He cut my toenail in three jagged cuts with a large clipper lifting the nail off the nailbed and causing some anguish. My nail is still growing out. I went one time in March. Ever since then Medicare had been charged $19 per month by Hoosier Foot and Ankle (alias Upperline). I notified Medicare and today I'll notify my secondary insurance as they are paying some also. I paid $102.28 in April for that March visit and haven't been back since.

Awful, Awful, Awful Dentist Office! these people have NO IDEA what they are doing…read more 1) They shined a light in my mouth screening for oral cancer -- it took 2 minutes. At the end they tried to charge me $50 out of pocket!!! THEY NEVER ASKED MY PERMISSION TO DO THE SCREENING. So, I refused to pay it. 2) I had a crown ordered and I showed up for my appointment to have it put in. THERE WAS NO APPOINTMENT on the books for me EVER. They had to fit me in since I drove there but I had to wait for an hour to be seen. 3) I showed up for another appointment and they had me scheduled for the same day the NEXT week. I put it into my cell phone when the appointment was made...so either they told me the wrong date or they put it in their computer wrong. 4) THIS IS THE BIG ONE. They will DO THINGS that will void YOUR INSURANCE! Insurance has guidelines that has to be followed in order to be accepted. Smiles for Life is so INCOMPETENT that they will schedule things and do things in such a way that your insurance will DENY the procedure and you will be stuck with the WHOLE COSTS OUT OF YOUR POCKET. Called their separate billing office and was told it should have been billed to both and they would have to submit it now to get a denial due to timely filing and write it off. Called them back, get a different excuse and they refuse to bill. Now I have to pay because their billing department messed up and it took them 8 months to figure out! Now I have to send the insurance after them.

Dr. Weber is an expert and his staff is amazing. He does such great work (many crowns and root canals for me, permanent dentures for my mom and stepdad). I needed a lot of work when I first arrived there and we made a multi-year plan taking into account my insurance and my ability to pay. I am super happy with everything that's ever been done there and I cannot recommend this place enough!
