One Star - Severe Neglect and Lack of Accountability…read more My uncle entered this assisted living facility three years ago with early dementia. At that time, he was a happy, easy-going man. What followed over the next three years was a steady and heartbreaking decline that we firmly believe was caused by neglect, poor management, and a complete lack of compassion. Employee turnover was constant, including higher-level administrative staff. The facility was consistently filthy. On one visit, I became physically ill after seeing how dirty my uncle's bedding was. His room and bathroom were also visibly unclean and poorly maintained. On one occasion, a staff member referred to my uncle as "nasty" in front of my sister. When confronted, other employees attempted to excuse the comment by saying the worker "didn't know English," rather than addressing the disrespect shown to a vulnerable resident. There were repeated failures in communication and medical care. When my mother called to speak with my uncle one day, we were told he couldn't come to the phone because he had fallen and was in bed. No one had called an ambulance. Only after my mother insisted did they do so. At the hospital, doctors confirmed his femur was broken and stated that the injury did not align with a simple fall. My uncle later developed skin cancer. The facility repeatedly mishandled his transportation to treatments, causing him to miss appointments until the cancer eventually became untreatable. They also lost his expensive dentures, leaving him unable to eat properly--one of the last simple pleasures he had left. Calls to nursing staff routinely went unanswered. We were often left in the dark about his condition. Most disturbingly, we recently learned that he had another fall and was sent to the hospital for stitches, and no family member was notified--despite both my mother and I calling the facility that same day. That night, at approximately 2:30 a.m., he was found unresponsive. Staff claimed they perform checks every two hours, yet no one can explain how or why he was "checked" at that hour while supposedly asleep, only to be found unresponsive. He was hospitalized and never regained consciousness. When I called the facility to inform them of his passing, the employee I spoke with offered no condolences, no acknowledgment--nothing. At no point did we feel that anyone truly cared about my uncle. Even a former employee told us directly that many staff members "don't care and shouldn't be working there." My elderly father was responsible for overseeing my uncle's care and trusted this facility to do their job. That trust was deeply misplaced. I would never recommend this facility to anyone who loves their family member and expects basic dignity, cleanliness, communication, and humane care. RIP Uncle Keith, you deserved better!

Rooms are small and everything is outdated you get what you pay for here…read more The smell is bad in the whole building and even some of the residents look dirty. The staff don't seem to be friendly or care about the residents. You can hear them speaking about the other residents and that's unprofessional!! I will be moving my family member out of here and hope this place gets a good deep cleaning for those that can't afford other places up the street or maybe they will get closed down. It's a shame to see health care providers not care about others. Where's their compassion

My mother was sent to Palms for rehab after a little illness. At first, this place depressed me,…read morebut after spending many days and weeks there, I must say this was a wonderful place. All of the staff, from front desk to nurses to therapists, were friendly and kind. Most of the other patients were equally friendly and I came to know them well. The food is good and bad. There were some meals that were outstanding and others that were bleh. (For $5 a meal you can join your loved one for any meal). The purpose of this place is to increase strength and the ability to function on your own. This they accomplished with my mother. There is also an area of apartments for those who wish to live there. So many activities: musicians performing, ice cream socials, games and puzzles, a library. I was truly sad to "leave" this place when my mother was deemed well enough to go home. These are truly caring folks and they make life for the elderly much more exciting. I can only hope to end up with wonderful folks as this when my time comes.

Please let me start this by saying the nurses and nurses aides were the best!. Now let me tell you…read morewhat a scheming lying company this is. My dad was diagnosed with terminal cancer in May. We met with one of their reps in the hospital. She drew us right in, telling us we would have round the clock care when he needed it. She told us that they would make sure the family would have overnight care so the family could rest. Funny that neither my brother or I were paid for this, because IT WAS US. They basically abandoned us with a dying man to take care of at night. It took me threatening a lawyer before we got overnight help during his "crisis care time". What a horrible lying bait and switch company, and very uncaring and vicious upper management. I did not name the two reps I spoke to, but it's not because they were great people. Karma does come around and I hope these two reps get the same help, that they lied about. One piece of advise to the upper management, tell your money hungry lying, reps to step up and BE HONEST.

This is not something easily said about even the worst of worst, yet, perfectly applies. There is no lower form of life to crawl the Earth than those of this palliative care and hospice group. They were prematurely asked to call a patient, by another practitioner. The patient and family politely refused "services". The calls keep coming. These "harbingers of doom" hide behind a "pretense of comfort provision" no matter how well you may be or what care you are currently receiving, their care is, supposedly, always, "better". You get this shoved down your throat in repeated phone calls until you hopefully cave-in. It's free... What have you got to lose? You will lose. These organizations have one purpose... To appear to provide compassionate care while methodically robbing a person of whatever remains, by any means. They take control of your 'case' and in a subtle way keep you or one your loved from testing or other medical procedures they deem unnecessary and a waste of time. It is effectively a "throttling-back" of your right to seek and receive the care of your choosing, you spent your entire life earning a right to. Suggestion: Keep your loved one as "functional" as possible with excellent home nursing/aides, caring family interaction, and a physician who takes the time to understand the disease process and will develop a care plan tailored to patient specific needs. To be clear, I am not speaking of absolutely appropriate and necessary agressive end-of-life issues. Although, even these cases have their issues. In this instance, it is the systematic "trolling", mostly of the elderly, looking for those easy to take advantage of. You become an economic target without patient-centered goals !
