History
Established in 2025.
Merlin Direct Primary Care opened in 2025 at 7978 Broadway, Suite 100, in Alamo Heights on the Broadway corridor.
The practice was built around one idea: primary care works when the doctor has time. Instead of billing insurance for routine care, Merlin runs on a flat monthly membership paid directly to the practice. That removes the coding, claims and prior authorization work that consumes most of a traditional primary care day, and the time it frees goes into appointments.
Membership covers unlimited office and telehealth visits, appointments of 30 to 60 minutes, same-day or next-day availability by request, and direct phone and text access to Dr. Ramón. In-office procedures are included.
Merlin does not bill insurance, Medicare or Medicaid. Membership is not health insurance and does not replace it. Most members keep a plan for hospital, surgical and specialist care and use their membership for everything else.
Meet the Business Owner: Dr. Ramon D.
I am Dr. Ramón Diez-Barroso, and I am the only physician at Merlin Direct Primary Care.
I am board certified in family medicine. I trained at Baylor College of Medicine and completed a general surgery internship before that. I grew up in Laredo, my family's roots run from Mexico City to San Antonio, and my wife and I chose this city to build a practice in.
I did not open a membership practice to be exclusive. I opened one because I could not practice the way I wanted inside a system that gave me ten minutes with each person. My appointments now run 30 to 60 minutes. My patients can text me. I know their histories without reading them off a screen while they talk.
I speak Spanish and I see patients in Spanish.
What I care about is healthspan, not just lifespan. Years lived well, not only years counted. That takes time and attention, which is exactly what this model is built to provide.
Because you deserve your doctor's time.
