After repeated phone calls and emails, they finally sent one form. They kept telling me they would send via snail and via email the second form. Both forms are necessary to got the state ID card. For many months they have continued to lie. This has been going on for over for months now. To this date, I never received the form. My application has been denied as I did not send in the second form. I will find another Doctor and start the process all over. I am working with my credit card company to have the charges reversed. DO NOT use this company. They are cheats and liars.

They had NO PHONE number to call so I did not want to go to the clinic about 15 mins after purchase I called the main number (not the clinic as there is no phone)and canceled I asked if there would be a penalty for cancel I was told no and I was told I was going to get a call back but I never did they did sent me an email with a refund receipt $99 I thought this was over and I moved on but sadly it was not over. They sent me an email and a text saying I had an appt the next day I responded to both saying No I cancelled and showed the refund receipt. Then the next day they charged me a no show $25. I figured it was a mostake after SEVERAL phone calls to the main phone and messages left and emails they tell me they cannot refund the $25 that they refunded $99 for cancelling but $25 is for NO SHOW! UMMMM IF I CANCELLED WHY WOULD WOULD I SHOW I am still arguing with them over $25. I will not stop until I get my refund. This is a JOKE
