    I've seen worse facilities but this place has many issues. It would seem obvious that people here…read morefor rehabilitation would be up and in their wheelchair in the morning so that physical therapy could work with them. However what really happens is that excuses are given and the patient is fed lunch before they are ever gotten up. Many times it is by therapy staff because nursing and techs are too busy. No schedule appears to exist for patient showers. Shower chair type wheelchairs are in rooms with showers. However the chairs do not fit into the showers. This results in staff preferring to do bed baths as opposed to an actual shower. Patient drugs that have been working for the patient for years are discontinued and replaced with other drugs. No notice to the patients or their family is given. When it comes to light that a change has been made, no specific explanation is available as to why the changes occurred. The staff only can provide theories as to why these things generally occur. Patient call lights are frequently ignored or someone comes in to turn off the light and tells the patient that they will tell the appropriate staff member. However patients end up sitting in their excretions for over an hour while techs sit and play on their cell phones. Physical therapy area has broken equipment that is necessary for the majority of patients. However instead of leasing equipment until replacements can come in, it is explained away as the replacement is on order.

    Actually want to give O stars…read more When admission came in on Monday they NEVER asked for insurance cards & I received a bill for over $5,000 for room & board for the whole month of January dated 1/5/2023. Received in Dec 2022. He was transferred out & back to hospital the end of Dec & spent NEW Year Eve & day 1/1/23 in hospital NOT in rehab center My husband was in for rehab It was the worst care I've seen. He 1st arrived & with the Director watching an aide manhandling him to change his bed even though he had just had back surgery & a head injury ; the Director said nothing to the aides to be gentle & then badgered my husband to talk to her when he was in a partial focal seizure as he couldn't talk & signed to me the room was spinning that he couldn't talk & she told him she knew he could talk & didn't want me answering for him. She keep badgering him to talk to her The Director demanded respect of their workers as they SUPPOSEDLY (LoL) her workers respect the patients. Then 1 night he's calling the nurse station saying he was having chest pains & they obviously didn't take him seriously, he called 911 & was transferred straight to the nurses station. He had chest pains again the next night & thankfully he had his cell phone & called me at home It took 5-10 min before the nurse station answered the phone when I called. I got a call 30 min later from nurse saying they were sending him back to hospital. ER dr said he was having multiple blood clots in his lungs, which could have killed him if moved to his heart. They also have a limited menu & EVERY person gets the same meal. There were even times they with held & refused to fill his water pitcher or get him anything to drink. "which is neglect in the states eyes." There is a report going to the state on neglect & DePaul social services is aware also. & says they are pulling them from the list to refer people to. They sent out an Email to families they had an ACTIVE case of COVID in the building & instead of going on lock down, they still allowed visitors in the building. Figured as long as you wore a mask & a face shield you were safe, but Patients were not required to wear protective mask or shields Phyllis M.

    Our NIGHTMARE started the first morning, my mom called me crying at 11:20am and said nobody got her…read moreout of bed yet, no meds, no breakfast. So of course i was so upset, ready to come up there and get her up myself. I called and talked to the front desk. Eventually, she did get out of bed maybe an hour later. When we inquired about the reason she was still in bed at 11:20am, we were told, We didn't know her preference."!We heard this statement many times during her stay. She was left in bed many times throughout her stay there and they always had the same excuse. Seemed aggravated we would even be concerned about it. There was not one week there wasn't an issue. From, CNA's being verbally abusive to CMT'and nurse's not giving her medication, nebulizer treatments and insulin, her not being checked on regularly during the night. Not receiving her dinner, to which we were told she received it and it came back untouched. No accountability at all, and i mean NONE. Every-time we would ask, Beth the director of nursing would say, "We have to educate staff." EVERY TIME. She was not given her textured altered (pureed) diet. I was so worried she would choke. They did not care AT ALL. Mauri, the day nurse supervisor never had a moment to address anything and always called my mom by a wrong name. She was mean to our mom, even other CNA's told me she (Mauri) "couldn't care less about your mom." So many incidents. She had scratch marks on her one time, and again we got"we will educate the staff." One time she hit her head on the headboard of the bed because a CNA threw her legs into the bed and no one let us know nor seemed to care. They seemed more aggravated that we were upset about it. We eventually installed cameras in her room, and that did help. We still had incidents though, and not minor ones. She had a CNA yell and pointed in her face, and yelled at her and told her to "eat your food and drink your drink" then walked out with my mom crying help me help me, again, we heard, "we will educate the staff." And also, the director of nursing, Beth went in further to tell us "she was not working with your mom that day, just helping out passing out trays and didn't know her". What!?'! So that makes it ok? The executive director, Michael, tried, i will have to say. I am sure i was a thorn in his side because I was in his office at least once or twice a week. He tried to rectify the problem.He would listen and generally seemed concerned. The problem was his directors didn't help him solve anything. The social worker had an attitude most of the time and was terrible at follow through. By the time my mom received her communication cards, she could no longer use them. I felt like i was always needing to fight just for adequate care for our mom. My mom had some fantastic staff, particularly, Dana, a nurse who worked in the rehab section but subbed on the nursing floor, was exceptionally great, and Lucy one of the overnight CNA's was good and my mom liked her. She had others that were good also, Victoria was another great CNA. It was my mom's home. She passed on June 13th, 2025. I did a lot of the CNA work (not just during her last days but the whole time she lived there)., when hospice came in, it did help, although when they were not there, I did the CNA's job most of the time. I needed to, otherwise she wouldn't have had the care. I rotated her, wiped her bottom, changed her, fed her, all things the CNA's are paid to do, but i needed her last days to be good. There were times i would go try to find someone to help me change her or rotate her, but to no avail, so i did it myself. The whole experience was horrendous. They were always saying to us they needed to educate staff, so hospice said they would be happy to inservice staff about ALS. They never took hospice up on that offer nor would they listen to the family when we tried to explain her needs. We appreciated the executive director coming to her room on what would be her last day, and ask how things were going and made sure we had snacks and drinks. But no one and i mean no one from administration ever offered their condolences, through any means, email, card, text, etc. No one. My mom paid $350 a day (private pay) to live there. Not one person from administration ever has contacted our family and said sorry for your loss. What does that say? Their were CNA's that offered their condolences and one nurse actually came to the wake. She became like family to us and we were so appreciative of her care for my mom and that she took the time to come to her wake. We didn't expect administration to come to anything, but we did expect at least a quick email"sorry for your loss." Please please don't make the same mistake our family did. Really reconsider if you want to place your loved one in this facility. You will have some good days but mostly horrible days that you will worry constantly if your loved one is safe or not. -The Kuhlengel Family

    I cannot stress how poor the care and lack of response from the administration was at this…read morefacility. The staff was not able to care for my mom's needs. For the one week that she was there, multiple trips by family members were made daily to assist her, a minimum of 3 hours each day. Witnessed staff sitting in their cars in the parking lot apparently getting high for extended periods of time. No response from administration after multiple outreach attempts made when it was clear they could not provide the care that was needed. After hospital transfer, mom went to a facility that was a higher level of care. This Facility looks nice but that's it. Awful experience at a very vulnerable time!

    On move in day (2/28/24) I was given an invoice that stated "Charges in Full" at the TOTAL DUE line…read more I wrote a check and they deposited it accepting as payment for 2 days in February and all of March. However, on my April Statement I saw a new charges for both Feb & March in the amount of $1,924.15. I then reached out to the admin staff inquiring as to what this was. I was informed that they increased her care level around 3/19/24 and I now owed this add'l amount even though not one person reached out to me. I have been disputing this ever since because when I pay a bill that says "charges in full" I believe that that means I have paid the bill to a zero balance. How can they justify increasing anything and back-dating it?? I have been paying the increased amount on April - July care months. Yet they continue to add late fees to this bogus add on bill. Now the newest thing is that they have neglected to reduce her med management fee on her monthly statement even thought she now qualifies for the 1-5 pill $300 charge not the 6-10 pill $400 charge. I have verified with the nurse practitioner that my mom now takes 5 daily meds not the 8 meds she was previously taking. This should've been adjusted on the June 2024 statement but it still hasn't been done and I just received the Aug 2024 statement. I have been deducting the $100 med management reduction from my payment(s) for both June and July....they have left those amounts on our account to accrue even more late fees. This is just scandalous. They are all about money and getting all they can from the families with loved ones living in their care. It's not enough that my mom has had multiple falls, gotten a boil on her upper thigh but when she was sent out to Mercy Hospital ER for aggression she arrived wearing multiple diapers so that they wouldn't have to change her as often. That is just inhumane and disgusting. I would not ever recommend this place to anyone. It looks fancy n pretty and they all seem nice.....don't be the fool I was.....they saw someone in a tight spot and they took advantage of my situation. Due to my mom's previous facility ceasing their Memory Care services I had 28 days from notification to move out deadline so I did not have the luxury of time on my hands. Unfortunately, I again find myself in the same situation just a mere 5 months later.

    Used this facility for my father when his Alzheimer's started to progress. They have 3 units based…read moreon their needs. Level 1 I would rate 4.5 stars. My father was still high functioning. He participated in a lot of the activities. Level 2 I would give 3.9 stars. When my father was moved to this unit I realized they don't have enough staff to keep the residents engaged. It's at this level that each resident needs more care but there isn't enough staff to deliver that care. This unit is locked so residents don't wander the facility. Level 3 is downstairs and is a little chaotic. I give this unit 3.1 stars. Not enough care for the needs of every resident exists in this unit too. When you take the tour I recommend spending 15 minutes within each unit so you get the feel for the level of care each resident receives. I do like the level of security they have in place and how clean they keep every room.
