My mom has lived here since 1996 and I wholeheartedly believe that the care and love she has received from this staff is what has kept her active and strong well into her 90th year. Powell Valley is about family and community and it's beautiful. Not only are their residents treated like gold, but they go above and beyond in efforts for the surrounding neighborhoods. Do NOT expect to receive the care your loved one is highly overpriced for here!! There is NEVER ENOUGH STAFF & an extreme turnover of employees, making it impossible to receive consistent care. The training is minimal, usually done by a new, inexperienced employee to another. The wait time for a call response can be an hour & a half! THIS IS UNACCEPTABLE! Someone could have a STROKE & MINUTES to respond ARE CRITICAL!! The dining room is a JOKE! Twice within 3 weeks, there was bo food prepared for dinner. The residents were served a single slice of pizza! There was an occasion where NO SERVERS showed up for work @ dinner. A resident's FAMILY MEMBER went into the kitchen (no food handlers license!) & got plates of food to serve the residents!! The cook & several servers QUIT @ same time. The employees are UNDER PAID & don't feel appreciated by staff. The excellent Director on staff when my Dad moved in 2021 was fired for trying to make an improvement that rubbed someone wrong. Ever since her termination, for about 1 1/2 years, ALL has gone DOWNHILL. We just turned the facility in to the STATE & OMBUDSMAN because a CONTRACTED MEDIC forced medication on my Dad that wasn't his! His medication is handled by family, not Powell Valley Living staff! He didn't even know what he took! It could have killed him!! In addition, His blood sugar & bathing schedule is never followed, however he's charged for it! I'm exhausted from micro managing a facility that my Dad is draining his life savings for! DON'T MOVE YOUR LOVED ONES HERE!! read more

My mother was there for 10 years and was afraid to move because she found it hard to adapt to change, so she stayed, as the service went downhill when the new owner took over in 2000. My mother wasn't given her showers on such a regular basis that I deducted it from rent each month. She remain lucid and I visited several times a week even during Covid ( me on her patio, masked and mom inside the screen door masked). They began to let dogs live there and one big dog ran up to my mom growling, showing teeth and tried to jump on her as I was strolling her outside at the facility (she was in a wheelchair paralyzed on one side) I jumped in and saved mom while yelling at the dog and at the resident who owned it. Someone put their head outside their apartment and shouted that the dogs needed to be on leashes. When I brought up the matter to the person who ran the facility, I was told that I was mistaken and that it's a nice dog. They also let violent memory care patients into the dining room and the assisted living facility who would shout obscenities at the chef and strike terror in the residents. Food wasn't delivered for up to an hour so mom commonly returned to her room without a meal. I kept her fridge stocked with healthy items so she could still be nourished. Meds were such a mess, I won't even start. They had the wheelchair residents mailboxes at a height of 6 foot so they had to dangerously try to stand, unlock their mailbox and retrieve their mail. When I tried to resolve this, they gave a different mailbox, lower, that mom could reach, but then ended up renting that corresponding room, they removed my moms name from that mailbox and they had the mail given to the receptionist (who sometimes delivered it, not always). Some important mail could not be delivered without an actual mailbox and that's when it came to my attention. I heard other residents struggle to try to reach the height of their mailbox. Once in the middle of the night, mom fell in her bathroom, breaking her arm, and Powell Valley didn't contact me. I found out when I talked to mom the next day after she had returned from the hospital! Another time, the maid, in changing the bed linens put her metal bedframe on her oxygen tube so that night when she couldn't breathe and was taken to the hospital. I discovered this as I was gathering things to take to her hospital room. She had to wait for more than an hour for assistance to use the bathroom very often. Once, when she had diarrhea, and had been waiting over an hour, and was crying when an aide (not assigned to mom's room) popped in to say hi. This particular aide was mom's friend and she immediately called the aide station to tell them to "get someone down here immediately" to help my mom. Someone came reluctantly. They let a mental patient live next door to my mom who made threatening remarks to her often. I brought this up but it fell in deaf ears. I do need to say that there were some wonderful individuals who I trusted and mom and I both loved, but the overall place was run horribly. Theft, losing clothes, wrong meds given, showers not given, bathroom assistance not dependable, Covid shots discontinued without notice, I wouldn't be informed when there were Covid cases in the building, they didn't require residents to mask during the height of the pandemic; these were just a small sampling of the problems. My goal was to try to keep my mom safe and them honest in what they had promised. I was on a first name basis with the state ombudsman. My mom was lucid and had family checking in constantly, I can't imagine what it's like for less fortunate residents. read more

Review of Powell Valley Assisted Living (Not Memory Care) Our family placed my sister at Powell Valley Assisted Living after she suffered a debilitating stroke in 2022. We chose this facility because it appeared clean and professional, and we were told there would be medical oversight. Unfortunately, what we experienced over the next couple of years was deeply upsetting and unacceptable. To be clear, this review is about Powell Valley Assisted Living only. We had no experience with the Memory Care Community, which is located in a separate building. We were initially told there was a doctor on staff, but that turned out to be misleading. The person overseeing medical matters had a doctorate, but he was not a licensed medical doctor. He refused to manage my sister's critical prescriptions-especially her long-term oxycodone for severe pain-and insisted her outside physician handle it, despite her being in their care full time. Medication management was consistently poor. There were repeated delays and missed doses. At one point, her medication was even stolen, and there was no accountability. Staff turnover was high, and untrained employees were training new hires. Medication changes from hospice were not being communicated, or faxes were ignored-once simply because there was no paper in the machine and no one noticed. One particularly alarming situation involved her nerve pain medication. I noticed she wasn't receiving it, and when I checked the medication log, a med tech had marked that they were out of the medication--yet on other days, another med tech had given it to her. When I asked about this discrepancy, I was told by staff that they had " borrowed" the medication from other residence, or from " somewhere," with no clear explanation. Shockingly, it turns out she hadn't been given her nerve pain medication for two weeks. Even when she was placed on hospice, the disorganization continued. Staff were confused about how often she could receive her pain medication. I had to call hospice myself and stand there on speaker phone to get clarity. Watching someone you love suffer in pain while staff delay care is devastating. Other issues included: - Personal belongings went missing, including six of her dresses. - Her laundry was mixed up with other residents' clothes. - Food quality declined sharply depending on who was cooking-at times, it was unrecognizable. - At the end of her life, when she could no longer feed herself, we were never informed-and she was left without help. One day I raised concern about the fact that her medications were not being given on time, and I asked if anyone was overseeing this. The staff member's response was, "We can't really do that-we have over 90 residents we give meds to." That kind of response summed up the lack of oversight and accountability we experienced throughout her stay. Families place loved ones in assisted living facilities hoping for compassionate, consistent care and peace of mind. But without constant family oversight, your loved one at Powell Valley Assisted Living may not receive the care they need-or deserve. Our mom has been an assisted living resident here for roughly 5 years. When we first placed her here, it was under much different, caring and involved Management. The new owner "Trevor" who took over about 3 years ago has no control over the staff and seems to not care. They are unqualified in the dispersement of medications (Mom's pills / med's found on floor NUMEROUS times so she never received them!), food service has to be at an all time low when you have residents leaving because they wait literally an hour or more to be fed. The food staff seem to be rude, severely under trained and just do not care. We have bought new clothing for mom and come back a week later and its gone. Money has been taken from her room along with the new clothing which we have to purchase every month or so as well. Poor care on doing laundry, bathing and personal care which she is charged EXTRA for. Our family is told we have to purchase wipes, gloves, bedding pads etc. which should be an "Operating Cost" not a burden to the paying residents who shell out thousands per month to be ignored and abused. Our family will be talking to Multnomah County DHS regarding Elder Abuse/Neglect for the poor residents that depend on this facility for their care. The residents cannot even depend on a timely meal which is the basic of functions in an environment of care. SHAME ON YOU Trevor for letting this formerly wonderful facility becoming the rock bottom of Senior Care. We have read the reviews of 4 and 5 stars and find it difficult to believe that those who write them are real or truly visit the facility the way it is now...maybe when Pam and her family owned it but not now! read more

My mother lived here for about a year. As long as you're in the main facility it's a good place, nice apartments, pretty courtyard, etc. But the memory care wing - not so much. read more

We moved my mom in February of 2021 when she was able to mostly function on here own. When we moved her in the food seemed decent but hi have since found out otherwise. I initially thought she was just complaining but have photos of some the food served. A burnt waffle for breakfast is not acceptable. She weighs 110 lbs so proper food and nutrition is so important to her well being. She became sick a few weeks ago needed additional care. She could no longer go to the dining area, so they were suppose to bring food. Some meals they would take the order and never bring it. Since being sick she need help with medications. This is a service that can be provide for extra cost, but the first couple of days have been screwed up, so were starting to feel care is not up to par as well. There telling us staffing is short due to covid. Not an excuse you want to hear when you paying $5000 a month for care. read more

Pleases think twice before putting your loved one here. They initially fawn all over you and pretend to cater to your every wish. After you have been there, every concern is treated as your fault, not theirs. The staff do not communicate well between departments and contradict each other constantly. The food is mediocre. The wait staff is composed of students. You are made to feel insignificant. read more
