Absolutely terrible care here I wouldn't let them take care of a sock ,my family member came here and was on the road to recovery doing great and able to communicate with us but within less than 48 hours ended up back in ICU because of the neglect at this facility. [[HIGHLIGHT]]The nursing staff don't care about the patients, I can't say that one nurse truly cares about any patient, if there's one that cares , then consider yourself lucky.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]I asked one nurse about medication for my family member and he snapped after I had asked and made it feel like you weren't allowed to ask any questions.[[ENDHIGHLIGHT]] They'll let your loved one sleep in their own defication and simply flip the blanket and make sure you don't see it, not one nurse will come in to check if your loved one needs to be changed or if they used the bathroom. When visiting hours is over heaven alone knows what they do to your loved one just so that they won't have to deal with them. Nurses take forever to do their rounds when it comes to giving medication and ultimately a truly horrific experience. [[HIGHLIGHT]]Even after speaking to the directors about what we had experienced and getting promised that things would change, nothing changed.[[ENDHIGHLIGHT]] Getting my loved one out of this facility was hands down the best decision i could've made and even when trying to get my loved one out of here they made it seem like you'd be doing the wrong thing but the wrong thing is leaving your loved one under their care. Short Staffed and rude staff.[[ENDHIGHLIGHT]] My father was there after hip surgery and he is a diabetic, he developed wounds on both his feet because they would drag his feet on the wheelchair to the therapy room, the wound care nurse just kept wrapping his toes and they refused to give him anti-biotics for the infection. The infection went to the bone which resulted in a partial amputation of his right foot on Christmas Eve, if you have loved ones, I do not recommend this place. read more

Long but worth reading. My Grandma was admitted here for about 3 weeks and she had a bad experience. DO NOT GO HERE! First week the night shift came in huffing and puffing and grabbed personal walker that was brought in with my grandma (not their equipment) and the lady grabbed it throwing it against the wall for no apparent reason. Mind you my grandma didn't speak any English and wasn't able to communicate well but this doesn't mean their staff should be able to grab clients personal belongings and try to destroy them. 2nd week here while they took grama to therapies within the building at some point the nurses left her hanging on the bed, if it wasn't for a family member that came in to visit her who knows how long she would have stayed like this. For your information, she wasn't able to pull herself up onto the bed as she had a shoulder injury and wasn't able to sustain herself up she needed assistance throughout the day. Lastly, the nurses never offered her a shower her, us the family members had to constantly be asking them to shower her (which they never showed up so we ended up showering her ourselves), clean the bathrooms, stock the briefs/wipes, we were doing their job during the day. Some nurses peeked in Avery 2 hours saw how she was doing and left. Didn't check if she was soiled, didnt check if cleaning was needed in the bathrooms etc. mind you each room is occupied by 2 patients. [[HIGHLIGHT]]They didn't use their interpreting system to communicate with her at all, which all this is paid for by her insurance so there is absolutely no reason for lack of communication.[[ENDHIGHLIGHT]] [[HIGHLIGHT]]I had left my contact information for them to call me and specifically stated to contact me anytime of day and my grandma had signed a release of information with my info. On it but apparently this means nothing to them.[[ENDHIGHLIGHT]] Family members were not able to stay overnight but rest asure we were there the second doors unlocked in the mornings. Morning shift was great, sure, I think because we would be on them asking for what was needed but when night shift came my grandma had the worst of experiences. Please don't leave your loved ones here. [[HIGHLIGHT]]Yes, we did complain but nothing was done about it. We even requested the name of the nurses and all we got was "ooh I'm not sure who it could have been we'll look into it though."[[ENDHIGHLIGHT]] Even though I provided the date and times this happened which each nurse is assigned to a specific hall/room. [[HIGHLIGHT]]I also gave score of 1's when they called me a week later to ask about my experiences. This past week, my dear friend was moved from her initial assigned room ( 'roommate was horrible. yelling relentlessly day and night) to an unoccupied room. My dear friend did not receive proper care from staff and was disrespected by one attendant. Staff did not provide dinner for my dear friend. This Nursing Rehab place is neglecting there patients. They are rude and unkind and stand around and talk. A family friend is there and they do not pay attention. There is a patient who has been waiting for over an hour for pain meds, and the staff -all that can say- Yeah... the nurse knows. I am reporting them to the Arizona Department of Human Health Services. If you receive terrible care report this facility for their license to be revoked. !! DO NOT GO There!!! read more

Please use caution and visit this place before you send your loved one here. My mom loves the PT and OT program. Very encouraging to her. Some of the other staff could use work on customer service. Three hours for water or ice, My moms meds mixed up, facility ran out of supplies told my mom to get up and go to the bathroom when she has not been cleared by PT to do so, setting up a appointment for my mom and did not even tell her about it, list could go on. I will leave you with this. If you have a family member here check on them often. Especially if they can't speak for themselves. I would hate to think of what could happen if my mom could not speak up for herself. read more

Let me start by saying we've had many family members stop here for therapy after surgery's. This first time during covid-19 were my brother had to be quarantine for 14 days ok understandable. But to selfishly tell us after insurance only pays upto 20 days then really only has 6 days of therapy if they even start with what workout. They tell me that he had while quarantine therapy in the hallway..HELLO how when he should be quarantine which I knew was untrue he told me just up and down off he's bed and that's it You know I can't go on let me just say this anyone who has to send a family member there, please have your radar on and standup for patient rights, because if we don't all these center take advantage of are people. read more

We took my father here after his hospital stay. Looking at the reviews our family was very skeptical about sending our father here. We are so happy we did. The staff was amazing. My father received the best therapy. The nurses and doctor in general were great. Everyone was super friendly and you can tell they really care about their patients. We want to say Thank you so much for taking good care of our father. If and when its needed again we are definitely going to take him there again. read more

Horrible Horrible Horrible experience! Recently a family member spent 2 weeks there after a critical surgery - very similar to heart bypass. Thank goodness it was only 2 weeks. The food was disgusting! One morning they gave him spoiled milk for his cereal and didn't bring him fresh milk. One day maybe 2 days, he didn't get dinner because they ran out. And no one offered to get him anything like a sandwich or something. The food was just BAD. So family had to take him food. Because of his chest being cut into, use of his arms is limited. Also his legs very weak and he needs knee replacements for both knees. So he depended on wheel chair to move around. Gradually, he was able to use a walker but for a very short distance and not unattended. His physical capabilities were very limited and often times he needed assistance with getting dressed. He couldn't take a shower on his own and the staff were too busy to assist him. Although they were scheduled. Any time he asked for help, it was very obvious they didn't want to help. There were a couple of cna's who were good care takers. But majority of the staff....NOT. Medication: for some reason, they decided to take him off one of his meds. When he went to the doctor for a followup visit, he gave them the medical folder that SMPC had put together. The doctor made the call the the facility right away and demanded they give him all meds ordered. There are lots of people with medical conditions requiring good care takers whether it's short term or long term. I was not impressed at all! He couldn't wait to get outta there. read more

*On 4/15 my Father was sent to SMPA for Physical Therapy. *On 4/16 Tyler called to see if I was bring in his home med because the Facility won't provide it because it cost to much (but its doctor ordered) I learned that in February 2019 because they stopped the meds without saying anything. I did take the meds in on 4/16 around 12pm On 4/21 I received a call from SMPA @ 1050pm that my Father was having seizures and was going to be taken to a hospital. (very hard to understand the lady that called, sounded like she was whispering) While at the hospital he was put on breathing machine and was unresponsive. They said they don't think he was receiving Seizure medication (waiting on lab work for that) On 4/22 I went to SMPA @815am to pick up my Father's belongings and his home medication that I took in. When I got home I counted the pills and out of 44pills that I took in on 4/16 there were still 43pills in the bottle. (nurse even wrote date i took pills in and how many on the bottle) On 4/23 I called SMPA around 1230pm to bring it to someone's attention and ask what happened. The Nurse in charge (Mary) took down my name and number and said she will look into it. Around 330pm my Brother called me and said Tyler from SMPA showed up at my dad's ICU room (didn't call me, just showed up) while he was unresponsive and on Breathing Machine. He was asking my brother questions about what happen. On 4/25 while I was sitting in main lobby at hospital I see Tyler walk in hospital. My dad was moved to a new floor the night b4 so if he tried to go to his old room he wasn't there. We also put my dad as a No information patient so if he was to just say his name with out password they would just say they have no1 there by that name. As I was leaving the hospital I saw Tyler sitting in lobby so I took a picture of him to send to my Brother (just to confirm that was him that went to my dad's room on 4/23) 15min after I passed (we made eye contact) by Tyler he ended up leaving (after making another phone call or two) As of today ( 5/01) I have yet to hear from the Facility as to what happened and why only 1 pill was missing from his bottle. My dad is still in the hospital trying to recover from the 5 Seizures he had back to back. I WILL NEVER SEND HIM BACK TO THIS PLACE MY TRUST WITH THEM IS GONE!! I FEEL LIKE THEY ALL FAILED HIM AND HIS HEALTH..... read more

I went after being severely dehydrated. There is a 12 to 15 hour gap of when you can get water. Because the kitchen is closed. This is terrible and unacceptable. The nurses and class can take an hour or two to answer your nurse light. Other than these two big factors they are good Goodwood and physical ans occupational therapy 5 days a week. read more

Unfortunately, due to Insurance issues, this was the only facility close to the hospital where she underwent colon cancer surgery and our home (27 miles away :( ). The facility, and grounds are very clean and well kept. And, the receptionist and "assigned" staff members (nurse, CNA, Therapists and cleaning) are polite and usually friendly. However, there are a lot of areas that caused me enough concern to where I couldn't give them a better score. When she was checked in, I told the nurse that due to bladder and bowel issues, she needed to be checked every 2 hours. But, on two occasions, when I leaned over to kiss her, I either felt the dampness, or could smell, where her briefs had overflowed. At least 5 times in 6 weeks, the bathrooms' Soap dispenser was empty. Twice, it was still empty after I told a staff member, and when I visited the next day. But, the one thing that really ticked me off, was when a staff member went to adjust my mom's O2 nasal tube. I noticed that mom flinched, but the staff member ignored her. A bit later, it was out of position again, so I carefully adjusted it. And, mom flinches again. Well, I checked behind her ear, and there was an abrasion area rubbed raw. So, I went to the nurses station and informed them. Non-assigned staff members were usually aloft, and basically ignored family members of patients in other wings. Also, and at least 8 different times, I had to call the main switch board. But, even after 10 rings, there was no answer. So, I was forced to call back a few minutes later. Now, I don't know about y'all, but when there's supposed to be 24/7 coverage on the phones, the phones are to be manned by someone at all times! It doesn't matter even if it's for a 4 minute potty break. And, one time after a visit to her Doctor's for a follow up, I noticed a couple of issues in the paperwork that caused me great concern. First, the SSN is shown (between that, and the patient's name, that's 3/4 way to ID theft) on the Transfer and/or Discharge documents. And, it said she was a long time Insulin user, when she never used it. Well, the SSN issue, the nurse said that it's standard procedure and a "confidential document". So, I went to the Medial Records Office. Indeed, the SSN IS NOT supposed to be shown. Now, with regards to the Insulin issue, when I checked the records the next day with the same nurse, she argued again about it being a "confidential" document. In a unsealed envelope. That make it as confidential as the person handling 'em is honest. But, the real humdinger here, is that the records that I looked at the next day, WERE NOT the same records that I saw during her doctor's visit. Those documents were in Portrait format, while the documents at the nurse's station was in Landscape format. Also, while it's great that they provide transport to off-site doctor appointments, they sure cut the time close. Luckily, most doctor offices allow for this. Of course, like most Rehab facilities, the food can be pretty bland most of the time. Update, 6/25/18: This update is to reveal their handling the situation after a patient of passes away. Or, insurance doesn't cover the entire amount. My mother passed away on Christmas morning, and sometime back in March, I got 2 statements (both dated within one day of each other; and 1 showing "Past Due"), showing a balance of $1,500 due that insurance did not cover. Well, I called and spoke to Yolanda in the billing department and told her that my mother is deceased and did not have any assets to cover the balance. She told me to send a copy of the death certificate, and they would be able to write off the balance. I mailed it to her the next day. Well, since that conversation, I have received 2 or 3 more statements (latest one was 2 days ago), and all of them show a balance due of $1,650 (1 day was left off the original statement). So, this morning, I called Yolanda at 9:05 a.m. She didnt answer, so I left a message; my call was not returned. Hence the downgrade in their rating from 2 stars to 1 star. So, think twice about using this facility, even if you have to pay extra for the care of a parent elsewhere. read more

DANGER DANGER DANGER If I could no stars I would. This place only cares about collecting the money from the insurance company. Do not send your loved in there. The nursing staff does NOT care. The case manager Danika is a joke. My mom who went there after a hip replacement and received her evening medication at 4am and then was given her morning medications at 8 am. This happened multiple times during her stay there. At night when she asked for assistance to the restroom she was told to use her diaper because there was no one to help her. Another night she was crying due to not getting her medicine in a timely matter and instead of the nurse doing her JOB she got irritated with my mom and asked her "what are you crying for" in a nasty tone. I don't understand how it's literally your job to care and have compassion for you patients. Most of them are already humiliated by needing certain types of assistance. Why make them feel worse. Why is it so hard for you to your job that you chose to do? Better yet why is this place even open! The state needs to do a real investigation not one where they have a heads up. If I could give this place a 0 I would. This place should be shut down.I Can't believe this place is even still open I have an Aunt that went here for care and was treated horrible. She was taken there after a very bad fall that caused her to break her leg and ankle also causing her damage to her hip were she needed surgery. So after her surgery and another week in hospital she was moved there.This was Thursday April 28th and was in so much pain because they didn't give her any of her medicine or pain meds. So she called my other Aunt and my mother crying about how she was treated and also that they havn't even changed her bandage since she been there. Mind you she is also diabetic.After see my mother distraught I ask what the name of the place and she didn't know all she new it was South Mountain something so I started looking up the name of facility with South Mountain and of course abunch of name pop up so I was looking for one that had to do with after care.So I started calling these facility,Lucky this was the first one I called and she was there when my phone called was answered I proceeded to ask them what was going on because my Aunt calling and saying she in pain,hasn't been changed or taken to Dialysis She Proceeds to tell me that breakfast was breakfast served not too long ago and there should be getting her meds soon when I call this was 9 - 9:30A.M So when I ask so then what is going on and she's not calling lying, Whoever answers the phone proceeded to tell me if she could put me on hold to get her nurse to see if they could take the phone call. After a couple minutes on hold another nurse got on the phone and said Her nurse He said that she had breakfast serve she just hasn't touched touched it And she was about to receive her payments. Which is her breakfast was some couldn't potatoes sour crowd and a sausage with bread thrown on it and that's what she had for lunch also they don't even let the patients know that the food is there and they place it on a trade far from them. She asked me if there's anything else she can do to help me I told her no my brother will be there after he gets off work and if he has any more questions or problems he will let them know. Now after my phone call she called my other aunt and was on the phone with her talking when her nurse went in there yelling at her telling her "why did she call her family and complain" So she called my mother back crying and more saying that they got yelled at her for having a family member call This is ridiculous!! So my brother did go there after work with my mother and other aunt with the fist of hell behind them and let me tell you he said that guy was rude as hell 1st trying to deny that he said anything or that he was even around then after my aunt told her she recorded her phone call because she feared for her being there he'd proceeded to tell her I give information to record me she's like oh I don't need your permission I was recording my sister-in-law When he came in yelling at her. This man not knowing my brother was there with my mother and aunt Try to start yelling at them and my brother walked in and told him oh no no this is not happening. Yeah he stopped real quick and was trying to say he didn't try to sound like that. And if we were unhappy there that we need to call 911 to get an ambulance to take her back so that's exactly what we did As they waited there for the ambulance to come pick my aunt backup that's when they finally changed her dressings And cleaned her up then they noticed that they started cleaning all the other patients up as well. The website paints a pretty picture of how this facility is but it to load of shit No supplies for the patients my brother said that they had to send someone else to go buy diapers This is a medical facility they should be stopped up all the time. The orderly that my mother was talking to had told her she lucky she has family Because the one that don't state to drop in health.So my mother told him well not her she had family who are and love her so they picked the wrong person to treat bad. if i could give them a minus several hundred i would. my husband was abused here, fed food not fit for human consumption much less for someone suffering with kidney disease. I am type 2 & was not fit for a diabetic either, too salty to desserts too much sugar, vegetables as hard as a rock. i kept every meal ticket & wrote what was wrong too many starches too much salt fat floating on the meat dishes, 2 days before he died of cardiac arrest someone had turned off the AC in his room. when i walked in with the nurse's assistant & the nurse it was just after 7, he was red in face i put cold towels over him to cool him down removed his socks. the nurses aid adjusted the ac. i told her it was like someone was trying to kill him. He seemed ok but i am no nurse & there was 2 of them there. what surprises me when i reported it to the administrator next am. there was no apology or offer to investigate. I had to go & feed him because he could not swallow & he could choke on food, or it could go down the wrong way. the Staff there did nothing to help just sat behind the nurse's station & maybe you could move them to service the patient. I had to continuously ask for help, that was part of their job. they had to feed my husband thickener in his water due to the problem with the throat developed at Desert Banner i had some envelopes until i got them to finally take care of it. I took some phosphorus binder i paid 100 for that they said they could not get. Someone stole it! He had a sticker under his foot. i asked the nurse to let me have tweezers i would try & hoping she would take care of it she told me to bring them the next day. I did but part stayed in the flesh so i asked a nurse to remove or had a Dr to remove. i saw a bandage but never removed to check if they had just bandaged. He had bruises where they were changing his brief as they called it he told me he had yelled at them for being so rough & i documented who they were. He also had a bruise on his heel when i noticed when i was trying to remove the sticker. It hurt him. He could not do therapy & they said he had stood up & was wobbly. Jerks did not check if he had issues like the bruise on his heel or the sticker. It is not the end of the abuse these people should be in prison. I buried my husband yesterday at the vet memorial. he was a marine & retired officer plus he worked at Hewlett Packard & state farm. He was a man of honor & these animals caused his heart failure he died before they discovered him. What kind of evil lurks here? do not send your loved ones here or move them out immediately. read more

This place is a joke, ( I would give zero if i could) when it comes to the care of patients and family. I currently have my dad here for PT and OT due to his lack of cooperation on his medical care (Dialysis) he was week and needed to get stronger after he was diagnosed with Pnemonia. Yes, my dad has been stubborn with treatment which is why its more important that when he does want to comply that we get him there. His phone in his room does not ring at all and i have had to call the main number to have them "call down the hall to have someonce check his phone" and thats the ONLY way i can reach my dad by phone. The food there is always cold and is served in styrofoam, as according to staff, the wing he is on ( Covid Wing since he is not vaccinated) is not allowed "Hot Plate". When he asks to have it heated up, some staff are great and will heat it up and others say no they can't cause there's no microwave on that wing and are not allowed to go back and forth from that wing. He has had to call family to bring him hot meals, which is very frustrating as the facility gets paid to provide meals. My dad says the food looks like "Pig Slop". He has dialysis 3 times a week and they are supposed to provide transportation to and from his dialysis center, however, on Friday their transportation was over an hour late picking him up and when they did show up, he was on the phone with me and said hold on one minute, the gentalman said no problem then left. when i called the center about this, hey arranged for the driver to come back but my dad kept telling them that the dialysis center wont take him cause he was already 2 hrs late yet they made him go anyway just to be turned away and had to wait for 2.5 hrs for transportation back to SMPA. Now we are on Saturday the next day and I called his dialysis center to see if they can fit him in and the nurse said yes at 3pm but if he is more than an hour late they cannot do a treatment so i called SMPA to see if they could arrange transportation for him to go but could not be late. The admissions person Alley, said she would check with his nurse and call me back, I waited over an hour for her to call back so i called her back and she proceeded to tell me that the dialysis center said "They could not get him in till monday" yet i called the dialysis center and they told me SMPA told then they could not provide transportation, so IMO the SMPA center just did not want to deal with the situation. So now at this point i am really frustrated cause now his health is at risk. I would NOT reccommend this place to anyone for themself or family member unless you dont want to see progress on their BBB, Board of Nursin, and his insurance company along with medicare. read more

My Abuelo came to this facility for therapy after his stroke. We could not have asked for better treatment. Getting him set up with a bed and comfortable in his room was a breeze and members of the staff are bilingual so he felt that everything was thoroughly explained to him. The nursing staff was incredibly helpful and made my Abuelo feel comfortable and cared for. We are very grateful for the entire team at South Mountain. read more

I was highly pressured by the hospital facility at St. Joseph's, Select Specialty, upon my husbands time of discharge from Select to go to South Mt. Acute. In my opinion South Mt. should be closed down by the State of Az. Thiey did not take care of my husband and his needs. After about 10 days my husband became violently ill. I sat with him for hours while he became worse as the day went on. The facility told me he had contracted a very light case of Pneumonia. I sat most of the day and watched my husband get worse. I had asked the facility what they could do for him and they said they were doing all they could for Pneumonia. After I left South Mt. they called me almost immediately to say they had transferred him to Bannon Medical. They didn't tell me he had Covid. I had been at the facility all day and they didn't say a word. Bannon Medical tested him and told me he had Covid. He had to have contacted it at the South Mt. They had done a very poor job taking care of my husband. When I arrived at the facility each day around 10:00 am to 11:00 he was slumped at the bottom of the bed. He had not been attended to that day. He was cold and needed some sort of attention and they hadn't taken care of him at any point. Do not use this facility you will be very sorry. read more

My grandpa has been here for about three years. When he first went we thought that we wouldn't have much more time with him but they have continued to amaze me! They keep him clean, they try to get him to do all of the activities that they have planned even though he is stubborn, and they deal with all the phone calls from my family. We were not allowed to see him for about a year and during that time he had Covid which they nursed him back to health from. I just went back to visit for the first time since covid and his room was set up and had all the pictures and post cards I've been sending on the wall. My mom was sent to this center from the hospital. The place was on a COVID lock down from the get go. We didn't hear from anyone for days. We did not get any information about my mom's care despite of leaving multiple messages. Leaving voice mails was useless, one case manager told me they were too busy. My mom would text us about eating chicken and pasta for 3 days straight. She was also given an inhaler from another patient. We made multiple trips to pick up her dirty laundry or drop off clean clothes and items would disappear and found days later. Upon discharge, she was handed to us on a wheel chair and 5 trash bags. When we inquired about her discharge post care, everyone told us they didn't know anything. After insisting on seeing a nurse in charge, someone came out to say that everything was on a manila envelope for us to read. We said we were concerned about questions we might have later. The nurse stated that everything was there in writing. I guess we needed to google any questions. Today is day 1 at home and not even a follow up phone call to see how she is doing. This is not a place you want to take a person you love. Do your homework and don't let a hospital force you into a place you do not know anything about. This place should be audited. read more

I come here regularly as a healthcare professional and I have noticed that the CNA or LPN named Andrew is very aggressive and apparently laughs at the patients when they scream in pain, as I needed assistance in one patients room he was very rude to the patient and laughed when she screeched in pain. How is this facility even open! I made the mistake of sending my grandmother here for rehabilitation so that she could get her physical and occupational therapy daily! Visiting is not allowed due to Covid which is understandable however I knew something was wrong when I could not get ahold of my grandma after a day I finally went to the office and demand to see or talk to her she was in the worst state I have ever seen her in ever! She was heavily sedated and traumatized to say the least! My Grandma had to be taken to the hospital! She was given medication that was not ever approved to sedate her! She mentioned she could not call us because the staff took away her phone and states some staff would even hit her! Please do NOT choose this facility! read more

My mom was there they did not treat her well didn't shower her brush her hair no normal hygiene when you visit u look like a minion and leave it in room before leave wihich makes no sense lets patient's needs light stay on for hours so unprofessional and just disappointing read more

If I can give this place a minus 10 stars I would. If you love your loved ones don't agree for the hospital to send them there. At the end of the day it's obvious it's about collecting money from insurance companies not care for your loved ones. The hospital sent my dad there after being in the hospital for 3 weeks to "get stronger and get rehab" he was there 20 days and nothing happened. When you come visit you quickly realize that the right hand does not know what the left hand is doing, meaning nobody knows what the hell is going on, specially the weekend staff. Sticky floors, horrible food, transportation company that is supposed to take my dad to dialysis is horrible, he gets picked up so early and gets dropped off so late that a 84 year old man has nothing to eat all day, of course he gets back and he missed dinner, they give him 6 medications in a cup and he is expected to take them all at once. I have so much more to say it would take all day to write. DO NOT send your loved ones there, they will get worse not better and maybe die there. read more

They sent my Tia to this place because she needed care 24/7 because she had slipped in the restroom causing her foot to turn all the way around to the point her bone was sticking out. She needed proper care and someone to always watch her because she is also overweight and can't help herself. We found out that they won't take her to dialysis like she needs to go and they did not change her bandages and she was still soiled in her urine and feces since the day she got there which was Thursday. She called us crying saying she doesn't like it there and that they're treating her bad. After the phone call she called another family member and when they were on the phone you can hear the staff yelling at her telling her "why did you call your family" and they didn't know someone else was on the phone at the time. Then we went right over and the man at first was denying he ever said anything and my other Tia said she recorded him yelling at her on the phone because she was scared for her sisters safety and when she told him he then turned around and tried to tell us "I didn't say it like that" . Also none of the other patients we seen were not being taken care of either, even a orderly came over and told us "she's lucky she has family to make sure she's okay, not many peoples families care and don't know how they're being treated here" read more

SeeThis was my first time in a home health care facility. I am very impressed with the treatment I received In my opinion the facility is very clean the nursing staff was always very helpful when I needed something. I have been around many nursing facilities and this one is top notch in my book. Being an retired firefighter and paramedic I can't say enough about the treatment I received during my stay there. I was asked by dear friends to help find a rehab facility for their mother. I can't say enough about how awesome Tina Bute handled my friend's transfer to South Mountain Post Acute. Not only did she ease the fear by answering all the family's questions but she welcomed their mother shortly after her arrival and continued to check in throughout the stay. The therapy she received was top notch, the food was great. Would highly recommend Tina and the staff at South Mountain. read more

I wouldn't take my dog here, let alone a family member. My mother was here for about a week. On the second day they lost her dentures, I'm guessing they just threw them away. As a result of that, she can no longer eat solid food or speak so that she can be understood. A new set of dentures is $3,500. The Executive Director has yet to return my phone call regarding the dentures. At least he should call and admit their mistake and apologize!! read more

The worst experience imaginable. My mother can't feed herself, and hasn't been able to for a month, nothing in her chart (family member just checked AGAIN tonight) in spite of repeated attempts by the family, to make sure she is helped to eat. They just put the food on her table and leave. She is supposed to have soup with each meal, nope! We show up and there is blended food sitting there, no soup, no one to help her eat. When I insisted on soup when I was there at lunchtime Saturday and she had no soup they brought it and HOORAY - she ate it! She has a bedsore. She is supposed to be repositioned every two hours, nope! Sitting right on it and the chart shows NO recorded of repositioning except once a day. She shouldn't have gotten one in the first place, and their non-attention to it is terrible. This past Saturday I was assured by an RN in the facility that she would be helped to eat, have soup, be repositioned every two hours, and have her med dose increased (that decrease was on hospice, but none of the nurses in the facility called our attention to it.). Her med dose got increased, but that was it according to her current chart. They are neglecting her. If you have a loved one in this facility and they tell you they are taking care of them I would advise that you double check it, again and again. If you are considering putting a loved one in this facility based upon their ratings thing again. If this doesn't get sorted out in 24 hours I plan to move my loved one, even though the move would be hard on her. It can't be as hard as residing in this hell hole. read more

This facility is nowhere near 5 star..The place is very under staffed. They hinder you from doing for yourself to keep you here longer. The care is as terrible as the food..Don't bring your loved ones here! Serious insurance fraud going on..the state needs to re-evaluate this place! read more

I had the worst experience with this center. They accepted my sister late afternoon and moved her from Thunderbird at about 6:30 in the evening. The room was not prepared and they had her dinner sitting there for who knows how long. She needed her meds which was a narcotic but was told that it wouldn't be ready until the next morning. I asked what other alternative and they told me the best they could do was tylenol. She also should have continued on antibiotics but said they had no order. They took some blood work and the doctor requested it early afternoon but was not done until the next morning because no one wanted to access the port. The first night she was there, the ratio on her wing was one nurse to 22 patients. It also took 20 mins for a nurse to come to in order to assist my sister in using the restroom. We then moved her to another acute facility and by that time she was already in the worst condition. Five days after her release from the hospital she had to be taken to back to Thunderbird and she passed away 2 days later. I don't want anyone to go through this ordeal. Never have your loved one moved late in the day, and I would not suggest moving them to this facility especially if you have other options. Another thing, we were assured that meds and other information would be sent with her and someone dropped the ball here. I believe that if this facility did what they were supposed to do, my sister would have still been alive. We would've caught her infection sooner, and therefore she would've gone to the hospital sooner to get her the help she needed. Again, do not have any of your loved ones go to this facility. read more
