The problem is that the mount pulled free of the wall (anchors didn't hold). Chris did a fine job with the initial install, but I have been unable to get him back to fix this issue and it has been weeks, I'm going to remove the mount and go back to just having the TV placed on the mantel which isn't ideal, but waiting 3+ weeks with poor communication to address this problem is just bad service.
