For all patients to receive hot meals that is at the proper temperature. No way to warm any food for a patient from their family. No access to hot soup or water for herbal tea. There needs to be better communication between the nurse on duty and the certified nursing assistant on staff. On a positive note the aids on the floor are compassionate and try to help as much as possible. Patients rooms need to be cleaned daily along with cleaning the floor and emptying the garbage. read more

My Mother in Law was admitted for hospice two weeks ago where she passed. The staff was amazing and there was excellent communication. I do believe she was well cared for. Her bed and person seemed clean. The facility was clean but her room was not. The floor was dirty and there were items that may have been left in both the closet and refrigerator from previous residents. She had a shared room with a person who was very mentally competent. I do not know how many beds were available when she was admitted but I would not be happy if my loved on was in the same room with someone who was dying. I can not speak for the food, she was a choking hazard on hospice care. Between the staff at William Barr LP and the hospice team we signed with both my Husband and I felt this was a fine choice for her. The front door staff was welcoming. The valet parking was helpful in the congested neighborhood but I always found street parking close by. read more

The place is clean. The rooms are nice. I am very pleased with the PT, and OT for my mom. ...But...the food is not quality, there is no consideration of dietary needs ( diabetics given starcy and sugary foods). Family membbers need to carefully monitor medications. Staff loose stuff and don't keep track. My mothers eye drops where misplaced daily. Staff is polite, but Night staff is LOUD. Nurse assistants need training in proper decorum in a medical facility. read more

This facility was great..special thanks to Theresa Palmer for a job well done. She did an excellent job with my grandma.

My mother is there. The night attendant does not respond to the call for service. This leaves patients laying in body waste.

Was there for a week it was the most terrible experience. I have had the places depressing. You share a room one wall is painted black. The other one is brown and gray. How depressing is that. The staff is not properly trained and slow to respond. We won't even speak about the food because it is horrible if you can recognize what it is, I can't understand how this place is even open. Is there anything less than one star read more

I just admitted my mother here. What a mistake the night nurse let the residents lay in urine and feces. You ring for service no one ever comes. I tried to call the supervisor to no avail. The attendants are more concerned about their cell phones than the patients. Do not be fooled by the neighborhood and façade of the building. This place is horrible!!! read more

with the exception of the receptionist at the front desk when you sign in ... she was very pleasant ..this place is horrible, I went to visit a dear friend, the rooms reak of urine, the floors were dirty, she said the food was horrible, and don't try calling for help or assistance; they need a total make over the area and the neighborhood is very nice , don't let that fool you lol when you walk in this place its a nighmare read more

DO NOT COME HERE! Terrible service and care. Grandmother was admitted and was not properly taken care of.

My mom spent some time here before she passed away. The staff is pleasant, they try to do comprehensive evaluations, and the valet and included medical transportation are nice. However, the rooms are extremely noisy. Almost constantly you hear the call buttons being rung for other rooms, people talking, carts rolling loudly through the hallways. The rooms are very small with only a curtain in between the two patients and almost no privacy - I could hear literally everything going on with the patient sharing my mom's room. In my experience my mom's caretaker was always quick to tell me all the things she could NOT do, but then I would see her sitting at the desk texting on her phone. One time we found that my mom's call button had been placed outside of her reach. We asked why and they said it was just for a little bit to control the volume of calls. But what if there had been an emergency? I do think they are understaffed. I was uncomfortable leaving my mom alone because I felt it was important for someone to be there to advocate for her needs as she couldn't do so for herself amongst the crowds. I was displeased because after my mom passed away, the staff misplaced her belongings after cleaning out her room. I was told they would reimburse me if I submitted receipts. I did so, and it took literally months of followup before I was finally told they couldn't reimburse me, but they could purchase the items back for me. I'm not sure how it makes sense to repurchase my late mother's items since she obviously won't need them anymore. In short, I advise not bringing any belongings on site that you would mind losing. read more

My main complaint is they could not keep the coffee maker working. They advertise as a top notch facility but how can you claim that when you cant keep the coffee maker working? So half of my days there I had to order coffee from Dunkin Donuts to be delivered which gets pricey. Other then that it is a decent facility. read more

I was very pleased with the care provided to my mom at this facility. Staff was professional and provided constant reassurance to my mom and our family. The facility was clean, welcoming, and comfortable. This whole pandemic thing has been scary, but staff have made us feel as safe as possible throughout. My mom was provided with basic medical care and also extensive physical/occupational therapy after her surgery. Overall a great experience. read more

My mother was recently in Warren Barr Lincoln Park--what a fabulous place! She has been in multiple nursing homes/rehab centers and this was the nicest staff ever! Everyone was so helpful and folks even volunteered to do extra help. She was so well cared for; it was such a relief off my mind. Food was really good, too. Only complaint, sometimes the food was not really hot by the time it hit the fourth floor. Can't recommend the place high enough! read more

My mother has been at this facility for over 3 yrs and throughout her time here I could always count on Theresa Palmer and Nicole to assist me with my mom's needs. Whether it was for personal items that she needed or a complaint about missing ones, Theresa always comes through. Also, Nicole is always lending a hand and asking if all is ok. I appreciate how both ladies handle their positions and look forward to working with them both in the future. read more

I will undoubtedly be in tears before I complete this review, but my mother's story must be told in hopes of sparing anyone else from the same painful, traumatic experiences that we had at this facility, The Grove at Lincoln Park and their sister facility The Grove at LaGrange. Please know that the corporation behind these two facilities, LEGACY HEALTHCARE, cares nothing about your loved ones. Your loved ones are no more than a bottom line and dollar signs to the people that own and run this company. I can attest to what other reviews have stated...the facilities are not at all pleasant places to be. They are dark, dingy, smelly and stuffy. They are extremely understaffed...my mother had to wait several hours sometimes to be changed out of a dirty diaper and to be turned. I personally sat in the room with her while the call light was on for 40+ minutes on more than several occasions. I would have to personally go seek someone out and/or if I was not there my mother would call me and I would call up there and demand that someone go attend to her. Understaffed...busy...whatever the reason, they get paid enough money to fix the staffing issue and deliver the quality of care that is expected. My mother had a progressive case of multiple sclerosis that rendered her 100% unable to walk, turn herself, sit up in bed...anything, she could do little to nothing by herself. All she could do was move her hands and even that on some days was not possible. My mother also had a stage 4 ulcer on her lower back/butt area...wounds this severe demand top notch care. She did not obtain the wound at this facility, but the level of care delivered did not improve things for her either, but rather made it worse. Additionally, after numerous requests that the nursing and physician staff review her long list of 20+ medications and eliminate those that were not needed, still nothing was ever done. So naturally being overmedicated, stressed out and in constant pain broke her down mentally as well. I imagine my demands for better care and attentiveness to my mother is one of the primary catalyst of the most disgusting/deplorable actions eventually played out by then administrator Shua Sief. Every dime of income that my mother received went directly to this facility. However, they had in their minds that there was more money out there somewhere. Mind you, my mother had been disabled and not working since 1998. After several months of providing proof of income, Legacy Healthcare still never corrected the billing issue, nor tried to rectify things on their end. Yet, the proof I scoured for and delivered to them wasn't enough. So....one day my aunt takes my mom our in her wheelchair to get some fresh air. Something I might add that their staff NEVER did for her. She was lucky if she even got the therapy or care that should have been included in her treatment. Shua Sief and staff took this opportunity to gather my mom's things, put them out in the back alley parking lot and refuse to allow her to return to her room...a room she shared with three other tenants I might add. No "private suites" here. My mother, 100% unable to walk due to MS...with a catheter...a feeding tube...a stage 4 ulcer...unstable blood sugars....diabetes....They put her out on the street. They then LIED and told us that another nursing home in Oak Lawn had accepted her. With no other choice but to leave, we took her there, via ambulance transport, only to find out that they had ZERO idea that she was even coming. And this other place was such an awful dump that my mother broke into tears the second we walked through the door. For those of you unfamiliar with MS...stress is the worst possible things for an MS patient. Having to leave her there overnight while I scrambled to find her someplace else to go was heartbreaking. I did eventually find her a better place, but by then the damage was already done. She was disheartened and broken by what The Grove at Lincoln Park had done to her. My mother died February 11, 2015....from the wound...from malnutrition...from lost hope. I wish I could recommend a fabulous nursing home, but I"m not sure they exist. Oakton Pavilion is where she spent her last days and they took truly great care of her, fixing the damage that the Legacy Healthcare facilities caused as best they could. If considering this place...I urge you to reconsider. If you still decide to go with a Legacy Healthcare facility, be prepared to work hard and stay on top of them. Best wishes in your search, God bless and praying your story ends better than ours did. read more

First the positives. The administrators and social services lead are good. Although they seem very young, they seem to genuinely care about the patients in Warren Barr and are trying to do their best by them. They were accessible by text and phone and tried to be responsive to our concerns, which helped. Unfortunately, they are let down by a nursing and PT/OT staff that appears to be undermanned and indifferent to the care of their charges and seems not to be trained in or supported by standard processes and procedures necessary to provide the bare minimum level of care and rehabilitation. My 93 year-old aunt was admitted to Warren Barr - Lincoln Park after a fall in late Oct 2020 in which she broke her hip and elbow. It was a terrible experience for her, made worse by Covid which meant we were unable to check up on her care in person. Although promised regular Physical Therapy (PT) and Occupational Therapy (OT) it is not clear that she received the PT/OT that was promised (or billed to her insurance)...she certainly did not recover her ability to walk. There were many days when the nursing staff left her in bed all day...even though she would ask to be helped into a chair. When we complained to the Administrator she apologized saying that the nursing staff had gotten out of the routine of helping people out of bed during Covid! As I mentioned above, Covid meant we were reliant on the phone to check in on her. Although my Aunt had a phone in her room, it often did not work and the maintenance staff seemed unable to fix it (we wondered if this was a deliberate attempt to prevent us from finding out what was going on with our aunt). So we would call the main number and often it would ring and no one would answer. Or if they did answer we would be on hold for a long time or they would transfer us to the nursing station where again no one would pickup. On top of all these issues they are trying to bill her for part of her care, even though It was supposed to be covered by Medicare/Medicaid. We moved her to a different skilled nursing facility in April and she is so much better off...she is more alert, cheerful and communicative (we can visit with her and talk to her on the phone). She gets out of bed every day and is getting PT again. She is engaging with other residents (something I don't think happened at WB)...in short she is back to her old self. In summary, I would never send a loved one to this place. read more

My mother came there in June of this year and is still a resident at Warren Barr in Lincoln Park. The people there are very nice, never had a confrontation with any of the staff. The administration doors are always open, if you want to talk with someone, someone is here to assist you. I ask for help with my mothers transportation to and from her doctors appointments and things are done smoothly. The only thing is the staff should do more with the odor on the resident floors. sometimes the odor is overwhelming. If staff would just monitor the trash where they place the used diapers of the residents this would help a great deal. I know that budget is tight in these type of places, but if administration could voluntary to bring odor fighting strays to be used on the residents floors that could help. read more

My mum had surgery at Northwestern Memorial Hospital and was transfered to Warren Barr Rehab yesterday, July 25 at 2.00p. Her room was 408. From the time she moved in there were other patients coming in and out warming their food in the microwave. The room smelt of fish. We were informed that this is the main room where the patients used the microwave. Around 9.30p when she was getting ready to sleep, she was disturbed by the loud tv from the next room. TVs need to be turned off by 10p. Also, my mum complained that the night nurse was very mean. She never had a wink of sleep due to tbe loud noise.Today breakfast was brought with meat despite informing tbe staff that she did not eat meat. The lady who brought the breakfast snapped at her to remove tbe meat off her plate and eat her breakfast. My mum refused and she was given bagels which she liked. She called me and informed to take her away from her dark, dingy and smelly room right away. My mum was discharged at 3.00 p today after just one day in this nightmarish place. read more

If I could give this place negative stars I would! They never have enough staff. It constantly smells like dirty diapers and garbage. The food, if that's what you can call it, is never cooked properly. If your loved one has trouble swallowing, they will not prepare soft food, they will simply blend up toast or chicken and call it "mechanical soft diet". I hope this place gets shut down, it is beyond disgusting how they run this facility. read more

Came to tour facility for my mother who needed short term rehab. If it wasn't for touring Warren Barr in the Gold Coast this facility was my second option! Keeping this place in mind! read more

Staff are lovely and friendly, and valet is a perk, but all positives end there. Rooms are dark and small, not conducive to healing in my opinion. Bathroom was shared with 2 other people in adjoining room. In my husband's 4 days there he never left the bed, even during PT and OT. Halls are noisy night and day. Please do yourself a favor and don't just go by the photos on the website. Visit, and insist on seeing an actual patient room. When people ask why I was so displeased, I show them the photo of the concrete block wall outside my husband's room. It pretty much tells the tale. read more

I'm giving The Grove at Lincoln Park a 5 star rating, that's because it is truly a fact in my case. What I'm going to tell you about is that my experience with nursing homes runs rather deeply, as I have been housed in 3 but visited more than I want to remember. I'm not going to mention names but the ugly rumors that you've heard about throughout the years are too often true. Every nursing home I've been to has a secret. The secret is kept on whatever floor the residents live for long term care. On this floor you can suspect neglect and/or abuse. You can feel it, you can hear it, you can see it, and worst of all you can smell it. On these floors there is never enough staff; either nurses, CNAs, social workers or housekeeping to keep up with the care the permanent residents and all who come through these doors deserve. Many of the long term patients are acutely aware that they will be spending their final days within these walls. It doesn't matter that it is The Grove at Lincoln Park when your heart is aching, your mind is wandering and your body has betrayed you; our golden years should be provided all the world has to give. The Grove does that. Your spirit needs that special dose of love, confidence and teamwork support that is always bursting at The Grove. The Grove exceeds expectations and provisions for my comfort. I've been here since March 31, 2015. This is my second rendezvous. I spent 100 days at The Grove last summer. My circumstances prohibit my being alone in my home, it would not be safe. Therefore I dread my own long term living activities but I feel very lucky and highly favored that The Grove at Lincoln Park is where I'll be. Frankie Franklin Official Welcoming Committee read more

location location location. Very convenient to restaurants and cta. Lakefront is a short beautiful walk away. The staff seemed very friendly and thoughtful when having my father readmitted to the ER after a 3 hour stay here. the decor is best described as that of a rundown motel from a horror movie. I would recommend a tour, possibly on Halloween. I can grade three items, care, location and ambiance. -2 for ambiance, 2 for location and 1 for care. read more
