    Review Title: Severe Medical Misdiagnosis & Violation of Rights…read moreI was brought to this ER in the middle of a life-threatening medical crisis caused by a toxic interaction between my daily Wellbutrin (bupropion), Effexor, and a standard dose of DXM. Wellbutrin is a known potent inhibitor that can cause DXM levels to spike to toxic, dissociative levels. Instead of identifying this pharmacological emergency (Serotonin Syndrome/Toxidrome), the staff dismissed my involuntary physical thrashing and confusion as "behavioral issues." I was denied a medical bed and involuntarily committed to a psych ward for over a week. While there, I was coerced into taking Olanzapine under the threat of a 2-week extension if I refused. This was a documented substance-induced episode, not a psychiatric one. The hospital's failure to screen for this well-known drug interaction led to medical trauma and a total violation of my rights. I am sharing this so others on maintenance medications know this facility may prioritize a psychiatric label over actual toxicological care.

When I moved to Virginia and enrolled for care, I made a simple and reasonable request for a male primary care provider because I communicate better that way. That request was ignored and I was assigned a female provider anyway. That set the tone immediately. During my first and only in person appointment, my provider asked basic questions but did not offer any real medical direction. The only consistent answer was medication, even after I explained my history of severe adverse reactions and that I had previously been put on nearly 20 medications at once that I was expected to take long term. When I raised concerns and declined medications based on those past experiences, I was met with visible frustration instead of any effort to explore other options. It didn't feel like a discussion about my care, it felt like pressure to accept the same kind of approach that had already caused serious harm. Those medications caused major problems. I was constantly vomiting to the point I couldn't leave my home, dealing with severe chronic pain, shortness of breath, and worsening symptoms from medications like omeprazole and other prescribed drugs, including highly addictive medications that led to severe withdrawal symptoms when I tried to stop them. The withdrawals were intense and at times felt life threatening. It got so bad that my teeth were literally chipping away while on those medications and I had to see a dentist because of the damage. Instead of addressing any of that with a real plan, I was pushed off to social services, which did not help, and generic programs like yoga or whole health classes. I made it clear during that appointment that I don't feel well most days, yet the only options offered were programs that required me to show up on specific days. When those days came, I was too ill to attend. That disconnect made it clear my condition wasn't being taken seriously. After dealing with that, I requested a basic medical accommodation letter for housing accommodations tied to my service connected disabilities, including PTSD and physical limitations that directly impact my safety and daily functioning in my apartment home. This should have been routine, but instead I was met with delays, deflection, and no resolution after multiple attempts. When I escalated to the Patient Advocate, it didn't fix anything. Instead of helping resolve my requests for basic accommodations like safe pest control, reasonable parking, front door package delivery, and proper notice and entry procedures due to PTSD, the focus shifted to my wording during a stressful moment. That was used against me instead of addressing the actual issue. I did not want to handle any of this over the phone, but that was the only option provided to report these issues. Due to my PTSD and anxiety, any communication other than written communication becomes much harder for me to manage when I am under stress. Being forced into that process only makes it more difficult for me to communicate clearly and explain what is going on. The VA reporting process does not properly accommodate veterans dealing with mental health conditions. At this point I am not asking for anything unreasonable. I am asking for basic medical documentation, a provider who actually listens, and communication that does not make my condition worse. Instead I have been met with resistance at every level, from my provider to the Patient Advocate process. Veterans should not have to fight this hard just to get basic care and support. This has been exhausting, frustrating, and far below what any veteran should experience from a system that is supposed to take care of them. God Bless America Semper Fidelis

    I've been a beneficiary of the Salem VAMC since moving here in 2015. While there is a lot of…read morenegative press on access to care, Salem does much better than provide outstanding care - they lead the way on how a VA Medical Center can set the standard for care for Veterans in the United States. I've seen other care areas that did not provide care to the level that most Salem VAMC beneficiaries take for granted. My providers care for me and I feel confident in their assessment of anything I come to them with. I have no problem talking with other Veterans who might not be utilizing their benefits there. I would say that my experience there has been exemplary. I highly recommend the Salem VAMC to anyone who served our country honorably for their care. I appreciate them and trust them with my care.
