We have also observed multiple care concerns that appear related to insufficient staffing within the facility: * She has been left sitting in soiled briefs for extended periods * She does not appear to be repositioned regularly * When family requests assistance, nurses state that only aides can provide that care, yet aides are not available or responding * Staff frequently appear overwhelmed and unable to meet residents' basic needs promptly I have attempted to address these issues internally by contacting Cat, the Assistant Director of Nursing, through both phone and email. The facility seemed clean, although there was a faint smell of urine in the hallways. She told me they were cleaning and the receptacles were in the hallway at the time. The room she showed me was remodeled. I sent over a list of required care items since my husband has Parkinson's disease and I wanted the nursing staff to have a heads up. I had asked if depends were supplied and she said yes. Upon arrival, no one greeted us or came out to help. He is hard for me to manage, but I got him in his wheelchair and took him up to his room myself. I then had to go back down and unload the car, using a cart that was in the lobby. His room was outdated and shabby. The dresser drawer was broken off the track. The shelving in the closet and bathroom were sticky. The floor was dirty and the room stunk. I went down to talk to the admin to relay this info. I had a flight booked for a family occasion and had to leave. The first time he needed to be changed, he said the nurse was not happy because he did not have his own depends. I had his daughter bring some, plus another pillow and better blanket, as well as some room spray. He was admitted on a Tuesday and by Sunday night had vomited several times, was running a fever, and couldn't breathe. He had no clean clothes left (I brought 11 changes of cloths, but he is totally incontinent. I was told laundry service was supplied, but that never happened until I complained) Thankfully, his daughter was there and she called me. I told them to call an ambulance ASAP. He is in the hospital with aspirational pneumonia. This place is totally negligent, and I feel that my husband was totally ignored. Upon talking to the floor nurse, she claimed she was not given the list of care needs. The admin acknowledged receipt of the list. I'm beating myself up over this decision but these people are deceptive. I would give 0 stars.

It turns out that my concussion and light sensitivity were so bad that they were beyond Dr. Pingree's scope. Dr. Pingree gave me a recommendation for a specialized eye doctor in Wexford, who subsequently sent me to vision therapy (to clarify, vestibular therapy gets your two eyes and brain to work together; vision therapy just gets your two eyes to work together, independent of the brain). Long story short, I completed the vision therapy sessions Dr. Pingree recommended and am now back to her. She said there was no comparison to where I began in April 2016 when we first met to now, February 2017. Complete night and day difference. Dr. Pingree is professional, cordial, and knows what she's doing. I didn't care for her in the beginning when she told me I wasn't allowed to wear sunglasses indoors, but now we laugh about that every single visit; I feel much better. Dr. Pingree is a tough love kind of person; do your exercises no matter how difficult they are, or how dizzy/nauseous they make you feel. She knows what's up and has 10+ years of dealing with difficult clients like me. I don't know why people skip out on her exercises; I mean, I want to be able to see again! LOL Thanks for everything, Dr. Pingree! If I ever go through another car accident, I'll be in touch (hopefully, that doesn't happen)!

They quickly and efficiently determined a plan to get me back to my normal self. They were able to individually work on both my knee and my elbow but in a timely manner within my allotted one hour sessions. Everyone that I worked with was very nice and professional. Each person knew what they were doing and gave me great advice as well as exercises for me to do on my own at home. My conditions were somewhat minor but still needed attention. I was able to get through PT in about a month's time. I couldn't have been happier with Steve + Team. I highly recommend this group if you're looking to effectively recover from an injury/condition and work with a congenial team that cares.

I suffered my head injury 30 years ago in the military and slowly declined over the years until the symptoms were so bad the quality of my life deteriorated to the point I could barely function. Unaware of what was happening I visited many doctors and specialists with everyone presenting the same conclusion they didn't know what was causing the symptoms and they had no answer or cure. I tried rehab and different vision therapies with limited results. Thankfully I found Dr Simkovich and immediately scheduled an appointment. If you are reading this post you have the same questions and concerns I did. Is this real and does it work? I can personally attest the treatment is real and works. I only wish I had found Dr Simkovich years ago and could have ended the symptoms and effects of my head injury when my accident occurred. Dr Simkovich is very caring and passionate about the patient and treatment. The office staff is fantastic and will ensure you are well taken care of during your visit. I usually do not post reviews however I know what it is like to suffer the affects of a concussion and promised myself I would do everything possible to let people know there is a legitimate treatment available that can alleviate your symptoms.     No one likes to be in pain. It's depressing. It wears on you. It has long term ramifications. So,…read morewhen I developed severe shoulder pain early in 2021 - I struggled to find some relief. I tried everything. Patches. Over the counter meds. Massage. Special pillows. After several weeks of that, I finally had to contact my GP. The first course of action was to schedule physical therapy. There was no question where I'd go - Breakout Advisors and Rehabilitation. My mom had been treated there when she'd had a full knee replacement and had a wonderful experience. She said they treated her like family and gave her excellent care. That is an understatement. From my first interaction with Brad Stotsky by email to my final phone call with Ms. Patty - I was treated with the utmost care and respect. My first consultation was very detailed and I felt like I was listened to and heard. Every staff member is friendly and pleasant...and knowledgeable. They know your case in depth and remember it at every appointment. Every time I went in, I felt like I was visiting with friends...not getting treatment. I felt like I was in excellent hands. After several weeks of PT and other medical appointments, it was deemed I needed surgery. Neck surgery. That's pretty scary. However, I had an excellent support team at Breakout. I had days where I was really down and in a lot of pain leading up to the surgery, but appointments lifted my spirits and gave me relief from pain. I had a few weeks off post-op when I really missed seeing everyone, so when I was able to go back for post-op it was not only great to learn some tools to cope with my post-op body...but to see everyone! In my time at Breakout post op, I was given the tools to strengthen and protect my body after an invasive surgery. I left their care feeling like I had been given the gift of health and wellness. I also feel like I was given an entire staff of new friends. I will miss seeing them regularly and catching up on their lives, but I know if I ever need their support and care again - they will be there for me. I cannot recommend Breakout Advisors and Rehabilitation enough for any PT you might need....and I thank them for their excellent care!
