The allegations include concerns that residents may be left in soiled diapers for extended periods, receive inadequate personal care, experience neglect, and may not be properly monitored for their safety and well-being. Additional allegations suggest possible inappropriate medication practices, including the use of medications in a manner that may unnecessarily sedate residents. Concerns have also been raised regarding staff professionalism, supervision, and overall compliance with standards expected in a long-term care facility. The reports further describe incidents involving resident safety, including claims of residents wandering away, escaping supervision, suffering injuries, and other preventable events. If true, these allegations would represent a significant risk to vulnerable elderly and disabled individuals who depend on facility staff for their daily care and protection. Residents of nursing homes and assisted living facilities deserve to be treated with dignity, respect, compassion, and professionalism. Family members place tremendous trust in these facilities to provide safe and appropriate care. Before I even entered the building, I saw, first hand how a staff member responded to a resident in need. The resident was enjoying the fresh air outside and had taken a fall in rounding the corner. I didn't see the actual fall but as I got out of my car, I saw that he was on the ground and bleeding on his forearm, rushed over to him as he was on his hands and knees in the driveway. I told him I would stay with him until help arrived. Just then a staff member was pulling into the parking lot, likely to start her shift. She saw us, immediately pulled her car up behind the resident to block/protect him from anyone else that might be driving in, jumped out of her car, rushed over to assess his condition, called for help with a sense of urgency and thanked me for waiting there with him. I was so impressed by the sense of urgency, confidence, obvious great training, empathy, compassion and dignity shown to this resident by this staff member, I knew right away that this organization is special. I've toured many communities in my career in healthcare and now that my family member is involved, I know what to look for. More than a basic, staged "tour" of a community, I actually got to experience what this community does to care for its residents on a daily basis. That, for me, was priceless. A care community is far more than a pretty entrance, with balloons and a grand piano. It's the staff who care for the residents day-to-day that counts. Instead, what we experienced left us heartbroken, frustrated, and ultimately fearful for her wellbeing. We had multiple meetings with management about her care plan, her daily needs, and the fact that she was a known fall risk (she was a dementia memory care resident) We made her needs very clear, repeatedly. Yet her care plan was rarely followed, and vital information was not passed down to the staff, or completely ignored by those who were supposed to be caring for her. Watching this play out time after time was devastating. Our biggest struggle was with management. Our interactions with Kim Briley, one of the owners, were discouraging and dismissive. Instead of responsibility or solutions, we were met with excuses and comments that made us lose confidence in the facility's leadership. She told us directly that many of her staff were lazy, improperly trained, or didn't have a high school education. She also made the comment that "they can't be watching her 24/7," which felt like a complete disregard for the fact that we were paying for supervision, safety, and adherence to her care plan -- especially given her being documented as late stage dementia and a fall risk. Amid the chaos, a few staff members stood out for their genuine compassion: Jessica, Tiffany, Colette, Christina, and Kiera. They cared about our mother and tried their best despite the lack of support and structure from management. They were the few bright spots in an otherwise deeply disappointing experience. The breaking point -- the moment we will never forget -- was when our mother fell and fractured her hip. In our view, this was preventable had her care plan been followed and communicated properly. After everything we had voiced and tried to address, this outcome was devastating. Obviously there was an ambulance ride to the ER followed by surgery and rehab. We removed our mom from Grace immediately. Not once did Kim reach out to check on Mom. Another disheartening find was learning that the facility is not even licensed by the State of Michigan! This only added to the pain and regret we felt. We trusted Grace Senior Living with someone we love dearly. That trust was broken. Based on our experience, we cannot and will not recommend this facility to any family seeking consistent safe, reliable, or compassionate care.

The facility called us saying my grandmother was found unresponsive, and they were calling the ambulance. A few minutes later, they called back saying "never mind, she is responsive now. We think she was unresponsive due to the position she was in (slumped over in her wheelchair)." My father requested she be put in bed, and some of the family went up to check on her. We found her still in her wheelchair and still unresponsive. She was unable to open her eyes, she couldn't speak, etc. Had we just taken their word for it, my grandma would have never been medically treated. Additionally, my grandmother waited over an hour in the rehab unit on several occasions when she hit her button to use the bathroom. Then she ended up being put in room where the button didn't work, and she had no way to request for assistance. We met maybe two people that actually cared for her and took good care of her. P.S. be warned, when they say they have nurses on staff during evening hours, they do not. It is aids only, and if you want your loved one to be seen by the doctor, forget about it.

Every senior should have one hands down for their protection. They are so under staffed and they have had lots of temporary agency's who don't care as they come and go. We had to get on them about making sure they were doing her laundry weekly. Her clothes went missing weekly and many showers were missed until we got on them about it and they put her on a weekly shower. To much laziness and personal past times going on the 3rd floor. I am distraught in the lack of care MIL received while she was having her stroke/UTI issues in their care 3 days before she was finally sent to the hospital where she died 11 days later. Better communication between the aids and floor managers/nursing staff and the residents Doctors. Recognizing that a resident needs more than just monitoring and maybe down playing the symptoms is not a good idea. When in any doubt send them to the hospital immediately. Many lessons were learned and retraining is needed. The camera told the story. Again it is all about communication. On the plus side she was happy even though her dementia was getting worse and she was hard to manage at times. The staff was always kind to us and accommodating when the family called a meeting. The home did work with us helping to make positive changes. So many lessons were learned from this experience. It is not easy placing a loved one in a memory unit hoping they will be loved and cared for. This home may be their last and you better make sure it is the right fit for all involved.
