    I would give CNAs, LVNs RNs 10 stars. Activities 3 1/2 stars Kitchen 3 stars Administration 1 star Physical therapy Dept 1/2 star If you or a family member are here for care and a little therapy to move around after a fall or surgery you'd do okay at this facility but if your going after a major event this place is not for you. I've been a resident here for over 2 years. The physical therapy Dept is ill equipped to take care of much more then walking you around until your injury heals. Make sure your family members keep on eye on your progress. This therapy Dept. Will go against your doctors orders look you straight in the eye and tell you this is what should be done. CNA left bed rail unlocked. When she rolled me off my bed pan, she rolled me out of bed and onto floor. Broke my hip. I had to have a hip replacement. When I went back to TOPA they were supposed to get me out of bed and standing....per surgeons orders, therapy 5 days a week, sit beside, use Hoya lift to get in shower chair and on commode. This physical therapy department looked right at me and said this was not the doctor's orders. The most they did was put me in a wheelchair to sit in garden half hour and put me back in bed. What was really going on? They're trying to find out who was going to pay for physical therapy. They broke my hip. They were responsible for my physical therapy. Instead they left me laying in the bed with some range of motion. Shame on this physical therapy department. And the doctors they come in spend maybe 3 minutes with their patients and leave. My doctor would not listen. When I told him I couldn't breathe. It kept getting worse and worse. He told me I needed to lose weight. Put me on appetite suppressant I ended up with pneumonia in the hospital. Do not trust a physical therapy department. Do not trust the doctors. Keep your outside primary care. Discuss things with them. The CNAs here work above and beyond what they should have to sometimes 12 patients for one CNA. They expect them to bring food trays, shower patients change bedding get ready for appointments. Get ready for people returning from appointments. Deal with family members. Deal with patience getting out of beds that shouldn't be standing point out. Tubes falling on the ground they are asked to stop what they're doing to go work in the dining hall. Then come back and work with their patients. Too much work for one person that you CNA is come back time and time again and work their butts off. You can always count on your CNAs

    2 janky TVs, 3 beds. Also, it's so noisy you have to blare the TV if you want to hear it.

    I've had experience with at least 4 local care facilities at this point, when I say I am appalled by this place I'm not judging them by impossible standards. The common areas look nice and new, but the patient rooms are disgusting. They pack 3 beds into rooms that are sized for 2 beds. It's loud and chaotic. There appear to be a lot of staff on duty when you go in, but not a single one of them asked me who I was or what I was doing there. My loved one started having anxiety attacks on the 2nd night there. If you have to send a loved one here, be vigilant.

Call button consistently went unanswered for over 25 minutes. Food trays left out of reach and spouse was diagnosed with malnutrition upon discharge. As the direct result of a 10 day stay here, my husband lost every gain that was achieved at Los Robles East Campus Acute Rehab. Note to East Campus Discharge Planner: concentrate less on your ensembles and more on speaking the truth about T.O.P.A. REPORTED VIOLATIONS- 1) "9:30 p.m. when Good Samaritan observed Resident 2 standing in the middle of a four lane road in front of the facility with a purse wrapped around wrist and a walker...you couldn't see the Resident because it was so dark..." 2) "Yesterday patient was found by the church. A lady walked in the door with Resident 1 and stated she saw Resident's name on her wrist band. The lady also stated, Resident was confused and did not know where (Resident) lived." "Resident was observed to have a urinary catheter...the treatment nurse confirmed there was no documentation for monitoring signs and symptoms of infection for 10/21, 10/25, 10/27, 11/7 and 11/9/21 in Resident's record." "Medication Administration Record indicated: Give one (1) tablet by mouth every four (4) hours as needed for severe pain 7-10/10...A further review indicated staff administered the medication to Resident...for a pain level at zero (0)..." "...the facility failed to ensure a medication cart and narcotic storage boxes were locked, secured, and inaccessible to unauthorized staff, residents, and visitors when one of five medication carts was left unlocked and unattended in a resident hallway." "Review of the past three months Resident Council meeting minutes indicated the following issues: call lights not answered timely, staff turned the call lights off without giving the care needed; wandering resident entered multiple rooms uninvited; missing items; food was cold, no flavor, rice and beans were undercooked, open faced sandwich were served closed and meat was tough."

    Most unprofessional, understaffed facility I've experienced. After my knee replacement on Feb. 2024 the hospital transported me there as I live alone. I was not given a ID band until the 3rd day. Shared a room with another person who was hard of hearing and wouldn't wear their hearing aides and had a lot of visitors any time of the day. There was a "thin" curtain between us. I rarely got my pain meds on time and there is no "in house" pharmacy and the weekend Rx came whenever the "person who handles them" came on their shift. I was on drip antibiotics and the tubing filled with blood several times and they would put wrappers on the foot of my bed. Sometimes it would take l/2 hr to 45 min to have them answer the call button. Because I couldn't walk to the bathroom they put a bedside toilet by my bed (without a cover) and I would have to ask them to please empty it when I had a BM. After a week and a half while I was throwing up I asked them to call 911, and the ambulance took me to Los Robles hospital where they literately saved my life. While in the ER my blood pressure raised over 200 due to my pain. When I asked a friend to pick my belongings, they didn't have to sign in and no one even talked to them, They just went to my room and bagged my things up and brought them to me. Through the help of prayer, the ability to be my own advocate I'm grateful I didn't die. SM 85 year old patient

Then my lower back got involved, and that caused hip bursitis to crop up. The latter I hadn't had to deal with for a number of years. Not too happy about having it flare up! Went to an orthopedist, who recommended physical therapy after doing diagnostic testing. I initially started going to a PT practice that I'd been to in the past, but they had downsized during the pandemic and I wasn't happy in their new facility. Too many patients in a small space. Too noisy / no privacy. Dear Hubby had just started PT for his own issues. He'd been referred to Westlake PT's Thousand Oaks location by his orthopedic surgeon. DH was so pleased with Bernie and his team that I wanted to give them a try, too. Although physical therapy isn't "easy", I feel very comfortable and cared for at this clinic. The staff all seem to be professional, while also being friendly and compassionate. Those of us who are seeing them to recover from injuries, surgeries, or just an accumulation of years' lived which is affecting our joints can appreciate this gentle, yet effective approach. It's a pleasure to start off with a session of heat in a private treatment room, followed by a targeted massage. Then it's out to the gym area for stretching and strengthening exercises. There's music playing, but at a nice volume level; not too loud or too soft. That encourages movement and an upbeat mood. Everything appears very clean and well-maintained. That's very nice to experience as part of the overall event of leaving the house for supportive medical treatment. Bernie is easy to talk with and go over questions which arise about my situation. He has a good team surrounding him and making this PT practice one to recommend.

The reason for my visits were to help prevent debilitating episodes of occipital neuralgia (ON) from reoccurring and to help address some recent C3-7 level sensation changes that could be related to disc herniation and some spinal cord stenosis. Initially, I was biased toward getting doctor recommended PT at the venerable SCOI - UCLA because of my overall experience with other UCLA providers and the fact that treatment, scheduling, documentation are easily accessed through the UCLA Health patient portal. Unfortunately, availability of the first appointment was many weeks out. Hence, I opened up my search for other nearby providers and found Omega Rehab & Sport, who by my first internet impression, seemed worth looking into, they were in-network with Blue Shield CA PPO, and were essentially immediately available. My initial consultation was with Dr. Sevak Khodabakhshian, who instantly recognized my symptoms. The consultation was very informative and confidence inspiring. I then proceeded to a comprehensive baseline exam with Dr. Zach Fulks so he could assess my condition and map out some parts of my recommended treatment plan. After he got my imaging data, he was able to recommend more and fine tune the treatment plan based on real criteria. Zach was extremely efficient, always had the answers to my questions at his fingertips, provided constant encouragement, and was a great conversationalist to boot. My treatment settled on chiropractic adjustment, soft tissue work, strengthening of certain muscle groups to improve my posture by an evolving set of exercises, cervical decompression, and a wind-down session that were mostly warm packs and e-stimulation to temper post-treatment soreness, which ended up being minimal to non-existent. The final assessment showed large objective improvements in my head/neck range of motion and I definitely felt the improvement; the reduction in occipitalis tightness was very apparent. I told Zach that since 2020 to before treatment, I had recurring episodes of ON but felt confident I would not have another after my treatment at ORS. I'd like to also thank numerous other assistants and staff: Grace, Karla, Jason, Majedeh, and front office staff Juliana and Joanne. Communications/awareness among assisting staff of my treatment was perfect. For example, it's amazing when I am about to transfer from/to my wheelchair to/from a bench for cervical decompression, 2-3 staff members instantly congregate around and help me get situated before dashing back to their patient; no call for help, it's just automatic. More than one staff member are checking on me every few minutes. Everything meshes and it's no wonder I mostly see the place with plenty of patients still coming in if not already there even at the tail end of the work day; and all are in a great mood.

Chiropractic. I need Physical Therapy. I fought like I was being buying a car and pushed into it I was there for physical therapy. Hes a chiropractor. The front desk booked me for Physical Therapy. My pain was severe. He was rude and out of line. I do have a long voicemail apology from him but that comes across unnerving. For any chiropractor to yell at a patent is not ok! He says he has problems. We all have problems. He needs serious anger work. He scared the heck put of me texting etc. I am 72.I never got Physical therapy, just the chiropractor that owns this placw,calling me honey. Again I am 72 and gone elsewhere.
