This location is in the Centennial Hills area off of the 215/Durango exit. It's in a plaza with plenty of parking spots. When I arrived at the facility for my first PT appointment on a Wednesday around 3:00 P.M, it was a breeze to check in. I had completed all of the paperwork required online. There are only a few seats in the facility though. Kiana is an amazing physical therapist! She is constantly providing challenges and is knowledgeable! My first appointment focused on getting a clear baseline of where my hand was at. Kiana measured my hand strength, assessed my mobility, and talked through my goals so we could create a plan for the rest of my PT sessions. By the end of that initial visit, we had a solid game plan in place, and I even started some targeted exercises for my right hand to begin improving strength and movement right away. This is the place to go if you need PT! The team members here are outstanding!

We asked them if we would be billed twice and they said no we should be fine since it would be a "transfer". Apparently not. They don't know what they are doing and just tell you what you want to hear.

I go so much it's like family there. Jena, Delilah, Jacob, Chris, Mark, Tyler, Bryce, Brendan, and I forgot 1 lady's name. But they all take turns coaching me. Now this is NOT a 1 on 1 place. Its never been structured like that. My insurance sends me here everytime and I'm ok with it. Some assistants are better than others-Tyler is really knowledgeable and kicks your butt, Chris and Bryce are also very knowledgeable and are ready for the ice packs at the end. Jasmine the actual pt can do dry needling and cupping. So if you need those specialized treatments be sure to ask when booking your first evaluation appointment. I love this group. So the person on the phone then sets up my appointment for the same time I had before 1/27 @ 3 pm with a person named Earl to do my evaluation. I take a few hours off work to get to my appointment and they can't find me again. So I would be an idiot to give them a third shot at wasting my time. Not only the clinic itself being a very clean, intimate, professional space; BUT the clinicians Cailin and Kameron being top notch. I've trusted both of these amazing humans with treating my family and myself. Cailin doing my wife's pelvic floor therapy post parting with our 2nd child, my stepdads back rehabbing, and treating me from small shoulder pain, a minor calf muscle (major pain) and my frequent back spasms. You got an injury new or old? Well they have a treatment and a plan. not only will you feel safe, and heard, and better after your visit. My 88‑year‑old dad spent 24 days at Trellis Centennial Rehabilitation Facility after a six‑day stay…read moreat Centennial Hills Hospital, where he was treated for malnourishment and severe dehydration caused by a brutal case of shingles. Once he was deemed "medically stable," the hospital transferred him here for physical rehabilitation. Like most patients arriving at Trellis, he came in on a gurney--weak, confused, and suffering from "hospital delirium," a condition I had never heard of until then. He had a long road ahead of him. The facility itself was nicer than I expected: large common areas with plenty of seating, big‑screen TVs at both ends, and a pleasant outdoor courtyard. But the real strength of Trellis is the healthcare staff. They were exceptional. My dad's primary CNA, Aleah, was the standout--truly the star of the show. Watching her care for my dad, I would have believed she was caring for one of her own family members. Even when he was ornery or threw her a curveball, she handled it with patience and grace. His occupational therapist, Raymond, and his physical therapist, Badge, were also excellent. And across the board--from nurses to CNAs to PTs, OTs, nutritionists, and housekeeping--the clinical team was skilled, compassionate, and consistently attentive. They are the reason my dad made real progress. Unfortunately, the administrative side of Trellis is a completely different story. From the moment we arrived, we were handed a stack (well, an electronic stack) of forms. I understand the need for paperwork, but the entire process felt designed more to protect the facility than to support my dad's recovery. The biggest issue involved my dad's macular degeneration treatments. He receives eye injections every seven weeks, and one of those appointments fell during his stay. I started trying to coordinate it a week in advance. His doctor's office explained that because he was a resident at Trellis, Trellis--not Medicare--had to be billed. If they billed Medicare, it would be denied. What followed was a frustrating loop of unanswered messages, unclear answers, and administrative runaround. Despite my efforts, I ultimately had to cancel the appointment. As we approached the 20‑day mark--the point at which Medicare stops covering 100% and the patient becomes responsible for $217 per day--the business office suddenly appeared to say my dad was "nearing his baseline." Interestingly, none of his healthcare providers agreed with that assessment. Still, after five additional days, the administrative staff declared he had reached maximum improvement and discharged him. My dad and I were both uneasy about it, though it did allow me to get him to his eye appointment that same day. Now, nearly two months post‑rehab, my dad is doing better than he was before he got sick--despite the admin team insisting he had "baselined" long ago. Go figure. I do want to acknowledge Julie at the front desk, who was consistently pleasant and helpful in navigating the administrative maze. In the end, I owe a tremendous debt of gratitude to the healthcare workers at Trellis. Their skill and compassion got my dad back on his feet. The administrative staff, however, made an already stressful situation far more difficult than it needed to be. Healthcare workers: 5 stars. Administrative staff: 1 star.

From the moment we walked through the doors, the care, professionalism, and compassion shown by the entire staff was evident. I want to especially recognize Jasmine, the Business Manager. She went above and beyond to help us navigate the financial and administrative side of care -- something that can feel overwhelming in a stressful time. Jasmine was not only knowledgeable, but also kind, patient, and incredibly clear in her explanations. She made the process smooth and took a huge burden off our shoulders. Post Acute Care (PAM) Rehabilitation Hospital of Centennial Hills is a new, highly efficient,…read morefriendly, and extremely supportive rehabilitation facility. I spent nearly 2 weeks there after an open chest artery aneurysm, a supportive surgical procedure requiring a 15 inch incision from my breastbone to below my navel. During the surgery, my abdominal organs were manipulated, and some structures were cut to improve access to a short, hard-to-reach artery that was inflamed and torturous. The procedure took about 3 hours with 3 vascular surgeons in the OR. Basically, PAM received a relatively immobile patient in a lot of pain, with significant complicating diseases (or comorbidities) - a challenge for any rehab hospital. The team that made the boo boo go bye bye. It may be hard to fathom, but the team at PAM is just that, a real team. Support teams work 12-hour days, but it seemed as though there were many, many more members of the team since my bell never went unanswered, even during shift changes. I required health care (daily doctor visits for general health and wound care), occupational therapy (OT) comprised of routines like toiletry, showering, brushing teeth, etc., and physical therapy (PT) that helped with walking with assistance, gaining strength to remain upright and balanced, to building stamina to increase mobility. Although none of these things were unfamiliar to me, getting any one of them done with a 15-inch chest wound and whatever went on under the skin made every action a challenge, and a team member substantially aided. With 44 beds in the facility, I can only give you some idea of the support I received. Marie did my initial interview, and Arrian was my case manager. Albert was ever-present for wound care. Nurses were largely responsible for medication distribution and directing others to my needs and included Bella, Ferna, Maita, Faye, Sandra, Winnie, Matthew, Fatima, Rhenzo, Lori, Jia, Imani, Michelle, Silas, Skylar, Cecelia, Marichu, Yow, Jean, and Sam, 'The Man', a notably dedicated nurse, and given that all the others were great, this says a lot for Sam. My PT team included Baraka, Bethany, Candice, Connor, Douglas, Eliza, George, Isacic, Lauren, Lucy, Mark, Matthew, Michael, Parisa, Rachel, Rony, Skylar, Tania, and last but not least, Vanessa and Winnie. Ivy and Cyhthia were my dietary champions, righting the wrong of a hospital to rehab error in orders that kept me on a liquid diet longer than needed. Robert kept my room as clean as the Plaza in New York City. Although I hope to stay out of PAM forever, if I need rehab again, I can't imagine a better facility for a speedy recovery. If I missed someone, I apologize. I'll make up for it after my next stay...oh boy. Most importantly, my patient advocate was and remains my wife, Margie, without whom I wouldn't still be around. She has spearheaded support for me and complemented the rehab team.

Don't do it don't let your family be taken care of by these people . His family is also told them repeatedly that I am the local contract person. I can't even find out where he's at the hospital tonight they moved him somewhere . He's been fighting for his life for 6 weeks we've been through hell as his people and doesn't matter to them.
