What you read below reflects my personal observations and experiences. This is what I witnessed: * I observed overnight staff placing 2-3 diapers on some patients, which appeared to reduce how often they needed to be changed. * I observed a nurse speaking with a patient who had been given Haldol and morphine regarding a DNR form. Based on what I witnessed, I was concerned about the patient's ability to make an informed decision at that time. * My loved one developed a bedsore that was approximately 4 inches across and 1/8 inch deep on his back. The wound was severe enough that I could see exposed tissue. * In my opinion, many (though not all) staff members appeared indifferent to patients' needs. * I'm not sure whether the facility has been remodeled since then, but when my loved one was there years ago, there was a persistent odor of urine throughout the building. * In my opinion, the food quality was extremely poor. Of course, there's more, but if you have other options, I would strongly encourage you to consider them. If you must place a loved one here, be a strong advocate for them. Know your loved one's rights. Do not sign anything unless you fully understand it and are comfortable doing so. Be extremely observant. Take pictures. Document everything. Communicate with your loved one and assure them that they are loved and can tell you anything. Ensure that they are not being intimidated, mistreated, or abused. A heartbroken family member.

I am an LTC nurse…read more(I do not and will never work at this facility), and I am aware of how the everyday routine of nursing care goes for Skilled facilities, as the facility where I am employed is both LTC and a skilled facility. At the end of March, my father was admitted to this place. He is recovering from a severe cerebral hemorrhage that left him with a tracheostomy and a peg tube, unable to speak, walk, move his arms, or do pretty much anything other than sleep and look around the room. At first, things seemed pretty normal for a Skilled Nursing Facility until I started to learn the truth. Such as using towels to clean patients rather than wipes or disposable clothes, not normal or sanitary. Walking down the halls and seeing naked patients being changed with their doors wide open for anyone to see, again, not normal and could be considered illegal, let alone dehumanizing. Call lights and respiratory alarms are being unanswered for hours, literally hours, with nurses, CNAs, and respiratory techs standing and sitting in the halls completely ignoring them. This was all just what I witnessed outside of my own personal experience with care for my dad at this facility. As I stated previously, my dad is recovering from a cerebral hemorrhage and can not speak or move. He was placed at the bottom of the building in the very last room, the furthest away from the nurses' station. If there was an emergency (and there definitely was), no one would have heard it or known until someone rounded on him. By the End of his 3rd week working with PT, OT, and ST, my father was beginning to emerge. He was starting to speak and assist in oral care, and following commands to some degree; it was amazing headway. However, the day Respiratory began trials on capping his trach, it all changed. We agreed to wait until his CPAP was brought from home to try an overnight capping trial, as before the brain injury, he was diagnosed with OSA. The Next day, when I arrived at the facility, I learned that despite our agreed-upon plan, my father had started on an overnight capping trial and failed due to becoming apneic. Its literaly in the name (Obstructive Sleep Apnea). My father was different than the day before. He was shaking, unable to speak at all, and his limbs were almost completely stiff. When I alerted the Nurse to his change and inquired about medication changes or an assessment. I was met with "this is normal," and I'll put him on the board for the NP to see." The nurse did no bedside assessment, she never documented the abrupt change and decline in his physical or mental status, she only charted that I asked about his medication, my father went 4 days without the NP seeing him. After that, I was called by his social worker and informed that he would be discharged home with home health, that he would not be receiving any new treatments, and that the therapies he was getting could be done with home health. While home health was our goal for discharge, it was quite abrupt, and he was not medically stable considering the decline. He was never sent for a CT to make sure there were no new brain injuries or changes. He was never assessed by the NP, when he did come, he only spoke to me at bedside about medication and completely disregarded what I said about his decline. The Process of discharge was just as horrible as the care my father received. I was contacted by his social worker 2 days before his discharge and informed that there was no home health agency set up, that I needed to find one, and that his insurance denied him being brought home via EMS, and that I would have to set up transportation. None of these things I was able to do, as it is required that the facility set up the transfer of care and transportation via non-emergency EMS. It took me calling other facilities and companies to get it sorted, and finally, the social worker supervisor started working on it and helped get things in order. As for a home health agency, I had to call a local company and explain the whole situation, and thankfully, they accepted him. This place took away the progress my father had made in recovering; it almost took away my family's hope that my dad could have a meaningful life. We still do not know how bad the damage is and how much he can recover, but since he has been home, he is making small improvements little by little. I could not ever recommend Standifer Place as an acceptable or safe SNF, LTC, assisted living, or business to anyone. The state should do some serious investigating of this place and get some help for the people who currently live there and have to receive services there. They have a right to live and heal in a safe, clean, and honest environment. Unfortunately, Standifer Place is none of these.

We expected another decline…read morebecause of the move but she has improved to the point that she no longer has to use her walker all the time. She is sleeping on a normal schedule. We have cut out some of her pain meds because she is so much more active now she does not need them. There is no doubt she was declining at Rockbridge Oaks because of the SEVERE NEGLECT. Don't listen to the words of management. Pay attention to their actions! If they say they will change but their actions do not reflect their words...RUN AWAY!!

We tried for months to get things addressed but their promises were consistently broken, and this was the final straw. The assisted side is ok, but the memory care side is critically understaffed and undertrained. The office has a very high turnover rate as well. They are trying, but unfortunately failing, and all the good staff seems to be burning out. I do hope they get it together soon though, for the sake of the remaining residents and few remaining great staff members. They deserve better.

This place is so unique and it is because of the staff: they are really committed to help the residents, they are kind and helpful and the care in this facility is simply excellent. Kendra is an angel, always so nice and helpful and April is particularly my mom's favorite (though like everyone is great here). My mom is comfortable and in good hands, and I am happy and grateful. As an RN with over 20 years experience, I did through research on Chattanooga area assisted living. I then took her around to visit these places so we could make make a choice. Although Summitview was an old building, the staff was GREAT! They were the most accommodating on things such as special diets, were the most affordable, and didn't have add-on or hidden charges Most important of all, my mother felt more comfortable there. After 2 years, she died at Summittview in her sleep. They had never raised their prices but more importantly, they provided excellent care. I will always be grateful to them and would highly recommend them.

The dining room/meals are hit or miss, but mostly enjoyable. My main concern: Staff. There are one or two superstars but for the most part, the staff are not well trained, not well supervised, and not willing to work hard. Perhaps my definition of "assisted" living is different than theirs. Things were very good for our loved one at MB....until they weren't. Classic case of one extreme to the other, AKA, the honeymoon is over. It's been a learning experience for sure, and one that I will continue to explore as I educate others about the lack of overall quality care for our senior citizens.

This (expensive) life-care facility is terrible. My parent has been there for about ten years, the…read morelast several of which have been in assisted living and occasionally skilled nursing. This facility is unable to accommodate even a common dietary restriction (gluten-free) with any consistency. My parent frequently gets sick from eating items that staff has said are fine and safe to eat. Their staff is so poorly paid that the turnover rate is astounding. They are constantly short of staff and Ascension (old name is Alexian) just deflects by saying it's hard to get people, when in reality they just don't want to pay enough to retain good employees. There are a few nurses in Assisted Living who are truly exceptional, but they are so overworked and underpaid, I honestly don't know why they stay. Alexian's staffing inadequacies are so bad that they sometimes have to bring in temps to do things like give showers, which should only ever be given by staff that the residents know and feel comfortable with, not total strangers! Food service is another place Ascension/Alexian cuts corners. Small illustration: At the price point that is charged, and the amount of money required to buy into lifetime care, I would expect at least one or two homemade salad dressings a day. But nope, all salad dressings come in a packet of low quality pre-prepared junk you would buy at Grocery Outlet. And salad dressings are SO EASY to make! Also, the main dining room serves a LOT of buffets to cut down on staffing requirements. These people are old, and navigating a buffet is difficult with a cane or walker. Many residents are too proud to keep asking for assistance so they either struggle, or just skip a meal. On the many times I have dined there, it has always been a most underwhelming experience with quality of food served. I swear, the food at Shoney's is better than Alexian. It is very clear that the new owners, Ascension Living, are in this to make money. The elevators are dirty and some are scratched up, many places are overdue for a new coat of paint, some areas are overdue for new carpeting, and much of this facility just looks "tired" and in need of attention. Look around the grounds and you will see piles of cigarette butts where employees gather to smoke. Employees park in the closest parking spaces so relatives and visitors have to hunt for a place to park. When my parent was in skilled nursing, the nursing staff were HORRIBLE. Sometimes I would finally have to go up to the desk to find out why no one was responding to a request for help using the restroom, and there would be the nurse, playing on her phone, irritated that I had interrupted her. The overall management is consistently non-existent. When calling the business office, no one ever bothers to answer, and calls go direct to voicemail. Same when calling the administrative office, straight to voicemail. How can you conduct business and impress prospective residents with your customer service when no one ever even answers a phone??? In the last several years, this place has gone so far downhill, I wouldn't place my worst enemy here. Compared with someplace well-run and well-managed like the Spires in Rome, Ga, (another continuing care community), Alexian is like the lowest bargain basement of a place that charges Four Seasons Hotel prices. If I could get my parent out of here, I would, but they're too old now to make that kind of big change. Stay away from this sorry excuse of a retirement facility. It seems to only be all about the money to them, not quality care for the residents.

Thankfully the state has finally listened to so many and have penalized them and placing no new resident sanctions on them for the time being. I had two family members there and during that time saw 6 executive directors and 7 wellness directors. Under each one of them, we saw constant neglect from food services, housekeeping, bugs, incorrect med administring or not at all, unsupervised meds, and not following doctors orders or plan of care. In the last state report, it was found that close to ten residents needed a higher level of care that they were receiving from the Terrace and had to be removed. So revenue has always been the priority over care and wellbeing of their senior residents. Their number one push this year was the renovation, to bring in more residents and to compete in higher sales profit. $$$ SALES $$$$ We had requested several times for carpet to be replaced due to being dangerous for potential falls and was ignored. (see photo) After more than 10 plus meeting over a 3-month span regarding incorrect meds, deplorable housekeeping, unacceptable dietary issues and finally, our family member being hospitalized and going to inpatient rehabilitation due to lack of care from the terrace her health and welfare where too much of a risk to trust with this facility. She hated leaving the residents she became friends with and we worry for them.
