My grandmother passed away on Sunday, and in the weeks prior, we came to learn of gross negligence by the contracted doctor at the Addison of Indian Trail, Amy P. The primary issues were: -Overmedication, to an extent that negatively impacted my grandmother's health outcomes. -Incomplete medical history and documentation, led to paramedics misdiagnosing my grandmother because symptoms that were associated with her medical conditions weren't disclosed. When the doctor was asked about why the conditions (dementia) weren't listed on her paperwork, she replied "We all know she has dementia". The family arrived on the scene and was able to intervene. Paramedics confirmed that my grandmother was in her normal state and did not need ER attention. -Confusing patient medical information - Amy confused my grandmother with another patient who had experienced a head injury, told our family that my grandmother had experienced a head injury, and made a recommendation to send her to the ER based on this. When questioned about it, she responded with "Wasn't she the one with the head injury?", showing confusion, lack of documentation, and lack of discretion at sharing information about the patients medical condition. Cissie V defended this mistake calling it "human error". -A lack of cooperation and even hostility towards communicating with the family - when Amy was asked to communicate with the family about patient care she responded "I have multiple facilities and six kids to take care of, I don't have time to talk to all my patients families." My grandmother was admitted to the hospital in recent weeks, and the hospital informed our family that she had been far overmedicated for her current conditions. This malpractice resulted in grave outcomes for her. Her well-functioning kidneys became barely functional, and eventually failed altogether, leading to what would ultimately be her death. It was slow and difficult. In the instance mentioned above regarding confusing my grandmother with the patient who experienced a head injury, the Addison called paramedics to come assess her and EMS was concerned that my grandmother had experienced a stroke because of her confusion. It was later identified that her paperwork from the Addison did not list her dementia and no member of the staff had informed the paramedics of her dementia. It was only after our family arrived and informed the paramedics of her dementia that they were able to accurately assess her symptoms. This is just one example of how the disorganization and incomplete medical documentation at the Addison directly impact medical professionals' ability to administer appropriate care to the residents living there. In the first month my grandmother lived at the Addison, my she experienced a fall. Despite excruciating pain, she was able to drag herself to the alarm hanging from the wall to call for help - no one came. After hours, she was discovered during regular rounds, on the floor and in pain. In assessing why no one came to her aid, two issues were uncovered: (1) the alarm on her wall was not working and did not send alerts to the main system and (2) the person on shift did not take their phone/walkie talkie with them on rounds, so even if the alarm had worked, they would not have been aware of it. This example demonstrates a failure on the Addison's part of ensuring safety equipment is working and functional and that staff members are trained and that operational standards (like a staff member taking the phone with them at all times) are followed - a combination of which leads to extremely dangerous conditions for the residents. If you have a loved one needing assisted living care, I would urge you to look elsewhere so your family member is not compromised by sorely inadequate and risky practices adopted by Addison of Indian Trail.

Living without my mom since 8/19/23 is an immeasurable loss. I would give a 0 rating- the review won't allow less than 1 star. The depths of despair, unimaginable absence of knowledge, lacking sense of urgency and, non-existent communication overshadow the hallways at The Addison. The only thing superseding the above is the retaliation put forth by The Addison against family members who advocate for their loved ones. This community should be condemned, (I did encounter a few genuine caregivers- those few sadly, were not enough). On 8/11/23, my mom was suffering a massive heart attack under the care of the memory care staff; I had to demand the Director of Nursing step up and give aid to my mother. She proceeded to place a bag of ice in my mom's lap and asked her to cough. Hesitation and excuses when I yelled at them to call 911. My mom would have died right there had I (and my mom's close friend), not been there to witness and act upon the situation. This was the best this community could offer my mom- I will never understand. I have many examples of what I witnessed here (wheelchair bound residents locked in their rooms w/ no key for staff, blood stained furniture for the residents to sit on, and much, much more.). I filed two separate investigations with Social Services, as a result. Specific to my mom (the staff missed ALL of the following): 1)massive heart attack, 2)high ambulatory to inability to walk the halls, 3)20lb weight loss, 4)20lb weight gain, 5)severe facial infection, 6)aspiration pneumonia (TWICE), 7)inability to breathe We had a division of Hospice assisting (not end of life care), they too, missed all of this Additionally, the staff here: 1)attempted to administer sedative without family consent, 2)-falsely accused family of administering meds, 3)stole my mother's dentures when she was sent to the ER, 4)retaliated as a direct result of reporting the community to social services, 5)abused their authority: disallowing me to enter my mom's shared room stating, policy does not allow family into a shared room (which, prior to my reporting the community, I never had an issue going into my mother's shared room). After 9 months of fighting with this community and, bringing Sr Leadership at their corporate office into the equation, it was determined said policy did not exist, instead, it was said preventing me access (while not preventing other resident family members access), was "a practice Monica Adams (on-site Director), uses". Monica denied me access to my mom's room for the better part of a year for no other reason than, she could. Think about that- I was deliberately denied access to my mom's room because it was decided it was in the community's best interest that I not be allowed to spend the last months of my mom's life with her, in the room she paid for. Monica had her entire staff enforce this non-existent policy denying me access to my mom's very basic needs. The Addison's employees stood at the front door literally barricading me with their bodies from entering when my mom was discharged from the hospital back to the Addison, stating to the CEO that they were instructed by Monica to do so- I had CEO on speed dial at this point- he insisted they allow me entry. This community does not champion a fearless leader instead, this community is home to a "lead by fear" mentality. With all their years of experience, they did nothing for my mother other than ensure her bed became available for the next resident. Please don't let my mom's death be in vain. As far as Social Services ..."we are not saying your claim/s aren't valid, we are simply saying at the time of our investigation, there was no evidence of any of your complaints." The system is broken and our family's suffer. To be in a position where you're chosen to provide a service to the members of a community is an honor. Being in the position of running a community such as The Addison, means that you provide a service to the community with a sense of community, passion, empathy, care and a sense of genuineness which, you also extend to the family- you will not find any of that here. Shame on you all at the Addison - may your loved ones never need the care which, my family entrusted to you.There is zero honor or dignity to be found in these hallways. The only thing the people who run this place can offer you is a shortened lifespan for your loved one and a consistent auto draft to their bank account. Your family deserves so much more, as my mom did. Following my mom's passing, not one individual from this community where my mom spent nearly two years of her life, reached out to offer acknowledgement, condolences or, support. Wishing The Addison of Indian Trail and their Corporate Offices a Christmas filled with all they deserve inclusive of the joys of their family.I'll be at the cemetery.

    Reviews are subjective of course but I believe they can be divided into sections eg operational…read moreprofessionalism/efficiency, subject expertise of provider, and quality of care/treatment/result. There are many challenges for patient and provider alike in these areas due to consolidation in many specialties, frequent turnover in organizations/offices and simply getting lost in the bureaucracy (insurance, follow-up care, patient volume etc). Modern medicine is remarkable for its wealth of knowledge, technology & efficacy but it's very easy to feel like a number instead of a patient at times. With all this in mind I'm happy to report that Gaitway & Alison Starkey achieve full marks in the areas outlined above while also avoiding avoid the pitfalls. Gaitway took the time to fully document & understand patient needs, history and desired outcome(s). Exams, analysis and advice were thorough and patient-specific (ie no boilerplate). Treatment was equally personalized and modified/augmented over time to achieve optimal results. Concerns & questions regarding treatment, appointments, billing, etc. were handled clearly & rapidly. Highest recommendation!

My husband suffered a massive brain hemorrhage due an AVM rupture on july 11, 2018. After doing Inpatient and outpatient therapy through Atrium, we knew we were ready for more and Gaitway and Alison came to the rescue. She is so patient and really understands my husbands needs and the way he responds to therapies. And she is super funny!! Alison's rolodex should be the envy of all PTs in Charlotte- she has referred us to Race to Walk (this was just during a consult prior to us using her services), helped with recommending equipment to make out home safer for my husband in all his stages from wheelchair to cane. And introduced us to a great speech therapist to help with his very high level executive function deficits as well as a therapist for me!! I really needed someone to talk to that understood what a caregiver goes through- and wouldn't you know it, Alison referred me to a Therapist who specializes in caregivers! My husbands progress since we've started using Gateway in July of 2019 has been remarkable. He did so well with his progress at Atrium but we knew he could be pushed further and having the therapy at home has really made it come together because it is truly functional therapy related to how we live, where we live, etc. I know that between my husband's tremendous dedication and determination and the dedication of Alison and her team of therapists, we would not be where we are today-- and where we are is- he is ready to stop using a cane in about a week, he is using his left hand/arm (he is left side affected) for function things like opening/closing the dishwasher, opening/closing refrigerator, he's washing himself in the shower with min to zero assistance, he's dressing himself fully 3 times a week, he's making his own lunch 3 times a week, the list goes on!
