FORMAL CONSUMER COMPLAINT Date of Incident: April 16, 2026 |…read moreTime: Approximately 2:15-2:45 PM Facility: Genesis Address: 7455 W Washington Ave # 140, Las Vegas, NV 89128 To Whom It May Concern, I am submitting this formal consumer complaint regarding a deeply concerning and potentially dangerous experience I had at Genesis on April 16, 2026. I have been a loyal patient at this facility and have consistently received excellent service from the medical assistant, nurse practitioner, Sapphire (the manager), and Heshimudi during his time there. This place felt like a second home to me -- which is why today's incident was especially troubling. I arrived for a biotin shot and a glutathione shot. The medical assistant who attended to me -- believed to be named Erica, wearing all-purple scrubs -- demonstrated a serious and alarming lack of sterile technique that puts patients at risk of infection. The following violations were observed:1. She removed my biotin and glutathione vials and took them into a separate room, out of my sight, before returning with the prepared shots. 2. She placed open gauze and an alcohol pad directly on her tablet -- the same surface used to check in patients -- and then used those items on my skin. 3. After administering the injection, she placed the used needle on the same dirty tablet, where it then rolled off and fell to the floor. 4. She left the room, retrieved a new gauze pad, and placed it in the same contaminated location. The side of the gauze that had been in contact with the tablet surface was then applied directly to my body. 5. The injection itself was administered abruptly and without care As someone who works in healthcare and is employed, I am well aware that these practices are a direct violation of basic sterile and safety protocols. Needles, gauze, and alcohol pads must never be placed on unsterile surfaces, and items that have contacted unsterile surfaces must never be used on a patient's body. I am formally filing this complaint and intend to report this incident to the State Board for further investigation, including a formal inquiry to identify the staff member involved. This is a matter of patient safety, and I will not allow this to go unaddressed. I sincerely hope this facility takes immediate corrective action to prevent other patients from experiencing what I did. Respectfully, Angeline Angie Villanueva

The swelling was unreal. I was trying to find lymphatic services to continue my Post-op care for what I had received of the country. Mimi took me in the same day, When I went to her the first time she was at the Sahara location. I have been following her since! Mimi is very knowledgeable, and intelligent. I have acquired a lot of knowledge from her regarding my lymphatic system. She is brilliant, caring, and such a professional. Her space is beautiful, you feel right at home. It is spotless, organized, and quite a gem! I was really impressed on the whole package. Needless to say I feel amazing. I have been going to Mimi since December, and I will keep continuing my post- op visits to her and routine ones as well. She has relieved me of so much pain , swelling, and she has really helped me with contouring as well. She saved my life and I am so grateful for Mimi for all her care and professionalism and her wonderful advice and tips. I rarely do reviews, however this is a special person and place. Mimi really is 5 star service. In a world where it is really difficult to get such amazing service, thank you Mimi from the bottom of my heart. You are a special one indeed. You are a diamond in the rough. Shine bright because you truly are a diamond! And last but not least... do not neglect your post- op care. It is just as important as the surgery!

Dr. Mall's professionalism and expertise made me feel not only confident, but excited about my decision to get work done for my first time. I went in about a month ago because I had dark circles form under my eyes that come with age and stress. Dr. Mall did Voluma fillers for my tear troughs and under eyes, and the difference is UNBELIEVABLE. I'm so satisfied with the results. I feel confident wearing no makeup and I look younger. It's had such a positive impact on my day to day. I could not be more grateful to Dr. Mall and every one of his staff members. I also went back for the Titan XL laser treatment with Michelle today and I'm absolutely loving the results of those as well! It enhances the amazing job Dr. Mall did with the Voluma even more!!! Michelle was also incredible, my treatment with her was so relaxing and rejuvenating. I didn't bother putting on makeup all the rest of the day, I felt so beautiful! I can't wait to go see everyone at Dr. Mall's again. I just love it there! I would HIGHLY recommend for anyone to go see him. I fully and happily put my trust in everyone there.

Lex texts RIGHT before my appointment to ask for my intake forms which I had filled meticulously ten days prior. She said she " can't find them, we didn't receive them ". EVERY professional clinic has reached out THE DAY BEFORE if there was any issue with the form. I told her I could sign the main docs when I'm there because I needed to get in the car. Drove 5 min, she texts me saying " we will have to reschedule you, dr had an emergency ". Nono no... YOU messed up because of terrible mgmt, YOU failed to fix YOUR mistake by offering a same day solution, then YOU LIED about so called emergency which - oops! - occurred right during the 10 minutes between your first text and the last one. Now why was it canceled? Because they don't want to risk performing a procedure without protecting themselves from legal issues. These people just suck and have no qualms about insulting patients' intelligence. The Dr is a regular family physician and obviously has no clue about how to run an office. Btw she will do her best to sell you her treatment with her " new Korean radiofrequency machine ". It's " new ", no one has heard of it, it it supposed to work the same way as Ultherapy for the same price. She claims she's had it done herself. If I'm going to invest $1500 for neck radiofrequency treatment, no thank you I'd go for Ultherapy. The clinic is being advertised as a " plastic surgery clinic " which if false. If they comment here with new lies, I'm adding updates to address them.
