Took an elevator to the second floor since the stairs looked exhausting. The front staff was very friendly and provided me the all too familiar registration packet to fill out. The waiting room was small but only a few people waiting. I was called into an examination room within ten minutes. Dr Pollak is a joy to speak with and is super knowledgeable in her profession.

The problem is that this policy is not clearly disclosed before the appointment, and the practice cannot explain what qualifies as a "high deductible." On May 6, I attended an appointment with Dr. Jackeline Castillo. Before the appointment, I completed all required paperwork and provided my insurance information. At no point during scheduling, registration, or in any forms was I informed that I would be required to pay the full cost of the visit because of my deductible. No one explained what deductible amount triggers this policy or what my financial responsibility would be before I arrived. When I arrived, I was told I had to pay $203.30. I was already there, I was sick, and I needed to see a specialist, so I paid $203.30 and proceeded with the visit. The medical care was excellent. My complaint is not about Dr. Castillo or the quality of care. It is about the office's billing practices. After receiving my UnitedHealthcare Explanation of Benefits (EOB), I learned that my actual responsibility for the visit was only $14.92. I was charged $188.38 more than my insurance determined I owed. On June 19, 2026, I contacted the office and asked where this policy is disclosed and what deductible amount is considered "high." The representative could not direct me to any website, form, or written policy and simply stated that it was the office's internal policy. I am requesting reimbursement of the $188.38 overcharge and that this practice be required to clearly disclose these billing policies to patients before appointments are scheduled.

I have been struggling to find a doctor that would help me with my severe & debilitating period & ovulation pain. After hearing what my symptoms were we decided that he would perform laparoscopic surgery on me. Finally after 2.5 years of pain and not knowing what was going on in my body he gave me a diagnosis of endometriosis. I am still going through treatment with him but I can confidently say that I trust him and his recommendations with absolutely no doubts. And Made me wait like two weeks for surgery even though I only have 1 kidney and it's jammed by big stone ..I asked him isn't time of the essence because of the solitary kidney ? He said "you'll be fine". Day of surgery after waiting and waiting someone from Aventura Hospital cancelled surgery after already being on the surgical table and had been drugged twice. I got up off the surgical table in the OR and drove myself to memorial hospital in renal failure. I will never understand how he can practice medicine like that. I acquired an acute kidney injury which has left me with compromised renal function and I had to have specialized surgery to remove damaged part of ureter because of damage the stone made on the ureter. I was forced to have my ureter resected and reconstructed because of the trauma from the stone and the length of time it was allowed to stay stuck . I would not recomendad Aventura hospital since the OR cancelled surgery abruptly because the ceiling was collapsing while I was on the surgical table. Just So you understand, the hospital called me the day before the surgery to cancel it because they lost the lazer to be used for surgery. Only to be called back 30 min later saying they had found the lazer and the surgery was back on. I know it sounds crazy but this happened to me a year ago and I never wrote a bad review nor sued the Dr. nor the Hospital but now I receive a bill in collections from Dr Levin for like $88. So you get the scathing review above. Dr. Levin, Sir, You suck! And so does Aventura hospital.
