From start to finish my mom was dischargued from St Mary(horrible hospital in High Desert) for one my mom blood pressure was not able to read it just said "high" second her sugar was 516 they stated they gave her insulin and her sugar was always high. I take care of my mom at home her sugar is NEVER OVER 200 which still isn't good but never that high. I left and later called they said her sugar was 500 like that's "normal" so I called and asked for the head nurse in charge and what they told me was "her sugar is normal for her " like I don't know my own mother or she don't live with me. I called 911 and the paramedics came the nurses told them I could not come in and the paramedics stated I was with them and she will come and check on her mother while they were here the paramedics asked for my moms chart and the nurse left the room saying she doesn't have one also my mom wears diapers she diaper was ALWAYS SOAKING WET which we caused her to have a UTi and the never treated it which turned into another issue to wear she has to be hospitalized read more

The Nurse's here are worthless. They act as if they're doing the patient a favor when it's their job to care for them ! Some of these Doctors are incompetent . I'm a nurse practitioner and the Doctors diagnosis is were way off base and did not make sense to. It was actually wrong with the patient at hand. A patient was experiencing symptoms of a certain prognosis to where this patient should have been transported to the emergency room for further evaluation and instead of doing such a thing like what normal doctors would do they told the paramedics that they could leave or leave it up to the patient decision where the he or she wanted to go. That is the doctors call not a patient call. Obviously there was a situation that 911 was needed to be called. Supervising nurse is there all lazy and considerate have the worst attitude and there's each and every individual that has to complain about them to call by director of this so-called convalescent home. I witnessed abuse towards patients definitely neglect and the pictures posted on this website are all false and lies and staged. I left the facility for the same reason. I could not risk my integrity and responsibility to my patient. The chair that they need just because the director instead of being a boss is everybody's best friend and doesn't know how to direct. If I are you and you have your elderly parents or grandparents in this facility, I would definitely remove them. This place is the the worst place ever! My 87 year old mother is currently there now ! she has fallen down and is in there for physical, therapy so she can't walk She just called me crying .. she said when the nurses finally changed her diaper after more than 12 hours she had been asking for a diaper change they were very rude and tossed her around like a sack of potatoes how unprofessional and uncaring can they be .. not to mention she just had a heart procedure the day before wow ! I urge you do not send your love ones to this terrible facility I will be moving her out asap and filling complaints !this place needs to be closed down!!! And the insurance company will be hearing from me and my attorney as well , this is a classic case of elderly abuse! Just beware!!! I would give zero stars if that option was available. read more

My Father 93 went for rehab after broken hip.. I was completely satisfied with this facility.. all the nurses, all the CNA's, all the physical therapist were very helpful and compassionate, his room was cleaned every day.. My mother was sent here after surgery (she had metastatic cancer and had just had nephrostomy bags placed) and this was an inhumane place. Many patients, including my mom, were left asking for help with no one coming to aide for hours. One night my mom used the request button because her urine bags were overflowing, and the evening/night nurse left her sitting in a puddle of urine for 6 hours. She said when a young nurse finally came at 4 in the morning, he complained that it stank and she felt humiliated. I would not recommend this facility to anyone. read more

Well since Google or this facility blocked me from doing the review they deserve I'm putting it on yelp. My wife ended up here around June/25/24 and everything was going great with the nursing staff on the help for my wife. My wife has a broken elbow and a fractured pelvis. Well my wife asked for a toilet for the side of her bed because she was embarrassed that they had to wipe her up and change her diapers. The physical therapist told her not to do it herself to make sure she gets help from a nurse. Well the time came my wife needed help and the nurse told her to do it herself and walked out her name is Carrie she has been helping my wife almost everyday and my wife even spent 20.00 dollars on a cream for her. At this time my wife can't walk at all by herself or put hardly any pressure on her left leg and left elbow in a temporary cast. Well the next day after my wife gave her the cream that's when she told her to do it herself on using the toilet next to the bed. Special thanks to Shaun,Brandi,Robert,Steven,Tiffany,Kathy for helping my wife get through this ordeal. This place has no air conditioning my wife's toilet didn't flush right took a couple times to flush food is terrible a lot of the time I had to bring her food to eat. Bed control wire outside coating gone in two spots down to the colored wires. This place needs OSHA or the State to check out and make them fix what ever needs fixing or before they electricute someone. Good luck people if you end up coming here. Ya they put that bad bed controller on the other bed next to my wife after the exchange. Oh ya almost forgot black rotten banana to go with the rest of her breakfast lol. She's home safe now. Where is the money going when people come in this facility that is covered by insurance. At the front desk where you check in at broken clock to see the time to write it down to check in cost for a clock maybe 20.00 dollars, no air conditioning, bad food even the patients that were going outside to smoke were complaining about what they get fed. I'm pretty sure the workers outside that help these patients will verify what was said because they said it right in front of the workers when I was outside smoking with them. They told my wife they were going to fix the toilet problem but after 25 days here at this facility never got fixed. Yes from my wife's roommate left poop in the toilet because it didn't flush even the girls that cleaned my wife's room and bathroom got grossed out. This place needs some attending to or checked out by OSHA or the State. read more

I spent a month in this place after I had a seizure from a stroke. The employees are rude and very unprofessional. The food is horrible and tasteless. The air-conditioning wasn't working in the middle of summer. The nurses fight with each other like a bunch of thugs. The TVs keep going off. The maintenance staff was nice but not the medical staff. The only good thing to say about this facility is that it smelled clean. My mother in law was brought here from St Mary's after hip surgery. There was no rehab given. The first time she sat up and got out of bed was 9 days after surgery when she had a follow up appointment with the surgeon. 3 days later she was rushed back to St Mary's with plummeting blood pressure, weak heart, collapsed veins, and septic shock. She passed a few hours later. It seemed more of an ignore you center and not a rehab center! read more

This place is horrible! They don't do nothing but medical billing. Televisions don't work for days at a time. I bought clothes for my family member. The staff stole the majority of it within a month. Also another resident walked in the room and was going through my family members things. Oh and if you call be prepared to get disconnected at least 8-13 times & deal with 80% of the staff giving you and attitude. Their were only a handful of the staff that actually had compassion the rest just show up to collect a checks with their name on it . (Chuckles) Martha's Stewart the Social Worker was no help! I see they changed their name. I also see they haven't changed, so here's how they played major role in my mother's death: I'd give it ZERO stars if possible. My mother came here after a back surgery at St. Mary's to rehab. I came to visit her, and was immediatley hit with the stench of feces. During my mother's stay here, she contracted MULTIPLE infections. She even had a stroke, that was misdiagnosed by these people. She could no longer move or speak, and they just said she was unresponsive, and waited almost two days to send her to St. Mary's. She was cared for, and sent back here, only to be sent back to St. Mary's and put on life support because the infections she contracted HERE, spread to her lungs. 3 hospitals later, and now, as of 11/07/13, my mother is GONE now. This place and these people are who I have to thank for her death. If you care for the people you love, DO NOT send them to this God-awful excuse of a convalescent hospital. read more

My dad was recently in their care, it was a nightmare. They put my Dad in the covid unit and he didn't have covid. He's 80 years old it would have killed him if he got covid. They didn't change his bandages he has wounds that need to be changed daily. My Dad couldn't lift his arms so he couldn't feed himself . They wouldn't help him. They didn't give him water. The staff continually mocked my Dad. It was hard to contact my dad it would take about 30 minutes to speak to someone and they would hang up. They didn't change his urine bag and the catheter feel out. He was over drugged he could barely speak and felt paralyzed. He could barely hold the phone to talk to his family. My dad was depressed he wanted to die. We took my dad out against Dr.orders. Had him transferred to hospital. He has congestive heart failure , he had fluid built up. He doing much better now and is able to have a conversation. read more

My 85 year old mother stayed here for 2 weeks and hated every second of it. Very outdated, and I had words with slow employees on more than 1 occasion. So when she needed care again in 2018 and wanted to send her there. I said absolutely no way. read more

AVOID THIS FACILITY LIKE THE PLAGUE, IF YOU CAN. The care here is horrific, almost non-existent. They are supposed to provide PT to help the patient recover, but my father experienced the same thing as many other reviewers here, and left in worse condition than when he went in after his knee replacement surgery. He was left in bed for most of the time there, almost no PT was actually provided, and was too weak to even walk when he left. I could list other terrible things, but the lack of PT is the worst, since that's the main purpose. They are clearly understaffed based on patients assigned to a worker, but that's no excuse for almost zero actual PT. This place should be shut down. Sadly, this is the service we'll all have if we go to single payer health care! read more

I highly recommend this facility I was skeptical at first and scared when I read the negative reviews to not have my 73 years old father go here. We gave them a shot and sent our father there and the staff was attentive caring and very supportive they made sure my father was watch carefully his meals were given his medication was given on a regular basis and he was able to leave the place a hundred times better than when he arrived. And if ever he needs to go back to a center this is the place we would choose I highly recommend this center and you will not be disappointed I hope everyone can have a positive experience like ours. read more

This team is going through a lot of struggles and transitions. Patients sometimes complain of horrible food, small portions and neglect.

While visiting my friend she complained of pain. When I searched and found her nurse I was told that she had her pain meds. 45 minutes ago and wasn't permitted more for 5 hours!! Needless to say her family moved her the next day. read more

My father was released from ICU and his entire body was atrophy. He was transferred to this facility to received rehab services. My family was really disappointed by the overall services from documentation to nursing care. I will follow with several examples: 1. Given my father was VA Vet they argued with my mother on the billing of his stay. VA nurse had to come down to assist in clarifying documentation my mom feel she was being pressured the wrong way. 2. Upon waiting two weeks in this facility to start physical therapy. My father suffered from bed sores due to lack of diaper changing. My mom request several time that he be changed because he suffer from intestine issues and agent orange. 3.Once PT was activated they lack basic resources such as wheelchair and transfer board. My mom had to begin a wheelchair from home. As you imagine we worked hard to get him out to get home care and PT where it would benefits. I hope this review helps the facilities to improvement their overall services. read more

DO NOT LET THE PHOTOS FOOL YOU! DO NOT put your family members in this place! My grandfather was released to this facility after being released from St.Marys Hospital. He was supposed to be doing physical therapy which they only did a few times when the order was addressed for 5 days a week. They missed him on days he was scheduled or they would put him in a wheelchair and throw him out in the hallway. That's NOT PHYSICAL THERAPY. Also the place had roaches on the ground. NOT SANITARY! My family and I went to visit him and they didn't feed him dinner. They told us it was because of a room change. When they brought his dinner (2 hours later) it was cold and his ice cream was melted. He also has a colostomy bag, so when I looked at that he had feces all over his back and stomach. I addressed the situation to the nurse and was told "I changed it." Well I didn't care when she changed it. He had feces everywhere, even on his bed sheets. Also non of the nurses read the patients charts. His roommate also commented as I collected his belongings "Help me. These people are abusing me. They won't let me out of my bed. They took my glasses, I can't see what they are trying to make me sign. " I saw another patient strolling the hallways in his wheelchair dragging his urine bag on the floor.. Did the two nurses that walked by him pick it up? Nope.. They continued walking. Well now where is he... Back in ER, being transferred to ICU because this place does not care about their patients. As my mother and I went to collect his belongings the nurses had no idea who he was. This place will be reported to state! read more

I have to give this place a 5 star rating. My dad was sent to this place by Saint Mary hospital so my daddy can get strong and better to come home. I was transported here following hip surgery. This place is in a dire need of updating g and refurbishing Very old and falling apart. Checked in at 9 p.m. Signed admittance papers. Was told I would be brought some water and ice. She never came back to my room. At 3 a.m. a man who said he was a "charge nurse" came in and asked if I was okay and then left. At 4 a.m. a man came in to administer a TB test. At 5 a.m. a lady came in to check my blood pressure with a very dirty and old piece of equipment. At 6 a.m. A cleaning woman came in to clean the room. She had a white washcloth and proceeded to clean the bathroom with it and all surfaces of the room: doors, furniture, bed railings and then lastly in my call button. I asked her to put a plastic glove on that! Then she swept and mopped the floor. At 6:30 a.m. my breakfast was brought in. Dirty looking tray with a scoop of scrambled eggs, a slice of white bread, carton of milk and a tiny glass of brown looking orange juice. I didn't touch a thing. They had NOT filled my morning medications The employees on the night shift did not want to be there. Felt vulnerable and unsafe. Called my daughter to come and bail me out. Left by 9 a.m. Do Not go here for any reason. read more

My Dad was in Spring valley post acute after he was taken to the E.R. after a fall. He was there for 2 weeks for physical therapy, he had one session of PT. And one time for range of motion in his bed. When i inquired about his PT. The RN. Got his records and showed me how many times were documented for pt. They keep great records to cover there ass. My Dad told me he went to the pt room once and rode the stationary bike. I have him home now and he is so sore and stiff from laying in bed for 2 weeks. He can no longer walk or even stand up. This has made my life hell. I am his daughter and care giver and he lives with me and my husband. One time on another visit that he stayed there he came home with a bedsore, they did not say anything about it on his discharge. My poor father is in such pain. He will start pt in my home tomorrow. The physical therapist said that he hears this alot from spring valley post acute leaving patients in bed. Im going to talk to an attorney. We don't have but 3 choices for rehabilitation facilities here in Victorville my Dad has been in all of them, and i have see so many bad things. read more

DO NOT LET YOUR LOVED ONES GO HERE. Rude, lying, unprofessional, inexperienced, awful, unloving nurses/CNAS! Dirty outdated facility.. My father went there after a knee replacement after being there for an hour we knew we had to get him out. Meds aren't given on time, if at all. there is no communication between nurses and patients.. my dad had received meds at 8am next one should had been given every 4 hours..after calling the nurses 3 times for meds and being ignored it was 3pm and all we got were excuses (shift change, busy, that's not my room) etc.. we were told by other staff just bring in your own meds and give it to him. So illegal but there notorious for ignoring patients. I had to get him fresh water everyday! They did not even know how to put on his ice pack or the CPM machine. They never once lifted the blanket up to look at his wound. They never changed his bedding. We witnessed his room mate wither away.. didn't eat for days finally his blood pressure was so low they told him they were sending him to the hospital, he responded and said "about time". We heard more than once "idk let me check I haven't even read his chart" WOW.. really. Patients wheeling around in his room! I saw one patient go into someone's room and pull off the pee pad and was holding it strolling around and nurses/CNAs laughing! Call buttons going off for several minutes (got that recorded). Heard people yelling for "help" everyday. The only good people here were the Physical Therapist. I can go on and on... Next stop reporting to the state. read more

NO! THE BAD REVIEWS ARE TRUE! My mom lasted 8 hours here before I pulled her OUT! It's shameful that there are not better choices in the high desert. This was supposed to be the better of the worst....wrong. This place is not fit for animals, much less humans. While the hallways are clean, that's it. The rooms were decorated in the 60's, curtains don't close, the bed didn't work, duct tape was holding down the floor! The call button was STICKY and gross! My mom asked a gal to wipe the room down (there are no disinfect wipes anywhere)...she used a SINGLE washcloth. Wow. They use VINYL gloves which tear easy and are cheap. Many of the staff have long talons/nails (inappropriate for caregivers) which will puncture them. My mom asked for water and ice, never came. She was supposed to have very IMPERATIVE medications which were never filled nor administered. My mom did not feel safe. There were a couple people who were nice, but it was limited. No one that came in her room identified who or what their tittle was. Breakfast was a piece of white bread (cheap brand) and ice cream scoop size of runny scrambled eggs, orange juice that was brownish color, and coffee. Facility doors are left ajar which means anyone could wander in at any time, unnoticed. When I did, after my mom was transported at 9PM,no one noticed me or seemed to give a damn. NONE of this is acceptable. read more

My family member was transferred to here from Arrow Head He was recovering from a brain injury. He has been receiveing rehabilitation care for several weeks.When he was first admitted he couldn't walk or sit up by himself, as of today I have seen positive great changes in him, because of his great care in this facility my family member is able to walk by himself with just a little bit of assistance with a walker. Good food, some really good CNA's, lots of backstabbing and racism among the staff. Bathrooms are not wheelchair accessible, and they don't have automatic door openers. Phone system doesn't work. I second the review that said the staff lies. The nurses also make assumptions about the causes of symptoms instead of calling the doctor. They don't make notes of patient medical complaints, so the doctors never know what's going on. They will let health deteriorate through ignorance or laziness. If you have to warehouse a loved one there, be there 24/7, take careful notes of all conversations, meds and therapies, and call the doctor yourself. Members of the staff will tell you that anywhere is better than Spring Valley. If you need someone to go to bat for you with the insurance company, the admin here isn't your advocate. They run out of medications and blame the pharmacy. Everyone says "i'll get someone to help with that" instead of taking two or three steps and taking care of something themselves. I agree with the ignorance of the staff and that the physical therapists are okay. Nurse call buzzers are going off constantly, when the patients can reach them ( another problem ) and if you are in the right place at the right time you can hear the cna's talking about why they aren't planning to answer the call lights. Take your loved ones down the hill if you want them safe and well. My family member was injured by a CNA who tried to pull them out of bed by yanking on their paralyzed forearm. They now have terrible pain and frozen shoulder, months later. The CNA's pretend to text the doctor and refuse to give patients the doctor's contact information. They tell the family that the doctor said to monitor them. Then you find out from staff planning to quit and from the doctor himself that no one ever called the doctor at all. Some of the RN's will say they will check on things but don't. Some are just plain MEAN to patients, like Tracy. read more

Ralph Selover Big Bear, CA I give this place no stars. Had I read any reviews beforehand I would not have gone there. But when you are in the hospital and having surgery and then need to go to rehab you are in no position to say where you want to go. Besides it depends on your insurance. I was at the facility for a week. I was told in the beginning that my insurance would pay for 5 therapy's a week. That sounded ok. I entered the facility on a Thursday night at 6:30 p.m. On Friday a therapist came to my bed and checked me all over and got me up and had me walk with a walker and told me I was doing good. Since the next day was Saturday I figured that must be there was no therapy on weekends because I saw none. On Monday a therapist came and put me in a wheelchair and left me there. After a couple hours I was tired and found my way back to my bed. There was no therapy on Tuesday so my wife called to complain to our doctor and insurance. On Wednesday there was a therapist to work with me. We had a good workout. She had me walk with my walker up and down the halls. The case manager came in and said why didn't you call me before. I would not think that I should have to call someone to find out why I am not getting the care I came here for. On Thursday I had a therapist who did very little. In the meantime on Sunday morning I complained about a sore throat and nothing was done. Monday morning I told them my throat is worse. Okay we will call the doctor. Monday night a doctor stopped by and evidently ordered chest xrays. Tuesday morning I was told they were waiting for the results of the xrays. I had never had any xrays. They tried to tell me I had but my wife was there and there were no xrays. Finally on Wednesday they came and did xrays. All that time I was having difficulty breathing, sitting up on the side of the bed all night because I couldn't breath. On Thursday they told me I would be going home Saturday or Sunday and two minutes later someone came in and said I was going to the hospital because I had pneumonia. I sure didn't have pneumonia when I came in there. That Thursday night I was taken back to the hospital and had a temperature of 103. At the hospital they were on it immediately. The following Saturday I was released to go home. All in all I spent one week at the Spring Valley torture house. Never while I was in there did they help me to the bathroom. I had to use urinal and bedpan even though I had my own walker and could have walked to the bathroom with maybe a little help. My wife had asked for a patient toilet seat which is higher but it never came. I was told do not try to go to the bathroom by yourself. There favorite words were: Just a minute, be back in a few, be right back, waiting orders from the doctor, will have to call the doctor, someone will be right here, in a minute. There seemed to be many employees and I do realize they had 165 beds. But as a patient or a family member you really don't want to hear "we are short two nurses this morning" or "we are short handed". I never had a good bed bath except when my wife did it. She changed my bed more than once. I could go on and on but I would never recommend anyone sending their loved one there. My doctor says they are working on improving the place. It sure needs a lot of improvement. Not all of the employees were bad. There were some really good people there but they seemed hindered in doing their jobs. They need a big reorganization at that place. I saw all of this and more with many other patients while I was there. If you complained they ignored you room. read more

This place is outdated, employees in admin are ok, admissions lady doesn't know what she's doing. My grandparents we're both placed there, each different times. The only reason we let my grandma go there after my grandpa was there was because of the CNAs and because of rehab was so good and helped my grandpa so well. Nursing was decent, administrator was different. He freely talks about patients in the hallways and in a rude way. The place is worried about where their employees park, will call a tow truck on their employees and they page overhead Letting everybody who is in building know their business. Very unprofessional. They should focus more on patient care than parking. I have been going there every day for the past 3 months because of my grandparents. The morale seems ok, CNAs all get along and help each other out which is good to see. Nursing is understaffed but the CNAs hustle to make everyone happy which is great to see. Rehab is good at what they do and they have fun which makes it fun for the person doing therapy. The administrator is just a very very different guy and not very professional. Cleanliness: I see the housekeeping clean so I know it gets cleaned although they clean with bleach which is very harmful to non sick people so I'm sure it's not healthy for people that are sick. I have headaches when I go there, the bleach smell is bad.Not sure I would have family back there if it weren't for a few of the CNAs and Rehab. read more

Mom came here after we refused to send her back to a facility she came from in Apple Valley. Much better overall treatment here. People here are caring. They introduce themselves upon entering the room. Everyone is eager to help, not making you feel like you're a bother. Great job with the dietary department. Even though mom didn't eat much, the food looked appetizing, making sure they customized the food to moms liking. The CNA's that assisted mom were always very kind and helpful. (Mom said James needs a raise). The therapists worked with mom only on a couple occasions due to mom's limitations. They were motivating in a gentle way. (Patrick and Chris). The wound nurse was very organized and caring. I can say I have no complaints other than mom having to wait for breathing treatment or pain meds and that's why I took off 1 star. Mom is coming home on Hospice today. This facility was a great place for mom to receive the assistance that she needed. read more

Stay away! Father in law was transfered here after surgey. He was sent there for after care and to regain his strengh after being sedated for 5 days after the surgery. He did not receive physical therapy the first week he was there and became weaker. We never had a meeting with anyone to discuss what the plan was for his care and when I asked a physical therapy assistant what the plan was he said they don't do that. He said it is up to who is on duty that day to do what they want. My father in law did not have an appetite and did not want to do pt because it hurt. We asked several staff members to give him more encouragement . They said they could not force him to do anything and treated him like an invalid instead of providing him with the rehabilitation he came there for. They let him wither away to nothing. He lost 40 pounds in the 7 weeks he was there. He complained of pain but the doctor never came to see him. Only gave the nurses instructions over the phone. We were there every day and he was always in bed. They very rarely got him into a wheel chair. Which we had to provide by the way. No mater how much we complained nothing changed. On our last visit he was very pale, could barely talk and complained of severe pain. I asked that an ambulance be called but the nurse said it wasn't necessary. He was just having a bad day and she would call the doctor. As we were leaving later that night a nurse said she would be giving him an IV to hydrate him and the doctor would check on him in the morning. The next morning they transfered him to the hospital where he died that same evening. The ER nurse asked why he wasn't transfered sooner. The staff at this facility was neglectful and did not provide the proper care which we believe contributed to his death. read more
