Warning: This review applies only to The Rivers Physical Therapy Wing (NOT their Independent Living, which is excellent). If you love your family member, DO NOT let them stay here. The food is miserable, and the care is worse. My mom was left for hours when she needed cleaning, which caused a severe rash. By the end of her two-week stay, she looked so neglected that I wanted to cry -- she left in far worse shape than when she entered. [[HIGHLIGHT]]Yes, there may be a few caring staff members, but overall the culture felt dismissive and bare minimum.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]Most of my requests were ignored.[[ENDHIGHLIGHT]] It was heartbreaking to watch. The day after leaving, my mom already looked 1000x better. That says it all. This facility is, in my view, a dangerous place for vulnerable people. I toured the facility and interviewed some staff. I also interviewed a few people with loved ones there. It might be ok if you are sharp but I wouldn't put never consider it for a dementia/memory care patient. It was quite the soulless place. Be warned, this facility we had to get our mother out of it. She got a UTI there and had to get her to a hospital after delusional activity that they basically ignored after family saying it was not normal. This place is dangerous, we were making her breakfast and bringing it to her, they would not even give her a 2nd tiny box of juice saying it was not allowed, She was not regularly cleaned we had to try and do this for my mom as they were to busy. [[HIGHLIGHT]]She stayed in a wheelchair one day for hours, the nurse got loud and nasty with me I had to hang up on her.[[ENDHIGHLIGHT]] The call button rings all day and night as we were there for breakfast, lunch, and dinner, plus! [[HIGHLIGHT]]I called Dr. Seth Parker for two days in a row with no return phone call[[ENDHIGHLIGHT]]. It looks good on the outside but once you get in for a day you see the cracks. Beware! Please read this... 10/29/23 Sunday, Dr. Seth Parker told nurse Jasmine that the only way they would call EMS for my mother who needed care that they could not give her, would be if we signed her out AMA (Against Medical Advice). That's illegal. You can't prevent someone from obtaining medical care. They were unable to get an IV or blood draws for more than a week. Her blood pressure was extremely low (systolic of 80... that's the high number). AND they let this go for a week and only when I was pushing to send her to the ED did Jasmine tell me. My mother could not take oral medicine. AGAIN this is illegal and not everyone knows this. Dr. Parker knows better and I'm sure he bullied people into staying, but I know better because I'm in medical field. Why would he do it??? -Protects the statistics of Rivers. Medicare looks at re-hospitalizations and pressure ulcers (my mother's steadily worsened under their care). TAKE A LOOK THEY HAVE A 4/5 STAR RATING. SO MANY OTHER FACILITIES HAVE 5/5 -Also, he was forcing us to use their hospice service for which his company Bay Hospice is the only hospice available to provide services at Rivers. He's forcing us to pay him when we want other services. Which brings up other issues with Rivers. 1. Social Worker (Taylor) will not do her job... she won't provide you information for find another facility. All she did for us was toss an out of date pamphlet about facilities. She even admitted the names were not up to date in the booklet! Again this sets up a course of action that residents will remain there because everything happens so fast that you don't have the resources to make an educated decision. MONEY IN THEIR POCKET 2. POOR DOCUMENTATION / LYING- When discussing with the dietician my mother's health, she was unaware of that she had any GI issues and stated that nothing was written in her chart!! This means that they are essentially lying...IN COURT if they didn't document it, it didn't happen...so they are protecting their mistakes. Also, a charge nurse (not Phil....He did his job) lied to me saying that labs were drawn on my mom when they had not been done (a big deal because my mom was essentially dying from congestive heart failure at this time because her blood pressure was so low and staff were UNABLE to EVEN GET A BLOOD DRAW WHICH EASIER THAN GETTING A LINE) 3. POOR COMMUNICATION- Called Nurse Practitioner, Elizabeth Chaps on a Wednesday 10/18/23, left a voicemail and did not hear from her until 10/23/23. Nursing did not inform family of dangerously low blood pressures until my mother was taken by EMS to St John Hospital where she was immediately take back and placed in the resuscitation room!!!! Jasmine (the nurse said she had those blood pressures "all week") 4. DANGEROUS- (see above)For a week my mother was hypotensive (she should have been ADMITTED INTO THE HOSPITAL WITH THE VITALS SHE HAD) and staff was UNABLE TO GET AN IV. read more

If you have a choice to go elsewhere, please do. My 85 year old mother came here after falling and breaking her pelvis and hip late in 2016. The goal of The Rivers is supposed to be rehab while getting skilled care. There is nothing skilled about their care. They are neglectful, incompetent, understaffed and uncaring. You have to constantly monitor what is happening every day to make sure the patient is fed 3 meals a day, taken to the toilet, bathed, dressed, etc. Pushing the call button elicits no response. I frequently had to wander the halls looking for anyone that could help me. They would forget to bring my mother her food and I would have to go track down a meal for her. Everyone is always apologizing but nothing ever gets any better. The meals were atrocious and non-nutritious for seniors. Hamburgers hard as a rock, hot dogs, diet pudding (what?), iceberg lettuce, just horrible stuff. They should be ashamed of themselves. They would put her in a wheelchair and then just leave her there even though she would be exhausted and want to get back in bed, but no one would come to her room. I would come in and find that they had moved her so that she couldn't reach the call button. Same thing when she was in bed, they would put the call button where she couldn't reach it. I guess that solves the problem of them not getting buzzed. I had to go every day to monitor and harangue to get anything done and to ensure her safety. There doesn't seem to be anyone accountable for what goes on there. There is the 'director' but she's only in charge of getting you checked in (even that didn't go smoothly) and doesn't have any other oversite. Seriously, there is no one to talk to to address issues. They have a head nurse, but her and all the aides are contract employees and she can only document the issues. I checked my mom out as soon as she was able to walk on her own at all with a walker and arranged for PT & OT at her assisted living facility instead. And let's not forget the 19" TV bolted to the wall across the room with the worlds most confusing remote, also bolted to the food tray cart. I could barely see it from my mom's vantage point, how in the world did they expect a senior to see it across the room. No decor, stark white rooms. I brought stuff in like most people to try to make the place look cheery. I was surprised to see so few reviews, but I see there are a few that are not recommended that are all 1 stars, not sure why they are hidden. This place is a hazard and should be fined or shut down by the state. read more

My elderly Mother had the misfortune of being a resident within the Rehabilitation section at the Rivers of Grosse Pointe from June 6 through July 3. I was with her most every day for 8-10 hours, and in the few days I could not be there, one of my siblings stepped in. Our overall experience with The Rivers left a lot to be desired due to the lack of consistent, compassionate, and professional care for my Mother. It became obvious very early that the Rehabilitation section is understaffed in all positions (Nursing, Technicians, Aids). It is even worse on weekends. This is based on my 28 days and over 250 hours of staying with my Mother to ensure she was getting the contracted services as promised. I give them a grade of 2/5 stars; and it would be worse except for the fact that most of the personnel seem to care about their residents and take personal accountability in their actions in the administration of competent and compassionate care. The problems we experienced were troubling and systemic. Their procedures are inconsistently applied or not known. Several of the employees lack adequate training, or they just do not care. It would be interesting to know what the average length of employment is for staff, but from talking with them there were several that were new or had less than 3 months. In two cases, they were temporary employees from an Employment Agency and it was their first day at the Rivers. Often, there were different interpretations about standard procedure depending on which employee entered the room. This ranged from personal hygiene (adult diapers/barrier cream/bandages/compression wraps), kitchen/nutrition (inconsistency in specified beverages), medication dosing, application of skilled nursing and availability of resident health information. Worse, some staff members would not take responsibility and blame it on someone/something else (another Technician or Nurse, the kitchen, etc.) In one example, it was suggested we inform the nutritionist regarding Mom's beverage needs essential for food swallowing with every meal, which were then added to her resident profile. Seems simple enough, but nothing is simple getting things done at the Rivers. Even with this information printed on each meal ticket, at least once a day we had to ask this to be honored. I found out it is the Techs responsibility to dress each tray with resident's beverages as it was received from the kitchen. The blame game went on most every day for this and other issues. I could always count on excuses from some staff members, not the accountability and competent service expected. Response times for assistance of any kind were poor, consistently averaging between 5 to 12 minutes. Upon arrival, they would press a button in the resident's room console to confirm the request and silence the chime and corresponding hallway light. I grew accustomed to the regular sound of chimes while residents waited for help. I will refrain from calling out the staff by name in this review that were routinely dismissive and rude, of which there were three that come to mind. Two of them were nurses and were very difficult to deal with the entire time. Fortunately, the Physical/Occupational Therapy personal attending to my Mother's needs were very capable and kind. Thank you, Amanda, Star, Mike, and Bert. Also, there are several Techs/Aids that were outstanding. Thank you, Armani, Carmel, Erica, Thea, Tenisha, Catherine (Cat), and Miss V. The same for Nurses Sade, Shakira, and Jasmin. The Rivers needs more people like this. Finally, I would not recommend The Rivers Rehabilitation Facility to anyone, regardless of age, condition or the availability of a family member or loved one to spend the time necessary for the advocacy of professional and compassionate care. It was a daily struggle with their systemic issues, inconsistency, and quality of care. Quite frankly, it was not worth it as there are other facilities available. I wanted to love this place. My 98 year old father was a patient at the Rivers rehab center for about 3 weeks in Feb/Mar of 2023. The facility is located in an upscale neighborhood in Grosse Pointe, near a private school and across from the Grosse Pointe Hunt club. Don't let the fancy address fool you. While the location is nice, the staff, care team, food and overall atmosphere are all horrible. Everyone working there seems to hate their job. It felt to me like wealthy investors own the place and are squeezing every last penny out to make a buck. The extremely slow response times to call buttons indicated that they are very short staffed, or the care team doesn't really give a hoot about the patients. There is a general absence of leadership and lack effective communication amongst team members. Either leadership is oblivious to the nastiness of the care team or they tolerate that type of behavior. I would cringe every time I would walk in the front door. Family members are required to stay in the patient's room at all times. I once went to the nurse's station because my dad needed help and I was quickly scolded for leaving the room. The response was "ring the call button" if you need help. Either they were ignorant that the call buttons went unanswered, or they just did not want to hear any complaints from the family members. One time I stopped into the administrator's office with a question and was also quickly reprimanded by the grumpy leaders of the facility. In the era of covid I understand the tight restrictions, but if there was an adequate response to the patients call button, family members would not need to leave the room to seek help. Often times when I was performing care tasks that I want not trained for such as transferring my dad from the wheelchair to the bed or wiping his butt, a CNA would walk by me, turn the call button off and tell me what a good job I was doing. The good thing is that my dad was still alive when he transferred to assisted living, beyond that, I don't really have any good things to say about the place. The bad thing is that he didn't really rehab very much from when he went in. The ugly is everything else. Thank goodness my dad had family members there most days to help perform basic care because the call button would typically go unanswered for hours. The care team mainly consists of CNA's, many who are contract or temporary workers and a nurse whose sole job is to pass out meds. The CNA's main responsibility is preparing meals, passing out trays and collecting trays. Everything else is secondary. Too bad if the patient has an emergency during the lengthy feeding cycle because the CNA's are not allowed to interrupt the food service duties until all trays have been collected the cleared. The food in brought to a central preparation area and the CNA's and possibly food service workers prep the patient's meals. The food is often served cold and inconsistent with the patient's dietary restrictions. It a huge ordeal to have the CAN reheat the food or ask for something else to eat. The therapy seemed to ok but was really limited. During my father's stay I think he only got maybe a dozen hours of actual therapy. The rest of the time they were perfectly happy with him not moving at all. His goal was to get strong enough to live independently but that goal was not realized during his stay at the Rivers. This was our families first experience with rehab so its hard to tell if its just the Rivers or if the whole industry is bad. I suggest looking around before landing on this place. read more

My husband went to the Rivers for rehab in the summer of 2021,and although the rehab company was recommended, I couldn't wait to get him out of this facility. Fortunately, he was sent out to a hospital after approximately 1 week, (did not notify me) and he was never cleaned up during his stay. The staff apparently did not know he was unable to stand due to surgery on the bottom of his foot, so they would attempt to stand him up all the time! He had rehab a couple of times, and when returned to his room was left in his wheelchair for hours. His call light was never answered in a acceptable time frame, his room and bath were never cleaned, the hours were regulated and we could not visit on the weekends, the food was terrible, the director of nursing hid out in her office and I never saw her doing rounds. The facility did not know that I had been a nursing home administrator for 20 years prior to retirement and I cannot figure out why the Michigan Dept. of Public Health didn't close down this facility. I went through many yearly surveys in the skilled facilities I worked in, and still can't understand how they continued to operate. I hate to admit I was a member of the Grosse Pointe Woods Planning Commission when the proposal for the Rivers was presented. The nursing home part of the Rivers needs to be closed if no improvements have been made. read more

Management is truly unprofessional. Left two voice mails with the director, Chris P., and one detailed email, and we received no response. It's shameful. It's heartbreaking. We were a full pay room and board. At times, couldn't even get through via phone. "Messaging mailbox full." Inexcusable. Ophelia is a Godsend. This precious woman belongs somewhere where she is appreciated! She was the only bright spot. She is a caring and wonderful person. We also want to commend Heart to Heart hospice who were always supportive and caring. The Rivers management always seemed determined to "throw them under the bus," when THEY are the ones who should be called out for truly heartless conduct. My mother spent just under 3 weeks there, She chose The Rivers solely because it was close to home. PLEASE reconsider before sending a loved one to this facility. read more

The Rivers is a nightmare. We had a truly horrific experience when my father went there for rehabilitation after a fall and some fractured ribs. The staff has an indifferent attitude. They are not invested whatsoever in the patients. They over medicate them, most likely to keep as long as possible, and bill Medicare. Patients call buttons are often ignored. I witnessed this first hand on numerous occasions. Cleanliness is not a priority, especially when concerning the sheets and towels of the patients. They were not changed as they should have been. They were thrilled when they found out my father had supplemental Blue Cross in addition to Medicare. Go figure. I wanted him out of there after a few days. Their "Social Worker" quite rudely told me that he needed to stay 21 days. I asked if she had spoken to my father, his doctor, or the physical therapists and she admitted no. Turns out that's how long his insurance would cover 100% of his stay. Interesting. It was as if he was their "prisoner". I had a very challenging time getting him released, even though I was his power of attorney and had all the necessary paperwork. This place is all about charging insurance as much as possible, while "warehousing" elderly patients. At the expense of their physical and mental health. Do not trust them with your loved ones! read more

I spent 7 days here and had my daughter take me out. Aside from the rehab team, which is great, everyone else is incompetent and neglectful. When I checked in they didn't have my meds, so they were 5 hours late. My light wasn't answered, and the poor woman in the room next to me was screaming for help for 45 minutes. I never saw a doctor, despite requesting to twice. An aide told me she's worked there 8 months and only seen the doctor in the building 4x. My daughter and I repeatedly called the social worker and the calls went unreturned. The wound care RN had no knowledge of my wound vac, and was stumped when it stopped working. A CNA did offer to remove it and google how to fix it. That's when, on my doctors advice I left. read more

The Rivers of Grosse Pointe is separated into 3 facilities under 1 roof: Independent Living, Assisted Living/Memory Care and finally a Rehab center. My grandma stayed at all 3 areas over the last 3 years so I feel confident that I can give you a comprehensive review of all facets of the Rivers. First the good. Walking into the independent living facility is like walking into a new hotel. It is light, bright, has high end finishes and furnishings, and the friendly staff at the front desk are always helpful. The apartments have tall ceilings, high end finishes and are quite spacious. They are able to get a nice cable TV package. It is a very nice place to live as long as your loved one will never need any further care. While living in the independent living apartment, my grandma fell getting out of bed. There is no way to contact the front desk or anyone from the floor of her apartment. It was hours before the neighbors below her heard her cries and tried to find anyone working at the 5am hour. They found a cafeteria worker coming in for the day who helped call the ambulance. After her stint in the hospital, my grandma was sent to the rehab unit. This is very similar to a hospital ward except it has about 1/10 of the staff. It looks like a nice place, especially since there is a huge Physical Therapy Gym attached to the facility. You can hardly find anyone working there and when you do, they are clearly overworked and overwhelmed. My grandma doesn't like to be a bother and was largely ignored, even taking a fall while there as she attempted to use the bathroom. I do not recommend coming here to rehab. It was determined that my grandma needed some assistance (getting out of bed, other tasks that could endanger another fall) so Rivers has NO option for that in the independent living side. You have to move everything in their apartment to the assisted living side largely on your own. When you walk into the assisted living side, you need to sign in with an security guard. You need to remember to take a key card as the residents are locked in their own hallway. The staff is also overworked but less so than the rehab side. The rooms at assisted living range from a double hospital room, single hospital room and a deluxe suite which has an area for sleeping a little further away from a living area. But with no kitchen, no storage and a view that consisted of a swampy lawn backed up by a parking lot, it is not a nice place to live. No cable package is available and the TV is analog. No options to upgrade and you can't get your own cable system. Why does the administration feel that the assisted living people don't deserve decent TV? Food is included, if you like what they are serving that day, and you enjoy sitting in the eating area next to the nurses station with the continual beeping of the unanswered call buttons. Finally, after living in the assisted living facility for 6 months, the administration increased my grandma's monthly rent by 63%! Preying on the elderly is a shameful practice. Everyone gets older. Think about that before deciding to live at Rivers as you may have to move your family multiple times as he/she gets older. Do yourself a favor and find an Independent Living facility that has onsite help that can be added on as your family member needs future assistance. read more

We checked-out quickly. Here is why: 1. They didn't verify if our doctor had permissions there. Too eager to bill insurance for their guy. Awful they'd not consider this as we wanted our trusted family doctor. 2. After repeated requests to speak with their doctor, we finally saw one 6 days later. He was disorganized, rude, and too brief. 3. Typically waited 40 minutes to an hour for help. Inattentive staff too engrossed in their smartphones. I saw this repeatedly for other patents. 4. My mom didn't recognize the pills they were giving her. We had a tough time learning what they were pushing. They wanted her to take stool softeners when she had diarrhea. They wanted her to take iron supplements when not anemic. They pushed sleeping pills, though she didn't have issues sleeping. Just a way to charge insurance and put a patent at risk. 5. The meals were often served cold and seemed greasy. Any requests for minor variations (dressing on the side, smaller portions, etc.) was ignored. 6. Dust and flies everywhere. Questionable cleanliness. 7. The rooms are stark. Only one visitor's chair and no pictures on the wall. read more

One word to describe this place. Awful. Food they serve turns your stomach looking at it. Most of the nurses are cut and surly, the CNA's are for the most part are probably the lowest paid in the industry and they show why. The physical therapy team is first rate 5 stars but the overall operation is so bad that 1 star is all they deserve. Stay away from this place. read more

I read the reviews before my grandma came here, advised my family, but we still decided that she go here to the rehabilitation center after surgery because it was so close to the house...wrong decision. Nurses never answered her call light and they forgot to give her one of her medications. To say the least, she has decided to leave after two short days and go to the Fraser Villa, which has given us exceptional service with family members in the past. read more

The most alarming problem with The Rivers GP is that the developer, Rich Levin, set out to create EXACTLY what it is - looks good from a distance, but fails to deliver quality on all levels. Everything from the roads and cement to the building craftsmanship to the care of the residents was done with one goal: profits for Mr Levin. Shame on GPW for allowing this to occur. It's always true you get what you pay for, except that when Medicare pays full board, we should all be getting something better in terms of care and attention. My experience is with independent living and skilled nursing, neither of which have delivered on any level other than disappointment. You have choices, so look around and ask good questions: what is the rate of infection in Skilled nursing? How often are residents checked? What is RN to bed ratio? Remember that Medicare pays handsomely and needs your feedback if there are glaring deficiencies. Complain here, but don't forget to enlist the local gov't offices. They have requirements that cannot be swept under cheap formica. The elderly deserve better than 'care' at The Rivers. read more

Incompetence, neglect, and abuse constitute a recipe for disaster, and that's what you'll find at The Rivers of Grosse Pointe. I was there for 9 horrifying days following surgery to receive physical therapy and recover some strength. The physical therapy was negligible, and it took all my strength just to survive the ordeal of being in this horrible place. Staff is rude, uncaring, and poorly trained. Some are downright abusive and hostile. Call buttons go unanswered, but if someone does show up it's to yell at you for pushing your call button. They say they'll get to you when they can, and sometimes they do...two hours later. I had bruises on my back and arms from being shoved, pushed, and grabbed. I needed assistance getting to the bathroom, and most often that assistance didn't arrive in time. More than once I sat for hours in my urine, wet and freezing. I reported The Rivers to the Michigan Department of Health and Human Services Abuse and Neglect at 855-444-3911. I urge others who have experienced the hazards of this place to do the same because sick and vulnerable people continue to suffer these abuses, and this must be stopped. read more

I am physically sick right now reading these reviews. My father has been in the cardiac rehabilitation center at The Rivers one week tomorrow and all I want to do is go take him out of there and have him examined at a hospital. My father was pretty weak when he arrived but since he has been there, he has fallen twice, I have come to see him and found him pants soaked with a diaper on he was never in diapers before this. There are no rails on the beds, the staff are short and uncaring. I have to call his room, his cell and the nurses station a million times before I can get someone to answer and when they do they just say whatever you want to hear to get you off the phone. I am honestly mortified reading these reviews cause I thought maybe I was just being hard on them cause its my dad and I'm emotionally vested. I want to just go get him out of there right now but can't without a back up plan...I honestly want to take him to the ER and have them check the levels of medication in his system and find out how much worse he is now than when he arrived at this DIVE. They make it look nice but the service is horrific. read more

Don't. Even. Think. About. It. You cannot even imagine the incompetence. When we arrived, the sheet wasn't even pulled out around the edges of the bed. No one came to the room to welcome the patient, go through his medications, give him an idea of when the doctor would be visiting, nothing. When you ask for a glass of water, it takes an hour. There is no sense of structure or organization at this place. The food is terrible. One day a plumber comes in to fix a toilet that during the entire duration of the stay was never working and when the plumber saw a pack of wipes which were not being used for the toilet but for another purpose, he admonished the patient and told him he could not have them in his room. When the patient agreed and asked to have them left in his top drawer, the plumber said that he could retrieve the wipes from the front desk. Well, you can imagine how well-tracked a package of wipes was at this place. We never saw them again. If you want your loved one to be disciplined by a plumber, send him/her to this place. If not, then skip this place altogether. It is truly the worst. read more

Stunning beautiful facility, but deficient in care. They all came in to introduce themselves on arrival, and then saw them less and less after that. Long stretches (4 hours) when we never saw anyone. We had private duty nurses with my loved one because of this. Had to ask for water, washcloths, coffee, turning patient, bed rails, diapers. Served pork chops and pork loin to very elderly and ill person with no teeth. Nurse with no name tag, said she didn't have to wear any id. Took her home to pass away in her own bed. They don't seem to know how to treat elderly, ill people, just plop them in a room. read more

My mother-in-law moved into the memory care with beautiful clothes and now they are all missing (bras, nighties, slacks, and sweaters). Had we known they couldn't be found we wouldn't have donated so much of her clothing so we could "shop" from home. I am disappointed that they are fed all their meals and drinks on plastic plates and styrofoam cups. And on the weekends in the dining area, the music that is played is rap (with inappropriate swearing) or current pop. 1930s, 1940s, 1950s music is very therapeutic and soothing to the elderly and often the only link to "life" for those with dementia. Caring for her clothing (shopping, handwashing, and hanging) was very important to my mother-in-law. Now the few remaining sweaters that are left have been beaten up and shrunk in the hot, rough washers. This atmosphere of not treating their era with dignity is very upsetting. Lastly and most concerning, the nurses and doctors seem to "forget" and not notify the Power of Attorney when a medical procedure is done. We find out when the bill comes. Ugh!
