Impressed. We all know the stereotypical negative reputations that go along with care at Skilled Nursing Facilities, but this place actually cares! My husband was transferred here from a rehab facility to receive rehabilitation for a below knee amputation. He was at deaths door, practically, when he was admitted. Incredibly weak, out of it, at times lethargic from the care he had just escaped from. At American River Center, it's lively, clean, the staff is exceptional, and above all the physical therapy has been the best my husband has received the whole year of this newly experienced amputation ordeal. Who ever oversees the management of this facility, my hats off to you! It smells GOOD in here, they take care with organizing the menu, they provide weekly newsletters with the menu to their patients, and you can make requests to the menu. They allow family or visitors to come and go. The staff is very friendly and very attentive. The facility is always being maintained whether it's inside or out. I really just don't have enough nice things to say about it here. By the time my husband was discharged he was able to walk with assistance of a rolling walker. Compared to when he arrived needing 3 people to help him out of the car, there was literally no need for help upon discharge. Physical therapy helped him reach his goals. I am so grateful as his caregiver! Above all, I felt comfortable knowing he was in good hands and never worried about his cared. This is saying alot for SNF's!!! It's certainly better than the three others my husband was unfortunate enough to have to endure in the past six years, but there were problems with a bed that injured him needlessly, and the food on his diet was very lacking. There is a clear disconnect between the kitchen and the other staff at the moment but the rehab. staff is mostly very good and as they don't have long term care they do seem make a genuine effort in that area at least. I'd recommend using it if the need arises, and keeping a close eye on it while you do. read more

My father was cared for here for a month following a fall where he fractured several ribs. He received excellent care at American River. My mother and I are both grateful for the diligent, caring, friendly and respectful staff. He received regular physical therapy that has helped him immensely. The facility is clean and does not smell. (Unfortunately, I can't say the same for other skilled nursing centers we visited). read more

NEGATIVE 5 STARS This is an update to my review a couple months ago. In the 3+ months my sister was in their care, her health and ability to care for herself deteriorated. She ended up hospitalized, after several trips to ER via ambulance for breathing issues, with MRSA and pneumonia. She passed away yesterday. I'm not completely blaming this facility for her death, she had several health conditions that fell like dominoes in her last months. However, in the midst of her decline they had plans to release her to live on her own. They even expected me to pick her up in my personal vehicle when she couldn't stand and walk. This all seems to be motivated by greed, Medicare was due to stop paying. I don't know how anyone with any medical training and especially those who took the oath could consider releasing someone in that condition by themselves. Fast forward to yesterday. I leave the hospital after paying my final respects to my sister and head to American River Center to pick up her belongings. I never received a call that her bed was given to someone else or that I should come get her things. When I get there, they don't have them or can't access them. There's a $400 wheelchair and several personal items missing. They were supposed to call me, they didn't, so I called them. Left another message. Still nothing. PLEASE BEWARE OF THIS FACILITY! The shadiness of their business practices continue after my sister has passed. read more

My brother was cared for here for over a year prior to his passing; he had multiple serious health issues. The staff was attentive and patient, as he was not always (usually!) an easy person to deal with. I appreciate that the staff kept me informed of his condition as I was only able to visit on weekends. The staff I encountered were hard working and cared for the residents with respect. One star is generous. Do not go near this place! It breaks my heart we sent our mother here. Shower every 5 days. She sat in a soiled diaper for hours. Food inedible. They waited 5 days before evaluated her for rehab. Started on the 6th day. She just laid in bed getting weaker and weaker. Not good for a 95 year old woman with a surgically repaired femur. I can go on and on. The worse... once in we couldn't get her out with Medicare. Avoid this place at all costs read more

Just got off the phone with the social worker here regarding her "recommendation" that my mom transfer to hospice care or get a feeding tube. We were supposed to speak with a doctor yesterday which did not happen. I called to follow up (as did my dad) and was told the social worker was not in. No one else would take our call. After speaking with the social worker today (who still didn't call back, I had to call her) I was advised the doctor is too busy to speak with us. I requested my mom's medical records and was told there's a fee. I advised that since I am unable to speak with the doctor, I should not be charged to view my mom's medical records since we're now being left in the dark regarding her health. This is just a small excerpt of how bad of service they provide. Horrible place, not even worth a star. I need to get my mom out of there! read more

I guess its a love or hate relationship. My dad looked alive after coming here from the hospital, i was worried until i saw him and met some of the staff. All his nurses, esp cna tiffany, freaking awesome. They love my father and treat him better than family. There's scheduled showers, wow, fantastic. There's a big living room for guests and patients, the place could use a facelift, but that can't take away nor replace the treatment. Ty guys and to Dr Yusufzai. read more

My mom has dementia. She had surgery on her left foot to restore blood flow & went to this facility to recover. While the nurses there were friendly, the other staff was not. I could never get answers about her foot care, PT, etc. I was told that she wasn't responding to PT. She was drowsy or asleep every time I visited. I found out 2 months later that they were giving my mom an antidepressant without telling me that has side effects of drowsiness and sleepiness. Further, there was no PT given, so my mom's ankle fused in the extended position. In this state she would never be able to walk again. She also had gangrene in her heel. There was a large hole with no skin on the side of her foot. They told me they thought she had an infection in her foot and scheduled a podiatrist appointment. I offered to meet at the facility to ride over to the appointment with my mom. They told me they would send a nurses assistant with my mom, so to just meet at the podiatrist's office. When I arrived, I found my mother there with no one. They just put her in the van and dropped her off alone. Then when the podiatrist surgeon told me he would have to cut her achilles tendon in order to bend her foot (since no PT was ever done), and that I had to go right away to the hospital due to the level of the gangrene. While at the hospital checking my mother in (less than 2 hours later), I received a call from American River telling me that my they would not be accepting my mom back there and I needed to find another care facility for her. Horrible!! Don't trust them! read more

My Mom was placed here after she fell and broke her hip. She had dementia. When I first called I explained in detail her situation as some of the rehab centers wouldn't take her as she was a memory care patient. They said no problem. They assessed her and accepted her. From there it was down hill. First time we went to visit she was soaking wet. She had spilled her full glass of water on her. They left her like that for over an hour. We hit the nurse button and nothing. We had to go get someone and even then I had to get angry to get anyone's attention. We went to see her a couple days later and she was sitting in her wheel chair. Had been there for hours. She was almost unconscious. They had her so drugged up she couldn't raise her head. They said she was being too loud so they had to sedate her. Her room mate said they hadn't eaten and it was an hour after dinner and no food was anywhere to be seen. Next time we went to see her there was a empty syringe on her eating tray where she could reach it. She was wet and unchanged. She was so skinny and they said they couldn't get her to eat. They didn't contact us to tell us that, we had to ask. We hand fed her and she ate. They basically kept her unconscious with drugs, never cleaned or changed her, never fed her, and were unresponsive. If we didn't have someone there with her almost every day, who knows what would have happened. They just set her aside to die. We finally moved her out, but unfortunately it was too late and she was not able to recover. If you love your parents DO NOT send them here. read more

Horrible place They don't clean the patients well, won't encourage them to get up, they don't follow up on medical issues and it is so noisy and disturbing. They basically just leave your loved ones to get worse and die. read more

OUTSTANDING is all I can say , from the administrator Brandon Stroughter who help secure a rehab bed for my 92 year old dad who had been hospitalize for 10 days and needed his strength and walking abilities again to the case management team that helped me thru processes allowing me to keep my dad in this A+++ facility. The nursing staff, Olivia especially , CNA's Baja all were magnificent with their care. Having visited 10 nursing facilities prior to admitting my Dad here, this by far is the cream of the crop for rehab facilities in the Sacramento area. Trust me, the cleanliness and the overall people skills at ARC have been top notched. Not once did I feel my dad was neglected, the staff worked as a team and I commend them for all the hard work they do to let your loved one feel important. The PT team has done a superb job of getting my dad back to where he was prior to being admitted to the hospital. The food service staff met my dads food needs and were helpful too. I don't write too many reviews as life is pretty busy but this one is that important. My mom has congestive heart failure and had an episode at the beginning of the year. From Mercy San Juan, she needed to pick a rehabilitation hospital that would help her gain strength so she could go home again. We ended up at Genesis American River. We had no idea what to expect. She was there for around a month. A month is a long time to get to know the place. I cannot say enough about how well they took care of my mom. Her nurses and her cried the day she left. That is a testimony to the caring and compassionate staff that work at Genesis. They were like family to her. Needless to say, the physical therapy staff were amazing also. Because of this facility, my mom was able to come home again. Thank you to you all from the bottom of my heart. read more

the cna's here need to be reminded that they are not gods.. they are assistants, and the patients are not well, do not expect the patient to inconvenience themselves just to make the cna's job easier. if i could tell my insurance to pay them less, i would. i was not here by choice, the nurses are, and they get paid for it. i dislike writing this review because half the staff id good and earned a good review, but the few not so good ones are so bad they bring the entire facility down. i have asked to speak to an admin, and was told one will speak to me, as soon as THEY have time and feel like talking to me. this was two weeks ago, and nothing since. the cna's bring food and just shove my laptop and phone out of their way, without regard for damage done or data lost. making matters worse, once they bring food, they try to remove it every 10 seconds even if you make it clear you are not done eating yet. they complain it is too much work to wait longer because if they do it makes them have to walk FIVE ADDITIONAL FRIGGEN STEPS TO THE CLEANUP CART. FAST FOOD employees get paid LESS and provide BETTER service and consideration. next time i get sick, i am going to McDonalds! if nothing else, i know if i ask to speak to mgmt/admin, they will make themselves avail quickly, not make it take days. my next step will be to find a state patient advocate/ombudsman, because all this has left me feeling abused, helpless and more unwell than i was before i was admitted here. read more

This facility is the worse in every category. Very few seem to care. The average employee is lazy; my friend has turned on call light and sometimes wait 40 mins. Asked for ice one day and none brought to him 30 mins. later. Started down hall to issue a 2nd request and CNA saw me coming and jumped up from sitting down and proceeded to get ice. Do they think people are ignorant? Meds take forever to get, even Tylenol. Many of the are CNA's rough (not gentle). We realize they can be assisting others but to the point of ignoring them completely. How often is everyone so busy? Then there is the food, unrecognizable. That should be a good enough description! There are a few good employees, don't get me wrong, but very few! Sorry to have to report such a bad review but actual physical care should have a separate IMPORTANT rating that counts more to being or not being related to star system. My husband was in the care center for 9 weeks. He was treated with respect and kindness. The nursing staff was efficient and courteous, mostly. There are always 1 or 2 who do not comply with the rules or who just don't get along with you. He was not fond of the food, it was cold when he received it. However, I was able to bring him anything he wanted. He got very good rehab and therapy. He is glad to be home now, and it is because of the caring people at American River Care Center. read more

THIS PLACE IS SO BAD - IT DOESN'T DESERVE ANY STARS! My husband was sent there for Rehab. Prior he had been at Mercy San Juan where he developed sepsis. While at Mercy, he had his knee aspirated because he could barely move it without excrutiating pain. We were told if the sepsis "seeded" in that knee (or any joint), it would require surgery to clean it out. As my husband stayed at this place, his knee got worse and worse, hotter and swollen to the point he could not do rehab because of the pain. No one could even bump against his bed without him experiencing lots of pain - even crying out in pain. On about day 7, I went to the Director of Nurses, Erin, and asked her to come look at his knee because I had no confidence the nurses there knew anything about sepsis in a joint even though I told every nurse on each shift exactly what Mercy had told us. It was October 31, Halloween. When I went to "Erin's" office about 12:45 pm, after waiting for her to come see my husband since 9:30 that morning, I asked her when she would be available to come see him. She had several responses to my requests, and I quote that she said, "That's what we have charge nurses for" and when I asked her if they told her what Mercy had told us she just shook her head "no" with distain. So I told her and her response was, "I have 99 other patients to care for." That is when I walked away with my hand up and told her, "I see you think I'm a pain in your butt, but the only person I care for here is my husband!" She NEVER came and saw my husband and the next day, 11/1/2017, we were told that the nursing students from the Job Corps could no longer come into my husband's room! My husband loved seeing the young students and their instructor who provided him with more care than anyone there! Even though I went to the Exec. Director on 11/1/17, and told her about my exchange with "Erin" as well as asking her about why the students could no longer go in his room, I told her that my husband wanted to see the students (and even though she responded that she would tell "Erin" that they could continue to see him), we never saw the students in the facility again. Later, we learned that Erin and/or the Exec. Director, or both, didn't want the students to go into my husband's room because they thought we were going to sue them! Also, when I went to the Exec. Director on 11/1/2017, she asked and I told her that I wanted my husband to go to the doctor's office who aspirated his knee at Mercy Hospital. He finally did, but not until days later, and we were told by the doctor there that my husband's knee was very bad and that he needed to go to the hospital ASAP or "it wouldn't be good." He did and the next morning my husband had to have surgery to clean the knee out. Well, this didn't end well. My husband at the young age of 68 and a very vibrant fun man DIED on Christmas Eve, 12/24/2017. If this AWFUL place even had a clue how to care for individuals, he could be alive today. They let him suffer for days, would not listen to family concerns, and eventually just ignored him and us! The Director of Nursing, "Erin" is a joke and should be fired, as well as other staff persons there. They did not like me because I was always there advocating for my husband, as were other family and friends. They even told a doctor friend of ours who came to see my husband that he had to leave because when he arrived he had his scrubs on! Guess what - it is a patient's right to see a doctor in the facility! The staff here does not even know what patient rights are according to federal and/or state regulations! There was so much more wrong doing at this facility towards my husband of 44 years! The bottom line is : PLEASE, PLEASE, PLEASE, PLEASE DO NOT PLACE YOUR LOVED ONES HERE! YOU WILL LIVE TO REGRET IT! read more

My Grandfather had a triple bypass surgery on his heart on june 1st, was not released from the hospital untill the 17th due to complications, My mother was taking very good care of him when he came home. Untill he ended back in the hospital due to an infection called Cellutlitis he was in the hospital for a week and wouldnt release him untill we found a rehab center he could go to. Arriving at this care center , my grandpa did not have his medicine it took two days for him to receive the medication he needed.. They were not checking my grandpa blood pressure, we always had to ask them if they checked it not to mention he had an incision that needed to be cleaned but they didnt clean it untill we continued to bug them! On the 4th day of him being there he needed oxygen and his health was going downhill, a doc never came to see him while he was there. this place is a joke. Finally my family came and wanted to take him to the hospital due to him looking in such a bad shape. they refused to call 911 because there orders are , they are not aloud to call endless the doc says to, so they told the nurse to go call the doc, finally they called 911, This place did such a horrible job, my grandpa is in worse shape because they refused to take care of all his needs. The nurses are a joke and very rude and dont know anything. If it wasnt for my grandfather having family that live him who knows what would've happened. read more
