My mom received care from Powerback Rehab in 2024. They sent me erroneous invoices for an entire year before seemingly admitting their mistake and letting me know that I owe nothing and they would take care of things at their end. They sent this in May, 2025. In March 2026, they sent an invoice as if I still owed the entire amount. I responded letting them know what had happened. In April I received a dunning letter saying they will turn over the invoice to collection and legal. I would stay away from Powerback! read more

My father was admitted to Berkshire Commons in November 2025. Soon after, we received a call from the Powerback Rehabilitation Director of Rehab, Kate Johnson, informing us that my father would be eligible for Physical, Occupational, and Speech therapy, which would be authorized and covered by his insurance. We would be responsible for any small co-pays. My father's condition continued to deteriorate, and he was ultimately placed on hospice care in February 2026. In March we received a bill from Powerback and were surprised to see that Powerback had billed our insurance for ~36 treatments for $7,400 in under 2 months - or almost a treatment daily. In addition to this we received numerous notices from my insurance company that they were declining additional treatments requested by Powerback. Ultimately we were responsible for $620 of these charges which was very unexpected. As I told the Director of Rehabilitation Kate Johnson via phone, my experience with Powerback left me with a bad feeling in my stomach. I feel they were very quick to bill almost daily services but provided little or no feedback on what services they were providing and the expenses being incurred. More importantly we received almost no formal updates on my father's treatments and progress, other than a single comment in their patient portal and after I had visited with my father. It was not until we received a bill in March that we figured out what had been going on. While the staff were pleasant and their intentions might be good, I would strongly recommend anyone who plans to engage with Powerback to clearly define the treatment plan and measures of success up front. My experience is that Powerback was very very good at getting insurance to approve and then execute costly treatments but was less concerned about ensuring the treatments/communication were transparent, appropriate and keeping the family engaged. read more

DO NOT COME HERE. RUN! This place is awful, at best. Dangerous at worst. Every SINGLE patient touchpoint is riddled with mistakes. You have to be constantly vigilant and always advocating for yourself. Everything from the food that you receive to your PT/OT to the cleanliness of yourself and your room to the availability of staff has large, egregious gaps. EVERYTHING. It is so disappointing that you can't count on them to do 1 thing right. That's not an exaggeration. If there is an opportunity for them to ignore something or do it wrong, everyone at ProMedica will choose to ignore/do wrong. There is no competency AT ALL. I had high hopes because it looks great. Want examples? * Room never cleaned during my mothers entire 9-day stay. Her toilet was stopped up and unusable for a day. A used rubber glove laid on the floor for 2 days, despite mentioning it to EVERY staff person that came in. There was food on the carpet the entire time. * Lunch would arrive late and without utensils. When my mother asked for them, they would act "put upon" and tell her she had to wait for them to finish their deliveries. Almost an hour later, they showed up. Her food was cold * She wouldn't receive the main course of her food. When I mentioned it to the CNA, she shrugged. I had to explain to her that my mothers needs to, well... eat. * My mother had a cough that grew worse and worse. No one took it seriously, you couldn't find medical staff. Finally, after 6 days of it getting worse, my sister had to call 911 because no one on staff would help and my mom was starting to have difficulty breathing. Turns out, she has RSV. Easily diagnosed, contagious and very dangerous for a woman with (drum roll please) LUNG CANCER * Once she was taken by ambulance out of the place, they are supposed to hold her room for 3 days (Medicare rules). They did not. Instead, within HOURS, they threw all of her stuff (including food in the fridge!) into plastic bags and threw them on the floor. Everything in the bags leaked and was ruined. See pictures. * My mother would be in her room for more than 3 hours without anyone coming in. * No one would help her brush her teeth. She could go all day asking if she could get help (my mother can't walk) and they would ignore her * If you raise any of these issues (and believe me, there are far more - this is just a sampler), everyone says "but its not my fault". And so all the issues remain. There are some decent people here (and some really awful people as well!). It is obvious that it is management. There is no scheduling (PT comes in during lunch time????), gross negligence and giant gaps in care. I've seen some bad places before, but I still can't believe that this facility is still in operation. On every level - competency, professionalism, cleanliness - they fail. What a wonderful place to visit. The highlight of each day was the different crafts that the crafts director Tenner. They were interesting and timely for the fall season. I even made a pumpkin! Leaves for window to hang, skeleton and the most favorite of mine. An EAGLES scarf with knotted fringes on ends. I made two of them. Jewelry was interesting and creative endeavor as I made two necklaces and a bracelet. All of which I am proud to wear and show Off!!! read more

So far uve had small great nurses in helping with my rehab. Bun-mi is one of the nurses. She is all in a delightful mood and take care of my needs with a very bad break of ankle. She is attentive and willing to help ne in any way. Lucy the RN is Also one if my favorites. I dont have a lot of the names. The food is great and they ask what you want before you get it. Lets see what's happens after I cone back for rehab on my next surgery! read more

I definitely do not recommend this place for rehab. It is very disorganized and chaotic feeling. My father's whiteboard in his room that was supposed to be updated daily with the date, his nurse and CNA, etc never changed after first couple days there. There is no daily schedule given to patients for OT and PT and available recreational activities so people sit around in their rooms waiting the whole time. The NP assigned to my parent was not a gero specialist, even though there is a knowledgeable one on the staff. The only reason I gave it 2 rather than 1 star is that there are a few very hard working and caring nursing staff (e.g. George, Jordan) who seem to pick up the slack for many of the others. read more

Maybe 1 and 1/2 stars because there are some very nice and caring staff, but on the whole be very wary of this place. You can see some good reviews, most of these are from a while ago under different management. Try to talk to people who have been there recently. Promedica has a nice building, they don't provide high level service. The therapy department (what they should be strong at) is mediocre at best. No communication of a plan, next to zero communication between disciplines, a few good therapists, but others that are actually surly, no progress reports except maybe internally and to Medicare so that they can get paid. The manager of the department appears to be completely disconnected from patient progress monitoring. Nursing barely held together by a very few strong individuals, when they are not there you are on your own. Way to much agency temp staffing, probably 2 out of every 3 shifts, and these people cannot be expected to be of any real help. CNAs are distracted and only come to a room after a while if called with the button, appear to be just plain understaffed at times, but maybe it is the very high level of agency temps that make this care ineffective. Housekeeping appeared to rarely come through, really very bad. Can't even imagine what is going on with this. Maybe the trash got emptied some days? Little else done. Case management is just pushing papers around, absolutely no clue about what is happening with the patient. They are required to report once a week, but this is a joke. Literally, they will read something to you about someone and it is not the right patient. Could not be worse. In general the whole building is dysfunctional, no coordination or communication, minimal therapy delivered, and basic daily living services barely above nothing. If you are young, in pretty good health, and can fend for yourself, you might survive being at Promedica. If you are older, have some medical issues, and/or need support, your condition will likely worsen rather than improve. Very sad situation. read more

The only reason I even gave it a one is the food is pretty good. Calls not returned. Family member developed bed sores due to in bed for more than 15 hours. Not always dressed or in same clothes for days. Call bell not left in reach and bed not always low so fell out of bed 3 times trying to get help. Therapy at the most for 15 minutes and only 5 times a week. Rehab?! Family now in Barclay for rehab. Wonderful! Get almost daily calls with updates. Physician on top of things. read more

One star is too high of a rating. The Exton location is terribly disorganized. They do not communicate within their own staff let alone with patients family members. You have to speak with several employees to get resolution to issues (several meals incorrect, several meals extremely late, lack of updates to the family, etc). When calling for an update on your patient, expect a minimum 20minute hold time. Hang up, call back, only to be transferred to the incorrect nurses station. A 5 day stay that was 5 days too long. The nursing staff was good to our patient but it does not outweigh the issues. read more

If I could, I would give this facility 0 stars. The facility in Exton is new and very nice but the staff, however, is incompetent and rude. My father was admitted to Powerback after having back surgery. He was supposed to be there 14 days as he had a spinal fusion and would need some extra time. After 3 days at the facility, they gave him discharge papers. Now keep in mind he couldn't walk 2 steps, was in severe pain and had memory issues from the anesthesia that he was still recovering from. His social worker, Karen, was incompetent and a liar. She claimed to have spoken to my father the very day that I was in visiting to discuss the hospital bed that was to be delivered to his home since they were forcing him to leave. I told her that it wasn't possible because I was there at that time and she stammered and said "Oh, you must have left the room or something." No, I never did. When she spoke to my mother, my mom tried to explain that they didn't have a bed on the first floor. The social worker, Karen, then proceeded to tell her to have him sleep on the couch. Almost 2 weeks out of a difficult back surgery and they think the couch is okay for a back patient who can't walk or pull themselves up. We also explained to the social worker not to have him sign papers alone as his memory wasn't great. It was mentioned multiple time. Wouldn't you know, that afternoon, while my dad was alone, they brought in paperwork for him to sign. We never received the proper discharge paperwork and have called requesting all documents that my dad signed as well as the standard discharge packet. So far nothing. Also, therapy was a total of 3 days. He was supposed to be there a week before they could even complete an evaluation to dismiss. This place is a hot mess. After my father's release, he has worked with a therapist not related to Powerback, and she, too, said that a few of her patients have had similar situations within the last 6 months and the quality of care has gone down the toilet since they opened the Exton center. My dogs are better cared for. Don't go to this heartless, uncaring facility. read more

My 92 year old mother was in this rehab after being in chester county hospital. Let's start with the positive, chester county hospital was great. My mother loves the attentive care and new facilities. However, power back was a real disappointment. I engaged care givers and medical care that was more protective of the facility than their patients. All in all pick Shannondell which she is in now and WAY better ! read more

Overall my loved one was cared for well by the nursing staff. Depending on who was taking care of him, depended on the amount of communication we got. The facility is new and it was clearly designed to make the patient feel at home. My loved one had a few outstanding nurses and aids during his three week stay. Overall the communication could of been much better and more efficient. They haven't identified how to communicate with the family during COVID-19 on a regular basis. The poor experience we had was with the social work area. The social worker didn't have the experience or capability to meet our loved ones needs, she didn't listen and because she took so long to address those needs, we had to do it ourselves. read more

If I could give this facility 0 stars I would! They transported my Aunt from CCH and my Aunt wanted to change her HIPAA from the 2 people that were on to me and another person. She did not feel safe with the ppl taking over her medical care. Within 24hrs I was REMOVED and blocked from any communication from my Aunt. When I tried calling the head of SS Kim she wouldn't reply until I went to the head administrator who finally put her thru to me. She was beyond rude it bordered on threatening. She would not answer my question as to WHY I was removed when there were 3 ppl in her room who listened to her fears. They witnessed her signature. I said do you care about her or just the $$$. I asked for a copy of the HIPAA and she hung up. I then called back and was treated very rude by the Head Nurse Jean who said you don't need to know the "plan that will come to fruition tomorrow". WHAT?? I can't even get thru to her room to speak to her nobody can. I am beyond worried and concerned. read more

Since they paid yelp to take my comment down I'll keep posting it. My name is Matthew. I use to work at PowerBack Exton for a little over a year taking care of your family members, people. I recently was terminated over the phone for using my phone on company time, during my year there I was never written up for any misconduct, attendance or any miscellaneous imperfections. Thao (center executive) and Heather (nursing director) were poor choices for this role. I would come to them with stories of people sitting in their own bodily fluids way into the afternoon, from the NIGHT SHIFT. From someone from who was on the inside and watch employees come in drunk or high, their jobs were safe. This place is not a safe place. The Department of Health should really look into these leaders and this building. I wouldn't put my worst enemy here. And the only regret I have is that I didn't find other work before I was wrongfully terminated. PLEASE don't send your loved one to Genesis/Powerback buildings, I have see my share of neglect and abuse, and it is right. (And I have reported it to Thao and Heather, and my direct boss Michele Nolan) terrible place. I promise one one cares about your loved one at this place. I PROMISE. Because everyone is over the abuse. I once seen a patient with shellfish allergy been given shellfish, TWICE! I also saw wrong meds distributed and potentially positive Covid people admitted before results were even revealed or test was even conducted. Horrible. My grandmother was there after she got hip surgery because she broke her hip and since she got there it went down hill. My grandmother does not speak English and my mother is her translator. She was back and forth from the hospital and the rehab 3 times. The first time was right after her hip surgery she had to stay there for 2 weeks and while there she felt fine, but out of no where she had an infection in her leg so they had to bring her back to the hospital. After that was cleared she went back and got worse at the rehab. She does have a lot of internal health issues but there was a huge lack of communication between the nurse practitioner and my mother. No one knew what medications she was taking, no one would get back to us in a timely manner. We had to keep calling to get updates, she was so sedated on pain meds and was not able to do her physical therapy which is why she was there. No communication between cardiologist and the nurse which should have happened right away since she has heart issues. To top it all off, my mother was called her in the morning to see how she was doing and my grandmother said they had the bed ready but didn't know why so she passed the phone to the nurse there and they were taking her back to the hospital and NO ONE called or let me mom know. My grandmother does not speak any English so they were just taking her to the hospital with out even telling us what is going on so we can communicate that with her. They assumed because she was on the phone with her that my grandmother would of told her but how would she of told her when she doesn't speak any English and is so out of it because of all her meds she is taking. Now she is back at the hospital for the third time. She is not going back to this rehab and I do not recommend it for anyone. I understand this is a rehab center and there primarily for physical therapy but there is a lack of care with everything else. read more
