My experience with Bailey Springs Nursing Center while my mother was a resident was extremely…read moreconcerning and, at times, alarming. Within just a few days of admission, I was informed that my mother was not eating. When I asked what alternatives or interventions had been attempted, I was told none. Despite this, she continued to be given medications that are intended to be taken with food, which led to repeated vomiting of whole pills and worsening nausea rather than addressing the root issue. I was also told directly by staff that residents are only given bed baths a few times per week. During my mother's stay, she was not taken to a shower at all. On one visit, she had a strong odor and staff rushed to clean her only after realizing I was present, which raised serious concerns about her ongoing hygiene care. Care coordination was also unreliable. I communicated well in advance about a scheduled neurology appointment and the need for transportation. This was not properly arranged, despite multiple follow-ups, and incorrect information about her insurance was given. More alarming were the changes in my mother's medical condition. At one point, she appeared heavily sedated without clear explanation, with noticeably dilated pupils and altered responsiveness. This was especially concerning given her condition and care needs. She had also been complaining of abdominal and pelvic pain for days without appropriate diagnostic follow-up. When I ultimately took her out for her appointment, her condition was so concerning that she had to be admitted to the hospital immediately. She was diagnosed with a severe UTI involving E. coli and had lost six pounds in approximately 10 days--something that speaks to a lack of adequate nutrition and monitoring. There were also clear safety issues. My mother requires a protective helmet due to a prior craniotomy, yet she was placed in a chair without it and left unattended. I have documentation and records supporting these concerns. Based on this experience, I strongly urge anyone considering this facility to be extremely vigilant, ask detailed questions, and closely monitor their loved one's care.

This was the worst first experience I could've possibly had for someone seeking treatment. I…read morecontacted Monte Nido last Wednesday after my ED got worse. The young lady I spoke to asked me 3 questions max, told me my insurance would cover everything, and then scheduled me with the intake counselor, Christine Regalado, spoke to me last Friday, and conducted my intake discussion. Originally, we were considering the PHO, but she was mentioning residential a bit, so I asked, if that was the recommendation, when would I be able to start. She reviewed her scheduling and told me "looks like there's availability next week, as long as you get the paperwork and necessary pre-reqs done as soon as possible." As I was still technically on track for the PHO until the recommendation was finalized, she said initially all I needed to send was my report from an Urgent Care visit earlier that week. I did it less than 10 min after our call. Monday comes, the recommendation becomes officially residential, I ask if there can be a few accommodations made, but I said in that email, that I accepted the residential. I followed up the next day, and she sent back a slightly snappy response basically saying they couldn't accommodate anything, and that she was trying to see if she could find me a single bed. I told her it wasn't necessary, as my symptoms were rapidly declining. I sent this in the email where I stated, in no uncertain terms, that I was committed and interested in the residential treatment. There was no room for miscommunication (this is important for later). On Wednesday, I had a substantially worse episode and I had to go to the ER. Before I did, I called the Monte NIdo line to see if they could help me understand what was happening as the symptoms heavily indicated the beginnings of refeeding syndrome. The reception lady (who was very sweet) said that Christine was in a meeting and she would be done in 10-15 minutes and she would call me as soon as she was out. I told her that was fine, and I waited for her to call to decide whether or not to go to the ER. After 30 minutes of no call and my symptoms worsening, we made the executive decision to go. Christine never once called me back. My situation was not a concern to her. It was apparent. Because of this, my husband was growing angry and asked for her number. I gave it to him. When he spoke to Christine, she claimed that 1. She never said that there would be avilability this week. 2. She said she wasn't under the impression I wanted residential. 3. I was delaying things by not filling out the form she sent- that she never actually sent. She attempted to pit my husband against me to hide her incompetence. When he started to lecture me on being dishonest, I lost my mind. I had emails backing up my comments. I also told him that I remembered the exact section of the call where she told me, and that the call was allegedly recorded (who knows if it actually was saved). I sent an email to her calling her out for deceitful and manipulative behavior. I politely referred to them as "miscommunications" just to keep the peace. There was no miscommunication. There was outright lies, and not just to me, to my husband to turn him against me. She responded with a half-assed apology that claimed that everything was miscommunicated and that things change all the time and blah blah blah. I am incredibly understanding, as long as you are honest with me. Never once did she say "oh, I'm trying to get you a spot, but it looks like it's going to be delayed" I would've told her that's fine and thank you for looking. But because of her decision, I called a new center before I had even hit send on the first email. DO NOT EVEN CONSIDER GOING HERE! Christine Regalado should be terminated immediately, and I don't say that lightly.

Update responsive to Mr. Martin's recent review: As many…read morepeople do when you can't fall asleep at night, we play on the Internet. I noticed this recent review from someone named Mike Martin. I think if anyone is misrepresenting the facts, it is not David Mesnick. I attended physical therapy with David for almost 2 years. I was told when I made the first appointment that they don't take insurance; I was also told again face-to-face when I was checking out on day one, plus I have seen it on their website. However, for a flat fee of $50 per year, they will help you with the paperwork if you want to file a claim against your policy, but they do not take insurance and they expect payment immediately after their service is rendered. Everyone who goes to PT 360 knows that, except Mr. Martin. What I don't understand, and what makes absolutely no sense, is that when you complete your session, you stop by the front desk to pay. I assume Mr. Martin paid on his way out for each of his five sessions. At a minimum, I assume he knew after the very first session that 1) they don't take insurance and 2) they charge an annual fee of $50 to help you process the paperwork if you want to make a claim against your policy. So why did he, therefore, continue with four additional sessions??? I'm writing this review because I hate to see a good quality business like PT 360 slandered by someone's "misunderstanding," intentional or otherwise. I fail to see why Mr. Martin can't simply own up to the fact that perhaps he misunderstood instead of trying to interfere with a small local business by telling reviewers a bunch of BS and saying, don't go to this business - that's a terribly vicious way to handle what is obviously a misunderstanding on his part and/or a possible problem with insurance forms. David and the physical therapists at PT 360 are exceptional, professional, and ALWAYS gracious (never ever rude). They provide a much more hands-on approach to each patient versus the big orthopedic groups - that's why people continue to go to them even though they don't take insurance. I've used David and Sam, and my mother used Amanda. I can't say enough good things about all of them, as do all of the many unanimous 5 Star reviews on Google. Looking at Mr. Martin's other Yelp review, he enjoys critiquing the businesses he visits and even went so far as to accuse the other business of being "criminal." I'm not sure what his personal issues are, but I recommend that you go to PT 360 and see for yourself. You'll find David to be a soft-spoken gentleman, not the person depicted by Mr. Martin. I'm confident your experience will differ greatly from the one portrayed by Mr. Martin. Even my 90-year-old mother understood their payment policy, from day one and was extremely pleased with the promptness of their submission of insurance forms.....

Please read this entire review to understand why I'm giving this practice one star. Let me start on…read morea positive note and say that Sam Kridl is a wonderful physical therapist. He is knowledgeable, friendly, and very effective. Now, on to the well-deserved negative portion of my review. My physician referred me to PT360, and when I called, I specifically asked, "Are you in-network with my insurance provider, Aetna?" And the staff member told me, "Yes". I went to my first session with Sam, and afterward was told, "It's our policy to charge customers $50 to file your insurance claims." I was caught a bit off guard and said, "Wait a minute, I have to pay you to file claims, for you to get paid?" I've never heard such a thing. At this point, I asked a second time, "Are you in-network with my insurance provider, Aetna?" and was once again told, "Yes!" I did five sessions, asking each time if the previous claim had been filed, and I was assured it was. Well, they weren't. And not only that, it turns out that PT360 does NOT accept insurance. I had met my out-of-pocket for the year, so instead of my sessions being fully covered, I owed 100% of the cost of those sessions. While their mishandling of the insurance was costly and certainly disappointing, that's not why I have written this critical review. I reached out and respectfully shared details about my experience and received the most unbelievably rude and disrespectful reply from PT360's owner, David Mesnick. He told me I had "...misheard or forgot" what their policy was. Furthermore, he said (and this is a direct quote), "We are unapologetic about how we run our business." There are so many great physical therapists in Atlanta. Why would any customer do business with a company that misleads customers and then has the nerve to treat them this way? If you do choose PT360, just know this is concierge therapy, and contrary to what they may tell you, they DO NOT accept your insurance, and you will pay 100% out of pocket. Do yourself a favor and look elsewhere. If you want further proof of this owner's unprofessionalism, just read his reply, "This man is on Yelp with bald-faced lies and clearly doesn't value the good care he received." As I said in my review, I truly value Sam's care; it's the deceptive business practices I take exception to. Thanks for proving my point, David.

I was sent here on March 11, 2014 by my husband. I was so angry with him. I hated having someone with me at all times. When I came back home 4-11-14 I wanted a divorce I was so angry. We had over a year of counseling with our pastor and today we're good. I thank him for sending me now for I know he did what he felt was right. Thanks that I have accepted Jesus as my Lord and Savior. I know everything happens for a reason. My suggestion if you have a loved one who sustained a TBI first have them put on an anti depression medicine that will help them. Fortunately I never had to have my head cut open for it stopped bleeding the next morning but most people at Restore have and they will help them get back to the new normal. God Bless all the great people who put up with me for I was so full of anger I wasn't very nice. Our adult daughter has been a resident at Restore Neurobehavioral Center in Roswell for more than…read morethree years. Traumatic brain injury is difficult to explain until you have lived it. For our family, the excellent program and caring staff at Restore have been a godsend. We have found both management and staff to be kind, responsive, and knowledgeable and with very little staff turnover. Our daughter has profound short-term memory loss but every single day she tells me again how much she enjoys Restore. We have eaten meals there with her many, many times and the food is very good and nutritious with several options for every meal. I don't believe I have ever seen them serve fried food and our daughter tells me every day what her evening meal was and how much she likes it. She goes on outings regularly, which she enjoys very much. The medical staff calls me for every incident of any kind that involves our daughter. The therapy staff members are extremely competent and able to discuss our daughter with me (I'm her guardian) in great detail. They KNOW her, respect her, and treat her like an adult. They accommodate family visits by driving her one-way more than 50 miles every other week (we return her in the afternoon). Restore has changed our daughter's life for the better and by doing so has given us great comfort that she is in good hands. We highly recommend it.

This review is of the Woodruff Family Housing at The Shepard Center in Atlanta, GA, for their…read moreoutpatient patient and patient families. This is mainly for informational purposes. We are staying here while my husband rehabilitates at Pathways. Before coming here, I searched everywhere for pictures of the housing so I would know how to prepare. The building is older but serves its purpose well. You'll need to bring cleaning products including a vacuum, mop, bucket, etc. This particular unit had a few of these items but we had to move rooms and the next one did not; be prepared for them to not be available. The kitchen is stocked pretty well although, ours needed measuring cups, spoons, mixing bowls, mixer, and kitchen towels. The refrigerator is full sized but, on the smaller side. There is an ice-maker. There is a standard coffee maker, no k-cups, only ground. There is a microwave and our kitchen happened to have a crockpot, although the first room did not. The hot water takes forever to heat up so run the water for about 15 minutes before stepping inside to shower. They will gladly provide you with any medical needs you may have such as a shower chair, lifted toilet, etc. The towels and washcloths are a step below a hotel and SMALL. My husband found them too rough for his sensitivities. Myself, I thought they did a great job exfoliating, so just your preference. We ending up buying some and made our stay more comfortable. Bedrooms have two twin beds OR one twin bed and one hospital bed. Please confirm with your case worker what your needs are!! Bring blankets and mattress toppers for your comfort. The beds aren't the most comfortable but will suffice; they remind me of dorm room beds. We brought a king sized tri-fold topper, pushed the two twin beds together and made a king sized bed. Obviously this required us to bring our own bedding but completely worth it to us. Hey, if we are going to be here a while, let's make it comfortable!! Keep in mind they can't take donated mattresses when you're done so either meet a neighbor who would appreciate it or be prepared to leave with it once it has expanded. Trash bins are on the ground floor through the parking lot. At night it's dark. The first time I chose to bring mine back in since it was a good walk in the dark and there was a running, white van parked nearby. Hey, I've seen what happens in the movies!! Lots of renovating/construction going on but they are doing an adequate job of rerouting you with very little inconvenience. Security is very friendly and helpful. There is a secured parking garage. You have to have a key card to access the garage, your floor, and your room. There's plenty of handicap parking right by the door of each floor. There is a mailroom. If you're ordering Amazon, I recommend the Amazon Lockers located about a mile or less down the street. There is a delay from the mailroom (I am on day 4 of packages I know have been delivered) and they are not there but a VERY limited time on weekends. They do bring your mail to your room and if you are out, they open the door and place it and any packages just inside on the kitchen table. Very convenient and takes any worry away of your packages sitting out. Be patient .. I was told many are volunteers. Internet is okay at best. The TVs are smart TVs so your apps are available. There is a TV in the living room and bedroom. There is a laundry room on each floor. Keep in mind you can only access your floor. There are 2 washers and 2 dryers. They actually provide the detergent. It's an off brand powder and obviously you can bring your own, as we did. I have not seen any fabric softener so you'll need to bring your own either way. There is a floor downstairs that has couches, games, movies, etc.. Should you need the Shepherd Center or want to utilize their cafeteria, there is a walkway located in the garage area. You are only outside of the building/inside the garage for a very brief walk. The Shepherd Center also sponsors a Family Dinner every Tuesday night at 5:00. The food is furnished by local restaurants and local people that generously donate. It's FREE! Afterwards, they play several rounds of bingo with generous prizes. Mrs. Shepherd, herself, attends almost every meal. She is the very elegant lady on her motorized cart assuring everyone's needs are tended to. Should you need Piedmont Hospital during your stay, if capable, I recommend walking there. It's less than an 8 minute walk. Piedmont makes you pay for parking every single time you leave the hospital. I think the first 30 minutes are free but, then it's $7-$10 according to your stay. I walked every time of day, from very early morning, midday, and late a night and felt very safe every time. I hope that you find this information helpful for your stay! The majority of the staff takes no pride in their work, they don't read patient's chart, they don't communicate properly and practically put the patient's wellbeing at risk. The 60 min therapy sessions are basically 15 minutes of work and 45 minutes of chit chat and set up. Waste of money and resources. I definitely wouldn't recommend this place.
