication not given when scheduled or sometimes at all, leaving her in a state of fear and agitation. She would be left in the same clothes multiple days in a row, not being provided a weekly shower which resulted in a horrible peri-area rash. It was only treated daily if I was there to apply the ointment. Meals not provided on time and often served cold. (I regularly helped serving) Her room was not cleaned, bedding changed, or laundry washed. I was constantly informing management, filing grievances as to what was happening and the horrific things I witnessed, including documentation and pictures. There was never any accountability, just excuses and finger pointing. Be patient I was told, give the new director of nursing and charge nurse time to get acclimated and implement the procedures. Instead of that happening, there was just a revolving door of both, each trying to reinvent the wheel without follow through. It got to the point where I didn't know where else to turn so, I contacted the state. That helped for a hot minute and then it was back to business as usual. Their mission statement "We strive to bring dignity, vibrance, and love to the lives of older individuals and the families we serve throughout the Midwest." My mom was not treated with dignity, I don't know how they sleep at night or are even able to continue to operate. They should be ashamed of themselves.

    My mom needed post-fall rehab. Whether in the Linden Grove Waukesha short-term rehab or in their…read moreLodges, the care by aides was terrible. Mom was not able to walk unassisted; aides would leave her laying in bed for hours. Continuously short-staffed, the facility routinely relies on contract help. Once upon requesting help to use the rest room, a staff person helped her sit up / perch on the side of her bed w/ feet dangling, then got called away; Mom sat like that from 9 am-3 pm. More examples... Mom expressed urgency to use the restroom; they told her she was too big to handle and that she should hold it or just go in her diaper and they would clean her up later. She had no other option than to go; the aides left her in her waste for 6 hours, starting a horrible skin infection from which she never recovered. Initially, she could use the restroom with help; however, it was routine for "busy" aides to let her sit on the toilet for an hour or more. Aides also did not help her re-position her clothes nor wheelchair for easy transition from the toilet, causing her anxiety. Mom heard staff talking about her in the hall and then - to her face - aides called her a moose. Aides would also place her phone and picker-upper stick so she couldn't reach them. Mom was initially tested for her glucose levels before each meal; once she was moved to the Lodges, they stopped checking her glucose because they said they had no standing orders - and administered insulin anyway, dropping an insulin-dependent diabetic's glucose to a level of just 60 on multiple occasions. They also said they didn't have staff with the credentials to take her glucose levels regularly. At least twice, Mom did not get her breakfast until noon and, many times, got cold meals. Mom had only 6 lower front teeth left and needed mechanically-assisted meals - she could not chew meat, for example. However, she got chicken and hamburger that she couldn't chew so she was slowly starving. She lost 25-30 lbs and became very weak within about 90 days. Mom was forced to leave short-term rehab because they said she didn't need daily PT/OT; they felt three times/week would be beneficial. But that was mysteriously dropped once she got to the Lodges. Mom said she never saw a doctor in the three months she was there. It is a mystery as to why there were no orders for glucose testing for an insulin-dependent diabetic nor for the OT/PT that staff said would be helpful and provided. Note: the directors of nursing seemed very capable, but the day-to-day care was unprofessional and disrespectful unless it was a contract staffer. We asked the directors of nursing to check in with Mom; however, Mom feared retribution from floor staff and did not share all the details of her negative experience with the aides. After sharing about one employee, the directors confirmed the employee was a known problem but felt they had to keep her due to being so short-staffed. The last few days of her stay, aides left Mom in bed 24/7. She was schedule for transport to an assisted living facility at 1:00 pm. That day, aides did not get Mom to the bathroom all morning. They threw her belongings in a clear plastic bag vs. using her suitcase, brought her lunch late (needed to accommodate the transport), and refused to pack her CPAP machine. They said they planned to leave her parked in her wheelchair in the lobby for two hours until the transport driver got there. During Mom's stay, she became depressed. Her mental state and memory declined; upon reaching the assisted living facility, she had tremendous distress resulting from Linden Grove. She lost the ability to mobilize a new wheelchair, had multiple significant skin infections caused by negligent personal care, and was physically debilitated. Altogether, it is clear to me that Mom's stay at Linden Grove was exceptionally detrimental to her physical, emotional, and mental health. In my opinion, the deterioration of her health as a direct result of Linden Grove's negligence ultimately precipitated her premature death. Bottom line: do not allow anyone you care about be placed in this facility.

We fought repeatedly for my mom's care. At every turn that involved Julie T. we were talked down to and our concerns were always minimized and our feelings were never acknowledged. We questioned communication between shifts which is very lacking. I would call frequently to check up on my mom after a fall, or after an allergic reaction or an illness. I would call during 2nd shift and the typical response was that 2nd shift didn't know anything about why I was calling. It was very disheartening to always see my mom in clothes that didn't belong to her. Because of my mom being in a shared medicaid room I had asked when she was moved into one what happens to privacy at the end of life. We were told OHT can provide "many things" to provide dignity and privacy for the family and my mom. We were offered a private room on a different floor but by the time that happened my mom was screaming in pain at the slightest movement. We could not do that to her. I got to sit with my dying mom as the roommate munched on food behind me. No privacy screen was provided. At one point I was crying and I had to sit in the hallway crying as people walked by because there was NO privacy to grieve. NO ONE deserves to die in a room with a stranger. We were grateful for the floor staff and all they did for my mom. I sure hope people will reconsider putting their loved here due to the management.

The interior door must be kept closed at all times, so your loved one's precious articles will be placed on the filthy floor while the door is being opened. The building access plan is inconvenient and frankly disgusting. I have no idea why they think this is okay.

While there were some positive aspects, significant concerns about staffing, communication, and quality of care ultimately led us to move our mother-in-law to another facility. On the positive side, the food was very good. Residents were offered choices and could eat in a restaurant-style setting, which she genuinely enjoyed. Additionally, some staff members were exceptionally kind and went above and beyond in their care. However, there were consistent issues that impacted her well-being. Planned activities were frequently canceled due to staffing shortages. Because she is not mobile and requires assistance to attend activities, she often depended on staff to inform and help her participate. Without regular family advocacy, she was not consistently included. About a month into her stay, she was moved from a shared dining table with other residents to sitting alone--this occurred on her birthday. Given that she was adjusting to a major life transition and grieving the recent loss of her husband, this change was especially difficult. During her stay she was not consistently being taken to meals and lost a lot of weight in a short amount of time. We also observed concerns with staff presence and responsiveness. At times, staff appeared disengaged, sitting on phones or eating in common areas, making it unclear whether they were on break. Staffing levels seemed particularly limited during evenings and weekends. More concerning were issues related to personal care. During her stay, she lost 10 pounds and experienced multiple UTIs. There were several occasions when my husband arrived in the morning to find her still in the previous day's clothes, in wet incontinence garments, and with soiled bedding. While staff sometimes stated she had "refused" care, we were not confident that consistent or sufficient efforts were made. The call button system was also problematic. It required residents to hold the button down for an extended period before activation, and response times were often delayed. Some staff members were unfamiliar with how to properly reset the system so my husband had to show them how to use it. Although we requested the simple old style call button, we were told none were available, despite observing newer residents being given the old style buttons. We also encountered issues with missing personal belongings. A gold chain was replaced with a non-gold one, and a new TV remote went missing. Additionally, our mother-in-law reported that a staff member asked her to write them a personal check, which we immediately reported. Another resident nearby also reported missing cash. As her condition declined due to UTIs, she required the use of a Sara Stedy transfer device. We had to purchase this equipment ourselves and train staff on how to use it, as they had not received prior training. Eventually, we were advised to move her to memory care for more support, but instead we chose to relocate her to another facility. Since moving, her condition has improved significantly. She has regained strength, is walking short distances, maintaining her weight, bathing regularly, and is consistently dressed and cared for. While there were some compassionate individuals on staff at Brookdale, our overall experience did not meet expectations for the level of care being provided, especially since we were paying for the highest level of care. In speaking with long-term residents, we also heard concerns that the quality of care has declined over time.

I was impressed with the intimacy of the facility because they only has 24 residents and they specialize in Dementia Care. Their staff is caring, attentive and they get to know each persons individual needs. They know my voice when I call and they are respectful to both his needs and mine as his caregiver - Are they perfect NO - Do they make every effort to take care of any situation and communicate - YES - If I had to do it all over again I would still pick Brookdale - Their Staff is the BEST

My mother in law…read morewas there from the summer of 2025 and left before Christmas 2025 because she was paying the highest level of care but not receiving care. My husband would visit daily. She was not getting her weekly showers and received frequent UTI's. She was not changed when wet and her sheets were soiled. She wore a gold cross necklace and her gold chain was replaced with a cheap chain. She lost 10 pounds in 3 months not always taken to the dining room. There were activities planned but they did not always happen because there was no staff available. PT's were not consistent and she was not making progress with walking. It was not uncommon to see employees sitting on their phones throughout the building. Not sure if they were on their breaks or not. Several residents said that things were better last year. We left Brookdale in Brookfield off Bluemound Road and took my mother in law to a different facility. At the new facility, she has always been dressed with her earrings on and eats in the dining room. At the new facility she is getting her baths and is able to walk the length of the PT room now. She eats in the dining room and participated in social activities daily.

Judging by just one visit, it seemed a little disorganized, but they are pretty new. Nice friendly group of residents, the kitchen dining area is very pleasant. The grounds still need landscaping (the expansion is very new), and the parking area is just barely adequate in size.
