I found this facility lovely on the outside and lacking , when quality care was most needed. There was disparity in quality of care form independent living, to nursing care, to long term care and assisted living. Mice and vermin were in nursing home rooms for over 2 years, an experience where a resident was not given accurate medication, and a family member argued with nursing staff when resident was in hospice, and staff reprimands to other staff to not engage family. Regular reports of cold food, inaccurate food delivered and dangerous broken serving trays. Family member had great care from some CNA and nurses, and housekeeping staff in Acute nursing care at Oak, and then to find they were fired. For the prices paid, and promises offered, independent livin is lovely, But when people need most help, this facility fell deeply short. I'd say "could be better" but really, depending on which you live in, i.e. independent, assisted, or skilled nursing, you're going to have a totally different experience & so I'm saying "Not good" because for Assisted Living & Skilled Nursing it really is NOT GOOD. FIRST you have to understand that, although it's "the Quadrangle," it's really SUNRISE SENIOR LIVING, a for-profit company that is expanding all across the USA with over 242 locations. So the money you pay in is really used for Sunrise to expand nationally. Once you get that, it all makes sense. Then you need to understand that Sunrise is an REIT, a Real Estate Investment Trust. It's all about the investors making money; it's not about your loved one, or you, getting the best care possible. They skimp in as many ways as they can so the investors make money. This place - beware. The Independent Living is okay, Like all the photos, everything is beautiful. But Sunrise, the Quadrangle, is cheap. The signs at the elevators are so confusing & years ago we asked them to update them & make them easier for residents & for guests to understand. But no, they're cheap. What would it cost them to improve these signs so people could figure out whether they need to go up one floor or down one floor? Pennies. Really, what would it cost them? But my biggest beef is really with the Assisted Living & even more, with the Skilled Nursing. It's all about SUNRISE. When you or your loved one go to AL, building 8, your rooms are smaller, but the monthly expense doubles, PLUS you ALSO have to pay for meals (whereas in IL, the one hot meal/day is included in your "rent." So be prepared to pay more than double what you did, and have to live in a space now half the size. I could go on & on. There is no exercise room in the Assisted Living. So you or your loved one are going to go physically downhill quickly. They have chair exercise but big deal. No recumbent bikes to pedal, nothing. Oh sure you can walk a mile (literally the other side of the property) through the hallways to get to the main building's fitness room, but you might need to hire an aid to walk you there/back; or maybe your paid aid won't even want to walk you or push your wheelchair that far. Expect to deteriorate. The aids are rude. Is it their fault? They're paid bottom barrel so that's what Sunrise hires. And for the most part they're rude to the residents. (The kitchen & dining room staff, on the other hand, was so lovely.) If you get a nice & caring aid, the Quadrangle rotates them so the nice aid you had, and who knows what you want and need, you no longer have. And you have to start all over again telling them what goes where, and w them learning your mother is a PERSON!!!!! My relative, my mother, lost SO MANY GARMENTS there bc these aids do all the laundry middle of the night & and other people's laundry ends up in your loved ones closets or drawers, and your loved ones clothing ends up G-D knows where. The man in charge was so rude. He basically was supporting the hires. He didn't advocate for the residents, ever. Another beef is you don't get an Information Booklet. The rules of this Assisted Living are totally different from the Independent, but the booklet is like white bread. Totally devoid of any helpful information, helpful for the resident, helpful for the family member. I tried for 2 years to get them to update the booklet, even telling them what information we needed, what was unclear about the other booklet, or even put it online . NOTHING. They don't really want you to know. I'm writing this based on the experiences of three relatives and based on my own experience taking care of one of them. read more

The rooms are nice and large, it's clean, most of the staff is friendly, the food is quite good, there are activities for the patients. The PT and OT seemed to do a good job. This was mainly positive. BUT and this is a big one: totally careless when it comes to monitoring opioid pain medication. They gave my mother four 5mg pills, 2 in the morning and 2 at night, of oxycontin per day for 3.5 weeks when her order on admission was to give her 1 5mg pill as needed for 5 days. There was zero oversight and she became dependent and had a nasty withdrawal experience. This is precisely why we have an opioid crisis and I can't believe the doctor didn't monitor, adjust her order or even address it in any way. We called the nurse directly to make a tapering plan when we discovered what was happening, but the staff wasn't informed of the plan and it was a really difficult experience for our mother and us. read more

My dad was there for about 2 weeks. The day shift staff was quite nice, but never came to help him to the bathroom in time. The night shift aides were unkind and said mean things to him when they had to change his linens because he had an accident. My mother and he were too afraid to complain because they were worried the staff would be even worse at night because of a complaint. He came home early because he was so unhappy and has never been able to regain his bladder and bowel control because he had to spend the 2 weeks essentially in diapers. My parents could afford to go to many places, but they would never go back to Quadrangle. I will say the physical therapy staff were very good and kind. read more

Pros: free parking Cons: This place, where do I start. Toxic environment, lower management inside the building cares and support staff. The senior Management in the Main building and the skilled administrator does not care. Everybody is new(senior management) because they cant keep people(anywhere with in the company honestly). All the new people are on a power trip with who's ego can shine the best. This is suppose to be a skilled unit but residents are really medically unstable on arrival to the unit. There are no supervisors during the day with just a manager to supports staff. Who helps with admissions? Who helps with discharges? Who helps with the behavioral residents? The Director, assistant director, and manager. Leaving no time to focus on other issues such as excessive call outs, training, or staffing issues. The DON, ADON, & Manager are usually also on a cart. All the staff are leaving/left, the building is run by agency and PRN because all the staff quit, due mostly because of the new administrator in the building who fits right in line with upper managements. Lack of caring but wants to be in charge. Rude and does not know how to talk to people. Not approachable and sits in the office all day. Staff cafeteria is left overs from the main dining hall. The staff cafeteria is run down and stained so bad it looks dirty at all times. The staff locker area looks like a garbage loading doc. I wouldn't put my trash in a locker here. Hoyer lifts? Broke. vital sign machines? Broke. sit to stand? Broke. wheelchair scale? Broke. sitting scale? Works at times. Forgot to mention the management has become so overwhelmed they are asking staff to do their own everything. You ask for help they will help but often get pulled away before being able to finish helping you. So short staffed that social workers are helping nursing and not with just answering a call bell. Dining?short staffed food is often just blocks of ice by the time the trays make it to the residents. Admissions bring anything that makes money(not bashing its their job) but maybe we should consider staffing issues before bringing in the 16 pills at 9am, who is unpleasantly confused, refuses to use the call bell, constantly falling, blood thinner resident, with a sacral wound and wound vac. THEY STILL EXPECT YOU TO COMPLETE YOUR OWN ADMISSION, DISCHARGES, all while passing meds, completing treatments, calling physicians, and calling resident families i forgot to mention the above mentioned resident is now being sent out because of a fall. But asking why are you still here at 430/5pm when my shift ended at 330. No worries lower management is there early and leaves late too, trying to clean up(treatments, charting, skin checks) behind the agency staff that didnt care but made twice the amount of the staff to do a subpar job(a few really good ones did come through)The unit manager comes in at 730 and does not leave until after 5. Ill stop here if you get the point if not, I hope this helps. If it passes a state inspection it is purely due to the "window" giving them a heads up. Would not pass a random inspection AT ALL!! I will be honest I took this job because I previously work with someone who was already there. She really liked the place(been there a long time) and said it was ok but used to be better when it was owned by a hotel company. Worst mistake in my career. read more

My mother is a resident. The staff response time is excellent, and she likes all of her caregivers. They stop by to check on her throughout the day, and greet her affectionately whenever she is in any public area. They also stop by to spend time visiting with her when time permits. Concerns have been relayed promptly to the medical staff and addressed in a timely manner. While recognizing that no place is perfect, my siblings and I are very pleased with the care our mother is receiving at The Quadrangle. read more

I was in skilled nursing for 2 weeks. While some of the aides were kind, there was one who abused me. She told me I shouldn't be there, even though I had a fractured leg. When I reminded her of that, she lowered her mask and spat on the floor right in front of me. This was very dangerous because we were in the middle of a pandemic and because I was a patient, I was not wearing a mask. I reported it the next morning. When the Sunrise corporate head of nursing interviewed me, she questioned my memory and my mental status, which was eventually cleared by their psychiatrist. Later when I was sharing my experience with another aide, I was told that this aide was mean to everyone. When I asked why she was never reported, the aide said that everyone was too afraid to do so. I am glad that the State department and OSHA eventually interviewed me. I pray that this aide is no longer working in the health care industry. read more

The skilled nursing facility is shockingly understaffed. Response time is poor. Nurses are not informed on vital medical information--including, believe it or not, diagnosis. There is no housekeeping at night, and if an accident happens, it's not going to get cleaned up. There are openly hostile care assistants who strike fear in your heart. Some of the staff are lovely, but they are so overburdened they can't provide care efficiently. The Quadrangle assisted living seems very nice, but the health center is a nightmare. read more

We moved to The Quadrangle this past fall and are very happy with this wonderfully welcoming community, great variety of activities, arboretum grounds with natural beauty everywhere, (the many varieties and numbers of cherry trees were fabulous this spring), our home filled with light and views of trees - the pond a short walk away, interesting and enlightening conversations, friendly and helpful staff, and delicious food! I've had to get some PT help which has been an excellent experience. The library is a popular part of the community, containing several bookcases filled with books by authors who are residents here. Celebrating its 30th anniversary, we highly recommend this caring community. read more

do not allow your loved one to go to rehab at the quadrangle!!! they are dangerously negligent!! (my father passed away in their hands!) the doctor has no compassion and there are not enough nurses to adequately care for their patients! (the physical therapists & a few other staff members were qualified & caring) i still feel you are better off sending your loved one elsewhere! My father was here for two weeks. The nurses are kind, PT/OT is excellent and most importantly- response to the call bell is immediate. He was in another skilled nursing facility that I had to pull him from due to the terrible neglect. When we arrived at the Quadrangle it was the complete opposite. His room was spacious and he enjoyed the food. I felt at ease with him at this facility. I would definitely recommend it. Also, it is dog friendly which helped lift his spirits. read more

Mom was here for rehab services. Communication from facility to family was almost non-existent. Discharge instructions were unclear and unhelpful. Be very careful when trusting your elderly family member to the care here. They talk a good game but deliver very little. read more
