The facility is lovely and when the sun is shining it's so pleasant in the courtyard. All the nursing staff are very pleasant, knowledgeable, efficient, and caring. There was a variety of food and very enjoyable. Also, the activities department does a great job with keeping the residents occupied. I also was able to attend a car show they hosted in their very own parking lot. I would give CNAs, LVNs RNs 10 stars. Activities 3 1/2 stars Kitchen 3 stars Administration 1 star Physical therapy Dept 1/2 star If you or a family member are here for care and a little therapy to move around after a fall or surgery you'd do okay at this facility but if your going after a major event this place is not for you. I've been a resident here for over 2 years. The physical therapy Dept is ill equipped to take care of much more then walking you around until your injury heals. Make sure your family members keep on eye on your progress. This therapy Dept. Will go against your doctors orders look you straight in the eye and tell you this is what should be done. CNA left bed rail unlocked. When she rolled me off my bed pan, she rolled me out of bed and onto floor. Broke my hip. I had to have a hip replacement. When I went back to TOPA they were supposed to get me out of bed and standing....per surgeons orders, therapy 5 days a week, sit beside, use Hoya lift to get in shower chair and on commode. This physical therapy department looked right at me and said this was not the doctor's orders. The most they did was put me in a wheelchair to sit in garden half hour and put me back in bed. What was really going on? They're trying to find out who was going to pay for physical therapy. They broke my hip. They were responsible for my physical therapy. Instead they left me laying in the bed with some range of motion. Shame on this physical therapy department. And the doctors they come in spend maybe 3 minutes with their patients and leave. My doctor would not listen. When I told him I couldn't breathe. It kept getting worse and worse. He told me I needed to lose weight. Put me on appetite suppressant I ended up with pneumonia in the hospital. Do not trust a physical therapy department. Do not trust the doctors. Keep your outside primary care. Discuss things with them. The CNAs here work above and beyond what they should have to sometimes 12 patients for one CNA. They expect them to bring food trays, shower patients change bedding get ready for appointments. Get ready for people returning from appointments. Deal with family members. Deal with patience getting out of beds that shouldn't be standing point out. Tubes falling on the ground they are asked to stop what they're doing to go work in the dining hall. Then come back and work with their patients. Too much work for one person that you CNA is come back time and time again and work their butts off. You can always count on your CNAs read more

I've had experience with at least 4 local care facilities at this point, when I say I am appalled by this place I'm not judging them by impossible standards. The common areas look nice and new, but the patient rooms are disgusting. They pack 3 beds into rooms that are sized for 2 beds. It's loud and chaotic. There appear to be a lot of staff on duty when you go in, but not a single one of them asked me who I was or what I was doing there. My loved one started having anxiety attacks on the 2nd night there. If you have to send a loved one here, be vigilant. read more

Our dad would have been here a year on May 29 of this year. Unfortunately, he passed on March 9. We saw dad the day prior on March 8, and my Sister noticed that he was not well. He seemed to be having difficulty breathing. When she told his nurse and asked her if she has noticed anything wrong with dad the nurse seemed annoyed and curt with her answers. After checking his vitals she ordered a breathing treatment and let us know she had called the doctor and was awaiting orders. We left dad shortly before noon as they were putting him to bed and continuing with the breathing treatments. At 4:15 I get the call that dad had passed away 1/2 hour before. How could professional medical caregivers not realize that our dad was extremely ill??? We feel it was negligence on their part to not have noticed how ill he was. In retrospect, he should have been hospitalized and we would have been with him. He wouldn't have died alone in his bed. I was transferred here after a six weeks at Kaiser. Had a variety of care, nursing , Physical Therapy, time alone as well as being in a room with a couple others. Have to say could not believe how outdated the sound system was on TVs, with sound coming from TVs instead of bed it was very hard to hear as typical had three different sounds going plus radios. Nursing (Myrlie Nicanor) was very attentive, as well as Melinda taking good care of me. Physical Therapy was top notch with Johnathan Yogore and team getting me back on my feet. I was not able to walk upon arrival. The setting outside was good with a natural setting of grass and trees in an open area. Spent lots of time in the wheelchair pushing myself around working out. A lady walked in the door with Resident 1 and stated she saw Resident's name on her wrist band. The lady also stated, Resident was confused and did not know where (Resident) lived." "Resident was observed to have a urinary catheter...the treatment nurse confirmed there was no documentation for monitoring signs and symptoms of infection for 10/21, 10/25, 10/27, 11/7 and 11/9/21 in Resident's record." This place should be shut down !!. Elder neglect and abuse . My grandma was a patient there and almost called the police she was so scared . They took away her call light . They wouldn't change her . They wouldn't help her for anything . They were mean . Etc etc . She was completely traumatized. Fortunately she could talk to us and tell us. But for others who can't communicate I would worry about them ; as they are probably being abused and neglected and no one knows . read more

Some of the hired employees were just plain sadistic, putting my walker just out of reach and then disappearing. Someone stole money from my purse. I caught Covid there and my roommate died. Except for breakfast the food was not good. I never saw a doctor while I was there and had to beg for my evening medicine. My daughter finally rescued me and brought me home. read more

I was an unfortunate resident of this place during the height of the pandemic. The amount of turnover of CNAs and nurses was incredible. They couldn't afford to pay CNAs lvns and nurses but had plenty of money to redo the flooring walls and repaint during a pandemic. I was there for a pressure ulcer and their wound nurse quit shortly after I got there because they were not treating her well. They handed the wound care position to a young unexperienced LVN with no wound care experience. From the moment she took over the wound stopped healing and continued to worsen. It was not until a few months later when they hired an actual trained wound nurse that things began to improve. Regardless I made plans to get out of there as soon as possible. The young inexperienced LVN had multiple complaints from other patients about her experience and bedside manner. Fortunately they fired her or she left, regardless she was finally gone. The only positive thing I will say about them is that they managed to keep the pandemic under control until February of 2021. I was one of a few residents at the facility that did not catch covid. Charge nurse is / was excellent and a handful of their CNAs were wonderful. If you do find yourself a resident of TOPA, do the best to get Manny as your CNA. He has been working there forever and is the best CNA I've ever had in any facility in my life. Sadly the long-term care side, East wing, feels like a morgue with unexperienced people that have trouble with English. One CNA in particular would hide during his shift or take 2 hour lunches to avoid taking care of his patients and working. This particular CNA left one patient laying in his own feces for 6 hours without cleaning and taking care of him. HR knew about him and did nothing. The doctors are inept and do not listen to patients complaints when they are experiencing health issues. My doctor disregarded my complaints of bowel discomfort for a month until my wound care doctor visited and tested me to find out that I had C diff. I was immediately put into quarantine for 2-3 weeks. While topa is considered one of the best in the area I would rather lay in a gutter then return to that place. read more

Most unprofessional, understaffed facility I've experienced. After my knee replacement on Feb. 2024 the hospital transported me there as I live alone. I was not given a ID band until the 3rd day. Shared a room with another person who was hard of hearing and wouldn't wear their hearing aides and had a lot of visitors any time of the day. There was a "thin" curtain between us. I rarely got my pain meds on time and there is no "in house" pharmacy and the weekend Rx came whenever the "person who handles them" came on their shift. I was on drip antibiotics and the tubing filled with blood several times and they would put wrappers on the foot of my bed. Sometimes it would take l/2 hr to 45 min to have them answer the call button. Because I couldn't walk to the bathroom they put a bedside toilet by my bed (without a cover) and I would have to ask them to please empty it when I had a BM. After a week and a half while I was throwing up I asked them to call 911, and the ambulance took me to Los Robles hospital where they literately saved my life. While in the ER my blood pressure raised over 200 due to my pain. When I asked a friend to pick my belongings, they didn't have to sign in and no one even talked to them, They just went to my room and bagged my things up and brought them to me. For those of you who know. I love my mother. I'm fact I am proud to call myself a mamas boy. Anyway mom has really bad back pain and recently started fainting. It was getting so bad we had to take her the hospital a few times. One of this times they discharged her to Camarillo Health Care center. She wasn't in there for more than 10 minutes and she wanted out. The place smelled there were people wandering the halls people screening and no one around to help. Well as it turned out we only had to go to that facility because Mom had advantage pro health care HMO insurance and the choice of post care was very limited. At the hospital they said if we cancel her Advantage pro and then she will just have Medicare A+B and with her medi cal she would be covered well. So we got lucky that we got this all fixed and she was admitted into TOPA. After the experience in Camarillo we were so thankful to be near the house here in TO. First of all the place is clean. It doesn't smell. The staff is nice and helpful. So I don't know what happened with everyone else here on Yelp but they must have been picky cause in the world of post care this place is nice! After her stay of 30 days I heard no complaints. She said the food was good and they had planned events. Like a mariachi band for Hispanic heritage month and taco Thursdays. On her way out everyone was saying good bye and hugging and kissing. So suffice it to say I recommend TOPA wholeheartedly. read more

My 93 year old mother was there for 4 days after a car accident. She is very very independent and snooty about her surroundings. On day one we we were all terrified...3 to a room, the rooms and bathrooms are a little run down and there are folks in really bad shape wandering the halls, but by the end of the 4th day she wanted to stay longer! We received excellent treatment from all the staff including the therapists and administrators. The CNAs especially were warm and caring. My mom loved the food and there is a nice outside garden. The facility rarely smelled bad and was, for the most part, kept spotless. I would give it 5 stars but for the cosmetic issues of the rooms. I wouldn't hesitate at all using this place myself...we were all joking by the end that we wanted to stay! read more

Today I picked up my recently deceased father's belongings from this facility. His stay was relatively short by SNF terms, just over 1 month. As a healthcare professional myself, I am appalled at the lack of ethics and professionalism displayed by the staff from the administrator to nursing and even the therapists. My father's needs were not addressed. Not the aspiration pneumonia. Not his increasingly scary delusions. Not the multiple pressure ulcers/bed sores he developed while at the facility. He was starving to death and barely alert when I arrived for a visit from Texas. And as I stood in the entryway of the facility, staff placed 2 boxes of his belongings on the bench closest to the reception desk and left. No one offered their condolences. PLEASE DO NOT PLACE YOUR LOVED ONE IN THIS FACILITY! My father was living independently in December, was hospitalized in January and was placed there in February. My father was at Thousand Oaks Post Acute (TOPA) for approximately 3 months. My mother & I cant recommend TOPA enough. As my mother said, "the staff are like angels." They treated my father like family, always encouraging him, tending to his needs & always keeping him clean. The facility was always pristine. A special thank you to CNA Luna & Teresa, RN/wound care Dani & Mariam, they were so kind & caring to my father. The PT staff, Brian, Brian, Jonathan & Luka are amazing & got my father to his next step to Los Robles Rehab & he will be coming home in a week after 6 months in various hospitals. This place is so AWFUL! My kids grandpa is there now. We are doing everything we can to get him release from this place. The day he arrived they left him alone in the bed. No one knew if he had dinner. I was with him when he told me he needed to use to the restroom. I ask one of the staff to help him, because he had no walker to wheelchair and the nurse told him to go to the restroom on his diaper. UNDER NEW MANAGEMENT !! Dirty tables and night table i have to use paper towels and soap from the bathroom to wash it . New owners want to make LOTS OF MONEY- they are going room to room and if it's zoned for 4 beds no matter how crowded they are they are putting 4 in that room! No cares to if the patients get along if they are yelling just fill the beds ! Big turnover in staff a lot of new people. On long term side no TVs unless you buy your own and hope there is a place to hang it. No Wifi for residents to use either. The bathroom in my moms room is also dirty not sure what happened to the cleaning crew here, but they are lacking BIG TIME. They also use to have a "family room" where families can reserve it for special occasions, and residents can go sit and visit with loved one ect...... I noticed they emptied it out and it's a office right now! So sad literally no place to have any gatherings at now. I mean they have a patio outside but it's really really hot so not a good place to sit and visit . Another problem each bed has like one outlet so if your loved one has a phone or tv and other machines that need to plug in you will find that you need navigate around that. Since my mom lives here I'm hoping they get there stuff together and realize the safety and well being is FIRST priority!!! read more

All the staff communicated with me through my my father's stay. It was a full team approach to provide care, therapy and stimulating activities to make the stay as comfortable and pleasant as possible. I cannot tell you enough how amazing our experience was with Thousand Oaks Post Acute!  My grandmother spent 4 months there and they made what is usually a difficult time as good as possible. The staff there were all so friendly, accommodating, caring and helpful. The Social Services Team went above and beyond to help us with all of our concerns and needs as the family of a patient there. She organized monthly meetings with the nursing dept., dietary dept., therapy dept., activities dept. to give me the update on my mothers progress as I requested.  Social Services was always reliable to give me any information that I needed and even offered me assistance that I didn't even know that I needed! After speaking with the Administrator, she actually recommended I tour different Board and Care and assisted living facilities so I know which one will fit my grandmother's needs (I had never heard of board and care facilities before and this was one of the most helpful recommendation ever). When my grandmother left Thousand Oaks Post Acute we ended up placing her in a nice board and care that she loves. She is comfortable and it is perfect for her. The Social Services Team has contacted me a couple of times since my grandmother left just to check on her! I feel so blessed that my grandmother was there, and especially during COVID.  I knew that she was in good hands and was being well taken care of.  The Activities team was also so good to my mother helping her make fun crafts, etc but she also helped her to call me on the phone or video call with the facility iPad just to catch up!  All the staff members were just so helpful along the way! 10 YEARS LATER, I AM THANKING YOU WITH A YELP! When I was choosing a SNF for my aged Grandmother, I visited a dozen or more places in the Ventura/LA County area. I believe my grandmother was there for nearly 7 years. As she aged and her health declined, slowly at first...and then quickly worse in the last few months of her life, the entire staff was lovely and supportive at each stage. While anyone looking needs to do their own review and choice, I can't say enough about my experience as a caregiver...just how wonderful the care was for my Grandmother, how clean the facility is kept, and how patient and loving the support staff was to me. They earned my trust and were there in difficult times. Give them a chance to host your loved one, review their facility, meet a nurse or two, and do your own evaluation. I recommend them highly, and thank them so much! read more

Update a year and a half after the ownership change. I didn't realize there was still a bad review from me here. I've upgraded it to four stars. I'm happy enough, they are dealing with covid fairly well, I think. It is hard to get in touch with them sometimes but they are incredibly busy right now. Update six weeks after ownership change: The review will remain one star. Update: I've changed my review to one star because the facility is under new ownership as of 05/01/2019. I'm not happy with the way the new ownershandled staffing at the outset. I'm also less than thrilled at the changed soap and paper towel dispensers. I can only hope that it gets better. I'm having some issues with getting the new staff to look in the records for a doctor's standing lab order. I can't help but think what is happening with the patients that don't have someone advocating for them. I didn't give a review until seven weeks when mom first moved in, so i will revisit this review in about six weeks. We'll see if the facility can earn the stars back under new ownership. read more

My mother stayed at Thousand Oaks Post Acute for approximately one month after an episode of pneumonia. I am thrilled to say by the time I took her home she was up and walking again. The care that she received afforded her too be able to leave the facility and return home to resume her normal life at 97 years of age. From day one, the admission staff, namely Chelsea and others were instrumental in moving my mother from one room to another upon my request. So thankful. Secondly, the social services group, namely Soledad and others attended to my every need and request. They were very supportive and caring. The nursing staff worked diligently and attended to all the patients' needs and requests. My interaction with the nursing staff was nothing short of much appreciation. They were accommodating and supportive. The dining room staff was wonderful. They worked often understaffed but never neglected their duties to the patients. The meals were prepared according to the design for each patient. Kudos to the chef. My mother's meals were appropriate for her needs and health. Physical therapy did their job and some. I watched my mother go from not walking to walking with a walker. Collectively, all have been instrumental in my mother returning home, and with much gratitude I say thanks to all. Everyone was very pleasant, patient, caring, and supportive. Thanks for all for giving me back my mother in such a special manner. God bless all!! read more

We first went with a facility that had a higher rating than this one. Big Mistake! We should have checked out the facilities before my surgery! But time did not permit. As much as we appreciate and rely on Yelp reviews, if possible, start with the Yelp reviews and then best way to finalize a decision of this magnitude is to physically go to the facility and ask for a tour. After 3 disastrous days at the first local facility, my husband had me laterally transferred to Thousand Oaks Post Acute facility. What an incredible improvement! The Thousand Oaks Post Acute staff were responsive day and night to my needs, didn't mess up my medications (actually they fixed the mess caused by the first place), never forgot to feed me a meal and never ignored when I pushed my call button like the first place. They were all very kind, understanding, respectful and attentive. I have special food needs and they took it very seriously, even went to the store to make sure I had what I could eat. My room mate had totally different medical and food needs and they treated her with the same respect to ensure she would recover to the best of her ability. The staff takes their roles very seriously and consider the patients their responsibility. They kept the patients and facility clean, constantly removing garbage to eliminate smells, helping distressed patients to reduce potential angst, delivering meals on schedule. They were always available to solve perceived and real problems, and trust me when it comes to sick people there are both. I could not ask for anything more. Wish I had started with Thousand Oaks Post Acute from the get go. I could see each day I was getting better, making progress. I actually was able to leave sooner than the initial expectation. I attribute that to them. I highly recommend the Thousand Oaks Post Acute facility to anyone who unfortunately needs a care facility to recover or reside. read more

I entered TOPA on 5/26/21 and screamed at the top of my lungs on 7/13/21 to get out of there! They are horrible. DO NOT EVER GO THERE! They give you a call light when you need help, but no one comes. I would sit in my urine and feces for 45 minutes to an hour. I received 2 bed baths in the 6 weeks I was there. I was told they would help me with my diet and when I got there they said everyone gets the same meal, just don't eat what you don't want. The doctor changed my medicine and had my BP and heart rate jumping all over the place and did not show for any meetings I set with her. When I needed a bed pan someone would come into the room and tell me they were just getting off shift, I had to wait for the new shift to come in. I truly did not believe if I really needed help someone would get to me in time to save my life. Then to make things even worse, they billed my insurance for $46,000 for the 6 weeks I was there and NOT provided any care! If you value your life, DON'T GO THERE! read more

They sucked up all the medicAre and I basically got no help to exercise and walk dont even consider them. I had a brace On my leg a month too long PT pT is ridiculous and administered by basically incompetent indi idual longer than was prescribed bty the doctor. And even when they found out from my calling it to their attention They stilll forced me to wear it. I wS far worse than when i went in. Beware of this plCe. No management that cares. Just want patients for grabbing more money. Level of cate is highly questionable. A few tried to help those with real needs Also beware of Atria corp uts budgets at outlying facilities so low that assisted care is refused a lot with excuse they dont have time to care for residents as they dont have enough people working. DO NOT TAKE YOUR LOVED ONES HERE! This place made my husband worse. The only people that helped were the physical therapists. The nursing staff is terrible. They are lazy and the night staff refuse to take care of my husband who was on oxygen. They never checked on him and some nights his oxygen/cpap machine came off and was never put back on. The staff never checked on him. We had to take him out and return him to the hospital due to bed sores and infections which were NEVER treated by the nursing staff. They always just the sores for someone else to deal with. I am furious with this place! Horrible management!!! read more

This place is not posh by any standards, it's not the best I've seen, but it's FAR, FAR from the worst.  The nurses and aides are very sweet to my dad who's been here for fourteen days now.  Chelsea and Yesenia, from admissions, are especially cheerful and helpful. They truly made the transition so easy for my dad and for me.  They have a physical therapy and occupational therapy department and dad's been working out almost everyday.  They have a hair dresser that comes in every Tuesday.  They have an outdoor patio that is beautiful with fresh flowers and trees.  They have a newly paved parking lot and plentiful street parking.  The food smells good and isn't bad at all.  They honor all kinds of dietary preferences.  They also have Activities for anyone who wants to participate. Mario from Activities is great! He is lively, cheerful, and always thinks of the most creative activities! I've read the other reviews, and I understand where they're coming from.  There are some patients who are long-term and short-term - who are in various stages of Alzheimer's, dementia, or just old physical pain.  There's some yelling for help from time to time.  But this happens in all nursing homes unless the patients are very heavily medicated. It's good that the patients are not heavily medicated. They're old and they have to be here. The care that this facility provides is far beyond my expectations and they definitely care about all their patients. The staff always has smiles on their faces and this is a tough, tough job.  I appreciate that we have this place, with skilled nursing care for dad in his transition from hospital care to home. When you have to place a loved one in a nursing home it is a tough decision and you do not have much time to do your research. My mom fractured her pelvis after a fall and she need to go to a skilled nursing facility for therapy and recovery.At first I thought I made the wrong choice, because the rooms looked like it was from the 70s and the entrance is not attractive. But, this is a GREAT facility. They are in the process of renovation, but COVID put a pause on it. The staff is very caring and the facility is extremely clean. My mom was there for about 8 weeks. I was able to visit her 6 days a week and I was able to really get to know the staff and residents. They provided very good care. They have great COVID precautions in place and prioritizes patient safety at all times. All visitors have to be screened upon entry and a proof of vaccination has to be provided. They also proactively communicate with me. I would like to acknowledge the physical therapy department, nursing department led by Emy and Jane, housekeeping staff, kitchen staff, maintenance, social services, and admin staff. They were very caring and always made sure that my concerns were heard and my mom was taken care of. They were all great.If you must go down this path with a loved one I highly recommend Thousand Oaks Post Acute. read more

I have this place a great review when my mom first went there. I have to say they have completely changed. MY BROTHER AND I CALL EVERYDAY TO TRY AND GET UPDATE ON MY MOM NO ONE WILL CALL US BACK. I have emailed nothing. All they say is we will have head charge nurse call you and no one EVER DOES. I have put in call to Doctor as something is amiss with my mom and no one there will help. read more

BEWARE!! These people did not catch my mom's sepsis and she died!! She was showing symptoms of sepsis -- sleepiness, clouded consciousness, confusion, hallucinations etc. and they did not take her to the hospital or even call us to ask if this was normal for her. She had been transferred there after having heart surgery and they only checked her vitals but did not check for any of the other signs of sepsis. The early warning signs are often psychological and the hospital is only a few blocks away! They did not transfer her immediately and by the time her blood pressure dropped and they transferred her she was in severe septic shock and later died. She also had a UTI that they did not treat with antibiotics and do not have doctors checking regularly. They would not let us visit her because of COVID so they should have been monitoring her more closely especially after heart surgery. We are devastated. Our beloved mom was only 75. read more

After visiting 3 facilities to bring my mom to after a short hospital stay from a fall, I chose Thousand Oaks Healthcare Center and I am so glad that I did. I chose it because of the warm and inviting vibe I felt when I walked in. It was just that for the 20 days that my mom was there. We had a great experience. The room was comfortable with a nice big window. The food was not so bad either. The nursing staff was diligent and very friendly. They made my mom feel loved. The Physical Therapy department and therapists were amazing! They worked with my mom everyday and gave her something to look forward to. Apparently, my mom "rocked" at pt. The administration staff was also very helpful and answered all my questions. The process played out just like they told me it would when we first came. I would not hesitate to use Thousand Oaks Healthcare Center again if needed. read more

My brother is in this facility TOHC. They advertise a class A, five star nursing facility. Today I tried to speak to the director of nursing and she was horrible. She also said that they accept anyone and they have that right to be there. Anyone!!!!!. I tried to tell her about a troubled patient who just went off on my family members by moving a stool from the corner of the room and started to swing his fist almost hitting my sister. This director of nursing would not even listen to me, take names or wonder if everyone is doing well. Immediately she said she is calling their attorney's office so they can call me. She stated that they have a right to admit any patient their board approves. To me it looked like they admit almost every patient with the bottom line being their motivation. All I was asking from her was to take measures to not mix angry patients with the general population. Now I'm angry of the way she treated me and will definitely meet their attorneys along with my attorney. I would like to also say that for the most part the nurses, attendants, and the general staff are great and care a great deal for the patients. My problem was based on last night's events and this morning's handling of that situation by the second person in command. Please let me know if you'd like me to clarify any part of this for you. I have also talked to a few of other patients in this facility and they have had the same type of patients in this facility as I have described, but a little afraid to say anything. Well, we shall see how all this resolves. Very unfortunate that our elderly have to live in a place and have to confront such circumstances. Just not right or fair. read more
