I don't really want to write this review, but I feel it's my last resource and opportunity to call…read moresomeone's attention at the funeral home. My adopted son passed away at the end of July 2024. Lietz-Fraze Funeral Home & Crematory handled the cremation. They were really nice and empathetic. I have no complaints about their customer service. Until we received the death certificate. On the death certificate, it was my son's biological parents who were listed as his parents, even though I had provided the funeral home with his birth certificate with our names on it, and the Courts paperwork making us as his adoptive parents. I called the funeral home several times, I left messages, I left voice mail messages, and I sent emails with copies of (again) the birth certificate and the adoption order signed by the judge. I even called Vital Records in Arizona to inquire whether I could request a change in the information. Their Office informed me that "only" the funeral home could make the changes by amending certain worksheet. It has been almost 16 months now since the passing of my son, and I am still waiting to receive a single communication from the funeral home. Needless to say that I have not received an amended death certificate, either. I hope that this review will be seen by someone at the funeral home, other than the director with whom I was communicating during all this time. Thank you for your attention to this matter.
