Staff is exceptional from the welcome/release staff , to the physical therapists, to those providing other services like meals, meds, other needs. The key is the top level is very serious that the patients have the most positive experience so that the patient can focus totally on recovery. Amazing Staff and the facility is nice also.

    My 76-year-old father was a resident at Ignite Medical Resort-Round Rock for 10 days before he…read moresuddenly and unexpectedly died overnight while in their care. I will not address the overall quality of care he received during his stay in this review. What I will address is what happened afterward -- or more accurately, what did not happen. At 1:30 a.m. on the morning of his death, I received a phone call from a constable, not from Ignite staff. The constable informed me that there had been a "pretty serious medical emergency" and that my father had been found unresponsive sometime between midnight and 12:30 a.m. He stated that staff had called 911 and that paramedics pronounced my father deceased around 1:00 a.m. I was in shock and did not know what questions to ask at that moment. It has now been three weeks since my father's death, and I have not heard from a single Ignite employee -- not a nurse, not a manager, not a care coordinator, not anyone. No one has contacted me to explain what happened. No one has offered condolences. No one has acknowledged that my father died while in their care. To be clear, I am not alleging that Ignite caused my father's death. However, the complete absence of communication is deeply disturbing and unacceptable. The expectation was recovery -- not death in the middle of the night with no explanation afterward. Why did no unit manager, social worker, or director of nursing contact me as the primary contact? Why was there no call explaining the sequence of events? A simple, humane conversation -- "The nurse entered his room to administer his midnight medication, found him unresponsive, we immediately called 911 and initiated life-saving measures, and we are so sorry for your loss" -- never happened. Instead, I am left to imagine what MAY have occurred. For a family who entrusted this facility with their loved one's care, that silence is unfathomable.

During that time my grandson was thriving in physical therapy. I recall his therapists Jessica and the other Kristen again he was thriving. During the first few week of Jan 2024, Kobe had been neglected By not being fed, Showered or being changed. The first incident of him not being fed, I asked the nurse and his technician, their response was they thought the night shift fed him. I made a report with the front desk who at the time was Miss Betty. She went on to tell me that this type of behavior was not tolerated. She asked me to fill out an incident report which I did, It was never addressed. Upon my grandson telling me he was being mistreated, I decided to stay Monday through Thursday his case manager Stephanie Carpenter knew of all the things my grandson was going through because I let it be known not only to Mrs. Carpenter but to the nurse manager Chris. Chris assured me that everything would be OK Well it was not Again I stayed Monday through Thursday because no technician was going into the room to check on my grandson nor to change him. I literally would sit back and wait to see how long it would take for someone to come rarely did anyone. I did I talked to the nurse manager Chris and I asked ,"are your technicians under the impression that because I'm in the room they don't have to come in and tend to him?" She responded, "Did you talk to them and ask them why? First of all it is not my job to tell your technicians what they're supposed to do My grandson had lost weight so mid January of 2024 they administered a picc line to feed him on his left arm. not knowing at the time they did not use sterile procedure .Fast forward he went from thriving to not being able to move, talk, open his mouth and unable to do any type of therapy w/Jessica Kristen and his speech therapist. made reference to they have no idea what happened to him as he was thriving His doctor did nothing to figure out what was going on other then to titrate his medications she either went up on milligrams or went down on milligrams but never did any blood work to figure out what was going on Weeks would go on and declined one particular night a tech came in James and he cussed out my grandson not caring that I was in the room we had words and I told Priscilla who was the night charge nurse he was not to come back to his room When I told Miss Carpenter she asked me to fill out an incident report and verbatim say what happened between James and i did on February 16th due to his decline and lack of care his nurse and his technician at that night Roxanne I Requested my grandson be discharged I had walked in on the nurse force feeding my grandson when I asked what she was doing she said I have to give him his meds and he wanted chocolate pudding I said to her how in the hell does he want chocolate pudding when he can't even eat she said he requested chocolate pudding my grandson only likes vanilla I asked them both to leave the room When James cussed us out and I did the incident report nothing was done Nurse manager Chris told me she could not get rid of him because she had to investigate that was never done upon discharging my grandson February 16th 2024 he would end up at the hospital in San Antonio at university after running test it's documented that he Had contracted gram positive and was sepsis The doctor went on to not only verbalize but to put in the report He got the infection via the PICC line administered at Texas Neural Rehabilitation Center I later called Mrs. Carpenter about his results of gram positive and corona virus She said no one at the facility was infected. lack of sterile procedure of his picc line led to sepsis due to the negligence of the nurses who managed his PICC line My grandson was on life support again and would be in the hospital for a month. After learning about the sepsis due to the PICC line I wrote a letter to the Health and Human Services Department to no avail They cited Texas Neural was doing everything they were supposed to do which is funny because they go during the day when they're fully staffed and not at night to see how technicians sit in rooms, empty rooms and watch TV call buttons are going off but they're watching TV or they're on their phones they're on social media I wrote a review back in August 25th of 2024 I'm praying that this review gets to the CEO Ed prettyman, Well I've seen many wonderful reviews It's unfortunate that we had a terrible experience what initially turned out to be great because of Jessica, Kirsten and his speech therapists Too many people knew what was going on including Stephanie Carpenter who Knew my grandson had been there two weeks and witnessed the neglect, but too afraid to advocate for her patients. SHAME ON YOU, JAMES, ROXANNE, CHRIS, ANDREW, ANNA, AND ANY OF THE NURSES WHO WERE PART OF THIS

    12/30/2021- My father was transferred here after contracting covid 19 to quarantine. He arrived on…read morea Thursday afternoon. It wasn't until 18 hours later that I was able to get in touch with him. The staff never gave him his personal cell phone, nor did they answer their phones at the facility. Trying to get in touch with anyone there was impossible after MANY MANY attempts. When I did talk to someone I was told "everyone was off due to the holiday". It wasn't until receiving a text the following Monday 1/3/2022 asking to sign admission paperwork for the transfer, I did actually speak to someone for longer than 1 minute. At that time I asked for a update and strongly voice my frustrations. My reply in text was enough to actually receive a phone call from someone. At that time is where I voiced my concerns about the lack of administration, communication, and it being impossible to reach anyone there. Heaven forbid there be an emergency on my end, or on his. I'm almost certain I wouldn't be able to get through, or even receive a call if something happened to him. I will say after voicing my concerns things got slightly better. But that was still after multiple attempts. Today my dad returned back to his initial facility. Upon his arrival the nurses, and wound care did a full evaluation, and documentation on my dad. He has bruising all over both arms (possibly due to blood work, yet he has a picc line), a fungal infection under his arm pits, and groin area, they noticed what looked to be bed sores on his back, bottom, and heels of his feet from not transitioning him, or offloading his weight during his stay. He literally was in bed and the same position for 10 days straight from the sounds of the report I got this evening 1/10/2022. The wound care he was receiving from an amputation was larger upon his return, than it was when he left only ten days earlier. Due to being a covid patient, I was unable to visit, and relied on him answering his phone to get any sort of updates. If your loved one doesn't have a cell phone, do NOT plan on any communication! Make sure it's plugged in and kept charging. AT ALL TIMES! PLEASE if you send a family member here, make sure to be there as much as you possibly can be!!! I'm sure if something did happen you'd never hear about it from anyone. From what I've experienced these last 10 days I wouldn't recommend having anyone stay at Legend Oaks North Austin. Maybe things would have been different if I was able to be there daily. But given him being covid positive no one was able to enter the 300 hall staff included, nor send me to the nurses station phone "because they don't have one in that hall". Keep researching!!!
