I will say that the speech therapist and physical therapist seem to be very kind and thorough, and one of the nurses/CNAs was very kind as well. They do have a large gym area for physical therapy. The facility, however, is depressing. When you enter, the lobby looks lovely with wood floors and a piano. The front desk staff range from mildly friendly to apathetic. On a recent visit a staff member was slurping soda through a straw from a can and just glared at us as we signed in without saying a word. As you move further from the lobby, however, the wood flooring is replaced with linoleum and the place looks increasingly dilapidated. The first room mom was placed in had a leaking HVAC unit with tons of towels piled under it apparently to soak up the leak. Gross. Eventually she was moved to a better room. And the food - yikes! Do they even have a dietician at this place? One dinner was a cheese enchilada - is that even a balanced meal? Last night it was a sad looking sausage sandwich with the driest and most over cooked french fries that you can possibly imagine. Anway, I guess she is getting the physical therapy which is the reason for being at TLC. But don't expect much more in terms of comforts...

It makes my skin crawl when I go visit him. I believe they have painted over mold in the bathroom. I couldn't tell you what year the place was last cleaned. This didn't happen overnight. Yesterday, (4/5/26) I called my friend to check on him and asked him how breakfast was. It was around 9:30 a.m.. He said he hasn't been served breakfast. He said they didn't mind flipping on the lights and waking him up at 5 a.m. saying, "I'm going to clean you up and change your bedding before your family gets here and complains". He told them to get out and turn out the lights. They came back in and changed his sheets at 9:16 a.m.. How do I know you ask? They literally write the date on whatever they do, i.e., put a bandage on, change a bandage, change the sheets, etc. I digress, back to breakfast. I told my friend I just need to get dressed and then I'll go get him a great breakfast. I got to his room around 10:30-10:45 a.m. with breakfast. They still had not brought him a tray. The nurse practitioner had come the day before and said he needs to be in the wheelchair a good part of the day and scoot himself around to build strength back up in his legs. So when another mutual friend showed up, we went to the desk to see if someone could come put him in the wheelchair. When someone showed up, I asked why at 11 a.m., had he still not been served breakfast? The employees response, "well he doesn't eat it anyway!" Not, oh my gosh, I'm so sorry, let me go get him something. I told them it is normal for him to let it sit awhile and then pick at it but that's his choice. They literally deprived him of food that morning. Remember, this is a human being, a senior citizen who is quite sharp but got an abscess that landed him in the hospital near death. He was diagnosed with sepsis from a grapefruit sized abscess on the back of his neck. After 5 nights in the hospital, he was transferred to TLC Care Center. His sister had been given a tour of the facility the day before. This was a hospital recommendation. They showed her some nice areas and even a nice vacant room. When they arrived at 1 a.m., they set him up in a room and slapped a diaper on him. His sister just broke down into tears when she saw this room. It was nothing like the room they showed her. As you will see in the video, it is disgustingly filthy. There is no way this room would pass a health inspection. Again, this is a man that has dignity, and although he can't walk alone, he wanted to be able to use the restroom to relieve himself. In the hospital, he'd had a catheter and used a bed pan for his other business. He now needed to use the restroom to do all of his business. He asked for assistance to the restroom and was told, "well, you have a diaper on, just go!" He was mortified. Once he did his business, he asked to be cleaned up. He was told he isn't the only one there. His sister said she wasn't leaving until they cleaned him up. He sat there in his own feces for an hour. His sister spent the whole next day with him and found out the care was as bad as the unsanitary conditions. This facility was one of two places recommended because he needed a facility that could take care of the drain that had been placed in the back of his neck where the abscess was. That didn't turn out well either. His sister had to go back to her home state, to a husband who had just had reconstructive surgery the day before her brother had called in critical condition. She had already left him alone for 7 days. She called and checked on her brother the next day. He still had not been showered or helped out with basic hygiene care. He had not been able to shower the five days he was in the hospital and he had now been at this facility for four days. He desperately wanted a shower. His sister reached out to a mutual friend and asked if she would go to see what is going on and why he isn't being taken care of. When she arrived and asked why he had not had a shower in four days, she was told he had had one the night before. He said he absolutely had not been given a shower, which was evident. Then she was told that showers are only given twice a week on certain days. She said, "well you better make an exception and get him a shower." They finally helped him to the shower. Us two mutual friends have been showing up just to make sure some of his basic needs are met, i.e., food, water, shower, a clean diaper while his sister is back home, tending her husband. The next day, someone came to get him for PT. They told us it was for 30 minutes. From the time he got wheeled out to the time he got wheeled back into the room, it was exactly 20 minutes. I guess what I'm saying is, if you want a clean, sanitary, well staffed place for your loved one, look elsewhere.

My beautiful lady, Mary Lou was here as a guest during her transition from being at home. Unfortunately, she's at the moment now where she needs assisted living and that extra care. It's been a long and stressful time. Rough couple of months. Finally, while this was not permanent the staff and facilities here truly were a "god send!" This is a place where your loved ones are in great hands. The rooms primarily are not private but accommodate two individuals. They are co- ed. You never know how long your neighbor in the next bed will be here. The telephone and television are on both sides of the bed, but the loudness can interfere with the patients comfort level. The doctor and nurses here are so overwhelming great the way that they handle the patients care and pain levels. The patient, even Mary Lou, my angel, does not want to be here. She wants to go home . There is no more home for her. It's assisted living. They make this transition easier for the patient. The visiting hours are longer than normal and all an individual needs to do is sign it. You're allowed to bring outside food and drinks for patients . You can decorate and adorn their space with flowers and other trinkets to make them feel a little piece of home. Love is the focus and makes the world turn. Sometimes we cannot decide where we will be. When it comes to the end we will go in and out of who and where we are ?! My Miss Marylou, my angel you are loved and will be continued to be loved as long as you are here. We will go beyond to make you happy.

My mom has…read morebeen here for several weeks and was transferred from an acute rehab for continued nursing and physical therapy. It seems as though the care provided has been no different than had we put her in hospice... mind you, we are nowhere near that point in her life. Her weakness has only gotten worse, physical therapy has not been done as we were advised it would be, and she continues to lose weight, her muscle strength, and has been laid up in her bed for more than three weeks now with very little movement. She has become stiff and rigid in the position that she's in and is very painful to even attempt to move her into a sitting position. Communication from the staff and her case manager has been extremely poor. I was notified last week on Wednesday that Medicare was only providing one more week of treatment for her and have yet to be provided a discharge date. I have left several messages for the staff with no response and still don't know when I need to set up transfer for her to depart that facility. Based on the timeframe provided, it would appear that I have 24 hours or so from now, yet no information has been provided to our family on when she will be discharged. Her time at this facility has been a complete waste of time and only further set back any progress she had made prior to admission. While Mom says "most of the people are nice," she also said that they very rarely even check on her and never move her. Please do your research to find the best fit for your loved one. We thought we had, and while we understand that skilled nursing is a lower level of care, we feel that there is zero excuse for the lack of treatment she has been provided. It is also important to note that many times when we have been there visiting, there have been other patients crying out for help from their rooms that has gone on for more than 10 minutes before anyone responds to their request or urgency. This is uncalled for! The staff is there to care for these patients and they hardly acknowledge the calls for assistance... definitely not in a timely manner. We are extremely disappointed with our experience and how our loved one has been cared for, or lack there of.

Neglect at its finest and I WILL know what occurred no matter what the cost... Once a brilliant intelligent resilient kind man, who as of now, has no memory of his existence and little ability to speak, think, see, move or function.     I had a great experience with Align Physical Therapy while recovering from severe tennis elbow and…read moresignificant forearm pain and limited mobility. The team helped me make meaningful progress through a combination of strengthening exercises, stretching techniques, and hands-on massage therapy. When I first started, the pain and immobility were really impacting my ability to use my arm normally, and I'm very happy with how much stronger and more functional it became throughout therapy. The combination of hands-on treatment and targeted exercises gave me tools I can continue using to prevent future issues. The only part of my experience that I wasn't particularly fond of was feeling like there was an effort to upsell me on additional TENS/electrotherapy treatments. I would have preferred that those options simply be presented as part of the available treatment choices without feeling like an additional sales pitch. Overall, I'm very grateful for the care I received and the progress I made. If you're dealing with an injury or mobility issue, especially as an active athlete, I would definitely recommend giving Align Physical Therapy a try.
