All the nursing staff were so kind and courteous with all their dealing and the office staff (social worker/chaplain) were excellent with answering any questions we had. The facility is amazing and clean with multiple areas for family to relax when they may need a small break. We are so thankful for the staff at Evelyn's house for easing this transition for our family.

He was in extreme horrible pain, hollering and moaning for hours & hours. The morphine they were giving him did not help-out at all with his pain. The nurse was suspose to inject the morphine in the side of his mouth towards the back, instead she injected it in the front of his mouth, causing him to aspirate (CHOKE) on it. It was absolutely one of the worst experiences of my life, and my siblings. I wouldn't never recommend this company to anyone. I got a different hospice care company for my Mom, they were great. They told me that Crossroads should have given my Dad a fentanyl patch, since the morphine wasn't working.

Someone needs to check on the residents throughly because residents go without hearing aids for…read moremonths, they don't get assisted with toileting, have rashes between their legs(I was told and a family member had shown me a picture, residents who needs assistance going back to their rooms after meals are left in dining room for extended periods of time even after calling for someone to assist them, resident with diet/texture restriction being allowed to sleep allowed with a lollipop in their mouth, staff uses and takes from the residents ( using their deodorant, takes their toothpaste, hair bonnet, Q-tips, phone chargers, body wash, and their change tucked away in their drawer. They falsifiy information do that it can pass with State. Breanna, Ast. DON and Nurse Sandy even changed my mothers medication pass time without our knowledge or her PCP's order. If State would not notify them and show up unannounced maybe thing wouldn't be allowed. I'd noticed that a lot of workers walk around with their ear buds in talking on the phone even when they should be assisting residents in the shower. Also, they will put your loved one on Psych meds without notifying family not even checking their health history. Maybe because the Chief of Psych is also part owner of Delmar Gardens, seems like a racket. No one cares, like I was told by Charge Nurse Sandy "I don't know and I don't care" when discussing my mother's ER diagnosis. It's all about having a body, even if not qualified. Richard( Admn), Brenda (DON), Breanna (Ast. DON,), Charge Nurse Sandy, Marci(Social Worker), Optum Nurses (Sara & Amanda) PCP Alex Rudoi will all burn in hell for how you have NEGLECTED my mother damn near killing her again with the same medication Levothyroxine that was changed to be taken at night but is suppose to be given in A.M. on an empty stomach. We were hopeful and supportive that Delmar Gardens West would be a better placement...and it was. Supportive nursing staff, quiet environment, clean and no offensive odors, supportive personnel from top to bottom. When reading nursing home reviews there is so much negativity that it can be frightening. The reality is that no one wants to be sick and in need of nursing home care. Go and observe, do your research and visit often. Be an advocate for your loved one. Delmar Gardens West was a good choice for my Mother in Law.

Let me start off by saying this is the first negative review I have ever written in my life. I have…read moreworked with many Hospice Companies for over 20 years during my career in the Senior Housing Industry as well as my own personal family end of life events. I recently have experience working with Caris as my loved one was put on hospice originally to help with the loved ones anxiety. Within a few weeks after my loved one had been approved for hospice with Charis, my loved one started to decline and was then assessed to continue on Hospice as an end of life service. At the beginning, the family member that had POA for healthcare had requested that medication be adjusted so the love one would be more alert during the day. As the loved one declined and they put her on end of life service, the medication that helped with the loved ones severe anxiety was discontinued. When I asked how this anxiety would be controlled, I was told that they would take care of it. After the change, My loved one became more anxious and agitated. She would moan, cry for help, and try to get out of bed on her own. It was clear that our loved one was very uncomfortable and in distress. When questioned again about the medication, the nurse became defensive and started accusing the family's decision to try to keep the loved one more alert which was before the end of life process had begun. The nurse would say, yes I hear her moaning and crying out, she does that all the time. That night, our loved one got out of bed, fell, cut her head and received a gash in her leg. After that, the following day, medication was finally addressed. Our loved one finally got the peace and dignity she deserved. Hospice workers should be trained to put the care of the patient first while educating family members on what should be done and why changes are important for their loved one to not suffer. Our loved one suffered for days because this simple rule was not followed . No family member should ever have to see their loved one's life end in this manner. After finally getting the peace and dignity our loved one deserved, end of life came. After the death, The misinformation continued. The cause of death was given by the medical director of Caris as dementia. Our loved one had never been tested for dementia, diagnosed with dementia, or shown signs of dementia. Being certified by the state to conduct assessments to find right placement for individuals and running Memory Care communities for years, I believe I would have recognized the signs. No, I will not use Caris or recommend Caris for end of life services to anyone.
