    I came by ambulance from the hospital and waited after ambulance driver buzzed to have the door…read moreopened. When no one came to let us in he called the front desk; still no one came. I suggested he call my son who had followed us to the facility. I figured he was probably in side looking for my room. He came and let us in. When we got to the room there was a nurse and an aide in the bathroom. There was no greeting. I asked the nurse to help me to the bathroom in my wheel chair. She said she couldn't help me because I "wasn't in the system yet". That was the tone of most of my stay (almost three weeks). An aide dropped me when transferring me to my bed. She had told me to to hold on to the drug cupboard handles and she pulled my up by the waistband of my pajama bottoms. I went into a full squat which had a terrible effect on my bone on bone right knee. Another aide left me on the commode for over an hour despite my using the call light and even called out. I was still using a wheel chair to get around. I struggled to get to the wheel chair as I hadn't used a walker yet and my feet were dangling from the the commode seat because it was too high. When she finally did come she said she just forgot. She asked me if I wanted anything else and when I said no she threw the light cord and bed position controller on the floor and left. The OT and PT people were wonderful and are the only reason I I stayed. They were so dedicated. I made real progress. The house Dr. came to my room to introduce himself and the second time he came he was their about 5 minutes when he said he would go and get a list of my meds because I had questions about why I wasn't receiving my basic meds. He never came back until a day before I came home and left when he saw the OT guy there and said he needed to talk to him. Never saw the man again and he billed my insurance for 6 35 minute visits . The experience was awful. I did meet a few people on the care staff who seemed to 'care' but by and large Not a very 'caring' group of people.

    My one piece of advice if you are considering Shelby Nursing Center for your loved one. DON'T. My…read moremother spent approximately three weeks at Shelby. The status quo for the staff is "I don't know" or "I haven't seen her yet so I am unable to assess what is going on" or misrepresenting the amount of food my mother was eating. I spoke directly with Nurse Practitioner Paula (I was out-of-state for the first week my mother was at Shelby) who reassured me that my mother was eating 50 to 75 percent of her meals, but family members reported that my mother was eating only a forkful or two, with limited water intake. Even when we reported to another nurse and Nurse Practitioner the lack of eating, we were brushed off. One nurse was more concerned that we sat on an empty bed than the state of my mother. The Nursing Director also claimed not to know my mother was not eating. Additionally, Charlotte in the 200 wing of the rehabilitation section is rude and uncaring and should not be working in a rehab facility. My mother continued to rapidly decline, and it is my understanding that after two days of not eating the facility should have transported her back to the hospital if they were unable to care for her. This did not happen. She was also moaning constantly. We had to have her admitted to the hospital ourselves, where she received in-hospital hospice care. Realize that if you admit your loved one to this facility you will need to advocate for your loved one continuously. The unpleasant experience with Shelby has shaken my faith in the medical profession. There were a few bright spots with the nursing staff. Many thanks to Howard, Jason and Chiquita.

    I WARN YOU IF YOU CARE ABOUT YOUR LOVED ONE STAY CLEAR FROM THIS FACILITY!!…read more ANYONE EXPERIENCING A SIMILAR SITUATION I HIGHLY ENCOURAGE YOU TO REPORT IT TO THE STATE. THIS IS THE ONLY WAY TO ENSURE THIS FACILITY WILL BE STOPPED!! State of Michigan Ombudsman 1-800-882-6006 Select Option #1 then Option #3 My mother was discharged from the hospital to this center, while it was the third choice on our list it still had decent Medicare ratings.... Boy was that wrong. Within 3 short days, my mother's health significantly declined and went unaddressed. By the 5th day I demanded EMS to be called even though "she was fine". Apparently a blood pressure of 70/50 is thriving. My mother was severely dehydrated, had blood in her stool and we were not informed, fluid overloading her lungs and had an infection on the verge of being sepsis. On top of her being put in diapers unnecessarily, she was left in a bed with urine soaked cloths for hours at a time because of their lack of response time and laziness. My mother was 1:1 assist to the bathroom due to fall risk after back surgery, but no where lacked the ability to get there or needing diapers. I gave the Administrator the opportunity to discuss this matter privately, however have not received a phone call back. I will make sure I make as many people aware of the safety & quality of care issues, lack of compassion, and inability to properly manage a facility know. It's quite apparent based upon LARA though the State of Michigan reporting (see pictures attached) that complaints are a common occurrence for this facility.

My sister who has ALS was brought here from the hospital based on a social worker recommendation as a good place to rehab after breaking her ankle. While PT and OT worked within her physical limitations no one ever knew when they would be coming. There is no schedule or feed back unless you happen to catch them with resident The nurses were pathetic. Of the 3 weeks she was there I can only count one instance where the nurse extended any hands on care to her. That nurse was a midnight shift nurse who had 30+ years of experience. Nurses distributed medication only and not in a timely manner. If meds were ordered for morning it was not unusual to receive them in late afternoon or not at all. Multiple times pain meds or regular scheduled meds when ask when she would receive them was answered with the "I'm busy and I'll try to get to her soon" Aides were deplorable . My sister was told many times to not ring for help between the hours of 4-6pm as they were too busy handing out trays and feeding people. With my sisters ALS somedays her arms could not move to feed herself. On those days her food tray was left on table with no help in sight. There was actually one aide who cried the whole time she gave my sister a bed bath - seriously wept. Call buttons go unanswered for an hour or more. Aides will come into room ask what you want and then leave for 2-3 hours. Honestly they would only help if family was standing there demanding help We had to hire people to come daily to help my sister with simple task and so that person could get help from staff for my sister and feed her. There was at least 4 residents who had outside help come in. If we would of known that they were not family members while touring that would of diverted us to another rehab facility immediately HUGE RED FLAG Up until my sister broke her ankle she was able to walk a few steps and sit in wheelchair due to her ALS It was her hope that this facility could help her learn how to transfer to wheel chair and hopefully her ankle would heal while there. None of that happened. Without outside help and occasional help from OT or PT she would of sat in bed 24/7 We brought these issues to the assistant director of nursing attention and she actually witnessed personally the deplorable condition my sister was left in after the aides put her in bed without having her breathing machine on, the tray table across the room so no access to call button or water and dirty linens left on floor Her answer was to hand the family a complaint form Don't be fooled by the staff schedule in front lobby- it was infrequent that people scheduled were actually there Terri the corporate person who toured us before admission told many lies. She had no knowledge of staff to patient ratios and answered many questions with "I'm just filling in here and don't know how it works in this facility" We should of ran then . Don't let the nice surroundings fool you if you want your loved one to heal and be cared for in a human way.

I did not understand how these stretching and posture exercises could help reduce the pain. I was wrong. The therapist targeted my exercises and stretching to alleviate my specific pain issues. The staff was confident, friendly and extremely thorough. They asked the right questions, evaluated my images and concerns. I experienced a slight reduction of pain after my very first session. Today, after four weeks of sessions, I feel like a brand new woman. My pain has been significantly reduced and I feel great. They taught me simple stretches I can do at home and work to stave off any flair ups. I've been converted. Thank you St. Mary's!!!

    Do not. I repeat-DO NOT bring your loved one here. There are only three aspects of this facility…read morethat deserve commendation - the physical therapists, occupational therapists, and social services. Unfortunately, the other aspects of the facility make it far from worth it to come here for rehab. Upon arrival, our family was given no general information on the facility, including facility rules and the intended plan for the coming weeks of my father's stay. We were not given any information on who his doctor would be, who his nurse practitioner would be, or where anything was in the facility. It's an extremely outdated building with a rank smell (SNFs should never have a smell). What we quickly came to learn is that the nursing staff only makes things more complicated and frustrating for residents and families. It's like pulling teeth to find out where things are in the building and getting information about where things are so that the families can at least get what patients need themselves. It took 30 minutes for an aide to bring water to my father. They do not change linens daily, even if there are stains on the sheets. They do not heed dietary restrictions. They are rude to families and amazingly lazy. Families should be paid instead of needing to pay because we essentially do the jobs of the nurses and aides when visiting. And finally-we've seen several instances of blatant neglect. Nurses ignore room calls. They do not periodically survey the halls to find out if patients need assistance (I called one evening and was told that surveying the halls once an hour was too demanding for them). We've seen patients sitting outside of the nurse's station calling for help with no one coming for half an hour. We attempted to take pictures of neglect taking place, only to be chased down by the head of nursing and told that they would call the police. Clearly they know that neglect happens every day here and do not want to be implicated for it. We are transferring my father to the Oakland Heartland, which is considered to be a much better SNF and we are hopeful that it will live up to its good reviews.
