Absolutely horrible! Worst place to send someone needing care. Staff is inexperienced doctor does not care for patient, staff are rude and slow patient I'm concerned with has gotten a horrible life threatening rash and bed sores on a water restrictions yet has gained over 40 lbs and is getting worse! Sent to this hell hole by Sutter Roseville. Good luck trying to talk to someone there put on hold 44 minutes then disconnected. On hold again 22 minutes and the staff can't even answer the questions! I was there twice for a total for close to a year, looking over the other reviews I wanted to add in my account: I HEAR that this place has a contract with Kaiser, and they seem to get the most difficult and undesirable patients. What the person said about the woman who walks around in and out rooms day and night is true and she steals food constantly from rooms and from the snack trays, those trays are for after meal snacks and she steals them, I guess that explains why I haven't gotten mine sometimes. Yes the staff knows and sees but you can't do anything about it, you can yell and scream at her, doesn't do any good. It wasn't unusual for someone to park their wheel chair in a hallway and start screaming and yelling for about a half hour, then move to another section. Overnight screaming was frequent and sometimes sounded violent many of my roommates didn't seem to know where they were and why, some went through mood swings depending on their medication level. Parking is atrocious between visitors and transports blocking all the spaces... good luck! The food was worse than normal my roast beef dinner had a half eaten chicken wing bone in the middle. I tried to save it but a CNA threw it away before I could complain to the kitchen and when I did of course they didn't believe me. I ordered out a lot I had never met the Doctor until months later on a Saturday when he was like running from room to room asking you a question and before you could bring up a concern, he was out of there. Yeah it's hard to get someone to answer the phone and if the receptionist is gone...forget it the person who does barely knows how to work the phone. Okay I may need surgery and despite my misgivings I may go back Why? Because the nursing staff is really nice and works hard despite the shortcomings. The therapy staff is tops with some of the smartest people I ever met, I often wonder why are they at a place like this or is they aware of the talent they have? This place has some major issues and the truth ain't in them. Their documentation/note are most likely half truths. 1. 1-11-2025. There's a patient who's allowed to travel up and down the hallways by himself. He goes into individual s rooms and picks items that don't belong to him. This is not a new occurrence. Before leaving my home, my mother calls and we're speaking, he comes into her room (4th time) and this times tries to take two items but get yelled out. There was even an accident were my mother woke around 2am and a woman patient was in her room. Where was everyone and why was this woman allowed to be walking around by herself ? 2. 1-11-2025 An employee runs into my mother's room upon my arrival and tells me in a low frantic voice "She has scabies, be careful" while pointing in the direction of the women in the other bed. I wasn't in the room 15 seconds, couldn't tell you what she looked like other than being African-American. All I know is that she flew in, jumped so close to me she could of kissed me. I have waited a couple of weeks to write this review. I wanted to make sure that I calmed down before reviewing my experiences. My husband was released from his second 2 week plus hospital stay to this place that I DID NOT pick. This was his 2nd skilled nursing facility. My suggestion is to avoid this place at all costs. I have a medical background and know correct medical practices. I also know FILTH and CHAOS when I see it. I personally witnessed filth on the floors. Your feet stick to the floor. I was told they are not allowed to use disinfectant. I watched a patient on several occasions walk down the hall and pee and it was wiped up with a CNA's foot with a patient towel. No disinfectant was applied to the urine. There was dried urine in my husband's sink- that I was told is stained. Mold was growing in his bathroom and room (he broke out in a rash from it). My husband had recently been septic due to some serious health issues he is still dealing with. They did not have appropriate supplies for his wound care. They missed his IV antibiotic for several days till a sharp admissions nurse caught it. He has dementia and did not have a name band assigned to him until I mentioned it while outside. I was lied to about his care...mostly because the nurse in charge had no idea what she was doing. Remember he was just treated for sepsis from a very serious abdominal infection. When I called the CA state omnibudsman, he was shocked at all I had to tell him that we encountered. He inspected the place and has filed a report and told me to notify the CA State Health Dept. When they knew I was filing complaints with the state I was followed and spied on while outside. The omnibudsman told me he believed a cover up was in going on. This place is horrible. Additionally, the patients are out of control. The staff chase after them but do not control them. Dementia patients are just wandering around and breaking out----a loud siren would go off constantly by my husband's room. They would scream and curse. The staff are busy dancing and laughing with one another rather than caring for the patients. Because he was not cared for properly, he had an infection relapse and ended up in the hospital for another week plus on IV antibiotics. He is now in another skilled nursing facility. Getting great care. It is clean, does no smell of piss (yes not urine-PISS). They have control over the dementia patients. They are loving and treat them with care, dignity and kindness.And they actually use disinfectant. DO NOT USE THIS FACILITY IF YOU HAVE AN ALTERNATIVES. First of all, I really hate to give places of business such a low rating however in this case I feel it is very important for others to understand our concerns about the quality of care and our experience with RiverPointe Post Acute. Our father was admitted after experiencing a fall and a moderate concussion and fracture of his lower neck as the result of the fall (in April 2025). While at the facility (2 weeks) My brother and I both noted how badly the facility smelled pretty much constantly. There was always the strong smell of urine and feces. We noticed so many of the people hanging out in the hallway smelled and their clothing looked filthy. Patients seemed to be wondering around inside and out without much notice from the facility staff. In the 2 weeks our Dad was there, we discovered later that he only received two showers. I observed 2 of the CNAs not seeming to know what they were doing (training issues?) and one in particular was extremely rude and abrupt with my Dad more than a couple of times. He is literally the nicest guy. We also discovered one week into it his roommate had pneumonia and it had not been noticed/caught nor treated the whole time he had been there and his daughter noted he had not been eating. He was in really bad shape and if his daughter had not been visiting almost daily, this may have continued to be untreated. I don't recall ever seeing the doctor on site either. We did however have great communication with the Nurse/Social Worker team. I was very uncomfortable with my father being here. We have a lot of concerns about the level of care received at RiverPointe Post Acute. read more

The staff here do not care about their patients, they sit around talking at the nurse station and ignore call lights. This is a training facility so there are student CNAS, and I notice they have them do most of their work for them. Sub-par facility and care. PT and OT staff are rock stars, and seem to care about the patients. The rest of the staff, when they bother to show up, are basically unmotivated. The building itself has severe issues, including exposed sharps screws in closets, holes in the drywall going to the outside (and simply covered with tape) and a lack of clean linens. You will need to be an advocate for yourself or your loved one here, especially when looking for timely administering of meds. read more

When it became apparent that I was going to have to go to a care facility after my surgery I was terrified from all the horror stories I had heard about them. But it was necessary. So I chose River Point because they had a better rating than most around me. To my surprise they were wonderful in every way in meeting all my needs not only with efficiency but also with complete compassion. Anytime I needed anything all I had to do was push my light button and they were right there. It was not only me but with other patients as well. From the RN's, the CNA's and even the physical therapist I could not have wanted better treatment. My 90 yr old mom was there last year while recovering from a heart attack. The place smelled horribly of urine. A staff member lied and told me that my mom had a shower when she did not. There was no rest for my mom in the evenings. The night staff carried on like unruly teenagers, they were very loud, room doors stayed open and hallway lights were bright. There was no way she was going to recover if she stayed there. She told me she would starve herself to death if she had to stay there. One good point is the PT staff, they were awesome. Once she began to walk using a walker, I got her out of there! read more

I worked for a short time at River Pointe Post Acute. I can honestly say the facility has the most caring and compassionate department supervisors who works really hard to give the best care for their residents. Mark, the administrator is a great leader and it shows through the dedication each staff give to their residents. Staff friendly upon first arrival but at second visit, staff were standoffish, unwilling to help and spent most of their time talking to each other. Unorganized operation. Did not know names of new patients. When told they needed to call an ambulance to take a patient to ER, staff forgot. Over an hour of waiting and asking where the ambulance was, they apologized and then called them. Patient was left alone in bed and did not answer call button, so they had an accident in bed. Patient had walked to bathroom three times without supervision because they did not want another wet bed. Staff was not there to help them walk. read more

The one star was for the excellent care provided by the physical therapy team. Manny the nurse was kind. 2023-The facility is horrible!!!!! Lets start with after being in the facility for two weeks my elderly mom got COVID!!! They moved her to their "COVID wing". (There were no signs warning anyone it was a COVID wing.) In this wing, her dinner was not delivered so for two nights in a row they feed her peanut butter and jelly for dinner. Because they gave her COVID they stopped providing physical therapy to stop exposure to others, understandable. They then had her walk up and down the hall once a day and called that physical therapy, so they could bill insurance. When I was picking up her laundry, I asked when I could safely be around my mom. The nurse stated that it would be at least 4 more days since testing positive and then they would retest. The next day the administration called to tell me she was being released in two days. I stated she has COVID and yesterday I was told 4 days. Riverpointe, "No she is being released". I again stated, you gave her COVID and now are sending her home when she is still contagious. She is being released in two days. They never retested and sent her home still positive. The call button in the room didn't work so they gave my mom a bell. When her room mate fell getting out bed because she could not longer wait for someone to assist her (30+ minutes) they ended up yelling help because no one responded. The toilet leaked onto the floor for two weeks and the solution was to put a sheet around it. Side door doesn't latch so is unlocked 24/7. In her COVID room you see the roof sliding off of the building. I called 6 times to schedule my mothers pick up, put on hold listening to music for 10+ minutes each time, the sent to voice mail. Left a message 4 times and no one ever called me back. There is more but you get the point. read more

my 93 year old mother was sent here for rehab initally i thought it was a nice place but after two weeks i couldnt get her out of there fast enough . while i did meet some excellent caring nurses there were alot inexperienced staff members working there. its a training facility- at one point i walked into my mothers room and she was wearing someone elses clothes and another time they changed her room and gave me the new extentsion but the phone rang and rang she never picked up when i came to visit her she didnt even have a phone next to her bed sadly on the weekends it understaffed i called the main number and asked to be connected to my mothers room and the receptionist told me she didnt have telephone listing so call back on monday . lots of staff members gather at the nurses station and sit around and talking unaware of call lights riverpoint is a frightening dehumanizing experience my mothers has ptsd from the experience there and so do i, i cant imagine why kaiser would even want to be associated with a place like this. read more

I could not call my husband. He has sepsis and is very ill. The staff is nice once youre inside. However, they don't seem concerned about the patients. I observed a lot of staff talking to each other while my husband was in a wet diaper. I would never recommend this place. Poor management and exquisitely poor ability to reach the patient. I will recommend Melissa, a CNA who has a heart of gold. Also, Roxanne who is an OT. Both seemed invested in patient care unlike the other staff. read more

My mom was assigned to this facility by Kaiser and we found the staff to be extremely helpful and provided us the needed education for her treatment plan. The care team (nurses, PT, OT, CNA) were very friendly, helpful, and accommodated the medical needs of my mom during her stay. We are a big family and there was a large room we were able to use when we came to visit and a large patio while the weather was nice. We appreciated the COVID protocols this facility followed. The negative thing is parking and sometimes the large outdoor patio is filled with smokers. The current administrator Mark, the nurses (Lexi, Mel, Jeannie), the CNA Bailey, and the entire care team are all awesome. read more
