After I expressed my concerns about having another surgery, particularly for cosmetic reasons, he remained steadfast. Throughout the visit, I had the impression that he was concerned not with providing quality care but with performing the most lucrative procedure. I printed my name on the form, but didn't sign. I told the receptionist about this when returning the forms. She said she needed to talk to the dentist and left. He came out looking confrontational and asked me "What can I do for you?, as if the receptionist didn't let him know the situation. He finally said that the forms needed to be filled out before he would see me. I said that I would pass on that and that I would take the forms home to shred since my info was on them. He started to tear the forms up and then tried to hand them to me. At that point I said "You get rid of them" and left.
