    How do I start? I had a friend that I watched pretty closely who was in the Preston would rehab for…read moreone week and then ended up in ER. When we got there the bed mattress that was supposed to be on the bed was on the floor for three days. No one took the time to put it on his bed to prevent bed sores. Next he was constantly being fed his evening meal at the same time of his breathing treatment. It was always at the same time and the food was cold when he was able to eat. He was told that was his set schedule for a breathing treatment. My question is, if his breathing treatment was at five and they bring the food in at five - why didn't they schedule that a little differently. The buzzers from all of the patient rooms were going off continuously and not a person in sight to help any of them. Lastly, he is one that had to have a man's diaper due to his kidneys. He had an accident and was trying to move around because of it being uncomfortable and fell off the bed sometime during the night. He laid there yelling help to around 7 o'clock in the morning. Which is why he ended up in the ER. He even had a touch of pneumonia because it was so cold in his room and his wife had to get him a blanket. As for me, I have spent many days in the hospital, nursing home, home healthcare and rehab centers because I am in ministry. This has been the worst experience I have ever had or seen in any of the facilities or stayed at with friends and loved ones. My thoughts are: they are not very attentive, nor caring for their patients. For the amount of money, the patient pays not including what Medicare or insurance pays, this is very poor in service and in care of the patients. My opinion is to put your loved one or friend in another rehab facility besides this one. They may change with leadership, but for now, it needs to be evaluated. Please check the facility yourself and ask lots of questions and make sure you're there wherever your loved one is to be sure they properly treated and cared for. I rated them a 2 because of thefood m. It was really good but the service of getting the food to the patient was unprofessional. Sometimes it would sit in the hallway for a while until the loved one went out to get it for the patient. I have to admit Physical therapy staff wasreally good with the patients and couldn't ask for better. If they could just get the rest of it to match up, this would be a good place to go.

I appreciated her diligence and the effort she put into my case. Old review : I had total knee replacement surgery. This company contacted me initially and we set up times. The RN came and she was Amazing! The PT came and he was great. What I have major issues with is I was supposed to be getting 3 sessions a week for two weeks minimum.. On my about to be third session , the PT said oh we don't have pre authorization. I wondered about this so I called. The company would not call me back. I called twice. I waited 7 hours and no follow up call. I called a third time around 3 in the afternoon . I finally got someone and they said, " Oh we don't have pre authorization." I said , "Then how did you guys send people to my house in the first place?" They still have not answered that question. I still should be having PT sessions , it was knee surgery, the Dr. office did send that over (I checked) , and still no services and no calls from this company when I should be having therapy. Knee surgery is a difficult surgery mentally and emotionally, and to have to deal with this is unacceptable. I rated the company a 1 only because I really liked the PT and OT. Their management , scheduling , return calls needs to be improved.

The companions have been wonderful. Our regular one Lisa D is a jewel! She keeps us informed of how he is doing, activities he is participating in and anything he needs. Elizabeth has been a godsend in what ever role she is doing for Dad. I highly recommend BrightStar!

We brought her home that weekend and the loving and caring staff not only got her comfortable but more importantly, she had a series of falls the last few months before this hospital visit, so she had not walked for almost 8 months. She was started on some muscle strengthening exercises, given some tender love and care, and not only did she make it through the weekend, but she walked about a month later for the first time in a long time, and lived almost 7 months after receiving such initial devastating news. We collectively know that it was Accord Palliative and Hospice Care that went above and beyond for our family. Thank you guys so much for caring for us in our time of need. -Davis Family

Within the first few days, I was struck by the way the residents of Carrollton support this nursing home. It really was quite amazing! It should be no surprise that while he was in the nursing home proper, I found it necessary to be there as much as possible, making sure that his care was as kind as possible. My job, then, was to introduce the staff to the man they could otherwise never know because he was quite non-verbal due to Lewy body dementia (LBD). He was a professional commercial artist and photographer who had destroyed most of his work before anyone realized how sick he was, so storytelling became my primary educational tool. As a healthcare professional well-versed in bedside patient care, if someone's care could use a little improvement, I would jump in to team with them, being gentle with this very thin man who was aroused to some level of consciousness if something hurt. All the while, I told stories about his surprising humor. Or how he walked so rapidly yet never missed a tiny flower that needed to be photographed. And what I knew of his work as an artist in the Army during WWII. One day he shocked me by saying that he wanted to go to a hospital back home; that was the opposite, really, of his Advance Directives I was following. But his brief return to conscious awareness trumped that, so I spoke to the RN on duty and we were quickly moved to the current Texas Health Presbyterian-Dallas, where I stayed in the room with him around the clock. Once his diabetes, nutrition and hydration were all optimized, he became more alert and one day told us that he wanted to stop all treatment. That immediately moved him to hospice status. We moved back to Heritage Gardens, this time to the hospice wing. Because the census was low, I was invited to stay with him, sleeping in the other bed in his room. Again, I watched over and participated in his care, which went much better on that wing. I will forever be so grateful for that kind offer from Heritage Gardens. That time in the hospital and that final month in hospice care proved to be some of the most meaningful of our lives and happened so easily because of the calm, peaceful, supportive environment we were given. Many thanks to God and Heritage Gardens!

    This place is what I would consider a prison must be like. My son put me in this horrible place…read morebecause he thought I had dementia. If I do it's mild. First, I stood up for a young lady who was being harassed by another female "inmate". Resultingly I was transferred to a different unit. This continued progressively as I was moved two more times. Each time it was to a more violent arena. The last was the most violent because most guys looked like they got incarcerated for bar fights, etc. The food was absolutely horrible. We had to line up and march to the cafeteria. I saw no one who seemed to have any dementia, other than a couple of young guys who constantly paced. My son was out of the country so I freed myself without medical approval. If you actually love someone don't put them in this hell hole. It's also like a maze with all these different hallways and units.i am 77 years old and it is like I say THE MOST HORRIBLE PLACE IN THE US.

    The worst company in healthcare. RUN! The company creates the budget from the top-down which sets…read moreridiculous census numbers to be obtained. 2/31 of their buildings actually meet census- TWO! They assume all ED's are terrible and that's WHY the numbers can't be met. It was explained repeatedly that we were getting, at least but often times more, than the market share. I'd suggest looking at the budgetary process! They pay the line employees less than a fast food restaurant and wonders why as a company there is 159% turnover.....ALL Corporate Office personnel are aggressive, territorial, and retaliate. The Area Director is rude, aggressive, screams at the employees and hangs up on the vendors when she doesn't get her way-she is still employed. The Director of Ops pacifies everything and everyone; he is a diplomat that is it-he is still employed. The Director of Clinical Services was fairly new at the time...he mainly communicated by forwarding notifications from TXHHSC/DADS (which everyone has access to). The facility team works short (by 10+) positions every day and the Corporate Team allows unlicensed family members of current employees to help because it's free labor. It was a fight-literally an argument, to get the patients supplies-air mattresses, briefs, medications, etc. The company had over 70 complaint surveys (31 buildings; average of two per buliding) in Q1. Clearly, their concern is for the bottom line and not for patient care. Patients would go days without pain medication (remember this is transitional care; post-op) so the pain meds should be a certain. The Pharmacy (also owned by Cantex) was a disaster. The EKIT was always wrong, inaccurate, the meds arrived late if at all......This is an NHA's/DON's worst nightmare and the families/patients new hell. The company's actions are a far cry from their mission and vision statement. I would never put my worst enemy at one of their locations!
